D/B/A CALLEN-LORDE COMMUNITY HEALTH CENTER (CALLEN-LORDE) IS APPLYING FOR FY 2021 SCREENING, BRIEF INTERVENTION AND REFERRAL TO TREATMENT FUNDING FROM THE SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATION TO IMPLEMENT AN AGENCY-WIDE, MULTI-SITE SCREENING, BRIEF INTERVENTION, AND REFERRAL TO TREATMENT (SBIRT) PROGRAM. CALLEN-LORDE'S PROPOSED SBIRT PROGRAM WILL BE IMPLEMENTED IN BOTH ADULT AND PEDIATRIC OUTPATIENT HEALTH CARE SETTINGS. CALLEN-LORDE'S POPULATION OF FOCUS IS COMPRISED OF LGBTQ ADOLESCENT AND ADULT PERSONS, AGED 13 AND OLDER, LIVING WITHIN THE FIVE BOROUGHS OF NEW YORK CITY (BRONX, BROOKLYN, MANHATTAN, QUEENS AND STATEN ISLAND), WITH AN EMPHASIS ON REACHING THE MOST UNDERSERVED MEMBERS OF THIS POPULATION, INCLUDING THOSE WHO ARE PEOPLE OF COLOR, LOW INCOME, LIVING WITH HIV OR UNINSURED. CALLEN-LORDE WILL EMPLOY SBIRT FUNDING TO ADDRESS THE DEMONSTRATED RISK AND ELEVATED PREVALENCE OF SUBSTANCE MISUSE AND BARRIERS TO TREATMENT ACCESS AMONG NEW YORK CITY'S LGBTQ COMMUNITIES BY INTEGRATING DEDICATED SBIRT PROGRAMMING WITHIN THE AGENCY'S ROUTINE PRIMARY CARE OPERATIONS IN EFFORT TO SYSTEMATICALLY IDENTIFY AND TREAT PATIENTS AT RISK OF/OR LIVING WITH ADDICTION. CALLEN-LORDE'S PROPOSED SBIRT SERVICES WILL BE DELIVERED BY AN INTERDISCIPLINARY TEAM OF MEDICAL PROVIDERS, BEHAVIORAL HEALTH PROVIDERS, ADDICTIONS COORDINATORS, AND PEER RECOVERY SPECIALISTS, WHO WILL WORK TOGETHER TO ENSURE PATIENTS SEAMLESSLY TRANSITION THROUGH ALL STAGES OF THE PROPOSED SBIRT CARE PATHWAY. CALLEN-LORDE'S PROPOSED SBIRT PROJECT WILL FURTHER INCREASE SUBSTANCE USE DISORDER TREATMENT ACCESS AND ENGAGEMENT FOR PATIENTS LIVING WITH SEVERE ADDICTION THROUGH PROVISION OF ON-SITE MAT SERVICES, AND BY LEVERAGING COMMUNITY PARTNERSHIPS WITH SPECIALTY SUBSTANCE USE TREATMENT PROVIDERS WITH EXPERTISE IN SERVING THE POPULATION OF FOCUS. OVER THE FIVE-YEAR PROJECT PERIOD, CALLEN-LORDE EXPECTS TO SERVE AT LEAST 20,000 SBIRT CLIENTS ACROSS THE AGENCY'S CITY-WIDE NETWORK OF FEDERALLY QUALIFIED HEALTH CENTERS.

