CARE MANAGERS AND RNS CONDUCT COMPREHENSIVE IN-HOME ASSESSMENTS, IDENTIFY SUPPORTS, ACCESS SUPPORTS AND, WHERE APPROPRIATE, COORDINATE SERVICES. IN-HOME SERVICES PROVIDED INCLUDE ASSISTANCE WITH PERSONAL CARE SUCH AS BATHING, DRESSING AND TOILETING, ASSISTANCE WITH HOMEMAKING, GROCERY SHOPPING AND LAUNDRY.
