SEEDCO’S CONNECTING WEST TENNESSEE KIDS TO COVERAGE (CWTKC) IS A $1,494,525 THREE-YEAR PROGRAM TO INCREASE ENROLLMENT AND RETENTION OF TENNESSEE CHILDREN IN MEDICAID AND CHIP. SEEDCO WILL EXPAND ITS TARGET SERVICE AREA FROM THE 2019 FUNDING CYCLE TO INCLUDE TWO ADDITIONAL RURAL COUNTIES OF HAYWOOD AND FAYETTE COUNTIES IN TENNESSEE. THESE TWO RURAL COUNTIES WILL BE SERVED IN ADDITION TO SHELBY COUNTY (THE LARGEST COUNTY IN WEST TENNESSEE) AND TIPTON COUNTIES. THESE CONTIGUOUS REGIONS WITH AN ESTIMATED 40,000 UNINSURED CHILDREN, WILL RECEIVE OUTREACH AND ENROLLMENT ASSISTANCE VIA THE PROPOSED PROGRAM. THE PROPOSED SERVICE REGION SPANS NEARLY 1,800 SQUARE MILES AND IS HOME TO MORE THAN 40,000 UNINSURED CHILDREN; THIS IS MORE THAN TWICE THE LEVEL OF UNINSURED CHILDREN IN 2019. SEEDCO WILL BE THE LEAD AGENCY OF THE COLLABORATIVE, CONTINUING WITH EXISTING CWTKC PARTNERS AND ADDING NEW PARTNERS BASED ON THE FIRST THREE YEARS OF COLLABORATION AND ENROLLMENT WORK. CWTKC WILL TARGET SUBGROUPS OF TN CHILDREN THAT EXPERIENCE LOWER-THAN-AVERAGE HEALTH COVERAGE RATES, WITH A FOCUS ON A) CHILDREN OF YOUNG PARENTS AGES 18-24, B) LATINO CHILDREN, C) CHILDREN LIVING IN RURAL AREAS, AND D) PREGNANT INDIVIDUALS. TENNCARE, THE STATE AGENCY RESPONSIBLE FOR CHIP AND MEDICAID, HAS COMMITTED TO CONTINUING THE EXISTING DATA SHARING AGREEMENT WITH SEEDCO TO VERIFY ENROLLMENT AND RETENTION DATA. THE PROPOSED PROGRAM WILL BE IMPLEMENTED PRIMARILY BY AN ENROLLMENT ASSISTER TEAM AND A FULL-TIME PROJECT DIRECTOR, WITH SUPERVISION FROM THE DIRECTOR OF SEEDCO’S MID-SOUTH REGION AND ADDITIONAL OVERSIGHT FROM THE SEEDCO MANAGEMENT TEAM. TENNCARE, THE STATE AGENCY RESPONSIBLE FOR THE MEDICAID AND CHIP PROGRAMS, CURRENTLY HAS IN PLACE, A DATA SHARING AGREEMENT TO VERIFY SEEDCO’S ENROLLMENT AND RETENTION DATA ON THE INDIVIDUALS ASSISTED UNDER THIS INITIATIVE. TENNCARE HAS INDICATED A CONTINUED/UPDATED DATA USER AGREEMENT FOR ONGOING ACTIVITIES. SEEDCO WILL CONTINUE TO PARTNER WITH THE FOLLOWING ORGANIZATIONS AND AGENCIES TO IDENTIFY AND REACH THE ELIGIBLE UNINSURED CHILDREN IN ITS TARGET AREA: MEMPHIS-SHELBY COUNTY SCHOOLS (THE LARGEST SCHOOL DISTRICT IN WEST TENNESSEE); TIPTON COUNTY HEALTH DEPT.; CHILDREN AND FAMILY SERVICES; THE CITY OF BROWNSVILLE, TN (HAYWOOD COUNTY); THE CITY OF COVINGTON, TN (TIPTON COUNTY) AND THE JACKSON/MADISON REGIONAL HEALTH DEPARTMENT. | $1.5M | FY2022 | Jul 2022 – Aug 2025 |