CALLEN-LORDE’S POPULATION OF FOCUS IS COMPRISED OF LOW-INCOME LESBIAN, GAY, BISEXUAL, AND TRANSGENDER, AND QUEER (LGBTQ) PERSONS AGES 13+ RESIDING AND SEEKING SERVICES WITHIN THE GEOGRAPHIC CATCHMENT OF THE BOROUGH OF BROOKLYN IN NEW YORK CITY, INCLUDING HOMELESS YOUTH AND ADULTS. THE TARGET POPULATION IS INCLUSIVE OF LGBTQ PERSONS OF ALL RACIAL AND ETHNIC BACKGROUNDS, WITH AN EMPHASIS ON REACHING BLACK, INDIGENOUS AND PERSONS OF COLOR, WHO ARE OVERREPRESENTED AMONG LOW-INCOME COMMUNITIES. CALLEN-LORDE PROPOSES TO IMPLEMENT THE CALLEN-LORDE TRANSCEND PROGRAM IN RESPONSE TO THE CENTERS FOR DISEASE CONTROL RFA PS22-2209, TRANSGENDER STATUS-NEUTRAL COMMUNITY-TO-CLINIC MODELS TO END THE HIV EPIDEMIC WITHIN 3 SITES LOCATED IN THE JURISDICTIONS OF NEW YORK, BRONX AND KINGS COUNTY IN NEW YORK CITY, WHICH ARE INCLUDED IN PHASE 1 OF THE ENDING THE HIV EPIDEMIC IN THE U.S. (EHE) INITIATIVE. THE PURPOSE OF CALLEN-LORDE’S PROPOSED TRANSCEND PROGRAM IS TO ADDRESS LONGSTANDING DISPARITIES IN HIV PREVENTION AND TREATMENT OUTCOMES FOR BLACK, MULTI-RACIAL AND HISPANIC TRANSGENDER WOMEN IN NEW YORK CITY. WITH THE TRANSCEND PROGRAM, CALLEN-LORDE WILL INTEGRATE BIOMEDICAL HIV PREVENTION AND TREATMENT INTERVENTIONS WITHIN GENDER-AFFIRMING MEDICAL CARE AND SUPPORTIVE SERVICES FOR A TARGET POPULATION OF TRANSGENDER AND GENDER DIVERSE (TGD) INDIVIDUALS, PARTICULARLY BLACK AND LATINX/HISPANIC TRANSGENDER WOMEN (TW). CALLEN-LORDE AIMS TO IMPROVE ACCESS TO HIV PREVENTION AND GENDER AFFIRMING CARE THROUGH TARGETED CASE-FINDING AND OUTREACH WITH COLLABORATING CBOS, RISK ASSESSMENT AND STRATIFICATION AND ENHANCED LINKAGE SERVICES PROVIDED BY DEDICATED CARE COORDINATION AND NAVIGATION STAFF. UNDER THIS INITIATIVE, CALLEN-LORDE WILL ALSO ENHANCE EXISTING PROGRAMMING BY PROVIDING ACCESS TO LONG-ACTING INJECTABLE (LAI) PREP, PROVIDING INTEGRATED INJECTION COORDINATION AT EACH OF OUR SITES ACROSS NEW YORK CITY. THE TRANSCEND INJECTION COORDINATORS WILL PROVIDE LAI PREP, ART AND OTHER INDICATED INJECTION NEEDS, INCLUDING COVID VACCINATION. FURTHERMORE, A VAST BODY OF RESEARCH DEMONSTRATES THAT ADDRESSING SDOH NEEDS SUCH AS HOUSING, DOMESTIC VIOLENCE, STIGMA, AND EDUCATION/EMPLOYMENT LEADS TO IMPROVED MEDICATION ADHERENCE, IMPROVED APPOINTMENT ADHERENCE, AND HIGHER RATES OF DURABLE VIRAL LOAD SUPPRESSION AMONG PWH. FUNDS WILL BE USED FOR STAFF TIME AND EFFORT TO BUILD THE SDOH SCREENING TOOL, FOSTER INTEGRATION OF SDOH SCREENING TOOL INTO THE TABLETS, THE EPIC EMR, AND OTHER DATA REPORTING AND ANALYTICS PLATFORMS LIKE CPCI, STAFF TRAINING ON HOW TO CONDUCT SCREENING AND PROVIDE RESOURCES/REFERRALS FOR IDENTIFIED NEEDS, AND THE STRENGTHENING OF EXISTING PARTNERSHIPS WITH COMMUNITY-BASED ORGANIZATIONS AND OTHER HEALTHCARE FACILITIES AS WELL AS THE DEVELOPMENT OF NEW PARTNERSHIPS THAT CAN ADDRESS SDOH NEEDS NOT SUPPORTED IN-HOUSE.
