NYCENTER FOR MINORITY HEALTH, EQUITY AND SOCIAL JUSTICE - OVERALL-ABSTRACT THE OVERALL GOAL OF THE NEW RCMI AT THE CITY COLLEGE OF NEW YORK (CCNY) IS TO CREATE AN INTEGRATED MULTI-TIERED CENTER, THE NYCENTER FOR MINORITY HEALTH, EQUITY, AND SOCIAL JUSTICE (NYC-MHESJ). THIS PROPOSAL IS FOUNDED ON THE HISTORIC COMMITMENT OF CUNY MED AND CCNY TO THE HARLEM COMMUNITY. LOCATED IN HARLEM AND LED BY THE CUNY MED, THE NEW RCMI COLLABORATES WITH CCNY’S DIVISION OF SCIENCE, THE COLIN POWELL SCHOOL FOR CIVIC & GLOBAL LEADERSHIP, AND THE GROVE SCHOOL OF ENGINEERING. IT SPANS BASIC BIOMEDICAL, BEHAVIORAL, SOCIAL SCIENCE, AND CLINICAL RESEARCH. IT FURTHER DEVELOPS AND STRENGTHENS RESEARCH INFRASTRUCTURE, IMPROVES THE QUALITY OF SCIENCE, AND ENHANCES THE PREPARATION AND CAPACITY OF EARLY-STAGE INVESTIGATORS (ESI) TO CONDUCT STATE-OF-THE-ART HEALTH DISPARITIES RESEARCH WHILE, CREATING STRATEGIC COMMUNICATIONS, AND IMPLEMENTING A DISSEMINATION PLAN FOCUSED ON UNDERSERVED NYC COMMUNITIES WHERE DEEP DISPARITIES EXIST AND PERSIST. THE SPECIFIC AIMS AND KEY ACTIVITIES FOR THE OVERALL CORE ARE TO: AIM-1 SUPPORT THREE OUTSTANDING R01-LEVEL RESEARCH PROJECTS CENTERED ON HEALTH DISPARITIES: • A BASIC SCIENCE PROPOSAL FOCUSED ON END-STAGE KIDNEY DISEASE, WHICH PREDOMINANTLY AFFECTS MINORITY POPULATIONS SUCH AS AFRICAN-AMERICANS AND LATINO(S); • A BEHAVIORAL SCIENCE PROJECT FOCUSED ON MENTAL HEALTH AND STIGMA IN ADOLESCENTS • A CLINICAL PROJECT FOCUSED ON UNDERSTANDING CLINICAL TRIAL PARTICIPATION AND RECRUITMENT PARAMETERS OF HISPANICS WITH PARKINSON’S DISEASE AIM-2 CREATE A RESEARCH CAPACITY CORE (RCC) TO STRENGTHEN CUNY MED AND CCNY’S RESEARCH CAPACITY BY EXPANDING ACCESS AND TRAINING IN MOLECULAR BIOLOGY/GENOMICS TECHNOLOGIES, ACCESS TO A BIOSTATISTICS, EPIDEMIOLOGY, AND RESEARCH DESIGN CORE, ARTIFICIAL INTELLIGENCE (AI), AND DATA AND COMPUTATIONAL SCIENCE TO STUDY HEALTH DISPARITIES. AIM-3 CREATE AND ADMINISTER AN INVESTIGATOR DEVELOPMENT CORE (IDC) LED BY EXPERIENCED MENTORS TO DEVELOP FACULTY, AWARD PILOT PROJECTS, AND ENHANCE TRAINING ACTIVITIES FOR UNDERREPRESENTED ESI AND FACULTY WITHIN AN EFFECTIVE INTEGRATED MENTORING PROGRAM. AIM-4 CREATE A COMMUNITY ENGAGEMENT CORE (CEC) FOCUSED ON MINORITY HEALTH AND HEALTH DISPARITES (HD) IN UNDERSERVED NYC COMMUNITIES. AIM-5 CREATE A RECRUITMENT CORE (RC) TO INCREASE THE CRITICAL MASS OF SENIOR-LEVEL HEALTH DISPARITIES INVESTIGATORS. OPTIMIZING HIV PREVENTION FOR HIGHLY VULNERABLE METHAMPHETAMINE-USING SEXUAL MINORITY MEN - SUMMARY THIS LITE-2 (RFA-AI-21-018) INITIATIVE RESPONDS TO A RESURGENT EPIDEMIC OF METHAMPHETAMINE (METH) USE IN SEXUAL MINORITY MEN (SMM), WHICH IS A PRIMARY DRIVER OF HIV INCIDENCE. THE OVERARCHING GOALS ARE TWO-FOLD: 1) IDENTIFY MULTI-LEVEL AND BIO-BEHAVIORAL DETERMINANTS OF AMPLIFIED HIV SEROCONVERSION RISK IN METH-USING SMM; AND 2) TEST THE EFFECTIVENESS OF TELEHEALTH MOTIVATIONAL ENHANCEMENT INTERVENTIONS FOR OPTIMIZING ENTRY OR RE-ENTRY OF SMM WHO USE METH INTO THE PREP CARE CONTINUUM. FINDINGS FROM OUR LITE-1 COHORT (UG3/UH3 AI-133675, RFA-AI-16-031) AND OTHERS PROVIDE COMPELLING EVIDENCE THAT METH USE IS INCREASING, DISPROPORTIONALLY IMPACTS RACIAL/ETHNIC MINORITIES, AND ACCOUNTS FOR ONE-IN-THREE NEW HIV INFECTIONS IN SMM. IN RESPONSE TO LITE-2 (RFA- AI-21-018) WE PROPOSE A MULTI-COMPONENT INITIATIVE ZEROING IN ON THE “WHERE,” “HOW,” AND “WHY” OF METH USE AND HIV RISK. WHERE: WHAT ARE THE GEOSPATIAL DETERMINANTS OF THE ASSOCIATION OF METH USE WITH HIV INCIDENCE? HOW: HOW CAN WE SUPPORT (RE-)ENTRY INTO THE PREP CARE CONTINUUM WITH THIS HIGH PRIORITY POPULATION OF SMM WHO USE METH? WHY: DOES METH AMPLIFY BIOLOGICAL RISK OF HIV BY POTENTIATING RECTAL IMMUNE DYSREGULATION? AIM 1: EXAMINE MULTI-LEVEL STRUCTURAL, PSYCHOLOGICAL, AND SOCIAL DETERMINANTS OF AMPLIFIED HIV SEROCONVERSION RISK IN SMM WHO USE METH. THE CENTERPIECE OF OUR LITE-2 INITIATIVE IS A NEW PROSPECTIVE, BIO-BEHAVIORAL COHORT WITH N=5,000 SMM (N=3,000 SMM WHO USE METH, N=2,000 WHO DO NOT). PARTICIPANTS WILL COMPLETE ASSESSMENTS OVER 36 MONTHS AND PROVIDE BIOLOGICAL SAMPLES FOR HIV TESTING, DRUG TOXICOLOGY TESTING, RECTAL STIS, AND RECTAL CYTOKINES/CHEMOKINES. OUR PRIMARY GOAL WILL BE TO INVESTIGATE THE ROLE OF GEOSPATIAL DETERMINANTS (E.G., BACKGROUND METH AND HIV PREVALENCE, URBANICITY) AND OTHER STRUCTURAL DETERMINANTS (E.G., STRUCTURAL STIGMA OF SEXUAL MINORITIES AS EVIDENCED BY POLICIES/LAWS) IN RELATION TO THE ASSOCIATION OF METH USE WITH AMPLIFIED HIV SEROCONVERSION RISK. AIM 2: TEST THE COMPARATIVE AND COMBINED EFFECTIVENESS OF TELEHEALTH MOTIVATIONAL ENHANCEMENT INTERVENTIONS FOR OPTIMIZING PREP USE. PREP READINESS INTERVENTIONS FOR SUPPORTING MOTIVATION (PRISM) IS A HYBRID TYPE I, MODIFIED FACTORIAL RANDOMIZED CONTROLLED TRIAL (RCT) OF TELEHEALTH CM THAT PROVIDES INCENTIVES FOR FILLING A PREP PRESCRIPTION, AND A 2-SESSION TELEHEALTH MI INTERVENTION THAT WE ADAPTED WITH METH- USING SMM (R34-DA046367, CARRICO/GROV). PRISM WILL ENROLL 840 METH-USING SMM WHO ARE NOT CURRENTLY TAKING PREP FROM THE LITE-2 COHORT (AIM 1) TO EXAMINE THE EFFECTIVENESS OF CM (N = 280), MI (N = 280), AND MI+CM (N = 280) ON THE PRIMARY OUTCOME – FILLING A PREP PRESCRIPTION. AIM 3: DETERMINE WHETHER GREATER RECTAL IMMUNE DYSREGULATION PARTIALLY EXPLAINS AMPLIFIED RISK OF HIV SEROCONVERSION IN SMM WHO USE METH. USING A CASE-COHORT DESIGN, WE WILL COMPARE HIV SEROCONVERTERS (N=450) WITH MATCHED SERONEGATIVE CONTROLS (N=450) TO EXAMINE THE CLINICAL RELEVANCE OF METH-INDUCED ALTERATIONS IN RECTAL CYTOKINES WITH RESPECT TO HIV SEROCONVERSION. THIS LITE-2 INITIATIVE COULD HAVE AN EXCEPTIONAL IMPACT BY TRANSFORMING HIV PREVENTION WITH SMM.

