FOCUS AREAS: WORKFORCE RECRUITMENT AND RETENTION; TECHNOLOGY ADVANCEMENTS; SERVICE DELIVERY; AND POPULATION HEALTH IMPROVEMENT. TARGET POPULATION: THE SERVICE AREA INCLUDES DUNN, MERCER, AND OLIVER COUNTIES, AND THE SOUTHEAST PORTION OF MCKENZIE COUNTY IN NORTH DAKOTA. ALL PERSONS IN THE SERVICE AREA ARE TARGETED FOR CARE, BUT SUBPOPULATIONS INCLUDE CHILDREN AND YOUTH, THE ELDERLY, AND AMERICAN INDIANS. NETWORK NAME AND MEMBERS: ENERGY CAPITAL HEALTH NETWORK; (1) COAL COUNTRY COMMUNITY HEALTH CENTER (CCCHC) – FQHC; (2) SAKAKAWEA MEDICAL CENTER – CRITICAL ACCESS HOSPITAL; (3) KNIFE RIVER CARE CENTER – SKILLED NURSING FACILITY; AND (4) CUSTER HEALTH – LOCAL HEALTH DEPARTMENT PROJECT ACTIVITIES/SERVICES: (1) RECRUITMENT AND RETENTION OF STAFF. (2) TECHNOLOGY ADVANCEMENTS INCLUDING REMOTE PATIENT MONITORING, A TELEHEALTH NETWORK, AND A UNIFIED ELECTRONIC HEALTH RECORD. (3) SOCIAL DETERMINANTS OF HEALTH SCREENING; IMPROVED OBSTETRICAL OUTCOMES; AND DEVELOPMENT OF A COMMUNITY HEALTH WORKER PROGRAM. (4) EXPAND PARTNERSHIPS, IMPLEMENT A MARKETING/BRANDING CAMPAIGN, AND FACILITATE A POPULATION HEALTH COMMITTEE. EXPECT OUTCOMES: THE OVERARCHING PROGRAM OUTCOME IS TO ACHIEVE THE QUINTUPLE AIM OF IMPROVING HEALTH OUTCOMES, DECREASING OVERALL COSTS OF CARE, IMPROVING PATIENT SATISFACTION, ADVANCING HEALTH EQUITY, AND IMPROVING CARE TEAM SATISFACTION. THAT OUTCOME ADDRESSES ALL FOUR RHND DOMAIN S. OTHER PROJECT OUTCOMES INCLUDE: RHND DOMAIN: (I) IMPROVE ACCESS; OUTCOMES: IMPROVED EMPLOYEE SATISFACTION SCORES (STAFF RETENTION); INCREASE IN PATIENTS WITH A MEDICAL HOME. RHND DOMAIN (II) EXPAND CAPACITY AND SERVICES; OUTCOMES: UNIFIED COMMUNICATION SYSTEM AND EHR AMONG NETWORK MEMBERS; IMPROVED CONTINUITY OF CARE; EXPANSION OF SERVICES; IMPROVED WORKFLOWS; IMPROVED COMMUNITY AWARENESS; RHND DOMAIN (III) ENHANCE OUTCOMES; OUTCOMES: DECREASED HOSPITAL AND ER VISITS; IMPROVED POPULATION HEALTH RHND DOMAIN: (IV) SUSTAINABILITY; OUTCOMES: NETWORK LEADERSHIP TRANSITION FINALIZED; NETWORK SUSTAINABILITY CAPACITY TO SERVE RURAL UNDERSERVED POPULATIONS: CCCHC IS A NON-PROFIT RURAL COMMUNITY HEALTH CENTER ORGANIZATION WHICH HAS SERVED WEST CENTRAL NORTH DAKOTA FOR NEARLY 20 YEARS. ECHN PARTNERS HAVE WORKED INFORMALLY TOGETHER FOR YEARS TO ACHIEVE EFFICIENCIES IN HEALTH CARE SERVICE DELIVERY, EXPAND ACCESS TO CARE, IMPROVE QUALITY OF CARE, AND STRENGTHEN THE RURAL HEALTH DELIVERY SYSTEM. ON BEHALF OF THE ECHN, CCCHC APPLIED FOR, AND RECEIVED, A RHND PLANNING GRANT IN 2019. THE PROJECT WAS SUCCESSFULLY COMPLETED, AND THE ECHN IS WORKING TO ACHIEVE THE STRATEGIC PLANNING GOALS THAT EMERGED FROM THE GRANT. PREVIOUS COLLABORATIONS AMONG NETWORK MEMBERS WHICH DEMONSTRATE THE CAPACITY TO WORK TOGETHER INCLUDE: (1) COMPLETION OF A SERVICE AREA COMMUNITY HEALTH NEEDS ASSESSMENT; (2) SHARED FACILITY SPACE AND STAFF POSITIONS; (3) DEVELOPMENT OF THE ENERGY CAPITAL CHILDCARE COOPERATIVE; AND (4) DEVELOPMENT OF A WELLNESS CENTER. FUNDING PREFERENCE: PREFERENCE REQUESTED BASED ON THE FOLLOWING CRITERIA: DUNN, MCKENZIE, MERCER, AND OLIVER COUNTIES ARE ALL DESIGNATED AS HEALTH PROFESSIONAL SHORTAGE AREAS. CCCHC IS ALSO LOCATED IN A MEDICALLY UNDERSERVED AREA AND SERVES A MEDICALLY UNDERSERVED POPULATION.
