(FHCSO), LOCATED IN RURAL SOUTHERN OKLAHOMA, HAS A SERVICE DELIVERY SITE LOCATED IN EACH OF THE FOUR COUNTIES IT SERVES: ATOKA, COAL, JOHNSTON AND MARSHALL. POVERTY RATES IN THIS AREA ARE EXTREMELY HIGH WITH AN AVERAGE OF 19.13% OF THE 47,269 RESIDENTS IN THE SERVICE AREA LIVING IN POVERTY WITH 20.63% UNINSURED (2019 ESTIMATES - US CENSUS QUICK FACTS). THE 2020 UDS SHOWS THAT ONLY 346 OF FHCSO’S 8,610 PATIENTS WERE ABOVE 200% FPL WITH 22.73% UNINSURED. FHCSO IS ALSO SEEING GROWTH IN ITS HISPANIC POPULATION (13.25% OF ITS PATIENTS), THE POPULATION EXPERIENCING THE MOST GROWTH IN HIV INCIDENCE (2013-2017 OSDH DATA). THE PROPOSED FHCSO PCHP PROJECT WOULD ALLOW THE HIRING OF AT LEAST TWO NURSES AND 1.5 OUTREACH STAFF TO ENGAGE NEW PATIENTS BY PROVIDING OUTREACH AND HIV PREVENTION EDUCATION AT COMMUNITY LOCATIONS ACROSS THE SERVICE AREA. THIS WILL HEIGHTEN COMMUNITY AWARENESS OF HIV RISKS AND RELATED CONDITIONS, INCLUDING MENTAL HEALTH ISSUES, SUBSTANCE USE DISORDERS, VIRAL HEPATITIS, ENDOCARDITIS AND SEXUALLY TRANSMITTED INFECTIONS. STAFF TRAINING WILL ALSO INCLUDE EDUCATION, CLINICAL CONSULTATION, PEER COACHING, AND OTHER TECHNICAL ASSISTANCE AS NEEDED TO ASSIST THOSE IN NEED OF HIV CARE. BY ENGAGING IN ACTIVITIES OF EACH FOCUS AREA – PREP PRESCRIBING, OUTREACH, TESTING, AND WORKFORCE DEVELOPMENT – FHCSO WILL INCREASE THE NUMBER OF PATIENTS COUNSELED AND TESTED FOR HIV TO 1,000 INDIV IDUALS BY DECEMBER 31, 2023. WITH THIS FUNDING, FHCSO STAFF WILL RECEIVE PROFESSIONAL DEVELOPMENT TO INCLUDE PREP PRESCRIBING PRACTICES AND BARRIERS FOR INDIVIDUALS WHO HAVE BEEN SCREENED AND IDENTIFIED AS BEING AT RISK OF CONTRACTING HIV. A PROJECTED NUMBER OF 100 PATIENTS WILL BE PRESCRIBED PREP OVER THE TWO-YEAR PROJECT PERIOD. FHCSO WILL WORK CLOSELY WITH THE OKLAHOMA STATE DEPARTMENT OF HEALTH (OSDH) NETWORK, INCLUDING THE OKLAHOMA HEPATITIS AND HIV PLANNING COUNCIL, TO ENGAGE IN COORDINATED RAPID START FOR THOSE DIAGNOSED WITH HIV AND IMPLEMENT PREP FOR THOSE AT RISK TO END HIV IN OKLAHOMA. BY WORKING WITH THE STATE NETWORK, FHCSO PROJECTS THAT 100% OF PATIENTS FOUND TO HAVE HIV WILL BE LINKED TO CARE AND TREATMENT WITHIN 30 DAYS OF DIAGNOSIS WITH A PROJECTED NUMBER OF 5 PATIENTS. HIV HOME TEST KITS WILL ALSO BE MADE AVAILABLE TO SUPPORT RAPID TESTING AND ACCELERATE ENTRY INTO CARE. FHCSO’S PCHP PROJECT WILL GEOGRAPHICALLY COVER THE SERVICE AREA AS DESCRIBED ABOVE, ROTATING PCHP STAFF TO THE FOUR DELIVERY SITES AND USING THE FHCSO MOBILE UNIT TO ASSIST IN COMMUNITY OUTREACH AND SCREENING EVENTS TO BROADEN ITS REACH AND HELP END THE HIV EPIDEMIC.