COMMUNITY-ENGAGED INTERVENTION TO OPTIMIZE THE IMPACT OF RENOVATEDNEIGHBORHOOD PARKS ON COMMUNITY WELLBEING - PROJECT SUMMARY PARKS ARE A KEY FEATURE OF THE BUILT ENVIRONMENT THAT FOSTER COMMUNITY ENGAGEMENT, MAKE COMMUNITIES VIBRANT, AND SUPPORT RESIDENT WELLBEING. THIS STUDY WILL TEST THE IMPACT ON COMMUNITY-LEVEL HEALTH-RELATED QUALITY OF LIFE AND MENTAL HEALTH OF A COMMUNITY-ENGAGED, COMMUNITY-LEVEL INTERVENTION THAT AIMS TO IMPROVE PARK-BASED HEALTH AND SOCIAL PROGRAMMING. THE STUDY WILL LEVERAGE NEIGHBORHOOD PARKS THAT HAVE RECENTLY BEEN REDESIGNED AND RENOVATED AS PART OF THE COMMUNITY PARKS INITIATIVE, A CAPITAL INVESTMENT AND PARK EQUITY PROGRAM IN NEW YORK CITY. WE AIM TO WORK WITH EIGHT LOWER INCOME AND PREDOMINANTLY LATINO AND BLACK NEIGHBORHOODS THROUGH AN ASSET-BASED AND HUMAN-CENTERED DESIGN PROCESS TO CO-CREATE AND IMPLEMENT ONE HEALTH AND ONE SOCIAL STRATEGY IN EACH NEIGHBORHOOD PARK. THE HEALTH STRATEGY WILL FOCUS ON PHYSICAL ACTIVITY AND THE SOCIAL STRATEGY WILL SEEK TO ENHANCE SOCIAL RELATIONSHIPS AND NETWORKS IN EACH NEIGHBORHOOD. ACHIEVING BROAD REACH, INCLUSIVITY, AND SUSTAINABILITY ARE KEY FACTORS THAT WILL BE TAKEN INTO ACCOUNT IN THE DESIGN PROCESS. THE EIGHT NEIGHBORHOODS WILL BE CLUSTER-RANDOMIZED TO ONE OF TWO STEPS IN A STEPPED WEDGE INTERVENTION TRIAL. THE INTERVENTION IS 24 MONTHS IN EACH COMMUNITY, WITH FOLLOW-UP TO 48 MONTHS. WE WILL STUDY OUTCOMES, INCLUDING HR-QOL, DEPRESSION, ANXIETY, STRESS AND LONELINESS, AT THE COMMUNITY LEVEL (NOT AMONG INDIVIDUALS PARTICIPATING IN PARK ACTIVITIES) VIA RANDOM, REPRESENTATIVE SAMPLES OF ADULT RESIDENTS IN PARTICIPATING COMMUNITIES AT INTERVENTION BASELINE AND MONTHS 24 AND 48 POST-BASELINE.IN ADDITION, WE WILL EXAMINE WHETHER THE INTERVENTION EFFECTS MIGHT BE MEDIATED THROUGH SOCIAL COHESION, NEIGHBORHOOD TIES, SENSE OF COMMUNITY, PARK USE AND PERCEPTION AND/OR HEALTH BEHAVIORS SUCH AS PHYSICAL ACTIVITY AND SLEEP. FINALLY, WE WILL CONDUCT QUALITATIVE RESEARCH TO EXAMINE THE IMPLEMENTATION OF THE COMMUNITY-LEVEL INTERVENTION BASED ON CONSTRUCTS FROM THE CONSOLIDATED FRAMEWORK FOR IMPLEMENTATION RESEARCH. STUDY FINDINGS WILL HELP INFORM FUTURE BEST PRACTICES IN URBAN COMMUNITY DEVELOPMENT AND PARK PROGRAMS AND POLICIES TO PROMOTE POPULATION HEALTH AND REDUCE HEALTH DISPARITIES.

EXPANDQISE: TRACK 2: LEVERAGING SYNTHETIC DEGREES OF FREEDOM FOR QUANTUM STATE ENGINEERING IN PHOTONIC CHIPS -NONTECHNICAL ABSTRACT: THIS EXPANDQISE PROGRAM AT THE CITY COLLEGE OF NEW YORK SEEKS TO ADVANCE THE FUNDAMENTAL UNDERSTANDING OF QUANTUM PHENOMENA IN ENGINEERED OPTICAL STRUCTURES ENDOWED WITH ADDITIONAL DEGREES OF FREEDOM BY MANIPULATING THE FUNDAMENTAL PROPERTIES OF LIGHT AND ITS INTERACTION WITH NANOMATERIALS. THIS INITIATIVE AIMS AT THE DEVELOPMENT OF NASCENT QUANTUM MATERIALS WITH NOVEL PROPERTIES THAT CAN BE ATTAINED BY COMBINING TOPOLOGICAL PHOTONIC PROPERTIES AND QUANTUM PROPERTIES OF LIGHT AND MATTER. THIS PROJECT ADVANCES THE FIELDS OF INTEGRATED QUANTUM PHOTONICS THROUGH THE SYSTEMATIC DISCOVERY OF NEW MATERIALS THAT POSSESS THE NECESSARY FUNCTIONALITIES TO ENABLE DEVELOPMENT OF NOVEL QUANTUM DEVICES. TO MAXIMIZE THE EFFECTIVENESS OF THE DISCOVERY PROCESS, THIS PROJECT COMBINES THEORETICAL AND EXPERIMENTAL EFFORTS FROM INTERDISCIPLINARY TEAMS, INCLUDING ACADEMIA (CITY COLLEGE AND UNIVERSITY OF CENTRAL FLORIDA) AND INDUSTRY. IN ADDITION TO ITS DIRECT SCIENTIFIC IMPACT, THE PROJECT WILL HAVE A BROAD SOCIETAL IMPACT THROUGH THE DEVELOPMENT OF EMERGING TECHNOLOGIES FOR QUANTUM INFORMATION PROCESSING AND ADVANCES ONGOING WORKFORCE DEVELOPMENT EFFORTS THANKS TO THE STRONG INVOLVEMENT OF UNDERGRADUATE STUDENTS IN ALL ASPECTS OF RESEARCH. OUTREACH PROGRAMS WITH ACTIVE PARTICIPATION OF HIGH SCHOOL AND UNDERGRADUATE STUDENTS, WITH FOCUS ON UNDERREPRESENTED GROUPS, WILL FURTHER BROADEN THE PROJECT IMPACT. TECHNICAL ABSTRACT: THIS EXPANDQISE PROGRAM AT THE CITY COLLEGE OF NEW YORK SEEKS TO ADDRESS FUNDAMENTAL QUESTIONS OF MATERIALS SCIENCE AND LIGHT-MATTER INTERACTIONS IN ARTIFICIAL QUANTUM OPTICAL MATERIALS ENDOWED WITH ADDITIONAL SYNTHETIC DEGREES OF FREEDOM ? PSEUDO-SPINS ? AND CHARACTERIZED BY NONTRIVIAL TOPOLOGICAL PROPERTIES. OUR RESEARCH TEAM BUILDS ON OUR EXISTING EXPERTISE IN THEORETICAL NANO-PHOTONICS AS WELL AS ADVANCED FABRICATION AND EXPERIMENTAL TECHNIQUES TO ATTAIN NOVEL MATERIALS CHARACTERISTICS AND FUNCTIONALITIES EMERGING IN QUANTUM REGIMES. SPECIFICALLY, THIS ACTIVITY FOCUSES ON DEVELOPMENT OF THE CONCEPT OF ACTIVE QUANTUM TOPOLOGICAL MATERIALS THAT WILL ENABLE CONTROL OVER QUANTUM EXCITATIONS OF BOTH LIGHT AND MATTER ON A PHOTONIC CHIP. THIS EFFORT ENABLES GENERATION AND MANIPULATION OF QUANTUM STATES OF STRUCTURED OPTICAL MODES AND TOPOLOGICAL BOUNDARY STATES ENDOWED WITH SYNTHETIC DEGREES OF FREEDOM ON A CHIP. ADDITIONALLY, BY HARNESSING THE FUNDAMENTAL PROPERTIES OF SUCH QUANTUM PHOTONIC STATES THIS PROJECT ENABLES NOVEL POLARITONIC STATES WITH TAILORED PROPERTIES THAT CAN BE USED FOR QUANTUM TECHNOLOGIES, SUCH AS CONTROL OF PSEUDO-SPINS WITH SYNTHETIC GAUGE FIELDS ENGINEERED AT NANOSCALE, INCLUDING ACTIVELY VIA LIGHT-MATTER INTERACTIONS. THE POSSIBILITY TO IMPRINT THE STATE OF A PSEUDO-SPIN ONTO QUANTUM STATES OF LIGHT EMITTED BY INTEGRATED QUANTUM EMITTERS ENABLES NOVEL OPPORTUNITIES FOR INTEGRATED QUANTUM PHOTONICS, WHERE QUANTUM INFORMATION IS ENCODED IN THE MODAL STRUCTURE OF OPTICAL STATES. OUR APPROACH TO QUANTUM MATERIALS DESIGN LEVERAGES A VARIETY OF QUANTUM EXCITATIONS IN MATERIALS INTEGRATED INTO TOPOLOGICAL PHOTONIC STRUCTURES, SUCH AS VAN DER WAALS MATERIALS, ORGANIC EXCITONIC MATERIALS, AND WIDE BANDGAP SEMICONDUCTORS. OPTIMIZING HIV PREVENTION FOR HIGHLY VULNERABLE METHAMPHETAMINE-USING SEXUAL MINORITY MEN - SUMMARY THIS LITE-2 (RFA-AI-21-018) INITIATIVE RESPONDS TO A RESURGENT EPIDEMIC OF METHAMPHETAMINE (METH) USE IN SEXUAL MINORITY MEN (SMM), WHICH IS A PRIMARY DRIVER OF HIV INCIDENCE. THE OVERARCHING GOALS ARE TWO-FOLD: 1) IDENTIFY MULTI-LEVEL AND BIO-BEHAVIORAL DETERMINANTS OF AMPLIFIED HIV SEROCONVERSION RISK IN METH-USING SMM; AND 2) TEST THE EFFECTIVENESS OF TELEHEALTH MOTIVATIONAL ENHANCEMENT INTERVENTIONS FOR OPTIMIZING ENTRY OR RE-ENTRY OF SMM WHO USE METH INTO THE PREP CARE CONTINUUM. FINDINGS FROM OUR LITE-1 COHORT (UG3/UH3 AI-133675, RFA-AI-16-031) AND OTHERS PROVIDE COMPELLING EVIDENCE THAT METH USE IS INCREASING, DISPROPORTIONALLY IMPACTS RACIAL/ETHNIC MINORITIES, AND ACCOUNTS FOR ONE-IN-THREE NEW HIV INFECTIONS IN SMM. IN RESPONSE TO LITE-2 (RFA- AI-21-018) WE PROPOSE A MULTI-COMPONENT INITIATIVE ZEROING IN ON THE “WHERE,” “HOW,” AND “WHY” OF METH USE AND HIV RISK. WHERE: WHAT ARE THE GEOSPATIAL DETERMINANTS OF THE ASSOCIATION OF METH USE WITH HIV INCIDENCE? HOW: HOW CAN WE SUPPORT (RE-)ENTRY INTO THE PREP CARE CONTINUUM WITH THIS HIGH PRIORITY POPULATION OF SMM WHO USE METH? WHY: DOES METH AMPLIFY BIOLOGICAL RISK OF HIV BY POTENTIATING RECTAL IMMUNE DYSREGULATION? AIM 1: EXAMINE MULTI-LEVEL STRUCTURAL, PSYCHOLOGICAL, AND SOCIAL DETERMINANTS OF AMPLIFIED HIV SEROCONVERSION RISK IN SMM WHO USE METH. THE CENTERPIECE OF OUR LITE-2 INITIATIVE IS A NEW PROSPECTIVE, BIO-BEHAVIORAL COHORT WITH N=5,000 SMM (N=3,000 SMM WHO USE METH, N=2,000 WHO DO NOT). PARTICIPANTS WILL COMPLETE ASSESSMENTS OVER 36 MONTHS AND PROVIDE BIOLOGICAL SAMPLES FOR HIV TESTING, DRUG TOXICOLOGY TESTING, RECTAL STIS, AND RECTAL CYTOKINES/CHEMOKINES. OUR PRIMARY GOAL WILL BE TO INVESTIGATE THE ROLE OF GEOSPATIAL DETERMINANTS (E.G., BACKGROUND METH AND HIV PREVALENCE, URBANICITY) AND OTHER STRUCTURAL DETERMINANTS (E.G., STRUCTURAL STIGMA OF SEXUAL MINORITIES AS EVIDENCED BY POLICIES/LAWS) IN RELATION TO THE ASSOCIATION OF METH USE WITH AMPLIFIED HIV SEROCONVERSION RISK. AIM 2: TEST THE COMPARATIVE AND COMBINED EFFECTIVENESS OF TELEHEALTH MOTIVATIONAL ENHANCEMENT INTERVENTIONS FOR OPTIMIZING PREP USE. PREP READINESS INTERVENTIONS FOR SUPPORTING MOTIVATION (PRISM) IS A HYBRID TYPE I, MODIFIED FACTORIAL RANDOMIZED CONTROLLED TRIAL (RCT) OF TELEHEALTH CM THAT PROVIDES INCENTIVES FOR FILLING A PREP PRESCRIPTION, AND A 2-SESSION TELEHEALTH MI INTERVENTION THAT WE ADAPTED WITH METH- USING SMM (R34-DA046367, CARRICO/GROV). PRISM WILL ENROLL 840 METH-USING SMM WHO ARE NOT CURRENTLY TAKING PREP FROM THE LITE-2 COHORT (AIM 1) TO EXAMINE THE EFFECTIVENESS OF CM (N = 280), MI (N = 280), AND MI+CM (N = 280) ON THE PRIMARY OUTCOME – FILLING A PREP PRESCRIPTION. AIM 3: DETERMINE WHETHER GREATER RECTAL IMMUNE DYSREGULATION PARTIALLY EXPLAINS AMPLIFIED RISK OF HIV SEROCONVERSION IN SMM WHO USE METH. USING A CASE-COHORT DESIGN, WE WILL COMPARE HIV SEROCONVERTERS (N=450) WITH MATCHED SERONEGATIVE CONTROLS (N=450) TO EXAMINE THE CLINICAL RELEVANCE OF METH-INDUCED ALTERATIONS IN RECTAL CYTOKINES WITH RESPECT TO HIV SEROCONVERSION. THIS LITE-2 INITIATIVE COULD HAVE AN EXCEPTIONAL IMPACT BY TRANSFORMING HIV PREVENTION WITH SMM.

