PROJECT SERVICES WILL INCLUDE CUSTOMIZED EDUCATION AND INFORMATION FOR UNDERSERVED COMMUNITIES, INCLUDING RURAL, MINORITY/ETHNIC, AND TRIBAL POPULATIONS. DURING THE THREE-YEAR PROJECT, 7,000 HEALTHCARE AND COUNSELING PROFESSIONALS WILL RECEIVE AUD TRAINING. STRATEGIES/INTERVENTIONS: THROUGH THE PROVISION OF FREE AND ACCESSIBLE EDUCATION, TRAINING, GUIDANCE, AND MENTORING ASAM'S PCSS-MAUD PROJECT WILL INCREASE THE NUMBER OF CURRENT AND FUTURE HEALTHCARE AND COUNSELING PROFESSIONALS THAT EFFECTIVELY IDENTIFY AND TREAT INDIVIDUALS WITH AUD. PROJECT IMPLEMENTATION WILL BE LED BY ASAM, WHICH IS A NATIONAL LEADER IN ADDICTION EDUCATION, IN COLLABORATION WITH A COMMITTED CONSORTIUM OF PARTNERS REPRESENTING INTERDISCIPLINARY HEALTH PROFESSIONS. TO ENSURE EFFECTIVE IMPLEMENTATION AND ONGOING MONITORING OF ASAM'S PCSS-MAUD PROJECT, A STEERING COMMITTEE OF LEADERS IN THE FIELD OF ADDICTION MEDICINE AND VARIOUS SPECIALTY AND HEALTH PROFESSIONS WILL BE ESTABLISHED IN THE FIRST QUARTER OF YEAR 1. THE COMMITTEE WILL BE RESPONSIBLE FOR OVERSEEING THE DIRECTION OF THE PROJECT, MONITORING PROGRESS, AND ENHANCING THE PROJECT AS NEEDED DURING THE THREE-YEAR GRANT PERIOD. PROJECT COMPONENTS INCLUDE THE PROVISION OF: 1) DISCIPLINE-SPECIFIC EDUCATION AND TRAINING ON SCREENING, ASSESSMENT AND TREATMENT OF AUD; 2) SKILL-BASED TRAINING AND RESOURCES ON TREATMENT AND MANAGEMENT OF AUD; AND 3) TECHNICAL SUPPORT AND CONSULTATION SERVICES ON TREATMENT-RELATED TOPICS AND OVERCOMING BARRIERS TO IMPLEMENTING MAUD IN PRACTICE SETTINGS. CONTINUING MEDICAL EDUCATION AND CONTINUING EDUCATION UNITS WILL BE PROVIDED FOR TRAINING OFFERED. PROJECT GOALS AND MEASURABLE OBJECTIVES: ASAM PCSS-MAUD GOAL: INCREASE EDUCATION AND TRAINING ON MAUD FOR MULTIDISCIPLINARY HEALTHCARE PROFESSIONALS AND COUNSELORS TO IMPROVE ACCESS TO AND QUALITY OF PREVENTION, TREATMENT, AND RECOVERY SERVICES AND SUPPORT FOR INDIVIDUALS WITH AUD. OBJECTIVE 1: DEVELOP AND DELIVER A DISCIPLINE-SPECIFIC TRAINING PROGRAM THAT INCLUDES MULTIDISCIPLINARY, INTERPROFESSIONAL TRAINING, GUIDANCE, MENTORING, AND ONGOING TECHNICAL ASSISTANCE TO EDUCATE THE CLINICAL WORKFORCE AND ENHANCE THE CAPACITY OF HEALTHCARE AND COUNSELING PROFESSIONALS TO SCREEN, DIAGNOSE, AND TREAT AUD TO 7,000 INDIVIDUALS BY 9/29/2026. NATIONAL PRACTICE GUIDELINES FOR SAFE TAPERING OF BENZODIAZEPINES - ABSTRACT BENZODIAZEPINE MEDICATIONS ARE COMMONLY PRESCRIBED TO TREAT A WIDE RANGE OF CONDITIONS INCLUDING ANXIETY AND MOOD DISORDERS, INSOMNIA, AND SEIZURE DISORDERS. THESE MEDICATIONS REPRESENT IMPORTANT THERAPEUTIC TOOLS; HOWEVER, THEY ARE ASSOCIATED WITH SIGNIFICANT ADVERSE EVENTS INCLUDING POTENTIALLY LIFE- THREATENING WITHDRAWAL, SUBSTANCE