PPSNE TITLE X TELEHEALTH ENHANCEMENT PROJECT - PPSNE PROPOSES TO USE $72,144 IN FEDERAL TITLE X FUNDS TO ENHANCE ITS TELEHEALTH SERVICES AND IMPROVE OUTREACH TO PEOPLE WHO ARE UNDER-REPRESENTED AMONG PPSNE’S CURRENT TELEHEALTH SERVICE USERS. OUR GOAL IS TO IMPROVE THE EFFICIENCY AND EFFECTIVENESS OF CURRENT TELEHEALTH SERVICES TO INCREASE THE ABILITY OF OUR TITLE PROGRAM TO REACH UNDERSERVED COMMUNITIES. HIGHLIGHTS OF OUR PROJECT INCLUDE: - CONVENE A TELEHEALTH MANAGEMENT TEAM TO DETERMINE HOW PPSNE CAN BE MORE PROACTIVE IN ITS USE OF TELEHEALTH, - HIRE A DEDICATED TELEHEALTH MANAGER TO PERFORM CHART PREP, OVERSEE WORKFLOW, ADDRESS TECHNICAL ISSUES, ARRANGE FOR STAFFING, AND COORDINATE TECHNICAL ASSISTANCE FOR TELEHEALTH CLINICIANS AND MEDICAL ASSISTANTS. - DEVELOP AND IMPLEMENT A TELEHEALTH SPECIFIC EVALUATION AND QI PLAN. - PROVIDE 7 TECHNICAL ASSISTANCE AND TRAINING SESSIONS TO TELEHEALTH TEAMS AND CENTER STAFF. - LAUNCH AN AWARENESS AND SOCIAL MEDIA CAMPAIGN DIRECTED AT PEOPLE CURRENTLY UNDER-REPRESENTED AMONG PPSNE TELEHEALTH USERS, INCLUDING WOMEN AND MEN WHO ARE UNINSURED OR ARE FROM FAMILIES WITH LOW INCOMES. WE STRONGLY FEEL THESE EFFORTS TO ENHANCE PPSNE’S TELEHEALTH SERVICES WILL ENABLE PPSNE TO MEET ITS GOAL OF PROVIDING 9,690 TITLE X VISITS VIA TELEHEALTH IN BOTH CONNECTICUT AND RHODE ISLAND. FURTHER WE FEEL THAT BY TAKING A PROACTIVE APPROACH WE CAN USE TELEHEALTH TO SUPPORT AND IMPROVE THE EFFICIENCY OF OUR IN-CENTER SERVICES AND MAXIMIZE OUR HEALTH CENTER STAFF RESOURCES. PPSNE HAS SUBMITTED TWO APPLICATIONS FOR THE TITLE X PROGRAM, ONE FOR ITS 14 CONNECTICUT HEALTH CENTERS AND A SEPARATE APPLICATION FOR WOMEN AND MEN SERVED BY PPSNE’S PROVIDENCE, RHODE ISLAND HEALTH CENTER. PPSNE’S TELEHEALTH PROJECT WILL SUPPORT THE TITLE X PROGRAM IN BOTH STATES.