ARTIFICIAL INTELLIGENCE, MODELING, AND INFORMATICS FOR NUTRITION GUIDANCE AND SYSTEMS (AIMINGS) CENTER - ABSTRACT – OVERALL AIMINGS CENTER THE VISION OF THIS PROPOSED ARTIFICIAL INTELLIGENCE, MODELING, AND INFORMATICS, FOR NUTRITION GUIDANCE AND SYSTEMS (AIMINGS) CENTER IS TO IMPLEMENT COMPUTATIONAL AND DATA SCIENCE APPROACHES AND TOOLS TO ADVANCE NUTRITION FOR PRECISION HEALTH IN A WAY THAT ACCOUNTS FOR THE COMPLEX SYSTEMS INVOLVED. MANY EXISTING DATA SETS INCLUDE EXTRANEOUS DATA, MAKING THEM DIFFICULT TO ANALYZE AT BEST, AND AT WORST, PRONE TO GENERATING MISLEADING OR BIASED INSIGHTS. THUS, THERE IS A NEED TO FOR NEW APPROACHES, METHODS, AND TOOLS TO COLLAPSE AND DISTILL DATA TO MAKE THEM MORE ARTIFICIAL INTELLIGENCE (AI)-READY AND READY FOR A RANGE OF DIFFERENT ANALYSES. THIS COINCIDES WITH THE GOAL OF PROJECT 1: TO DEVELOP AND UTILIZE THE DATA DISTILLER FOR PRECISION NUTRITION, A SET OF APPROACHED AND TOOLS THAT CAN COLLAPSE AND DISTILL NUTRITION-RELEVANT DATA TO CREATE DATASETS THAT ARE AI- READY AND READY FOR A RANGE OF OTHER ANALYSES. THE FIRST OBJECTIVE OF THE NUTRITION FOR PRECISION HEALTH (NPH) PROGRAM IS TO “EXAMINE INDIVIDUAL DIFFERENCES OBSERVED IN RESPONSE TO DIFFERENT DIETS BY STUDYING THE INTERACTIONS BETWEEN DIET, GENES, PROTEINS, MICROBIOME, METABOLISM AND OTHER INDIVIDUAL CONTEXTUAL FACTORS.” GIVEN THE TYPE OF MISSING DATA WE FACE IN NUTRITION, AND THE IMPORTANCE OF ESTABLISHING CAUSAL RELATIONSHIPS RATHER THAN CORRELATIONS, THERE IS A NEED FOR NEW IMPUTATION METHODS. TO ADDRESS THIS, PROJECT 2, THE CAUSAL RELATIONSHIP DISENTANGLER, WILL INTRODUCE NEW APPROACHES FOR HANDLING MISSING DATA WHILE PRESERVING CAUSAL STRUCTURE. LEARNING HOW TO TRANSFER CAUSAL KNOWLEDGE AND DOING SO WITH MISSING DATA IS CRITICAL FOR REALIZING THE POTENTIAL OF NUTRITION FOR PRECISION HEALTH. THE NPH PROGRAM’S OTHER OBJECTIVES ARE “TO USE AI TO DEVELOP ALGORITHMS TO PREDICT INDIVIDUAL RESPONSES TO FOODS AND DIETARY PATTERNS,” AND “TO VALIDATE ALGORITHMS FOR CLINICAL APPLICATION.” THIS REQUIRES BRINGING DIFFERENT CAUSAL PATHWAYS TOGETHER TO UNDERSTAND HOW THEY INTERACT. AGENT-BASED MODELS (ABMS) CAN HELP AND SERVE AS "VIRTUAL LABORATORIES" TO PREDICT HOW DIFFERENT PEOPLE MAY RESPOND TO A PARTICULAR DIET UNDER DIFFERENT CIRCUMSTANCES. THEREFORE, THE GOAL OF PROJECT 3 (THE VIRTUAL HUMAN FOR PRECISION NUTRITION) IS TO DEVELOP AN ABM TOOL THAT CAN HELP BETTER UNDERSTAND AND PREDICT AN INDIVIDUAL'S RESPONSE TO FOOD AND DIETARY PATTERNS, WHILE BRINGING TOGETHER AND ACCOUNTING FOR THE INTERACTIONS BETWEEN GENETIC, PHYSIOLOGICAL, AND BEHAVIORAL FACTORS. HOWEVER, FOCUSING ON THE INDIVIDUAL ALONE WILL NOT BE ENOUGH TO ADDRESS ALL ASPECTS OF NPH. THEREFORE, THE VIRTUAL PUBLIC HEALTH PRECISION NUTRITION LABORATORY (PROJECT 4) WILL DEVELOP ABMS THAT REPRESENT AND ACCOUNT FOR THE SYSTEMS OUTSIDE INDIVIDUALS SUCH AS THEIR SOCIAL, ECONOMIC, AND BUILT ENVIRONMENTS. AN ADMINISTRATIVE AND COORDINATION CORE WILL OVERSEE ALL OPERATIONS AND A PILOT PROGRAM. A DATA SYSTEMS CORE (DSC) WILL LEVERAGE THE SUBSTANTIAL COMPUTING RESOURCES OF CUNY, WEST POINT, AND THE DEPARTMENT OF DEFENSE TO CREATE A FLEXIBLE CLOUD-BASED ARCHITECTURE FOR DATA FLOW AND A COLLABORATIVE WORKSPACE. A COMPUTATIONAL SYSTEMS CORE WILL PROVIDE RESOURCES AND PERSONNEL TO SUPPORT THE DSC AND TOOL DEVELOPMENT/DEPLOYMENT.

THE CENTRAL BROOKLYN VIOLENCE INTERVENTION COLLABORATIVE WILL PROVIDE TECHNICAL ASSISTANCE, CAPACITY BUILDING AND WORKFORCE DEVELOPMENT SERVICES TO A CORE TEAM OF COMMUNITY VIOLENCE INTERVENTION AGENCIES OPERATING IN CENTRAL BROOKLYN, NY. A KEY FOCUS OF THESE EFFORTS IS TO POSITION CVI TEAMS AS A PILLAR OF PUBLIC SAFETY AND PUBLIC HEALTH IN THEIR COMMUNITIES AND TO BETTER INTEGRATE THEIR TEAMS WITH THE EXISTING INFRASTRUCTURE OF PUBLIC SAFETY, HEALTHCARE, HOUSING, AND SOCIAL SERVICE PROVIDERS. IN WORKING TOWARDS THE GOAL OF REDUCING GUN VIOLENCE IN CENTRAL BROOKLYN, THE KEY OBJECTIVES OF THE COLLABORATIVE WILL BE TO: 1) ENHANCE STRATEGIC PLANNING AND PURPOSEFUL COLLABORATION AMONG CVI TEAMS THEMSELVES AND WITH KEY PUBLIC SAFETY STAKEHOLDERS; 2) BUILD A CULTURE OF CONTINUOUS WORKFORCE DEVELOPMENT OPPORTUNITIES TO STRENGTHEN THE TEAMS AT ALL LEVELS; 3) INCREASE CVI CAPACITY TO NAVIGATE AND PARTNER WITH LOCAL HEALTH, MENTAL HEALTH, SUBSTANCE USE, HOUSING, EDUCATION AND OTHER SOCIAL SERVICE PROVIDERS; 4) IMPROVE CLIENT NEEDS ASSESSMENTS AND CASE MANAGEMENT INFRASTRUCTURE; AND 5) DEVELOP STANDARDIZED DATA COLLECTION AND SHARING PROTOCOLS.