USE DISORDER, AND OVERDOSE (PARTICULARLY WHEN COMBINED WITH ALCOHOL, OPIOIDS, OR OTHER CENTRAL NERVOUS SYSTEM DEPRESSANTS). OVER THE PAST TWO DECADES, FATAL OVERDOSES INVOLVING BENZODIAZEPINES HAVE INCREASED MORE THAN 10-FOLD, OFTEN INVOLVING THE COMBINATION OF OPIOIDS AND BENZODIAZEPINES.1 BENZODIAZEPINE PRESCRIBING RATES ARE HIGHEST2 FOR OLDER ADULTS, AMONG WHOM THESE MEDICATIONS POSE ADDITIONAL RISK FOR FALLS, HIP FRACTURES, AND COGNITIVE IMPAIRMENT.3 WHILE THERE IS LITTLE RESEARCH ON THE EFFICACY OF BENZODIAZEPINE USE FOR LONGER THAN THREE MONTHS AND THE RISK FOR ADVERSE EVENTS INCREASES WITH DURATION OF USE, LONG-TERM USE IS COMMON.3 DESPITE THE PREVALENCE OF PRESCRIBING AND THE SIGNIFICANCE OF THE RISKS, THERE ARE CURRENTLY NO NATIONAL CLINICAL GUIDELINES TO INFORM CLINICIANS IN DETERMINING WHEN AND HOW TO TAPER BENZODIAZEPINE MEDICATIONS. SAFE TAPERING OF BENZODIAZEPINES CAN BE CLINICALLY COMPLEX SINCE RAPID DOSAGE REDUCTIONS MAY PRECIPITATE ACUTE WITHDRAWAL, WHICH CAN BE LIFE- THREATENING. PATIENTS ARE ALSO AT RISK FOR RECURRENCE AND EXACERBATION OF THE SYMPTOMS FOR WHICH THE BENZODIAZEPINE WAS PRESCRIBED (E.G., ANXIETY, SEIZURES, INSOMNIA). IN ADDITION, INADEQUATE TAPERING STRATEGIES MAY PUSH PATIENTS TO THE ILLICIT DRUG MARKET WHERE COUNTERFEIT PILLS LACED WITH FENTANYL AND OTHER OPIOIDS ARE COMMON, PRESENTING A RISK FOR OVERDOSE AND OVERDOSE DEATH.4 TO SUPPORT THE FDA’S GOAL OF “SAFELY REDUCING INAPPROPRIATE PRESCRIBING OF MEDICATIONS WITH POTENTIAL FOR MISUSE AND DEPENDENCE”, THE AMERICAN SOCIETY FOR ADDICTION MEDICINE (ASAM) WILL DEVELOP A CLINICAL PRACTICE GUIDELINE (CPG) TO GUIDE CLINICIANS IN DETERMINING WHEN AND HOW TO TAPER BENZODIAZEPINE MEDICATIONS. ASAM HAS SIGNIFICANT EXPERTISE DEVELOPING CPGS AND ESTABLISHED PARTNERSHIPS WITH OTHER MEDICAL SOCIETIES WITH EXPERTISE RELEVANT TO THIS TOPIC INCLUDING THE AMERICAN PSYCHIATRIC ASSOCIATION, THE AMERICAN ACADEMY OF NEUROLOGY, THE AMERICAN SOCIETY FOR GERIATRICS, AND THE AMERICAN COLLEGE OF GYNECOLOGY AND OBSTETRICS, AMONG OTHERS. ASAM WILL WORK WITH THESE PARTNERS TO CONVENE AN EXPERT PANEL WITH MULTIDISCIPLINARY MEDICAL EXPERTISE TO DEVELOP A CPG USING A MODIFIED GRADE METHODOLOGY. A SYSTEMATIC REVIEW OF THE EVIDENCE RELATED TO BENZODIAZEPINE TAPERING WILL BE CONDUCTED TO INFORM THE WORK OF THE EXPERT PANEL. PEOPLE WITH LIVED EXPERIENCE WILL BE ENGAGED THROUGHOUT THE PROCESS TO ENSURE THAT THE CPG ADDRESSES PATIENT CONCERNS AND NEEDS. ONCE THE GUIDELINE IS COMPLETE, ASAM WILL WORK WITH THE FDA AND OUR PARTNERS TO WIDELY DISSEMINATE THE GUIDELINES AS WELL AS TRAINING AND CLINICAL DECISION SUPPORT TOOLS TO FACILITATE IMPROVEMENTS IN BENZODIAZEPINE PRESCRIBING PRACTICES.

