HOPE SERVICES’ COUNSELING CENTER (SAN JOSE) IS A SPECIALTY PROVIDER OF MENTAL HEALTH SERVICES WITHIN SANTA CLARA COUNTY THAT FOCUSES ON PEOPLE WITH DEVELOPMENTAL DISABILITIES (DD) AND CO-OCCURRING MENTAL HEALTH NEEDS. MENTAL HEALTH SERVICES PROVIDED WILL INCLUDE PSYCHIATRIC MEDICATION SUPPORT, INDIVIDUAL COUNSELING, CASE MANAGEMENT, AND PSYCHOSOCIAL AND PSYCHIATRIC ASSESSMENTS. THIS INTERVENTION PROJECT IS BASED ON AN AVERAGE TOTAL OF 5.5 HOURS OF SERVICE PER CLIENT PER MONTH. AN AVERAGE OF ONE-HALF HOUR (0.5 HOURS) OF PSYCHIATRIC MEDICATION SUPPORT, FOUR (4) HOURS OF INDIVIDUAL COUNSELING, AND ONE (1) HOUR OF CASE MANAGEMENT WILL BE PROVIDED TO EACH CLIENT PARTICIPATING IN THE PROJECT, AS WELL AS AN INITIAL TWO (2)-HOUR PSYCHOSOCIAL ASSESSMENT AND AN INITIAL ONE (1)-HOUR PSYCHIATRIC ASSESSMENT PER CLIENT. FOR THESE INDIVIDUALS, THESE SUPPORTS CAN MEAN THE DIFFERENCE BETWEEN THEIR BECOMING ACTIVE MEMBERS OF THEIR LOCAL COMMUNITIES OR HAVING A PERMANENT DISABILITY. THE SHORT-TERM INTERVENTION WILL LAST FOR AN AVERAGE OF 3 MONTHS PER CLIENT. FOUR (4) TO SIX (6) CLIENTS WILL BE SERVED EVERY TWO (2) MONTHS, FOR A TOTAL OF UP TO 22 CLIENTS PER YEAR. THIS PROJECT WILL INCREASE SOCIAL SKILLS AND REDUCE SOCIAL ANXIETY AND DISRUPTIVE BEHAVIORS AMONG PARTICIPANTS. THE GOALS OF THIS PROJECT ARE: A. REDUCE SUBJECTIVE SUFFERING FROM MENTAL ILLNESS B. INCREASE MEANINGFUL USE OF TIME AND CAPABILITIES IN SCHOOL, WORK, AND OTHER TYPES OF ACTIVITIES C. INCREASE NATURAL NETWORKS OF SUPPORTIVE RELATIONSHIPS SINCE HOPE CLINIC IMPLEMENTS EVIDENCE-BASED PRACTICES, THE SUCCESS OF THIS PROJECT CAN BE MEASURED BY: A. ENGAGEMENT IN SERVICES (TIMELINES/ACCESS): FIRST APPOINTMENT TO PATIENTS WITHIN TEN (10) BUSINESS DAYS FROM REFERRAL NO SHOWS BELOW 25% B. SUCCESSFUL DISCHARGES (QUALITY): NUMBER OF CLIENTS SUCCESSFULLY DISCHARGED AT 60% OR ABOVE C. ACUTE CARE RE-ADMISSIONS (QUALITY): NO MORE THAN 5% OF CLIENTS ADMITTED TO EMERGENCY PSYCHIATRIC SERVICES WITHIN THIRTY (30) DAYS OF THEIR SUCCESSFUL DISCHARGE ACTIVITIES IMPLEMENTED IN THE PROJECT: MEDICATION SUPPORT: MEDICATION WILL BE PRESCRIBED AND ADMINISTERED TO CLIENTS PARTICIPATING IN THIS PROJECT UNDER THE CLOSE CARE AND DIRECTION OF A PSYCHIATRIST. INDIVIDUAL COUNSELING: ONE-TO-ONE INDIVIDUAL COUNSELING WITH A TRAINED MENTAL HEALTH CLINICIAN IN A SAFE, CARING, AND CONFIDENTIAL ENVIRONMENT. ASSESSMENT: SCREENINGS, CONDUCTED BY A PSYCHIATRIST OR MENTAL HEALTH CLINICIAN, TO GATHER INFORMATION ABOUT INDIVIDUAL CLIENTS RECEIVING TREATMENT AND TO MAKE A DIAGNOSIS. CASE MANAGEMENT: ONE-TO-ONE INDIVIDUAL SESSIONS WITH A TRAINED CASE MANAGEMENT SPECIALIST TO LINK CLIENTS TO AVAILABLE COMMUNITY RESOURCES, AND ADVOCATE FOR CLIENTS AND THEIR FAMILIES. THE FOLLOWING KEY STAFF WILL PARTICIPATE IN THE PROJECT: THE PROJECT DIRECTOR, ASSISTED BY THE ASSISTANT DIRECTOR, WILL PLAN, COORDINATE, OVERSEE, AND EVALUATE THE EXPANSION OF HOPE’S MENTAL HEALTH SERVICES IN SAN MATEO COUNTY. A MANAGER WILL SUPPORT THE ASSISTANT DIRECTOR IN THESE ACTIVITIES. THE MENTAL HEALTH CLINICIAN WILL PERFORM NEEDED PSYCHOSOCIAL ASSESSMENTS AND DIAGNOSE CLIENTS, AND WILL PROVIDE INDIVIDUAL COUNSELING, PREPARE INDIVIDUAL SERVICE AND COORDINATION PLANS, DOCUMENT CLINICAL OUTCOMES, AND MAINTAIN APPROPRIATE TREATMENT RECORDS AND DOCUMENTATION, AS REQUIRED. TO EXPAND MENTAL HEALTH SERVICES FOR INDIVIDUALS WITH CO-OCCURRING DEVELOPMENTAL DISABILITIES AND MENTAL HEALTH NEEDS IN SAN MATEO COUNTY, CALIFORNIA. - HOPE SERVICES WILL EXPAND ITS MENTAL HEALTH SERVICES PROGRAM IN SAN MATEO COUNTY. HOPE SERVICES’ COUNSELING CENTER (SAN JOSE) IS A SPECIALTY PROVIDER OF MENTAL HEALTH SERVICES WITHIN SANTA CLARA COUNTY THAT FOCUSES ON PEOPLE WITH DEVELOPMENTAL DISABILITIES (DD) AND CO-OCCURRING MENTAL HEALTH NEEDS. MENTAL HEALTH SERVICES PROVIDED WILL INCLUDE PSYCHIATRIC MEDICATION SUPPORT, INDIVIDUAL COUNSELING, CASE MANAGEMENT, AND PSYCHOSOCIAL AND PSYCHIATRIC ASSESSMENTS. THIS INTERVENTION PROJECT IS BASED ON AN AVERAGE TOTAL OF 5.5 HOURS OF SERVICE PER CLIENT PER MONTH. AN AVERAGE OF ONE-HALF HOUR (0.5 HOURS) OF PSYCHIATRIC MEDICATION SUPPORT, FOUR (4) HOURS OF INDIVIDUAL COUNSELING, AND ONE (1) HOUR OF CASE MANAGEMENT WILL BE PROVIDED TO EACH CLIENT PARTICIPATING IN THE PROJECT, AS WELL AS AN INITIAL TWO (2)-HOUR PSYCHOSOCIAL ASSESSMENT AND AN INITIAL ONE (1)-HOUR PSYCHIATRIC ASSESSMENT PER CLIENT. FOR THESE INDIVIDUALS, THESE SUPPORTS CAN MEAN THE DIFFERENCE BETWEEN THEIR BECOMING ACTIVE MEMBERS OF THEIR LOCAL COMMUNITIES OR HAVING A PERMANENT DISABILITY. THE SHORT-TERM INTERVENTION WILL LAST FOR AN AVERAGE OF 3 MONTHS PER CLIENT. FOUR (4) TO SIX (6) CLIENTS WILL BE SERVED EVERY TWO (2) MONTHS, FOR A TOTAL OF UP TO 22 CLIENTS PER YEAR. THIS PROJECT WILL INCREASE SOCIAL SKILLS AND REDUCE SOCIAL ANXIETY AND DISRUPTIVE BEHAVIORS AMONG PARTICIPANTS. THE GOALS OF THIS PROJECT ARE: A. REDUCE SUBJECTIVE SUFFERING FROM MENTAL ILLNESS B. INCREASE MEANINGFUL USE OF TIME AND CAPABILITIES IN SCHOOL, WORK, AND OTHER TYPES OF ACTIVITIES C. INCREASE NATURAL NETWORKS OF SUPPORTIVE RELATIONSHIPS SINCE HOPE CLINIC IMPLEMENTS EVIDENCE-BASED PRACTICES, THE