CHARTING THE EVOLUTIONARY DEVELOPMENT OF NOVEL GENES AND THE MOLECULAR MECHANISMS OF GLAND TISSUE ORGANIZATIONIN CEPHALOPODS - ABSTRACT VENOM IS A COMPLEX TRAIT THAT HAS CONVERGENTLY EVOLVED IN OVER 200,000 ANIMALS ACROSS THE TREE OF LIFE, TOTALING APPROXIMATELY ~15-30% OF ANIMAL BIODIVERSITY. THE PREVALENCE OF THE VENOM PHENOTYPE DEMONSTRATES ITS MOLECULAR SUCCESS AND STUDYING ITS EVOLUTION HAS BROAD APPLICATIONS TO UNDERSTANDING THE DEVELOPMENT OF HOMOLOGOUS TISSUES, THE ORIGINS OF NOVEL GENES, AND THE MOLECULAR MECHANISMS BEHIND THE REGULATION AND EXPRESSION OF BIOACTIVE COMPOUNDS. CONSIDERABLY, VENOMS ARE THE PROTOTYPE OF PRECISION MEDICINE: INDUCING A HIGHLY SPECIFIC AND IMMEDIATE RESPONSE. THESE ATTRIBUTES HAVE FUELED DRUG DISCOVERY EFFORTS, LEADING TO BREAKTHROUGH VENOM-DERIVED THERAPEUTICS FOR A WIDE RANGE OF CONDITIONS, FROM DIABETES TO HEART DISEASE TO PAIN. HOWEVER, THE FULL POTENTIAL OF VENOM, IN BOTH MEDICINE AND BIOLOGICAL RESEARCH, IS UNTAPPED. THIS UNMET NEED ARISES BECAUSE OF THE LACK OF ROBUST MODELS FOR GENETICALLY MANIPULATING THE DEVELOPMENT OF VENOM GLANDS AND REGULATION AND EXPRESSION OF VENOM BIOACTIVE PEPTIDES. THE WORK OF THE HOLFORD GROUP WAS THE FIRST TO CHARACTERIZE TEREBRID VENOM PEPTIDES AS BIOACTIVE IN MITIGATING ANALGESIC AND ANTITUMOR ACTIVITY. HOWEVER, WITHOUT GUIDING PRINCIPLES FOR HOW VENOMS AND VENOM GLANDS DEVELOP IN VIVO, WE, AND OTHER VENOM RESEARCHERS, HAVE JUST SCRATCHED THE SURFACE. WE NEED MODEL SYSTEMS TO REVOLUTIONIZE THE STUDY VENOM GLAND BIOLOGY, SO THAT WE CAN RADICALLY TRANSFORM HOW WE GENERATE, MANIPULATE, AND UTILIZE VENOM ARSENALS. THE CEPHALOPOD BREEDING PROGRAM PROVIDE THE TOOLS AND MODELS NECESSARY TO TACKLE BIOLOGICAL AND TRANSLATIONAL QUESTIONS THAT HAVE REMAINED UNAPPROACHABLE, SUCH AS: WHAT DRIVES THE EXPRESSION OF PREDATORY VERSUS DEFENSIVE VENOM COMPONENTS? CAN WE MANIPULATE THE PRODUCTION OF SPECIFIC TOXINS WITH A DESIRED FUNCTION, SUCH AS THOSE TARGETING RECEPTORS INVOLVED IN ANALGESIC ACTIVITY? ADVANCEMENTS IN GENETIC ENGINEERING, GENOMICS, TRANSCRIPTOMICS, AND PROTEOMICS WILL ALLOW US TO GENERATE THE FIRST MARINE INVERTEBRATE TRANSGENIC CEPHALOPOD ORGANISMS THAT PRODUCE VENOM IN SPECIALIZED GLANDS THAT CAN BE INVESTIGATED TO EXPLORE FUNDAMENTAL QUESTIONS ABOUT TISSUE DEVELOPMENT AND GENE REGULATION AND EXPRESSION. SPECIFICALLY, WE WILL: (1) DETERMINE GENES AND PROTEINS RELEVANT TO VENOM GLAND DEVELOPMENT, MAINTENANCE, AND SECRETION ACROSS DIVERSE CEPHALOPOD TAXA. THIS OBJECTIVE WILL DETERMINE THE EVOLUTIONARY UNDERPINNINGS BETWEEN VENOM SALIVARY GLANDS AND OTHER EXOCRINE TISSUES ACROSS TAXA. (2) TRACE THE DEVELOPMENT OF CEPHALOPOD SALIVARY GLANDS. THIS OBJECTIVE WILL REVEAL GENETIC PATHWAYS THAT CAN BE LEVERAGED TO DETERMINE THE FORMATION AND FUNCTION OF VENOM GLAND FROM DIVERSE TAXA. (3) ESTABLISH COMPARATIVE CEPHALOPOD TRANSGENIC MODELS. THIS OBJECTIVE WILL ESTABLISH TRANSGENIC CEPHALOPODS ALLOWING US TO OPTIMIZE THE UTILITY OF VENOM GLANDS FOR UNDERSTANDING THE DEVELOPMENT OF HOMOLOGOUS TISSUES, THE ORIGINS OF NOVEL GENES, AND THE MOLECULAR MECHANISMS BEHIND THE REGULATION AND EXPRESSION OF BIOACTIVE COMPOUNDS. THE PROPOSED RESEARCH IS A NEW DIRECTION FOR THE PI THAT IS HIGH RISK-HIGH REWARD, AND WILL BENEFIT DISPARATE FIELDS AND INDUSTRIES, INCLUDING DEVELOPMENTAL CELLULAR AND MOLECULAR BIOLOGY AND DRUG DISCOVERY AND DEVELOPMENT. MOST DISEASES, LIKE ALZHEIMER OR CANCER HAVE COMPLEX TRAITS WHOSE GENETIC CHARACTERIZATION IN MODEL SYSTEMS HAVE BEEN ESSENTIAL TO FINDING EFFECTIVE THERAPIES. STUDYING THE EVOLUTIONARY GENETICS IN THE COMPLEX TRAIT OF VENOM IN A RELIABLY, CULTURED CEPHALOPOD SYSTEM WILL BROADLY IMPACT RESEARCH TOWARDS THE NIH’S MISSION OF ENHANCING HUMAN HEALTH.

BRIEF DIGITAL INTERVENTION TO INCREASE COVID-19 VACCINATION AMONG INDIVIDUALS WITH ANXIETY OR DEPRESSION - PROJECT SUMMARY DESPITE INCREASED RISK OF COVID-19 INFECTION, SEVERE COMPLICATIONS, HOSPITALIZATIONS, AND DEATH, PEOPLE WITH MENTAL HEALTH DISORDERS REPORT GREATER VACCINE HESITANCY AND HAVE LOWER COVID VACCINATION LEVELS THAN THE GENERAL POPULATION. INDIVIDUALS WITH MENTAL HEALTH DISORDERS ARE MUCH MORE LIKELY TO ENDORSE COVID-19 VACCINE MISINFORMATION, WHICH MAY MEDIATE THE RELATIONSHIP BETWEEN MENTAL HEALTH AND VACCINE HESITANCY. INTERVENTIONS CAPABLE OF MITIGATING THE IMPACT OF VACCINE HESITANCY, MIS/DISINFORMATION, AND LOGISTICAL BARRIERS AMONG UNVACCINATED PEOPLE WITH MENTAL HEALTH DISORDERS ARE AN URGENT PRIORITY. ATTITUDINAL INOCULATION IS A BRIEF, SCALABLE STRATEGY TO ADDRESS MIS/DISINFORMATION. IN A QUASI-EXPERIMENTAL TRIAL, OUR TEAM FOUND THAT A BRIEF ONLINE ATTITUDINAL INOCULATION INTERVENTION SPECIFICALLY ADDRESSING COVID-19 VACCINE MIS/DISINFORMATION SIGNIFICANTLY DECREASED COVID-19 VACCINE HESITANCY AND INCREASED RESISTANCE TO VACCINE MISINFORMATION AMONG UNVACCINATED US ADULTS. HOWEVER, ITS EFFECTIVENESS AMONG INDIVIDUALS WITH MENTAL HEALTH DISORDERS IS UNKNOWN. INFORMED BY INOCULATION THEORY, ATTITUDINAL INOCULATION LEVERAGES THE POWER OF NARRATIVE, VALUES, AND EMOTION TO STRENGTHEN RESISTANCE TO MISINFORMATION AND REDUCE HESITANCY AND IS WELL- SUITED FOR LOW-INFORMATION AUDIENCES AND IDEOLOGICALLY POLARIZED OR CONSPIRATORIAL GROUPS. THE PROPOSED RESEARCH PROJECT WILL LEVERAGE THE INFRASTRUCTURE OF THE NATIONAL CHASING COVID COHORT, A LARGE AND GEOGRAPHICALLY DIVERSE COMMUNITY-BASED US COHORT, TO TAILOR AND TEST THE EFFECTIVENESS OF A BRIEF DIGITAL ATTITUDINAL INOCULATION INTERVENTION TO INCREASE VACCINATION AMONG ADULTS WITH ANXIETY OR DEPRESSION SYMPTOMS. AIM 1: CHARACTERIZE THE RELATIONSHIP BETWEEN SYMPTOMS OF ANXIETY AND DEPRESSION AND VACCINE/BOOSTER UPTAKE, AND OTHER RELATED DETERMINANTS OF VACCINE UPTAKE (INCLUDING ENDORSEMENT OF COVID-19 MISINFORMATION AND VACCINE HESITANCY) AMONG THOSE WITH AND WITHOUT ANXIETY/DEPRESSION AIM 2: ADAPT AND PILOT AN EVIDENCE-BASED ATTITUDINAL INOCULATION INTERVENTION TO INCREASE COVID-19 VACCINATION AND BOOSTING AMONG ADULTS WITH SYMPTOMS OF ANXIETY OR DEPRESSION. AIM 3: DETERMINE THE EFFECTIVENESS OF TWO BRIEF DIGITAL ATTITUDINAL INOCULATION INTERVENTION STRATEGIES COMPARED WITH CONVENTIONAL PUBLIC HEALTH MESSAGING FOR INCREASING VACCINE UPTAKE IN UN/UNDERVACCINATED AND UNBOOSTED ADULTS. THIS STUDY DIRECTLY ADDRESSES THE PRIORITIES OF THE NATIONAL INSTITUTE OF MENTAL HEALTH AND THE PAR: COVID-19 MENTAL HEALTH RESEARCH (PAR-22-112) TO CONDUCT “INTERVENTION EFFECTIVENESS RESEARCH TO ADDRESS VACCINE HESITANCY, UPTAKE, AND IMPLEMENTATION AMONG MENTAL HEALTH POPULATIONS.” THIS RESEARCH WILL RAPIDLY GENERATE EVIDENCE TO INFORM THE DEVELOPMENT AND IMPLEMENTATION OF STRATEGIES TO INCREASE VACCINATION UPTAKE AND MITIGATE THE IMPACT OF COVID-19 AMONG MENTAL HEALTH POPULATIONS. BEYOND THE COVID PANDEMIC, THIS RESEARCH HAS DIRECT APPLICABILITY TO FUTURE PANDEMICS AND ROUTINE VACCINATION CAMPAIGNS.