INTERNATIONAL SOCIETY OF ADDICTION MEDICINE ANNUAL CONFERENCE - PROJECT SUMMARY/ABSTRACT PROJECT ABSTRACT THE INTERNATIONAL SOCIETY FOR ADDICTION MEDICINE (ISAM) IS A PROFESSIONAL ORGANIZATION, WITH OVER 10,000 AFFILIATE MEMBERS, COMPRISED OF MEMBERS SITUATED IN CLINICAL AND ACADEMIC SETTINGS IN THE US AND OVER 40 OTHER COUNTRIES WORLDWIDE. SINCE 1999, THE ISAM ANNUAL CONFERENCE HAS BROUGHT TOGETHER A CROSS-SECTIONAL, MULTI-NATIONAL GROUP OF PROFESSIONALS WITH A COMMON INTEREST IN RESEARCH, CLINICAL PRACTICE AND TRAINING ASPECTS OF ADDICTION MEDICINE. THE LOCATIONS AND CONTENT OF THE ANNUAL MEETING HAVE PROMOTED THE DEVELOPMENT AND EXPANSION OF AN INTERNATIONAL NETWORK OF ADDICTION MEDICAL SPECIALISTS/SCIENTISTS. OF IMPORTANCE, THE MEETING IS ATTENDED BY BOTH RESEARCHERS AND CLINICAL PRACTITIONERS FROM THROUGHOUT THE WORLD, ALLOWING FOR A RICH EXCHANGE OF IDEAS AND BEST PRACTICES IN THE TREATMENT OF SUBSTANCE USE DISORDERS. NIDA SUPPORT HAS BEEN CRITICAL TO THE SUCCESS OF THE ISAM ANNUAL MEETING. SPEAKERS FROM NIDA HAVE ADDED TO THE SCIENTIFIC PROGRAM THROUGH PLANNING OF AND PARTICIPATING IN PLENARY LECTURES AND SYMPOSIA. IN ADDITION, THE NIDA GRANT SUPPLIES FUNDING FOR TRAVEL FELLOWSHIPS WHICH MAKE THE MEETING ACCESSIBLE FOR INDIVIDUALS WHO MIGHT OTHERWISE HAVE INSURMOUNTABLE FINANCIAL BARRIERS TO MEETING ATTENDANCE. THROUGH NIDA FUNDING, THOSE AWARDED TRAVEL FELLOWSHIPS ARE ABLE TO FORM RELATIONSHIPS WITH OTHER ADDICTION PROFESSIONALS WORLDWIDE THAT EXTEND BEYOND THE MEETING WITH CONTINUED MENTORSHIP AND EXCHANGE OF IDEAS VIA E-MAIL, TELE- AND VIDEO-CONFERENCING. IN THIS RENEWAL PERIOD, WE PROPOSE TO ENHANCE THE EXPERIENCE OF THE TRAVEL FELLOWS THROUGH INVOLVEMENT OF ISAM INTEREST GROUPS. WE WILL CONTINUE MENTORING AND NETWORKING EVENTS THAT SPECIFICALLY TARGET EARLY CAREER INVESTIGATORS AND CLINICIANS. THE ISAM ANNUAL MEETING OFFERS A SINGULAR FORUM FOR THE INTERNATIONAL EXCHANGE OF RESEARCH FINDINGS, CLINICAL EXPERIENCE AND SOCIETAL PERSPECTIVES. NIDA FUNDING IS VITAL IN SUPPORTING THE MEETING.