PROJECT ACTIVITIES INCLUDE A CONTINUUM OF PRE- AND POST-RELEASE SERVICES TO FACILITATE SEAMLESS TRANSITION BACK TO THE COMMUNITY: CRIMINOGENIC RISK AND NEEDS ASSESSMENTS, INDIVIDUAL SERVICE PLANNING, INTENSIVE CASE MANAGEMENT, JOB TRAINING, CAREER PLANNING, JOB PLACEMENT, RETENTION, AND POST-EMPLOYMENT SUPPORT. EXPECTED OUTCOMES INCLUDE: 190 PARTICIPANTS COMPLETE ASSESSMENT PROCESS (ENROLLED), INDIVIDUAL SERVICE PLAN, AND PRE-RELEASE JOB TRAINING; 95 (50%) ENROLL IN A TRAINING OR CERTIFICATION PROGRAM; 76 (80%) PLACED IN EMPLOYMENT AND RECEIVE EMPLOYMENT SUPPORT; AND 57 (75%) RETAIN EMPLOYMENT FOR AT LEAST 6 MONTHS; AND LESS THAN 5% RECIDIVATE WITHIN 12 MONTHS POST-RELEASE. ADULT MEN AND WOMEN AGES 25 OR OLDER ARE THE INTENDED BENEFICIARIES OF THE PROJECT. SEEDCOS IMPACT PROGRAM WILL PROVIDE SERVICES UNDER CATEGORY 2: IMPROVING EMPLOYMENT SERVICES AND CONNECTIONS. GIVEN PROPOSED IMPLEMENTATION IN THE MEMPHIS METROPOLITAN REGION, ALL PARTICIPANTS SERVED WILL REPRESENT HISTORICALLY UNDERSERVED AND MARGINALIZED POPULATIONS. CAREER PLANNING WILL BEGIN IMMEDIATELY FOLLOWING COMPLETION OF THE ORAS ASSESSMENT AND WILL BE FURTHER REFINED DURING SEEDCO UNIVERSITY, AN INTERNALLY DEVELOPED JOB READINESS AND SOFT SKILLS TRAINING CONDUCTED PRE-RELEASE, AND DURING JOB PLACEMENT AND ON-GOING DEVELOPMENT SUPPORT POST-RELEASE. IMPACT WILL BUILD SEEDCOS STRONG RELATIONSHIPS WITH CORRECTIONAL FACILITIES AND COMMUNITY-BASED PARTNERS AND THE MID-SOUTH CAREER PATHWAYS COLLABORATIVE (CPC), A LOCAL EMPLOYER NETWORK WITH 100+ EMPLOYER PARTNERS AND COMMUNITY- AND FAITH-BASED ORGANIZATION PARTNERS, THAT IS OPEN TO HIRING INDIVIDUALS WITH JUSTICE INVOLVEMENT. SEEDCO UTILIZED LABOR MARKET INFORMATION AND OBTAINED DIRECT INPUT FROM EMPLOYERS AND TRAINING PROVIDERS IN THE MEMPHIS REGION AND THE CPC TO IDENTIFY TRAINING PROGRAMS AND JOB CATEGORIES IN HIGH-GROWTH, HIGH-DEMAND SECTORS. THE ANALYSIS INCLUDES CURRENT AVAILABILITY WITHIN THE FOLLOWING INDUSTRIES: TRANSPORTATION/ WAREHOUSE/ LOGISTICS, FOOD INDUSTRY, CONSTRUCTION, RETAIL, HOME HEALTH, AND MANUFACTURING. TENNCARE, THE STATE AGENCY RESPONSIBLE FOR CHIP AND MEDICAID, HAS COMMITTED TO CONTINUING THE EXISTING DATA SHARING AGREEMENT WITH SEEDCO