MACHINE LEARNING FOR RISK-ADJUSTED BREAST MRI SCREENING - SUMMARY MAGNETIC RESONANCE IMAGING (MRI) IS THE MOST SENSITIVE IMAGING MODALITY FOR BREAST CANCER DIAGNOSIS TO DATE. WOMEN WITH A STRONG FAMILY HISTORY OR RELATED GENETIC MUTATIONS HAVE AN ELEVATED RISK OF BREAST CANCER AND ARE RECOMMENDED TO PARTICIPATE IN YEARLY MRI SCREENINGS. HOWEVER, THE RATE OF DETECTION IN THIS HIGH-RISK COHORT IS SMALL, PROMPTING A DESIRE TO REDUCE UNNECESSARY MRI EXAMS. THE BASIC HYPOTHESIS OF THIS PROJECT IS THAT WITHIN THE SCREENING COHORT THE INDIVIDUAL RISK OF A FUTURE CANCER CAN BE ESTIMATED BASED ON THE APPEARANCE OF BREAST MRI AND MAMMOGRAMS TODAY. IN PRELIMINARY WORK WE HAVE ALREADY IDENTIFIED LOW-RISK WOMEN THAT COULD HAVE OMITTED A SCREENING SESSION WITHOUT MISSING A NEW CANCER. THE DISCOVERY OF THIS LOWER-RISK SUBGROUP WAS MADE POSSIBLE BY MODERN DEEP-LEARNING TOOLS DEVELOPED IN PRELIMINARY WORK. MEMORIAL SLOAN KETTERING CANCER CENTER (MSK) HAS ACCRUED A DATABASE OF APPROXIMATELY 70,000 BREAST MRI EXAMS OVER 18 YEARS ALONG WITH THE PATIENTS’ CLINICAL OUTCOMES. THIS UNPRECEDENTED RESOURCE ENABLES THE TRAINING OF MODERN MACHINE LEARNING “FROM THE GROUND-UP” TO EXTRACT AND CLASSIFY VOLUMETRIC MRI FEATURES. THE SPECIFIC AIMS OF THIS PROJECT ARE AS FOLLOWS. AIM 1 (DATA CURATION): SYSTEMATIC ANALYSIS OF THE LARGE DATASET ACCRUED AT MSK REQUIRES CAREFUL CURATION INCLUDING IMAGE CONTENT, IMAGE QUALITY, PATHOLOGY RESULTS, CLINICAL FOLLOW-UP, AS WELL AS DEMOGRAPHIC AND GENOMIC INFORMATION. THE OUTCOME OF THIS AIM IS A CURATED DATASET THAT CAN BROADLY BENEFIT FUTURE TECHNICAL EFFORTS IN BREAST DIAGNOSIS. AIM 2 (DEEP LEARNING): TO MAKE RISK STRATIFICATION QUANTITATIVE WE PROPOSE TO ANALYZE THE MRI SCANS USING MODERN DEEP NETWORKS THAT HAVE BEEN TRAINED TO IDENTIFY THE LOCATION AND EXTENT OF A CANCER. WE WILL THEN TRANSFER THE MRI FEATURES OF THESE TRAINED NETWORKS AS WELL AS NETWORKS TRAINED ON MAMMOGRAMS TO THE TASK OF DIAGNOSIS AND RISK ASSESSMENT. THE INTENDED OUTCOME OF THIS AIM ARE PREDICTIVE MODELS WITH HUMAN-LEVEL PERFORMANCE AT DIAGNOSIS AND SEGMENTATION. AIM 3 (RISK ADJUSTED SCREENING): TO REDUCE THE BURDEN OF SCREENING WHILE MAINTAINING SENSITIVITY WE WILL ESTIMATE THE RISK OF FINDING A MALIGNANT TUMOR IN THE FUTURE, BASED ON THE PRESENT MRI EXAM AND MOST RECENT MAMMOGRAM AS WELL AS PATIENT INFORMATION. THE MACHINE-ESTIMATED RISK WILL BE USED IN A RETROSPECTIVE ANALYSIS TO DETERMINE THE PRIMARY OUTCOME, NAMELY, THE NUMBER OF EXAMS THAT COULD HAVE BEEN OMITTED BY SCHEDULING A LONGER SCREENING INTERVAL WITHOUT COMPROMISING SENSITIVITY. THIS WILL BE REPEATED ON NEWLY ACCRUED DATA AT MSK, DUKE AND JOHNS HOPKINS UNIVERSITY (JHU) AS SECONDARY SITES. ONCE VALIDATED, THE RISK-PREDICTION MODEL WILL BE PUBLICLY RELEASED TO ENCOURAGE DATA SHARING AND CLINICAL ADOPTION. THE PRELIMINARY WORK PERFORMED OVER THE LAST TWO YEARS HAS BROUGHT TOGETHER A UNIQUE INTERDISCIPLINARY TEAM INCLUDING CLINICAL INVESTIGATORS ON BREAST MRI AT MSK, AND MACHINE-LEARNING AND MEDICAL IMAGING EXPERTS AT CCNY, DUKE AND JHU. THE PLATFORM TECHNOLOGY THAT WILL BE DEVELOPED HERE IS APPLICABLE BEYOND BREAST CANCER, AND THE TRANSFER LEARNING APPROACH APPLICABLE IN PARTICULAR TO CANCERS WITH MORE LIMITED DATASETS.

AS RWHAP SERVES >50% OF US PWH, RWHAP OUTCOMES ARE VITAL TO ACHIEVING “GETTING TO ZERO”/ ENDING THE HIV EPIDEMIC (EHE) PLAN TARGETS. AS A GRANTEE FOR RWHAP PART A (RWPA) FUNDING DISTRIBUTED TO THE COUNTIES/CITIES SEVERELY AFFECTED BY HIV, NEW YORK CITY (NYC) CONDUCTS REGULAR HIV CARE CONTINUUM MONITORING CITYWIDE AND IN ITS RWPA PROGRAMS, WHICH OFFER SUPPORT SERVICES TO REDUCE SOCIAL AND BEHAVIORAL BARRIERS TO CARE/TREATMENT. TO ADDRESS LOCAL OUTCOME DISPARITIES AND TO FILL GAPS LEFT BY DATA-TO-CARE (D2C) STRATEGIES AND RESEARCH FOCUSED ON MEDICAL CARE (RE-)LINKAGE, WE PROPOSE TO IMPLEMENT AND RIGOROUSLY EVALUATE THE EFFECTIVENESS OF A NOVEL ‘DATA-TO-SUPPRESSION’ (D2S) INTERVENTION AMONG ~1,300 RWPA CLIENTS IN HIV CARE BUT UNSUPPRESSED. SURVEILLANCE-BASED REPORTS ON UNSUPPRESSED CLIENTS PLUS D2S CAPACITY-BUILDING ASSISTANCE WILL GUIDE RWPA PROVIDERS IN TARGETING AND DELIVERING EVIDENCE-INFORMED STRATEGIES TO IMPROVE VS. OUR PROPOSED AIMS ARE TO: 1) MEASURE D2S INTERVENTION EFFECTS ON TIMELY VS AND TIME TO VS, IN A STEPPED- WEDGE HYBRID TYPE 1 TRIAL; 2) IDENTIFY MODIFIABLE DETERMINANTS OF D2S RESPONSE, BY COMPARING CHARACTERISTICS OF D2S-EXPOSED CLIENTS WHO DO AND DO NOT ACHIEVE VS, TO RECOGNIZE OPPORTUNITIES TO TAILOR AND STRENGTHEN THE INTERVENTION; 3) ASSESS D2S ACCEPTABILITY AND PARTICIPANT PREFERENCES AND PRIORITIES FOR ITS IMPLEMENTATION, IN EIGHT (CLIENT AND PROVIDER) FOCUS GROUPS AND IN A DISCRETE CHOICE EXPERIMENT (DCE) WITH RWPA STAFF (N=200). THIS PROPOSAL ANSWERS THE CALL IN PAR-20-036 FOR RESEARCH TO DEEPEN UNDERSTANDING OF THE BROADER CONTEXT OF VS, BY LEVERAGING EXTENSIVE/MULTIPLE DATASETS AND RESOURCES AND APPLYING IMPLEMENTATION SCIENCE METHODS TO TEST AN INTERVENTION TO IMPROVE SERVICE DELIVERY AND ULTIMATELY ACHIEVE VS. THE STUDY IS ALSO ALIGNED WITH THE NATIONAL HIV/AIDS STRATEGY GOAL TO REDUCE HEALTH DISPARITIES – IN THIS CASE, A VS GAP BETWEEN RWPA CLIENTS IN CARE AND OTHER PWH IN CARE IN THE SAME JURISDICTION. THE PROPOSED STUDY’S HYBRID, TYPE 1 TRIAL DESIGN SUPPORTS RAPID DISSEMINATION AND UPTAKE BEYOND NYC, SHOULD THE INTERVENTION PROVE EFFECTIVE. THROUGH THE D2S TRIAL, ANALYSIS OF FACTORS DRIVING D2S RESPONSE, AND PRIMARY DATA COLLECTION IN NYC RWPA SUPPORT- SERVICE SETTINGS, THE PROJECT SEEKS TO CLARIFY FUTURE INTERVENTION DIRECTIONS FOR LOW-INCOME, BLACK AND LATINX PWH, WHOSE CARE CONTINUUM ADVANCES ARE VITAL FOR MEETING 90-90-90, “GETTING TO ZERO” AND EHE PLAN GOALS. WE WILL TEST THIS UPDATED CURRICULUM IN A RANDOMIZED CONTROLLED TRIAL (RCT) AMONG 856 ADOLESCENTS AND YOUNG ADULTS (AGED 18- 27 YEARS) WITH MILD TO MODERATE I/DD WHO RECEIVE SERVICES FROM DISABILITY PROVIDERS IN FOUR OF THE FIVE DEVELOPMENTAL DISABILITIES REGIONS OF THE NEW YORK STATE OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES. RANDOMIZATION WILL BE STRATIFIED BY REGION, SEX (MALE/FEMALE) AND AGE (18-22, 23-27). HALF OF THE PARTICIPANTS WILL BE RANDOMIZED TO RECEIVE THE UPDATED STEPS-2 SOCIALIZATION AND SEX EDUCATION CURRICULUM IN INDIVIDUALIZED SESSIONS DURING HOME VISITS, AND THE OTHER HALF WILL RECEIVE A GROUP-BASED INTERVENTION ON PHYSICAL EXERCISE AND NUTRITION IN A LOCAL COMMUNITY CENTER; ONLY ONE ROOMMATE WILL BE ELIGIBLE TO BE ENROLLED FROM ANY GROUP LIVING ARRANGEMENT TO MINIMIZE SPILLOVER EFFECTS. INTERIM MEASURES WILL BE USED TO ASSESS KNOWLEDGE AND TO REDUCE THE LIKELIHOOD OF RECALL BIAS FOR THE PRIMARY ENDPOINT. SPECIFICALLY, WE