WE PREPARE AND DELIVER NUTRITIOUS, HIGH-QUALITY MEALS TO PEOPLE WHO, BECAUSE OF THEIR ILLNESS, ARE UNABLE TO PROVIDE OR PREPARE MEALS FOR THEMSELVES. WE ALSO PROVIDE ILLNESS-SPECIFIC NUTRITION EDUCATION AND COUNSELING TO OUR CLIENTS, FAMILIES, CARE PROVIDERS AND OTHER SERVICE ORGANIZATIONS. LIKE ANY LARGE ENTERPRISE, GOD’S LOVE WE DELIVER DEPENDS ON TECHNOLOGY SYSTEMS TO HANDLE LARGE VOLUMES OF WORK. OUR EMERGING ROLE IN REGIONAL HEALTHCARE AND NUTRITION INITIATIVES REQUIRES INCREASINGLY COMPLEX DATA MANAGEMENT AND COMMUNICATIONS CAPABILITIES THAT MEET THE NEEDS OF OUR PARTNERS AND CLIENTS AND PERFORM AT THE LEVEL EXPECTED WHEN WORKING WITH CLIENTS, HEALTHCARE PROVIDERS, GOVERNMENT ENTITIES, MAJOR REGIONAL FUNDERS, AND OTHER COMMUNITY LEADERS. IN THIS ENVIRONMENT, OUR CURRENT SYSTEMS ARE OUTDATED. WE ARE UNABLE TO COLLECT ALL THE DATA THAT IS NEEDED FOR HEALTHCARE INTEGRATION, AND OUR SYSTEM IS NOT ADAPTABLE FOR EASY REPORTING OR RELATIONSHIP MANAGEMENT. FURTHER, OUR SYSTEM DOES NOT ALLOW CLIENTS OR REFERRING ORGANIZATIONS TO USE TECHNOLOGY TO SIGN-UP FOR THE PROGRAM, REFER CLIENTS TO THE PROGRAM, SEND DOCUMENTS AND PROTECTED HEALTH INFORMATION THROUGH TECHNOLOGY IN ANY WAY. THEREFORE, IT IS NECESSARY FOR GOD’S LOVE TO REPLACE ITS OUTDATED CUSTOM-MADE CUSTOMER RELATIONSHIP MANAGEMENT SYSTEM (CRM) WITH A NEWLY BUILT, STATE OF THE ART ELECTRONIC HEALTH RECORD SYSTEM (E HR) TO BETTER SERVE OUR CLIENTS, IMPROVE THE COLLECTION AND STANDARDIZATION OF CLIENT NUTRITION ASSESSMENT DATA, WORK MORE EFFECTIVELY WITH OUR PARTNERS, ADVANCE THE EVIDENCE BASE FOR MEDICALLY TAILORED MEALS, AND PROVIDE THE BEST AVAILABLE INFORMATION TO PUBLIC AND PRIVATE FUNDERS REGARDING OUR WORK. GOD’S LOVE WORKS WITH PEOPLE WHO LIVE AT THE INTERSECTION OF POVERTY AND ILLNESS IN THE NEW YORK METROPOLITAN REGION. MORE SPECIFICALLY: - 90% LIVE AT OR BELOW THE FEDERAL POVERTY LEVEL; - 97% OF OUR CLIENTS ARE NEW YORK CITY RESIDENTS: 23% LIVE IN THE BRONX, 24% IN BROOKLYN; 23% IN MANHATTAN; 23% IN QUEENS, AND 4% IN STATEN ISLAND. THE REMAINDER ARE IN SUBURBAN COUNTIES INCLUDING NASSAU, SUFFOLK AND WESTCHESTER IN NY AND HUDSON IN NJ; - WOMEN MAKE UP 57% OF OUR CLIENTS AND MEN ARE 43%; - 72% OF OUR CLIENTS ARE 60 AND OLDER. - OF THE CLIENTS WHO REPORT RACE AND ETHNICITY, 47% ARE BLACK/AFRICAN AMERICAN, 34% WHITE/CAUCASIAN, 6% ASIAN, .3% NATIVE AMERICAN OR ALASKAN NATIVE, AND 25% IDENTIFY AS LATINO ETHNICITY; - CLIENTS LIVE WITH ONE OR MORE SERIOUS ILLNESSES THAT INTERFERE WITH THEIR ABILITY TO SHOP OR COOK, INCLUDING HIV/AIDS (10%), CANCER (14%), CARDIOVASCULAR DISEASE (16%), CHRONIC KIDNEY DISEASE (12%), TYPE 2 DIABETES (10%), MUSCULOSKELETAL DISORDERS (10%), AND OTHER ILLNESSES. WITH A NEW EHR, GOD’S LOVE WILL BE IN THE POSITION TO: PROVIDE EXCELLENT SERVICE TO OUR CLIENTS; BETTER MANAGE ALL OUR EXTERNAL RELATIONSHIPS; AND ENABLE STAFF TO ACCESS THE INFORMATION THEY NEED TO HAVE GOD’S LOVE OPERATE OPTIMALLY. ADDITIONALLY, WE WILL EXPAND EVALUATION CAPACITY BY IMPROVING DATA COLLECTION FROM BOTH CLIENTS AND PARTNERS TO MEASURE THE IMPACT OF MEDICALLY TAILORED MEALS AND THEREBY CATALYZE OUR GOAL OF INTEGRATING MEDICALLY TAILORED MEALS INTO THE STANDARD OF CARE FOR PEOPLE LIVING WITH SEVERE AND CHRONIC ILLNESS.