SUCCESS OF THIS PROJECT CAN BE MEASURED BY: A. ENGAGEMENT IN SERVICES (TIMELINES/ACCESS): FIRST APPOINTMENT TO PATIENTS WITHIN TEN (10) BUSINESS DAYS FROM REFERRAL NO SHOWS BELOW 25% B. SUCCESSFUL DISCHARGES (QUALITY): NUMBER OF CLIENTS SUCCESSFULLY DISCHARGED AT 60% OR ABOVE C. ACUTE CARE RE-ADMISSIONS (QUALITY): NO MORE THAN 5% OF CLIENTS ADMITTED TO EMERGENCY PSYCHIATRIC SERVICES WITHIN THIRTY (30) DAYS OF THEIR SUCCESSFUL DISCHARGE ACTIVITIES IMPLEMENTED IN THE PROJECT: MEDICATION SUPPORT: MEDICATION WILL BE PRESCRIBED AND ADMINISTERED TO CLIENTS PARTICIPATING IN THIS PROJECT UNDER THE CLOSE CARE AND DIRECTION OF A PSYCHIATRIST. INDIVIDUAL COUNSELING: ONE-TO-ONE INDIVIDUAL COUNSELING WITH A TRAINED MENTAL HEALTH CLINICIAN IN A SAFE, CARING, AND CONFIDENTIAL ENVIRONMENT. ASSESSMENT: SCREENINGS, CONDUCTED BY A PSYCHIATRIST OR MENTAL HEALTH CLINICIAN, TO GATHER INFORMATION ABOUT INDIVIDUAL CLIENTS RECEIVING TREATMENT AND TO MAKE A DIAGNOSIS. CASE MANAGEMENT: ONE-TO-ONE INDIVIDUAL SESSIONS WITH A TRAINED CASE MANAGEMENT SPECIALIST TO LINK CLIENTS TO AVAILABLE COMMUNITY RESOURCES, AND ADVOCATE FOR CLIENTS AND THEIR FAMILIES. THE FOLLOWING KEY STAFF WILL PARTICIPATE IN THE PROJECT: THE PROJECT DIRECTOR, ASSISTED BY THE ASSISTANT DIRECTOR, WILL PLAN, COORDINATE, OVERSEE, AND EVALUATE THE EXPANSION OF HOPE’S MENTAL HEALTH SERVICES IN SAN MATEO COUNTY. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. LIMITATION ON USE OF FUNDS: NO ASSISTANCE PROVIDED UNDER PROGRAM (OR ANY STATE OR LOCAL GOVERNMENT FUNDS USED TO SUPPLEMENT THIS ASSISTANCE) MAY BE USED TO REPLACE STATE OR LOCAL FUNDS PREVIOUSLY USED, OR DESIGNATED FOR USE, TO ASSIST HOMELESS PERSONS OR PERSONS AT-RISK OF HOMELESSNESS.; EXPECTED OUTCOMES: DECREASE IN THE NUMBER INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS, MORE SPECIFICALLY USING PERFORMANCE INDICATORS SUCH AS THE LENGTH OF TIME HOMELESS, RETURNS TO HOMELESSNESS OVER TIME, AND EXITS TO PERMANENT HOUSING. PURPOSE: THE CONTINUUM OF CARE (COC) PROGRAM IS DESIGNED TO PROMOTE COMMUNITY-WIDE COMMITMENT TO THE GOAL OF ENDING HOMELESSNESS; PROVIDE FUNDING FOR EFFORTS BY NONPROFIT PROVIDERS, STATES, AND LOCAL GOVERNMENTS TO QUICKLY HOUSE HOMELESS INDIVIDUALS AND FAMILIES WHILE MINIMIZING THE TRAUMA AND DISLOCATION CAUSED TO HOMELESS INDIVIDUALS, FAMILIES, AND COMMUNITIES BY HOMELESSNESS; PROMOTE ACCESS TO AND EFFECTIVE UTILIZATION OF MAINSTREAM PROGRAMS BY HOMELESS INDIVIDUALS AND FAMILIES; AND OPTIMIZE SELF-SUFFICIENCY AMONG INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS.
