WE ALSO PROVIDE ILLNESS-SPECIFIC NUTRITION EDUCATION AND COUNSELING TO OUR CLIENTS, FAMILIES, CARE PROVIDERS AND OTHER SERVICE ORGANIZATIONS. LIKE ANY LARGE ENTERPRISE, GODโS LOVE WE DELIVER DEPENDS ON TECHNOLOGY SYSTEMS TO HANDLE LARGE VOLUMES OF WORK. IN THIS ENVIRONMENT, OUR CURRENT SYSTEMS ARE OUTDATED. WE ARE UNABLE TO COLLECT ALL THE DATA THAT IS NEEDED FOR HEALTHCARE INTEGRATION, AND OUR SYSTEM IS NOT ADAPTABLE FOR EASY REPORTING OR RELATIONSHIP MANAGEMENT. FURTHER, OUR SYSTEM DOES NOT ALLOW CLIENTS OR REFERRING ORGANIZATIONS TO USE TECHNOLOGY TO SIGN-UP FOR THE PROGRAM, REFER CLIENTS TO THE PROGRAM, SEND DOCUMENTS AND PROTECTED HEALTH INFORMATION THROUGH TECHNOLOGY IN ANY WAY. THEREFORE, IT IS NECESSARY FOR GODโS LOVE TO REPLACE ITS OUTDATED CUSTOM-MADE CUSTOMER RELATIONSHIP MANAGEMENT SYSTEM (CRM) WITH A NEWLY BUILT, STATE OF THE ART ELECTRONIC HEALTH RECORD SYSTEM (E HR) TO BETTER SERVE OUR CLIENTS, IMPROVE THE COLLECTION AND STANDARDIZATION OF CLIENT NUTRITION ASSESSMENT DATA, WORK MORE EFFECTIVELY WITH OUR PARTNERS, ADVANCE THE EVIDENCE BASE FOR MEDICALLY TAILORED MEALS, AND PROVIDE THE BEST AVAILABLE INFORMATION TO PUBLIC AND PRIVATE FUNDERS REGARDING OUR WORK. GODโS LOVE WORKS WITH PEOPLE WHO LIVE AT THE INTERSECTION OF POVERTY AND ILLNESS IN THE NEW YORK METROPOLITAN REGION. MORE SPECIFICALLY: - 90% LIVE AT OR BELOW THE FEDERAL POVERTY LEVEL; - 97% OF OUR CLIENTS ARE NEW YORK CITY RESIDENTS: 23% LIVE IN THE BRONX, 24% IN BROOKLYN; 23% IN MANHATTAN; 23% IN QUEENS, AND 4% IN STATEN ISLAND. THE REMAINDER ARE IN SUBURBAN COUNTIES INCLUDING NASSAU, SUFFOLK AND WESTCHESTER IN NY AND HUDSON IN NJ; - WOMEN MAKE UP 57% OF OUR CLIENTS AND MEN ARE 43%; - 72% OF OUR CLIENTS ARE 60 AND OLDER. - OF THE CLIENTS WHO REPORT RACE AND ETHNICITY, 47% ARE BLACK/AFRICAN AMERICAN, 34% WHITE/CAUCASIAN, 6% ASIAN, .3% NATIVE AMERICAN OR ALASKAN NATIVE, AND 25% IDENTIFY AS LATINO ETHNICITY; - CLIENTS LIVE WITH ONE OR MORE SERIOUS ILLNESSES THAT INTERFERE WITH THEIR ABILITY TO SHOP OR COOK, INCLUDING HIV/AIDS (10%), CANCER (14%), CARDIOVASCULAR DISEASE (16%), CHRONIC KIDNEY DISEASE (12%), TYPE 2 DIABETES (10%), MUSCULOSKELETAL DISORDERS (10%), AND OTHER ILLNESSES.