FHCSO OPERATES FOUR PERMANENT CLINIC SITES AND ACCORDING TO THE 2023 UNIFORM DATA SYSTEM REPORT SERVED 9,734 PATIENTS. SERVICES PROVIDED INCLUDE PRIMARY AND PREVENTIVE MEDICAL CARE, DENTAL, MENTAL HEALTH SERVICES (SUBSTANCE USE DISORDER SERVICES ARE NOT CURRENTLY PROVIDED OTHER THAN MEDICATION FOR OPIOID USE DISORDER), AND PHARMACY SERVICES. ACCORDING TO THE CENTERS FOR DISEASE CONTROL AND PREVENTION, RATES OF DEPRESSION ARE HIGHER IN SERVICE AREA COUNTIES COMPARED TO THE NATIONAL AVERAGE. DEPRESSION AFFECTS HOW A PERSON FEELS, THINKS, AND HANDLES DAILY ACTIVITIES, SUCH AS SLEEPING, EATING, AND WORKING. WHEN A PERSON SUFFERS FROM DEPRESSION, THEY OFTEN THINK MORE NEGATIVELY ABOUT THEMSELVES, AND IT CAN LEAD TO THOUGHTS OF SUICIDE OR SELF-HARM. RATES OF MORTALITY FROM INTENTIONAL SELF-HARM (SUICIDE) ARE SUBSTANTIALLY HIGHER IN SERVICE AREA COUNTIES AND CORRELATE WITH THE AREA’S HIGH RATES OF DEPRESSION. OPIOID USE DISORDER IS ALSO IMPACTING THE SERVICE AREA WITH 11 OPIOID-RELATED DEATHS IN SERVICE AREA COUNTIES DURING 2018-2022. DURING THE SAME TIMEFRAME, THERE WERE 185 NONFATAL DRUG OVERDOSE DISCHARGES, OF WHICH 42 WERE DUE TO OPIOIDS. THE TOTAL COST OF HOSPITALIZATION DUE TO THE NONFATAL DRUG OVERDOSES WAS $10 MILLION. THROUGH THIS BEHAVIORAL HEALTH SERVICE EXPANSION PROJECT, FHCSO WILL EXPAND ACCESS TO MENTAL HEALTH, SUBSTANCE USE DISORDER (SUD), AND MEDICATION FOR OPIOID USE DISORDER (MOUD) SERVICES. STAFFING TO ACHIEVE THESE EXPANDED SERVICES WILL BE THROUGH DIRECTLY-EMPLOYED PROVIDERS AND SUPPORT STAFF INCLUDING: 1.0 FTE PHARMACY TECH, 1.0 FTE PSYCHIATRIC NURSE PRACTITIONER, 1.0 FTE LICENSED PROFESSIONAL COUNSELOR (LPC) SUBSTANCE ABUSE COUNSELOR, 1.0 LPC, 1.0 FTE CASE MANAGER, 1.0 FTE PATIENT INTAKE COORDINATOR, AND 1.0 FTE PHARMACIST. FUNDS WILL BE UTILIZED TO EXPAND ACCESS TO MENTAL HEALTH, SUD, AND MOUD SERVICES VIA TELEHEALTH, ENSURING ACCESS TO THESE SERVICES THROUGHOUT THE RURAL SERVICE AREA. AS A RESULT OF EXPANDED SERVICES, FHCSO WILL IMPLEMENT SUD GROUP COUNSELING SERVICES IN ALL COUNTIES. TRAINING WILL BE PROVIDED FOR FHCSO MEDICAL PROVIDERS TO INCREASE THE NUMBER OF MOUD PRESCRIBING PROVIDERS, AND MENTAL HEALTH FIRST AID TRAINING FOR RURAL COMMUNITIES WILL BE PROVIDED FOR FHCSO STAFF. THROUGH THIS PROPOSAL, FHCSO WILL INCREASE THE HOURS OF CLINIC OPERATION AT ITS TISHOMINGO AND ATOKA CLINIC LOCATIONS, INCLUDING EXTENDED HOURS ON FRIDAY UNTIL 6:00 P.M., SATURDAYS FROM 8:00 A.M. TO 6:00 P.M., AND SUNDAYS FROM 12:00 P.M. TO 4:00 P.M. FHCSO’S CLINICS ARE NOT CURRENTLY OPEN DURING THESE TIMES, AND THESE WOULD BE EXPANDED HOURS OF OPERATION FOR EACH OF THESE CLINIC SITES. THROUGH THIS EXPANDED HOURS PROJECT, FHCSO WILL EXPAND ACCESS TO PRIMARY MEDICAL CARE SERVICES DURING NON-TRADITIONAL CLINIC HOURS. STAFFING TO ACHIEVE THESE EXPANDED HOURS WILL BE THROUGH DIRECTLY-EMPLOYED PROVIDERS AND SUPPORT STAFF INCLUDING: 1.0 FTE NURSE PRACTITIONER (0.5 FTE AT EACH CLINIC LOCATION), 1.0 FTE NURSE TO SUPPORT THE PROVIDER STAFF (0.5 FTE AT EACH CLINIC LOCATION), 1.0 FTE PATIENT SUPPORT STAFF (0.5 FTE AT EACH CLINIC LOCATION), AND 1.0 FTE OFFICE MANAGER TO SUPPORT BOTH CLINICS DURING AFTER-HOURS.
