GARDNER PHONE NUMBERS: (508)539-6000 X1109 E-MAIL ADDRESS: KGARDNER@CHCOFCAPECOD.ORG WEB SITE ADDRESS: WWW.CHCOFCAPECOD.ORG CONGRESSIONAL DISTRICT(S) FOR THE APPLICANT ORGANIZATION AND PROPOSED SERVICE AREA: MASSACHUSETTS 9TH AMOUNT OF HEALTH CENTER PROGRAM FUNDING REQUESTED IN THIS APPLICATION: $500,000 ORGANIZATION HISTORY, COMMUNITY SERVED, TARGET POPULATION: COMMUNITY HEALTH CENTER OF CAPE COD (CHC) IS A PATIENT-CENTERED MEDICAL HOME (WITH RECOGNITION BY THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE) PROVIDING INTEGRATED PRIMARY CARE, DENTAL, AND BEHAVIORAL HEALTH SERVICES TO CHILDREN, ADULTS, AND FAMILIES ON CAPE COD. CHC’S PRIMARY SERVICE AREA IS THE BARNSTABLE COUNTY TOWNS OF BOURNE, FALMOUTH, MASHPEE, CENTERVILLE AND SANDWICH, WHICH HAVE A POPULATION OF 99,400. ADDITIONAL PATIENTS COME FROM THE NEARBY TOWNS, MARTHA’S VINEYARD, AND THE “OFF-CAPE” TOWNS OF WAREHAM AND PLYMOUTH. IN 1998 CHC WAS FOUNDED AS AN ALL-VOLUNTEER, FREE CLINIC AND HAS EVOLVED SINCE IN RESPONSE TO THE COMMUNITY’S NEED FOR AFFORDABLE, HIGH QUALITY, COMPREHENSIVE PRIMARY AND PREVENTIVE CARE SERVICES. IN ADDITION TO OUR CORE SERVICES, CHC PATIENTS ALSO RECEIVE INSURANCE ENROLLMENT ASSISTANCE, PHARMACY, WALK-IN CARE, IMAGING, CASE MANAGEMENT, EARLY CANCER SCREENING SERVICES, AND INTERPRETER SERVICES. SPECIALTY PROGRAMS ARE OFFERED TO REDUCE BARRIERS TO CARE AND ADDRESS THE NEEDS FOR ADOLESCENT BEHAVIORAL HEALTH, ADDICTION RECOVERY TREATMENT, PRIMARY CARE FOR VETERANS, AND DIAGNOSTIC AND SPECIALTY CARE FOR THE UNINSURED. CHC CARES FOR ALL THOSE IN NEED OF SERVICES, ESPECIALLY THE UNINSURED, UNDERINSURED, THOSE COVERED BY PUBLIC INSURANCE, OR WHO FACE LINGUISTIC, CULTURAL OR OTHER BARRIERS TO CARE. PROJECT: OVER THE LAST TWO YEARS, OUR HIGH PERFORMING HEALTH CENTER HAS WITNESSED DECLINING HEALTH OUTCOMES IN OUR COMMUNITIES ACROSS CAPE COD. THROUGH THIS OPPORTUNITY, CHC WILL TARGET THE EXACT GOALS OF THIS FUNDING OPPORTUNITY. SPECIFICALLY, WE WILL PARTNER WITH DANA FARBER CANCER INSTITUTE (ONE OF THE NCO DESIGNATED CANCER CENTERS) AND THE IMPLEMENTATION SCIENCE CENTER FOR CANCER CONTROL EQUITY (ISCCCE) TO INCREASE EQUITABLE ACCESS TO CANCER SCREENING, REFERRAL TO CARE AND TREATMENT BY ENHANCING PATIENT EDUCATION, CASE MANAGEMENT, OUTREACH AND OTHER ENABLING SERVICES. CHC HAS ENTERED IN TO A MEMORANDUM OF AGREEMENT WITH ISCCCE TO TRAIN AND DEPLOY OUTREACH SPECIALISTS, COMMUNITY HEALTH WORKERS AND NAVIGATORS FOR POPULATIONS ON CAPE COD. THIS PARTNERSHIP WILL INCLUDE THREE ADDITIONAL HEALTH CENTERS LOCATED ON CAPE COD, DUFFY HEALTH CENTER FOR THE HOMELESS, HARBOR HEALTH SERVICES AND OUTER CAPE HEALTH SERVICES. IN ADDITION, WE WILL LEVERAGE THE LESSONS LEARNED REGARDING EMPLOYING A SIMILAR OUTREACH MODEL TO IMPROVE COVID 19 TESTING IN EIGHT COMMUNITIES IN MASSACHUSETTS. WE WILL WORK CLOSELY WITH OUR PRIMARY CARE ASSOCIATION, THE MASSACHUSETTS LEAGUE OF COMMUNITY HEALTH CENTERS (MASS LEAGUE). FINALLY, CHC WILL LEVERAGE PRIVATE FUNDING RECEIVED TO SUPPORT COVID 19 RESPONSE AND RECOVERY EFFORTS AS WELL AS TELEHEALTH BEST PRACTICES AND EVIDENCED BASED STRATEGIES TO ACHIEVE PROGRAM GOALS. AFTER FULL PROGRAM IMPLEMENTATION AND EVALUATION, CHC WILL EVALUATE AND CONSIDER BRINGING BREAST CANCER SCREENING ON SITE AT THE MAIN LOCATION IN MASHPEE AND OUR WOMEN’S HEALTH CENTER IN NORTH FALMOUTH.