AIM TO: (1) UPDATE AND PILOT AN EXISTING SOCIALIZATION AND SEX EDUCATION CURRICULUM (STEPS-2) FOR 18-27 YEAR OLDS WITH I/DD IN NEW YORK STATE THROUGH A PARTICIPATORY PROCESS CENTERING THE VOICES OF DIVERSE LIVED EXPERIENCES; (2) TEST THE EFFICACY OF THIS SOCIALIZATION AND SEX EDUCATION CURRICULUM IN A RANDOMIZED INTERVENTION TRIAL USING INTENTION-TO-TREAT AND INSTRUMENTAL VARIABLE ANALYSES TO ADJUST FOR COMPLIANCE. WE HYPOTHESIZE THAT AT LEAST AN ADDITIONAL 10% OF THE INDIVIDUALS IN THE EXPERIMENTAL GROUP WILL HAVE HAD A REPRODUCTIVE HEALTH DISCUSSION WITH A MEDICAL PROVIDER BY MONTH 12 COMPARED WITH THE CONTROL GROUP; AND (3) TEST WHETHER THE EFFICACY OF THE INTERVENTION DIFFERS BY SEX AND AGE IN STRATIFIED ANALYSES. WE HAVE POWERED THE STUDY FOR SEX-SPECIFIC EFFICACY ESTIMATES AND WILL EXPLORE WHETHER EFFICACY DIFFERS BY AGE. HISPANIC SERVING INSTITUTIONS STEM HUB FOR INNOVATION IN NUMERACY EDUCATION (HSI-SHINE) -WITH SUPPORT FROM THE IMPROVING UNDERGRADUATE STEM EDUCATION: HISPANIC-SERVING INSTITUTIONS (HSI PROGRAM), THIS HSI PROGRAM RESOURCE HUB AIMS TO IMPROVE STUDENTS' NUMERACY SKILLS, TRAIN FACULTY AT HISPANIC-SERVING INSTITUTIONS TO TEACH THESE SKILLS MORE EFFECTIVELY AND DEVELOP A NATIONAL REPOSITORY OF NUMERACY RESOURCES FOR FACULTY AND STUDENTS AT HSIS. THE STEM HUB FOR INNOVATION IN NUMERACY EDUCATION (SHINE) WILL ADDRESS WIDESPREAD ILLITERACY IN QUANTITATIVE SKILLS BY EQUIPPING FACULTY WITH PROFESSIONAL DEVELOPMENT IN TEACHING QUANTITATIVE REASONING AND DATA ANALYSIS, DEVELOPING AN ENGAGING ONLINE NUMERACY TRAINING PLATFORM, AND ESTABLISHING COMMUNITY-BASED DATA ANALYSIS RESEARCH PROGRAMS (DARP) THAT APPLY QUANTITATIVE RESEARCH SKILLS TO REAL-WORLD PROBLEMS. SHINE WILL ALSO INTEGRATE CO-REQUISITE MATH INSTRUCTION AND CREATE A REPOSITORY OF RESOURCES CONNECTING STUDENTS WITH INTERNSHIPS, JOB OPPORTUNITIES, AND EDUCATIONAL PATHWAYS. FACULTY PARTICIPANTS WILL DESIGN INNOVATIVE INSTRUCTIONAL MATERIALS, MENTOR STUDENTS IN MEANINGFUL RESEARCH PROJECTS, AND IMPLEMENT EVIDENCE-BASED APPROACHES TO STRENGTHEN NUMERACY EDUCATION. THESE EFFORTS AIM TO BUILD SUSTAINABLE COMMUNITIES OF PRACTICE THAT PROMOTE COLLABORATION AMONG FACULTY AND INSTITUTIONS, IMPROVE EDUCATIONAL OUTCOMES FOR STUDENTS, AND PREPARE PARTICIPANTS FOR SUCCESSFUL CAREERS IN STEM. BY ADDRESSING THE NEEDS OF BOTH STUDENTS AND SURROUNDING COMMUNITIES, SHINE WILL CREATE A LASTING EDUCATIONAL AND SOCIETAL IMPACT, BRIDGING GAPS IN NUMERACY EDUCATION. THE SHINE PROJECT AIMS TO STRENGTHEN FACULTY CAPACITY FOR TEACHING QUANTITATIVE REASONING AND DATA ANALYSIS WHILE IMPROVING STUDENTS? NUMERACY SKILLS WITHIN THE LARGER PROCESS OF LIBERATORY IDENTITY DEVELOPMENT, INCLUDING NOT ONLY ACADEMIC (STEM IDENTITY, IN PARTICULAR) VIS A VIS COMMUNITY-BASED INITIATIVES. PARTICIPANTS WILL DEVELOP INSTRUCTIONAL MATERIALS, LEAD STUDENT ASSESSMENTS, AND MENTOR STUDENTS IN THE DARP, WHICH CONNECTS CLASSROOM LEARNING TO REAL-WORLD APPLICATIONS. THE PROJECT WILL ALSO LAUNCH AN ONLINE NUMERACY TRAINING PLATFORM WITH ACCESSIBLE FEATURES AND CO-REQUISITE MATHEMATICAL SUPPORT, PROVIDING STUDENTS WITH EFFECTIVE TOOLS FOR SKILL DEVELOPMENT. SHINE WILL EVALUATE ITS IMPACT THROUGH A MIXED-METHODS APPROACH, INCLUDING SURVEYS, SKILLS TESTS, INTERVIEWS, AND FOCUS GROUPS, TO MEASURE FACULTY PREPAREDNESS AND STUDENT QUANTITATIVE REASONING SKILLS, STEM IDENTITY, AND LIBERATORY OUTCOMES. KEY RESEARCH QUESTIONS WILL ADDRESS HOW SHINE BUILDS EMPOWERING OUTCOMES BY INFLUENCING COGNITIVE SKILLS, ATTITUDES, STUDENT RETENTION, GRADUATION RATES, AND CAREER TRAJECTORIES. ANTICIPATED OUTCOMES INCLUDE ENHANCED FACULTY EXPERTISE, IMPROVED STUDENT NUMERACY AND STEM READINESS, AND A NATIONAL REPOSITORY OF INSTRUCTIONAL RESOURCES TO SUPPORT QUANTITATIVE REASONING EDUCATION ACROSS INSTITUTIONS. DISSEMINATION OF FINDINGS WILL OCCUR THROUGH SCHOLARLY ARTICLES, CONFERENCE PRESENTATIONS, AND OPEN-ACCESS MATERIALS, ENSURING SCALABILITY AND BROAD IMPACT. A FUNDAMENTAL UNDERSTANDING OF NATURE?S ARCHITECTURES AND PROCESSES CAN BE APPLIED TO CREATE NEW BIOLOGY-INSPIRED AND GREEN TECHNOLOGIES FOR THE BENEFIT OF HUMAN AND ENVIRONMENTAL HEALTH. HOWEVER, THIS CHALLENGING TASK REQUIRES INTEGRATING ASPECTS OF RESEARCH IN BIOLOGY, CHEMISTRY, AND PHYSICS FOCUSED DOWN TO THE SMALLEST FUNCTIONAL SCALE: THE NANOSCALE. THIS NSF RESEARCH TRAINEESHIP (NRT), NANOSCIENCE CONNECTED TO LIFE: AN INTERDISCIPLINARY TRAINEESHIP PROGRAM IN BIOINSPIRED NANOTECHNOLOGY, WILL PROVIDE EDUCATION, RESEARCH, AND CAREER DEVELOPMENT TO HELP BUILD A NEW WORKFORCE IN THE EMERGING FIELD OF BIOINSPIRED NANOTECHNOLOGY. AS THIS FIELD UNDERGOES RAPID EXPANSION AND INDUSTRY EMBRACES THESE NEW APPROACHES, SOCIETY NEEDS A UNIQUELY TRAINED AND DIVERSE WORKFORCE. BUILDING ON THE CITY UNIVERSITY OF NEW YORK?S (CUNY) REPUTATION AS AN ENGINE FOR UPWARD MOBILITY OF A DIVERSE STUDENT POPULATION, THIS TRAINEESHIP WILL PROVIDE A NOVEL AND COMPREHENSIVE GRADUATE EDUCATION FOR 125 GRADUATE STUDENTS. THE PROGRAM WILL SERVE 25 DIRECTLY FUNDED TRAINEES, INCLUDING INDIVIDUALS FROM MINORITY POPULATIONS UNDERREPRESENTED IN THEIR PURSUIT OF STEM FIELDS OF STUDY, WOMEN, AND MEMBERS OF THE LGBTQ+ COMMUNITY. GRADUATES WILL BECOME EFFECTIVE LEADERS WHO WILL BE ABLE TO SOLVE COMPLEX PROBLEMS FACING THE U.S. AND PROVIDE NEW SCIENTIFIC BREAKTHROUGHS IN ENERGY HARVESTING, AGRICULTURE, MEDICINE, AND ENVIRONMENTAL REMEDIATION. THE BROAD EDUCATIONAL VISION OF THIS PROJECT IS TO TRAIN A GENERATION OF STEM PH.D. STUDENTS AS COLLABORATIVE, SYSTEMS-LEVEL THINKERS WHO CAN TRANSLATE CONCEPTS ACROSS SCALES AND DISCIPLINES. THE PROJECT STRENGTHENS AND INTEGRATES CHEMISTRY, BIOCHEMISTRY, AND PHYSICS PH.D. PROGRAMS AT CUNY THROUGH CONVERGENCE AROUND COMMON GOALS. STUDENTS WILL BECOME WELL-POSITIONED TO WORK ON CONVERGENT TEAMS TO ADDRESS URGENT SOCIETAL CHALLENGES RELATED TO ENVIRONMENTAL INSTABILITIES AND HEALTH CRISES. AT THE HEART OF EACH STUDENT?S EXPERIENCE IS A USE-INSPIRED PROJECT IN ONE OF SEVERAL DIFFERENT AREAS OF RESEARCH EMPLOYING BIOLOGY-INSPIRED APPROACHES. THESE INCLUDE: I) HARVESTING AND CONVERSION OF ENERGY, II) DESIGNING NANOSCALE INTERFACES TO MEASURE AND INFLUENCE LIVING SYSTEMS, III) NEWLY DESIGNED SUSTAINABLE GREEN MATERIALS, AND IV) DEVELOPMENT OF ADAPTIVE METABOLIC SYSTEMS AND FUNCTIONAL MATERIALS FOR THERAPEUTIC AND INDUSTRIAL USE. THESE INVESTIGATIONS WILL SPAN THEORETICAL, COMPUTATIONAL, AND EXPERIMENTAL APPROACHES. THE PROJECT WILL PREPARE CUNY PH.D. STUDENTS FOR STEM CAREERS IN INDUSTRIAL, GOVERNMENT, ACADEMIC, AND ENTREPRENEURIAL TRACKS BY INTEGRATING CAREER AWARENESS THROUGHOUT THE TRAINING PERIOD. THE TRAINING PLAN IS BASED ON AN