TO VERIFY ENROLLMENT AND RETENTION DATA. THE PROPOSED PROGRAM WILL BE IMPLEMENTED PRIMARILY BY AN ENROLLMENT ASSISTER TEAM AND A FULL-TIME PROJECT DIRECTOR, WITH SUPERVISION FROM THE DIRECTOR OF SEEDCO’S MID-SOUTH REGION AND ADDITIONAL OVERSIGHT FROM THE SEEDCO MANAGEMENT TEAM. TENNCARE, THE STATE AGENCY RESPONSIBLE FOR THE MEDICAID AND CHIP PROGRAMS, CURRENTLY HAS IN PLACE, A DATA SHARING AGREEMENT TO VERIFY SEEDCO’S ENROLLMENT AND RETENTION DATA ON THE INDIVIDUALS ASSISTED UNDER THIS INITIATIVE. TENNCARE HAS INDICATED A CONTINUED/UPDATED DATA USER AGREEMENT FOR ONGOING ACTIVITIES. SEEDCO WILL CONTINUE TO PARTNER WITH THE FOLLOWING ORGANIZATIONS AND AGENCIES TO IDENTIFY AND REACH THE ELIGIBLE UNINSURED CHILDREN IN ITS TARGET AREA: MEMPHIS-SHELBY COUNTY SCHOOLS (THE LARGEST SCHOOL DISTRICT IN WEST TENNESSEE); TIPTON COUNTY HEALTH DEPT.; CHILDREN AND FAMILY SERVICES; THE CITY OF BROWNSVILLE, TN (HAYWOOD COUNTY); THE CITY OF COVINGTON, TN (TIPTON COUNTY) AND THE JACKSON/MADISON REGIONAL HEALTH DEPARTMENT.

STRUCTURED EMPLOYMENT ECONOMIC DEVELOPMENT CORPORATION (SEEDCO) WILL SERVE 325 FATHERS ANNUALLY IN THE GREATER MEMPHIS METROPOLITAN REGION (SHELBY COUNTY AND TIPTON COUNTY, TENNESSEE, AND CRITTENDEN COUNTY, ARKANSAS) THROUGH A COMPREHENSIVE PROGRAM COMBINING PARENTING AND HEALTHY RELATIONSHIP EDUCATION AS WELL AS ECONOMIC STABILITY SERVICES. THE PROGRAM WILL IMPLEMENT 24/7 DAD FOR PARENTING SKILLS, THE BLUEPRINT FOR HEALTHY RELATIONSHIPS, AND SEEDCO U FOR EMPLOYMENT READINESS WITH CASE MANAGEMENT AND JOB PLACEMENT ASSISTANCE. SEEDCO WILL ENGAGE FATHERS BY PROVIDING TOOLS TO STRENGTHEN FAMILY RELATIONSHIPS, IMPROVE PARENTING SKILLS, AND ACHIEVE ECONOMIC SELF-SUFFICIENCY. THE STRUCTURED ECONOMIC EMPLOYMENT DEVELOPMENT CORPORATION (SEEDCO) PROPOSES TO IMPLEMENT THE INTERSECTING MEANINGFUL PLACEMENT AND CAREER TRAINING (IMPACT) INITIATIVE, ADAPTING 18 YEARS OF EXPERIENCE OPERATING REENTRY PROGRAMS IN SHELBY COUNTY, TN AND OTHER SERVICES IN THE MEMPHIS REGION. THE PURPOSE IS TO INCREASE ITS ABILITY TO MEET THE INTENSE DEMAND FOR ITS REENTRY EMPLOYMENT-BASED SERVICES FOR INDIVIDUALS WITH DIFFERING LEVELS OF JUSTICE INVOLVEMENT IN SHELBY COUNTRY, AND TO EXPAND ACCESS TO SUCH SERVICES TO INDIVIDUALS IN THE UNDERSERVED CONTIGUOUS COUNTIES OF FAYETTE, TIPTON, AND HAYWOOD (GREATER MEMPHIS METROPOLITAN AREA). PROJECT ACTIVITIES INCLUDE A CONTINUUM OF PRE- AND POST-RELEASE SERVICES TO FACILITATE SEAMLESS TRANSITION BACK TO THE COMMUNITY: CRIMINOGENIC RISK AND NEEDS ASSESSMENTS, INDIVIDUAL SERVICE PLANNING, INTENSIVE CASE MANAGEMENT, JOB TRAINING, CAREER PLANNING, JOB PLACEMENT, RETENTION, AND POST-EMPLOYMENT SUPPORT. EXPECTED OUTCOMES INCLUDE: 190 PARTICIPANTS COMPLETE ASSESSMENT PROCESS (ENROLLED), INDIVIDUAL SERVICE PLAN, AND PRE-RELEASE JOB TRAINING; 95 (50%) ENROLL IN A TRAINING OR CERTIFICATION PROGRAM; 76 (80%) PLACED IN EMPLOYMENT AND RECEIVE EMPLOYMENT SUPPORT; AND 57 (75%) RETAIN EMPLOYMENT FOR AT LEAST 6 MONTHS; AND LESS THAN 5% RECIDIVATE WITHIN 12 MONTHS POST-RELEASE. ADULT MEN AND WOMEN AGES 25 OR OLDER ARE THE INTENDED BENEFICIARIES OF THE PROJECT. SEEDCOS IMPACT PROGRAM WILL PROVIDE SERVICES UNDER CATEGORY 2: IMPROVING EMPLOYMENT SERVICES AND CONNECTIONS. GIVEN PROPOSED IMPLEMENTATION IN THE MEMPHIS METROPOLITAN REGION, ALL PARTICIPANTS SERVED WILL REPRESENT HISTORICALLY UNDERSERVED AND MARGINALIZED POPULATIONS. CAREER PLANNING WILL BEGIN IMMEDIATELY FOLLOWING COMPLETION OF THE ORAS ASSESSMENT AND WILL BE FURTHER REFINED DURING SEEDCO UNIVERSITY, AN INTERNALLY DEVELOPED JOB READINESS AND SOFT SKILLS TRAINING CONDUCTED PRE-RELEASE, AND DURING JOB PLACEMENT AND ON-GOING DEVELOPMENT SUPPORT POST-RELEASE. IMPACT WILL BUILD SEEDCOS STRONG RELATIONSHIPS WITH CORRECTIONAL FACILITIES AND COMMUNITY-BASED PARTNERS AND THE MID-SOUTH CAREER PATHWAYS COLLABORATIVE (CPC), A LOCAL EMPLOYER NETWORK WITH 100+ EMPLOYER PARTNERS AND COMMUNITY- AND FAITH-BASED ORGANIZATION PARTNERS, THAT IS OPEN TO HIRING INDIVIDUALS WITH JUSTICE INVOLVEMENT. SEEDCO UTILIZED LABOR MARKET INFORMATION AND OBTAINED DIRECT INPUT FROM EMPLOYERS AND TRAINING PROVIDERS IN THE MEMPHIS REGION AND THE CPC TO IDENTIFY TRAINING PROGRAMS AND JOB CATEGORIES IN HIGH-GROWTH, HIGH-DEMAND SECTORS. THE ANALYSIS INCLUDES CURRENT AVAILABILITY WITHIN THE FOLLOWING INDUSTRIES: TRANSPORTATION/ WAREHOUSE/ LOGISTICS, FOOD INDUSTRY, CONSTRUCTION, RETAIL, HOME HEALTH, AND MANUFACTURING. SEEDCO AND THE CPC WILL ENGAGE WITH PARTICIPANTS EMPLOYERS TO ENSURE PROPER PLACEMENTS, WORKFORCE DEVELOPMENT SUPPORTS, AND EMPLOYMENT MENTORING SUPPORTS, AS NEEDED, SERVE PARTICIPANTS ECONOMIC MOBILITY.
