CONGRESSIONALLY DIRECTED SPENDING FOR CONSTRUCTION PROJECTS - HRSA-24-110 CPF/CDS: CONSTRUCTION PROJECT CONGRESSIONAL DISTRICTS IMPACTED: NH-002 THE ORGANIZATION: MID-STATE HEALTH CENTER IS AN INDEPENDENT, NON-PROFIT ESTABLISHED IN 1998 WHOSE MISSION IS TO DELIVER COMPREHENSIVE PRIMARY CARE SERVICES TO ITS COMMUNITY REGARDLESS OF ABILITY TO PAY. AS AN FQHC, MID-STATE OPERATES WITH A CLEAR AND COLLECTIVE VISION OF PROVIDING HIGH-QUALITY PRIMARY CARE SERVICES TO ITS GEOGRAPHICALLY ISOLATED, RURAL COMMUNITY THROUGH ITS FOUR SERVICE DELIVERY SITES. AS A RECOGNIZED PATIENT-CENTERED MEDICAL HOME RECOGNIZED BY THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE, MID-STATE IS COMMITTED TO THE DELIVERY OF A COORDINATED, PERSONALIZED CARE EXPERIENCE AS PART OF AN INNOVATIVE COMMUNITY HEALTH MODEL. MID-STATE FOSTERS STRONG COLLABORATIVE RELATIONSHIPS WITH A WIDE VARIETY OF PUBLIC AND PRIVATE ENTITIES WITHIN ITS SERVICE AREA AND THE STATE TO SUPPORT ACCESS TO HEALTH CARE SERVICES IN ITS EFFORT TO PROMOTE A HEALTHIER COMMUNITY AND POPULATION. SERVICE AREA AND TARGET POPULATION: MID-STATE DEFINES ITS SERVICE AREA AS 30 GEOGRAPHICALLY ISOLATED, RURAL COMMUNITIES; ALL ARE DESIGNATED MEDICALLY UNDERSERVED POPULATIONS OR AREAS. THE TARGET POPULATION IS THE ENTIRE AREA WITH A PARTICULAR FOCUS ON THE LOW-INCOME RESIDENTS AND SPECIAL POPULATIONS OF THE SERVICE AREA. CENSUS ESTIMATES REPORTED IN THE UDS MAPPER INDICATE 27% OF SERVICE AREA RESIDENTS, AS OPPOSED TO 19% OF NEW HAMPSHIRE RESIDENTS, WERE CONSIDERED LOW INCOME (<200% OF FPL). ADDITIONALLY, 10% OF SERVICE AREA RESIDENTS ARE LIVING IN POVERTY WITH HOUSEHOLD INCOMES LESS THAN 100% OF THE FEDERAL POVERTY LEVEL (FPL). THE PLANNED PROJECT WILL BE IN THE LINCOLN REGION WHICH IS HEMMED IN BY THE NATIONAL FOREST AND SURROUNDED BY SEMI-MOUNTAINOUS, SPARSELY POPULATED TERRAIN AND IS DESIGNATED BY HRSA AS A LOW-INCOME POPULATION PRIMARY CARE HEALTH PROFESSIONAL SHORTAGE AREA AND A HIGH-NEEDS GEOGRAPHIC MENTAL HEALTH SHORTAGE AREA. ACCESS TO SAFETY-NET HEALTH SERVICES OFFERE D ON A SLIDING FEE SCALE IS LIMITED. US CENSUS ESTIMATES IN 2020 FOR LINCOLN & WOODSTOCK DEMONSTRATE A STAGGERING 25% INCREASE IN POPULATION. THE DEMAND FOR SERVICES COUPLED WITH A SHORTAGE OF WORKFORCE IN GENERAL, PUTS SIGNIFICANT STRAIN ON THE REGION’S LIMITED RESOURCES. PROJECT SUMMARY: MID-STATE WILL COMPLETE A CAPITAL INFRASTRUCTURE PROJECT AND BUILD AN 8,000 SQ FT. HEALTH CENTER FACILITY IN THE LINCOLN REGION DESIGNED TO SUPPORT AN INTEGRATED PRIMARY CARE DELIVERY MODEL THAT INCLUDES MEDICAL, BEHAVIORAL HEALTH, SUBSTANCE USE DISORDER TREATMENT, PHYSICAL THERAPY, AND ENABLING SERVICES FOR THIS UNDERSERVED REGION. THE CONCEPT FOSTERS A SINGLE LOCATION OFFERING HEALTH CARE FOR ALL AGES. IN ADDITION TO THE FEATURES TRADITIONALLY FOUND IN PRIMARY HEALTH CARE DELIVERY, PLANS INCLUDE ONSITE LABORATORY AND PHARMACY SUPPORT SERVICES, A GROUP MEETING SPACE TO OFFER HEALTH EDUCATION TO THE COMMUNITY AS WELL AS PROCEDURE ROOMS DESIGNED TO SUPPORT ITS VISITING SPECIALISTS’ PROGRAM. MID-STATE ENVISIONS A WELLNESS CENTER OFFERING HIGH-QUALITY PRIMARY CARE SERVICES TO SUPPORT THE HEALTH NEEDS OF THE COMMUNITY. MID-STATE’S STEADFAST COMMITMENT TO “WHOLE” PERSON CARE WILL IMPROVE THE HEALTH AND VITALITY OF THIS GROWING, RURAL REGION AND THE PROPOSED HEALTH CENTER FACILITY WILL BRING COMPREHENSIVE, INTEGRATED PRIMARY CARE SERVICES TO THIS GROWING REGION ENSURING EQUITABLE ACCESS TO HIGH QUALITY HEALTH CARE FOR ALL.