INNOVATIVE INTEGRATING MENTAL MODELS AND EXPERIMENTAL RESEARCH IN SCIENTIFIC EDUCATION (IMMERSE) TRAINING MODEL, DESIGNED?TO?CREATE A LEARNING COMMUNITY OF SYSTEMS THINKERS. THE NETWORK WILL FOSTER ENTRY OF A DIVERSE CADRE OF RECENT BACHELOR?S DEGREE GRADUATES INTO CAREERS IN DEMANDING SCIENTIFIC FIELDS SUCH AS BIOTECHNOLOGY, BIOENGINEERING, AND SCIENCE EDUCATION, AMONGST OTHERS. THIS APPROACH IS GROUNDED IN THE EVIDENCE-BASED ASSUMPTION THAT TRAINING COMMUNITIES FACING HEALTH DISPARITIES TO SERVE AS COMMUNITY HEALTH WORKERS WILL FOSTER ECONOMIC MOBILITY, INCREASE ACCESS TO CARE AND QUALITY OF CARE, AND HELP CLOSE HEALTH DISPARITIES. LAGUARDIA WILL RECRUIT LOW-INCOME, UNEMPLOYED AND UNDEREMPLOYED NEW YORKERS FOR CHW TRAINING AND EMPLOYMENT, WITH A FOCUS ON THE FOLLOWING THREE POPULATIONS EXPERIENCING HEALTH DISPARITIES: - PUBLIC HOUSING RESIDENTS - IMMIGRANTS WITH LIMITED ENGLISH PROFICIENCY - COMMUNITIES OF COLOR IN NYC’S HIGHEST-POVERTY NEIGHBORHOODS THIS WILL BE ACCOMPLISHED THROUGH PARTNERSHIPS WITH THE FOLLOWING THREE ORGANIZATIONS: - NYC HOUSING AUTHORITY RESIDENT ECONOMIC EMPOWERMENT & SUSTAINABILITY (NYCHA REES) WILL HELP TO RECRUIT PUBLIC HOUSING RESIDENTS FOR ALL FOUR TRAINING TRACKS. - COMMONPOINT QUEENS (CPQ) WILL HELP TO RECRUIT ENGLISH LANGUAGE LEARNERS AND TO OFFER CONTEXTUALIZED ENGLISH LANGUAGE BRIDGE INSTRUCTION SIMULTANEOUS TO THE CHW TRAINING. - 1199SEIU TRAINING AND EMPLOYMENT FUNDS (1199) WILL SPONSOR THE CHW REGISTERED APPRENTICESHIP, MANAGE THE RELATIONSHIP WITH THE APPRENTICESHIP EMPLOYER, AND TRAIN INCUMBENT WORKERS. OVER THE COURSE OF THREE YEARS, LAGUARDIA WILL TRAIN 255 STUDENTS, INCLUDING 195 JOB-SEEKERS AND 60 INCUMBENT WORKERS, ACROSS FOUR DIFFERENT TRAINING T RACKS: TRACK A: STANDARD CHW TRAINING AND INTERNSHIP PROGRAM TRACK B: CHW FOR ENGLISH LANGUAGE LEARNERS TRAINING AND INTERNSHIP PROGRAM TRACK C: CHW REGISTERED APPRENTICESHIP PROGRAM TRACK D: CHW INCUMBENT WORKER TRAINING LAGUARDIA’S CHW TRAINING CURRICULUM INCLUDES FOUR CLASSES, DELIVERED OVER 200 HOURS OF INSTRUCTION: FOUNDATIONS OF COMMUNITY HEALTH WORK, PUBLIC HEALTH, HEALTH & WELLNESS, AND TECHNOLOGY FOR COMMUNITY HEALTH. TOPICS INCLUDE MOTIVATIONAL INTERVIEWING, ACTIVE LISTENING, INTER-PERSON AND COMMUNICATION SKILLS, INTER-DISCIPLINARY TEAMS, OUTREACH AND ENGAGEMENT, ADVOCACY, SERVICE COORDINATION, HEALTHCARE AND SOCIAL SERVICE SYSTEMS, HEALTH INSURANCE, CULTURAL COMPETENCE, RACISM, SOCIAL DETERMINANTS OF HEALTH, POPULATION HEALTH, CHRONIC DISEASES, INFECTIOUS DISEASES, MENTAL HEALTH, SUBSTANCE USE, DIET, EXERCISE, SEXUALITY, MICROSOFT OFFICE SUITE, TELEHEALTH, ELECTRONIC HEALTH RECORDS, AND MORE. THE COURSES CARRY NINE CREDITS FOR THOSE CONTINUING ON TO COLLEGE AT LAGUARDIA. INCUMBENT WORKER TRAINING WILL FOCUS ON TECHNOLOGY SKILLS, EMERGENCY PREPAREDNESS, AND VACCINE HESITANCY, AS WELL AS OTHER SPECIALTY TOPICS DEVELOPED BASED ON EMPLOYER FEEDBACK. HRSA ORGANIZATIONAL PRIORITIES IN TELEHEALTH AND BEHAVIORAL HEALTH WILL FEATURE PROMINENTLY IN THE CURRICULUM AND FIELD EXPERIENCES. STUDENTS WILL RECEIVE STIPENDS TO ALLOW THEM TO DEDICATE TIME TO TRAINING AND INTERNSHIPS AS WELL AS EDUCATIONAL CASE MANAGEMENT TO CONNECT TO OTHER RESOURCES ON CAMPUS AND BEYOND. OVER 90% OF STUDENTS ARE EXPECTED TO COMPLETE TRAINING, WITH 100% OF TRAINING COMPLETERS IN THE JOB-SEEKER COHORTS COMPLETING INTERNSHIPS OR APPRENTICESHIPS AT ONE OF SEVERAL DOZEN INTERNSHIP AND APPRENTICESHIP PARTNER ORGANIZATIONS. ASSOCIATION OF THE FRAGILE X PREMUTATION WITH COGNITIVE AND BEHAVIORAL SKILLS IN CHILDREN - PROJECT SUMMARY THE OVERARCHING GOAL OF OUR PROPOSAL IS TO ANSWER THE CLINICALLY SIGNIFICANT QUESTION: DOES A FRAGILE X PREMUTATION (PM; AN ALLELE OF 55-200 CGG REPEATS IN THE X-LINKED FMR1 GENE) POSE A SIGNIFICANT RISK TO CHILDHOOD DEVELOPMENT? WHY IS THIS IMPORTANT? AT LEAST ONE IN 300 WOMEN IN THE US CARRIES A PM ALLELE. BECAUSE MOST OF THESE ALLELES ARE SMALL AND NOT PRONE TO LARGE EXPANSIONS, NEARLY HALF OF CHILDREN BORN TO THESE WOMEN WILL INHERIT A PM, WHEREAS FEWER THAN 6% WILL INHERIT A FULL MUTATION (>200 REPEATS), THE ALLELE THAT CAUSES FRAGILE X SYNDROME. PM ALLELES ARE KNOWN TO INCREASE THE RISK FOR TWO LATE-ONSET DISORDERS: FRAGILE X ASSOCIATED TREMOR/ATAXIA SYNDROME (FXTAS) AND FRAGILE X-ASSOCIATED PRIMARY OVARIAN INSUFFICIENCY (FXPOI). ADULTS WITH A PM ALLELE WHO DO NOT HAVE FXTAS OR FXPOI ARE STILL REPORTED TO HAVE EXECUTIVE SKILL AND WORKING MEMORY DEFICITS, VISUOSPATIAL WEAKNESS AND POOR MATH SKILLS, ANXIETY, AND DEPRESSION. FOR CHILDREN WHO CARRY A PM, THERE IS A SIGNIFICANT GAP IN OUR KNOWLEDGE. SOME REPORTS FIND HIGHER RATES OF DEVELOPMENTAL DELAY, ATTENTION DEFICITS, AUTISM, AND ANXIETY AMONG CHILDREN WITH A PM VERSUS NON-CARRIERS, BUT THESE REPORTS ARE LIMITED BY ASCERTAINMENT BIAS AND SMALL SAMPLE SIZE. WE HYPOTHESIZE THAT THE TOXIC EFFECT OF A PM CAN MANIFEST EARLY IN LIFE, LEADING TO ADVERSE NEURODEVELOPMENTAL OUTCOMES AND WE PROPOSE TO TEST THIS RIGOROUSLY AND THOROUGHLY. TO ADDRESS THIS GAP IN KNOWLEDGE, WE HAVE AN OUTSTANDING OPPORTUNITY TO STUDY A LARGE COHORT OF CHILDREN, NOW AGE 3 -12 YEARS, WHO WERE IDENTIFIED PROSPECTIVELY: SPECIFICALLY, 582 CHILDREN WITH A PM AND 958 NON-CARRIERS, ALL IDENTIFIED WHEN THEIR CARRIER MOTHERS UNDERWENT PRENATAL DIAGNOSIS FOR A FRAGILE X MUTATION. THIS UNIQUE COHORT OF CHILDREN HAS MINIMAL POTENTIAL FOR ASCERTAINMENT BIAS. IN AIM 1, PARENTS WILL COMPLETE ONLINE QUESTIONNAIRES TO DESCRIBE THEIR CHILD’S GENERAL HEALTH AND HISTORY OF DIAGNOSED NEURODEVELOPMENTAL PROBLEMS AND TO DETAIL—USING STANDARDIZED QUESTIONNAIRES—THE CHILD’S BEHAVIOR, EXECUTIVE SKILLS, AND SOCIAL RELATEDNESS. IN AIM 2, A SUBSET OF SCHOOL-AGED PARTICIPANTS WHO LIVE IN THE MID-ATLANTIC REGION WILL BE ASSESSED INDIVIDUALLY PRIMARILY VIA REMOTE-ADMINISTERED SEMI-STRUCTURED INTERVIEW AND NEUROPSYCHOLOGICAL MEASURES EXAMINING COGNITIVE, ACADEMIC, SOCIAL AND PSYCHOLOGICAL FUNCTION. MOTHERS WILL ALSO COMPLETE QUESTIONNAIRES REGARDING THEIR OWN PSYCHOLOGICAL WELL-BEING AND PARENTING STRESS. WE WILL EVALUATE ALL PHENOTYPES WITH RESPECT TO SEX, CGG REPEAT LENGTH, AND REPEAT STRUCTURE TO GAIN INSIGHT INTO THE MECHANISMS AND ASSOCIATED RISKS (AIM 3). OUR PRIMARY HYPOTHESIS IS THAT CHILDREN WITH A PM HAVE A HIGHER FREQUENCIES OF NEURODEVELOPMENTAL DEFICITS THAN NON-CARRIER CHILDREN, STRATIFIED BY SEX. THE COMBINATION OF PARENTAL REPORT AND DIRECT ASSESSMENT WILL PROVIDE A COMPREHENSIVE EVALUATION OF COGNITIVE AND BEHAVIORAL SKILLS. THIS WILL RESULT IN A POWERFUL COMBINATION OF DETAILED PHENOTYPIC AND GENETIC INFORMATION THAT WILL UNAMBIGUOUSLY DEFINE THE RISK OF ADVERSE CHILDHOOD DEVELOPMENT POSED BY A PM ALLELE.

