POPULATIONS SERVED WILL INCLUDE THE FRAIL AND ECONOMICALLY DISADVANTAGED SENIORS. THE PRIMARY FOCUS AREA WILL BE HEALTHY FUTURES. VOLUNTEER SERVICE ACTIVITIES INCLUDE FOOD DELIVERY, TRANSPORTATION, COMPANIONSHIP, COMPANIONSHIP WITH VETERANS, FOOD DISTRIBUTION AND COLLECTION, FOOD PANTRY SUPPORT, ASSISTING SOUP KITCHENS, DISTRIBUTING INFORMATION, AND LEADING AND ASSISTING TAI CHI FOR ARTHRITIS, MATTER OF BALANCE AND BONE BUILDERS CLASSES. ADDITIONALLY, WE WILL HAVE WORK PLANS WITHIN THE ECONOMIC OPPORTUNITY FOCUS AREA, INCLUDING ASSISTING VITA, PREVENTING ELDER ABUSE, AND PROVIDING FINANCIAL LITERACY EDUCATION; THE ENVIRONMENTAL STEWARDSHIP FOCUS AREAS, WITH A SERVICE ACTIVITY TO IMPROVE TRAILS; AS WELL AS NUMEROUS SERVICE ACTIVITIES WITHIN THE COMMUNITY PRIORITY AREA. LOCATIONS OF ALL SERVICE ACTIVITIES ARE WITHIN AROOSTOOK COUNTY MAINE, AND INCLUDE SITES SUCH AS AAAA, AROOSTOOK REGIONAL TRANSPORTATION, THE MAINE VETERANS HOME, UNITED WAY OF AROOSTOOK, AROOSTOOK NATIONAL WILDLIFE REFUGE, NUMEROUS FOOD PANTRIES, HOSPITALS, AND OTHER COMMUNITY NON-PROFITS. EXPECTED RESULTS AND OUTCOMES OF THE ACTIVITIES INCLUDE THE FOLLOWING: 150 HOME-BOUND ADULTS WILL RECEIVE NUTRITIOUS FOOD, 250 HOME-BOUND ADULTS WILL RECEIVE MEDICAL TRANSPORTATION ASSISTANCE AND OTHER SERVICES THAT ALLOW THEM TO LIVE INDEPENDENTLY, 100 HOME-BOUND ADULTS WILL REPORT HAVING INCREASED SOCIAL SUPPORT, 1500 INDIVIDUALS WILL RECEIVE FOOD FROM FOOD BANKS AND FOOD PANTRIES, 800 CLIENTS WILL RECEIVE INFORMATION ON HEALTH INSURANCE, HEALTH CARE ACCESS AND HEALTH BENEFITS, AND 150 CLIENTS WILL PARTICIPATE IN HEALTH EDUCATION PROGRAMS. AROOSTOOK RSVP AND AAAA HAVE BEEN THE SOLE PROJECT AND SPONSOR FOR THE IDENTIFIED GEOGRAPHIC AREA OF AROOSTOOK COUNTY MAINE SINCE BOTH OF THEIR INCEPTIONS IN 1972. AN ESTIMATED 440 VOLUNTEERS WILL SERVE. OF THIS NUMBER, 352 VOLUNTEERS WILL BE PLACED IN OUTCOME ASSIGNMENTS. THE PRIMARY FOCUS AREA IS HEALTHY FUTURES. POPULATIONS SERVED WILL INCLUDE FRAIL AND ECONOMICALLY DISADVANTAGED SENIORS. VOLUNTEER SERVICE ACTIVITIES WILL INCLUDE FOOD DELIVERY, COMPANIONSHIP, COMPANIONSHIP WITH VETERANS, FOOD DISTRIBUTION AND COLLECTION, FOOD PANTRY SUPPORT, DISTRIBUTING MEDICARE INFORMATION, PREVENTING ELDER ABUSE SCAM AND FRAUD, AND LEADING AND ASSISTING NUMEROUS EVIDENCED-BASED ACTIVITIES INCLUDING LIVING WELL FOR BETTER HEALTH, LIVING WELL WITH CHRONIC PAIN, LIVING WELL WITH DIABETES, A MATTER OF BALANCE, TAI CHI, AND BONE BUILDERS CLASSES. IN 2023, THE AGENCY, WORKING THROUGH THE MAINE RURAL COMMUNITY HEALTH IMPROVEMENT PARTNERSHIP (R-CHIP), CONVENED STAKEHOLDERS THROUGHOUT AROOSTOOK COUNTY, MAINE TO BEGIN DISCUSSIONS ON DEFINING AND IMPLEMENTING TRANSFORMATIVE APPROACHES TO WHOLE-PERSON CARE. THE AGENCY CREATED THE AROOSTOOK COUNTY HEALTH IMPROVEMENT PARTNERSHIP (ACHIP) AND BROUGHT TOGETHER MORE THAN TWENTY PARTNERS IN THE HEALTH CARE, PUBLIC HEALTH, AND SOCIAL SERVICES SECTORS TO COMPLETE A YEAR-LONG PLANNING PROCESS AND SUBSEQUENT READINESS ASSESSMENT. THAT ASSESSMENT IDENTIFIED THE INTERSECTIONS BETWEEN TRANSPORTATION, SOCIAL CONNECTION/COMMUNITY BELONGING, AND HEALTH CARE ACCESS AS CRITICAL, YET UNADDRESSED, HEALTH-RELATED SOCIAL NEEDS. THROUGH THE HEALTH RESOURCES AND SERVICES ADMINISTRATION’S (HRSA) GRANTS.GOV NOTIFICATION OF THE “RURAL HEALTH NETWORK DEVELOPMENT PLANNING PROG RAM”, ACHIP PROPOSES TO DEVELOP A NETWORK TO LINK RURAL HEALTH CARE STRATEGIC PARTNERS TOGETHER TO ADDRESS THE ISSUES OF TRANSPORTATION, SOCIAL CONNECTION/COMMUNITY BELONGING, AND ACCESS TO HEALTH CARE. ACHIP BELIEVES THIS WORK WILL HELP RURAL STAKEHOLDERS ACHIEVE GREATER CAPACITY TO OVERCOME CHALLENGES RELATED TO SERVING THE NEEDS OF A RURAL POPULATION FOR WHICH GEOGRAPHICAL DIVERSITY AND ECONOMIES OF SCALE LIMIT SUCCESSFUL IMPLEMENTATION OF TRANSFORMATIVE HEALTH CARE. THE STAKEHOLDER PARTNERS FOR THIS PROJECT (NORTHERN MAINE MEDICAL CENTER, CARY MEDICAL CENTER, NORTHERN LIGHT AR GOULD HOSPITAL, HOULTON REGIONAL HOSPITAL, THE AROOSTOOK MENTAL HEALTH CENTER, THE AROOSTOOK TRANSPORTATION SYSTEM, AROOSTOOK EMS, AROOSTOOK PARTNERSHIP, AND THE AROOSTOOK AGENCY ON AGING) ARE ALL EXPERTS IN THEIR FIELDS AND ARE WELL-POSITIONED TO MOVE ACHIP FROM ANALYSIS OF THE ISSUES TO CREATION AND IMPLEMENTATION OF WORK PLANS THAT WILL IMPROVE TRANSPORTATION TO SERVICES, ENGAGEMENT IN THE COMMUNITY AND CONSISTENT ACCESS TO ONE’S PRIMARY CARE PROVIDER.
