COMMUNITY PROJECT FUNDING/CONGRESSIONALLY DIRECTED SPENDING - NON-CONSTRUCTION - SOUTHEASTERN MASSACHUSETTS VETERANS HOUSING PROGRAM (VETERANS TRANSITION HOUSE, “VTH”) WAS FOUNDED BY LOCAL VETERANS AND HAS PROVIDED TRANSITIONAL AND PERMANENT HOUSING SERVICES AND SUPPORTIVE HEALTH AND HUMAN SERVICES TO HOMELESS VETERANS SINCE 1990. VTH HAS SERVED MORE THAN 3,000 VETERANS IN THIRTY YEARS AND CURRENTLY SERVES 100 UNIQUE HOMELESS VETERANS RESIDING IN SEVEN REVITALIZED PROPERTIES THROUGHOUT NEW BEDFORD. THE VTH TELEHEALTH AND HEALTH INFORMATION TECHNOLOGY INITIATIVE WILL IMPROVE REPORTING TO FEDERAL, STATE AND COMMUNITY HEALTHCARE PARTNERS, AND ENABLE VETERANS TRANSITION HOUSE TO MAKE DATA-INFORMED DECISIONS REGARDING HEALTHCARE SERVICES PROVIDED THROUGH INTENSIVE CASE MANAGEMENT. THIS PROJECT WILL SECURE OPERATING AND FINANCIAL EFFICIENCIES FOR VETERANS TRANSITION HOUSE AND ITS NUMEROUS PARTNERS, INCLUDING THE U.S. DEPARTMENT OF VETERANS AFFAIRS, MASSACHUSETTS DEPARTMENT OF VETERANS SERVICES AND LOCAL HEALTHCARE PROVIDERS INCLUDING SOUTHCOAST HEALTH SYSTEM. THIS IS AN INVESTMENT IN HEALTHCARE INFRASTRUCTURE NECESSARY FOR HIGH QUALITY CARE OF U.S. ARMED SERVICES VETERANS WHO HAVE EXPERIENCED THE ADDITIONAL TRAUMA OF HOMELESSNESS AND SUFFER FROM COMPLEX HEALTH AND MENTAL HEALTH CHALLENGES THAT CREATE BARRIERS TO RECOVERY AND RE-INTEGRATION INTO THE COMMUNITY. THIS PROJECT IS DIRECTLY ALIGNED WITH THE DEPARTMENT OF HEALTH AND HUMAN SERVICES HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) CATEGORY OF TELEHEALTH AND HEALTH INFORMATION TECHNOLOGY FUNDING FOR TELEMEDICINE, DISTANCE LEARNING, OR USE OF INFORMATION TECHNOLOGY TO IMPROVE HEALTH CARE. THIS PROJECT-READY, CAPACITY-BUILDING INITIATIVE WILL ACCOMPLISH THE FOLLOWING GOALS: 1. IMPROVE THE COLLECTION AND MANAGEMENT OF CLIENT HEALTH INFORMATION TO ENHANCE THE QUALITY OF SERVICES. 2. INCREASE STAFF AND OPERATING EFFICIENCIES ACROSS MULTIPLE PROGRAM SITES TO ACCOMMODATE MORE CLIENTS. 3. EXPEDITE ACCURATE DATA REPORTING TO FACILITATE MORE EFFECTIVE HEALTH CARE OUTCOMES. THE KEY ELEMENTS TO BE FUNDED BY THIS REQUEST INCLUDE IDENTIFYING TWO FULLY COMPATIBLE SOFTWARE PLATFORMS THAT CAN SERVE THE HUMAN SERVICE CASE MANAGEMENT REQUIREMENTS (CLIENTS) AS WELL AS THE CRM AND WORKFLOW REQUIREMENTS (REFERRALS, VENDORS, ETC.). FEATURES WILL INCLUDE: - CLOUD-BASED CUSTOMIZED ELECTRONIC HEALTH, CASE MANAGEMENT AND WORKFLOW MANAGEMENT SYSTEMS WITH TRAINING, SUPPORT AND ADD-ON FEATURES AS NECESSARY - INTUITIVE, EASY TO USE DASHBOARDS - INTEGRATED CLIENT DATA STORAGE AND MANAGEMENT - TASK MANAGEMENT FUNCTIONS FOR STAFF AND CLIENTS - COMMUNICATIONS FUNCTIONS FOR STAFF AND CLIENTS (EMAIL, TEXT, MOBILE PORTALS) - FORM GENERATION AND MANAGEMENT (INTAKE, VA, REFERRAL, ETC.) - CRM/CONTACT MANAGEMENT - OUTCOMES ANALYTICS AND REPORTING VTH WILL ASSUME ONGOING COST OF LICENSING AND MAINTAINING THE SOFTWARE AND EQUIPMENT NECESSARY TO LONG-TERM SUCCESS OF THIS INITIATIVE.