EPIGENETIC SUSCEPTIBILITY OF BEHAVIORAL AND ADDICTIVE DISORDERS DURING PRE/PUBESCENCE AFTER NATURAL DISASTER EXPOSURES IN-UTERO - SUMMARY/ABSTRACT: THE RISK FOR DEVELOPING FUTURE PSYCHIATRIC AND ADDICTION DISORDERS STARTS EARLY IN LIFE, WITH EVIDENCE EMPHASIZING THE CRITICAL CONTRIBUTION OF THE IN UTERO ENVIRONMENT. MATERNAL PSYCHOSOCIAL STRESS HAS A SIGNIFICANT IMPACT ON THE PRENATAL ENVIRONMENT AND CONTRIBUTES TO NEGATIVE HEALTH AND NEUROBEHAVIORAL OUTCOMES IN OFFSPRING. OUR RESEARCH (STRESS IN PREGNANCY, SIP STUDY), ALONG WITH THAT OF OTHERS, HAS SHOWN THAT IN UTERO EXPOSURE TO MATERNAL PSYCHOSOCIAL STRESS IS A MAJOR PREDICTOR OF THE RISK FOR FEAR-RELATED AND IMPULSE CONTROL-RELATED PROBLEMS DURING EARLY CHILDHOOD. WE ARE NOW WELL POSITIONED TO CONDUCT A LONG-TERM FOLLOW-UP OF OUR UNIQUE RACIALLY AND FINANCIALLY DIVERSE SIP STUDY POPULATION TO IDENTIFY BIOMARKERS OF CHILDREN’S BEHAVIOR AND PHYSIOLOGICAL REACTIVITY RELEVANT FOR FUTURE SUBSEQUENT BEHAVIORAL DISORDERS AND ADDICTIVE RISK. DESPITE ITS IMPORTANCE, FEW STUDIES HAVE INVESTIGATED PLACENTAL MOLECULAR SIGNATURES TO BIO-BEHAVIORAL ASSESSMENTS FROM EARLY LIFE THROUGH THE PRE/PUBERTAL PHASE. EXPLOITING THE SIP STUDY'S BIO-REPOSITORIES AND DEEP-PHENOTYPED CHILD BEHAVIORS, OUR PRELIMINARY RESULTS SHOW THAT EPIGENETIC CHANGES OF THE HYPOTHALAMIC-PITUITARY-ADRENAL (HPA) AXIS AND IMMUNE SYSTEMS ARE ASSOCIATED WITH FEAR, ANXIETY, AND EMOTION DYSREGULATION AMONG CHILDREN EXPOSED TO IN UTERO MATERNAL STRESS. THE SIP STUDY COHORT HAS MANY STRENGTHS: ITS CONSISTENT LONG-TERM FOLLOW-UP, FROM IN UTERO, ITS DIVERSITY (FINANCIALLY AND RACIALLY), ITS EXTENSIVE REPOSITORY OF STORED BIOSPECIMENS (PLACENTA, TOENAIL, HAIR SAMPLES), AND ITS QUASI-EXPERIMENTAL DESIGN. THE COHORT IS NOW ENTERING PRE/PUBERTY (AGES 9-13), A TIME FOR RAPID SOCIAL AND BIOLOGICAL TRANSITION AND A PEAK TIME FOR THE EMERGENCE OF MALADAPTIVE, RISK-TAKING BEHAVIORS WITH THE BRAIN STILL DEVELOPING COGNITIVE FUNCTIONS. USING THIS UNPARALLELED OPPORTUNITY, WE WILL FOLLOW THE COHORT TO 1) EXAMINE THE ROLE OF EXPOSURE TO SS IN UTERO ALONG WITH THE EFFECT OF POSTNATAL STRESS (NORMATIVE STRESS, PARENTING) ON NEURODEVELOPMENTAL FUNCTIONING, AS MEASURED BY MULTIPLE BEHAVIORAL, PHYSIOLOGICAL, AND NEUROPSYCHOLOGICAL ASSESSMENTS; 2) DETERMINE THE RELATIONSHIP BETWEEN THE PRENATAL EPIGENOME SIGNATURE IN THE PLACENTA (ALONG WITH AN A PRIORI FOCUS ON THE HPA-AXIS AND IMMUNE NETWORK GENES) ON THE SUBSEQUENT RISK FOR NEGATIVE BEHAVIORAL CHARACTERISTICS (I.E., EXCESSIVE ANXIETY AND DISRUPTIVE BEHAVIORS) AND IMPAIRED EXECUTIVE FUNCTIONING, KNOWN TO ELEVATE THE RISK FOR EARLY INITIATION OF SUBSTANCE USE. FOLLOWING OUR PRIOR WORK, WE WILL ALSO EXAMINE SEX-SPECIFIC MANIFESTATIONS OF THE BEHAVIORAL PHENOTYPES (MORE INTERNALIZING PROBLEMS IN GIRLS AND EXTERNALIZING IN BOYS). WE WILL FURTHER EXPLORE THE MODERATING ROLE OF SOCIOECONOMIC STATUS WHEN INTERSECTING WITH PRENATAL SS-STRESS AND POSTNATAL STRESS ON INDIVIDUAL DIFFERENCES IN COGNITIVE NEURO-BEHAVIORS, WHICH MAKE PRE/PUBESCENT CHILDREN MORE VULNERABLE FOR A SUBSEQUENT ONSET OF BEHAVIORAL AND ADDICTIVE DISORDERS. WE WILL BUILD ON OUR UNIQUE REPOSITORY OF STORED BIOSPECIMENS AND PROFILE CHROMATIN ACCESSIBILITY ACROSS THE EPIGENOME AND RNA EXPRESSION NETWORK IN THE PLACENTA. GIVEN THE STEEPLY INCREASING INCIDENCE OF ADVERSE STRESSORS, ESPECIALLY NATURAL DISASTERS (E.G., HURRICANES, WILDFIRES AND PANDEMICS), INVESTIGATION OF THE SHORT- AND LONG-TERM IMPACT OF SS-STRESS ON NEURODEVELOPMENT FROM IN UTERO THROUGH PRE/PUBERTY, WILL HELP US TO IDENTIFY PROBLEMS AND POTENTIALLY LEAD TO SIGNIFICANT INTERVENTIONS FOR MOTHER AND CHILD.

PURPOSE: THE FUNDING WILL BE USED TO UPGRADE OUTDATED LAB EQUIPMENT IN TEACHING LABS THAT SERVE SIX ACADEMIC DEPARTMENTS: BIOLOGY, CHEMISTRY, COMPUTER SCIENCE, ENGINEERING AND ENVIRONMENTAL SCIENCE, MATHEMATICS, AND PHYSICS AND ASTRONOMY. THESE INVESTMENTS WILL MAKE IT POSSIBLE FOR CUNY COLLEGE OF STATEN ISLAND INSTRUCTORS TO CREATE IMMERSIVE LEARNING OPPORTUNITIES FOR GENERAL-EDUCATION COURSES AS WELL AS MAJOR COURSES IN STEM SUBJECTS. ACTIVITIES TO BE PERFORMED: THIS PROJECT WILL PROCURE EQUIPMENT, SOFTWARE, AND SUPPLIES, TO SIGNIFICANTLY IMPROVE THE INFRASTRUCTURE IN SEVEN DEPARTMENTAL TEACHING LABS. SIX DEPARTMENTS ARE HOUSED IN THE DIVISION OF SCIENCE AND TECHNOLOGY AND ONE DEPARTMENT IS PART OF THE SCHOOL OF HEALTH SCIENCES. THE PROJECT IS PLANNED TO BE COMPLETED IN 12 MONTHS. THE LEAD PI AND HIS PROJECT TEAM WILL WORK CLOSELY WITH BENEFITING DIVISIONS AND DEPARTMENTS TO ENSURE THAT PURCHASING AND PLACEMENT OF EQUIPMENT IS DONE WITHOUT CAUSING DISRUPTION TO THE ON-GOING TEACHING PROGRAMS THAT OCCUR IN THESE SPACES. ADDITIONALLY, AN EXTERNAL EVALUATOR WILL CONDUCT PRE-AND POST-ASSESSMENTS THAT ARE BOTH FORMATIVE AND SUMMATIVE TO ASSIST THE COLLEGE IN MEETING ITS STUDENT SUCCESS AND RETENTION METRICS.EXPECTED OUTCOMES: OVERALL, THE GOAL OF THESE STRATEGIC INVESTMENTS IS TO PREPARE STUDENTS TO BE CAREER READY GRADUATES BY TRAINING STUDENTS USING EQUIPMENT THAT ADHERES TO CURRENT INDUSTRY STANDARDS AND INTRODUCES THEM TO EMERGING IDEAS AND DEVELOPMENTS IN THESE FIELDS OF STUDY. SS RANDOMLY “ASSIGNED” STRESSFUL CONDITIONS TO PREGNANT WOMEN AND THEIR OFFSPRING IN OUR COHORT, GIVING US A UNIQUE OPPORTUNITY TO CONDUCT A QUASI-EXPERIMENT OF IN UTERO STRESS. USING OUR DEEPLY PHENOTYPED COHORT, WE PROPOSE TO INVESTIGATE WHETHER IN UTERO SS EXPOSURE MODULATES THE IMPACT OF THE POSTNATAL STRESSFUL ENVIRONMENT, EITHER AMPLIFYING (DOUBLE-HIT ACCELERATION MODEL) OR SHIELDING (STRESS INOCULATION MODEL) THE EFFECTS OF POSTNATAL STRESS ON ANX AND DB SYMPTOMATOLOGY. WE WILL CONTINUE TO MEASURE DIFFERENT TYPES OF POSTNATAL STRESS (NORMATIVE, POOR PARENTING, AND COVID-RELATED) TO EXAMINE IF MAGNITUDE, NATURE, TIMING AND DURATION OF THE POSTNATAL STRESSORS INFLUENCES THE DIRECTION AND IMPACT ON SUCH SYMPTOMATOLOGY. AS WELL AS THE DSM-BASED DIAGNOSTIC OUTCOMES, WE WILL UTILIZE THE NIMH RDOC AND FOCUS ON NEGATIVE VALENCE AND COGNITIVE CONTROL SYSTEMS (NVS/CCS), WHICH WILL ENABLE A TRANSDIAGNOSTIC PERSPECTIVE OF PRE- AND POSTNATAL STRESS-RELATED CHANGES IN BEHAVIOR AND THE BRAIN. FOLLOWING OUR PRIOR WORK, WE WILL ALSO EXAMINE SEX-SPECIFIC MANIFESTATIONS OF THE BEHAVIORAL PHENOTYPES (MORE INTERNALIZING PROBLEMS IN GIRLS AND MORE EXTERNALIZING IN BOYS) AND EXPLORE THE MODERATING ROLE OF SES WHEN INTERSECTING WITH INDIVIDUAL DIFFERENCES IN BEHAVIORS. AS THE BRAIN IS THE CENTRAL MODULATORY ORGAN OF STRESS/ADAPTATION AND REGULATION, WE WILL USE NEUROIMAGING TO ASSESS CHANGES IN BRAIN STRUCTURE, FUNCTION, AND CONNECTIVITY IN LIMBIC-FRONTAL CIRCUITRY DURING EARLY PRE- AND POST-PUBERTY, USING A SUBSAMPLE OF OUR COHORT. OUR COHORT WAS FOLLOWED FROM IN UTERO, WITH COMPLETED FOLLOW-UP ASSESSMENTS (NEUROBEHAVIORAL, CLINICAL, AND BIOLOGICAL SPECIMENS), WHICH WILL CONTINUE AS THEY ENTER PRE/PUBERTY (AGES 9-13), A TIME OF PEAK RISKS FOR SUCH PROBLEMS AND SEX-DIMORPHISM. SPECIFICALLY, WE WILL: 1) EXAMINE THE EFFECTS OF SS-RELATED STRESS AND THE POSTNATAL PSYCHOSOCIAL ENVIRONMENT ON TRAJECTORIES OF CLINICAL AND ADAPTIVE NEUROBEHAVIORS; 2) STUDY THE IMPACT OF PRENATAL SS STRESS AND POSTNATAL STRESS ON NVS/CCS BRAIN OUTCOMES (STRUCTURE AND FUNCTION) AND EVALUATE THE EXTENT TO WHICH NVS/CCS STRUCTURAL CHANGES MEDIATE BETWEEN PRENATAL SS-EXPOSURE AND CHILD ANXIETY AND DB SYMPTOMS; AND 3) DEVELOP PREDICTIVE MODELS FOR NVS/CCS OUTCOMES IN EARLY ADOLESCENCE BASED ON BIOLOGICAL AND BEHAVIORAL MEASURES OBTAINED IN UTERO AND/OR THE FIRST DECADE OF THE CHILD’S LIFE. AS PRE/PUBERTY IS AN IMPORTANT PERIOD OF DEVELOPMENTAL TRANSITION AND HEIGHTENED RISK FOR ANX AND DB, WE WILL BUILD ON OUR UNIQUE REPOSITORY OF BOTH CLINICAL AND STORED HAIR SAMPLES AND CONTINUE TO CHART CHILDREN’S NEUROBEHAVIORAL DEVELOPMENT AND RISK FOR PSYCHOPATHOLOGY THROUGH AGES 9-13. TOGETHER WITH THE INCLUSION OF NEUROIMAGING TO INVESTIGATE NEURAL MECHANISMS CENTERED ON THE NVS/CCS AND USING TASKS FROM THE ABCD STUDY, OUR STUDY IS UNIQUELY POSITIONED TO ELUCIDATE THE MECHANISMS OF FETAL PROGRAMING AND THREAT SENSITIVITY IN THE NVS/SSV THAT MODULATE THE RISKS FOR ANXIETY AND REACTIVE AGGRESSION.
