(REFUAH) SEEKS $600,000 IN FISCAL YEAR 2024 BEHAVIORAL HEALTH SERVICE EXPANSION FUNDING TO INCREASE THE NUMBER OF PATIENTS RECEIVING MENTAL HEALTH SERVICES AND SUBSTANCE USE DISORDER (SUD) SERVICES, INCLUDING THOSE RECEIVING TREATMENT WITH MEDICATIONS FOR OPIOID USE DISORDER (MOUD). THIS FUNDING WILL ENABLE REFUAH TO EXPAND ACCESS TO COMPREHENSIVE MENTAL HEALTH SERVICES FOR LOW-INCOME AND MARGINALIZED COMMUNITIES IN ROCKLAND COUNTY AND RURAL SULLIVAN COUNTY IN NEW YORK’S LOWER HUDSON VALLEY. REFUAH HAS PROVIDED COMPREHENSIVE HEALTHCARE SERVICES TO LOW-INCOME NEW YORKERS SINCE 1992 AND BECAME A SECTION 330(E) HEALTH CENTER PROGRAM GRANTEE IN 2002. CURRENTLY, REFUAH OPERATES FOUR PERMANENT SITES IN ROCKLAND, TWO PERMANENT SITES IN SULLIVAN COUNTY, AND A FLEET OF SEVEN MOBILE MEDICAL UNITS. REFUAH SERVES OVER 80,000 PATIENTS ANNUALLY, OFFERING PRIMARY CARE, DENTAL, BEHAVIORAL HEALTH, AND A BROAD RANGE OF SPECIALTY MEDICAL SERVICES. REFUAH’S TARGET POPULATION INCLUDES LOW-INCOME RESIDENTS AND FAMILIES LIVING AT OR BELOW 200% OF THE FEDERAL POVERTY GUIDELINES (FPG) AND THE UN/UNDERINSURED IN ROCKLAND AND SULLIVAN COUNTIES. REFUAH SERVES A DIVERSE COMMUNITY INCLUDING HISPANIC AND HAITIAN IMMIGRANTS AND ULTRAORTHODOX JEWS. MANY OF THESE FAMILIES FACE HEALTH EQUITY CHALLENGES AND REQUIRE CULTURALLY SENSITIVE AND LINGUISTICALLY APPROPRIATE CARE. OVER 40% OF REFUAH’S PATIENTS ARE CHILDREN UNDER THE AGE OF 18. THE PROPOSED SERVICE AREA REPORTS DISMAL MENTAL HEALTH OUTCOMES AND LACK OF ACCESS. BOTH ROCKLAND AND SULLIVAN COUNTIES ARE DESIGNATED AS MEDICALLY UNDERSERVED AREAS (MUA) AND MENTAL HEALTH PROFESSIONAL SHORTAGE AREAS. LOCAL AND REGIONAL COMMUNITIES HEALTH NEEDS ASSESSMENTS IDENTITY MENTAL HEALTH ACCESS AND SUD SERVICES AS TOP HEALTH PRIORITIES. TO INCREASE PATIENTS RECEIVING MENTAL HEALTH AND SUD SERVICES, REFUAH’S PROJECT PROPOSES THE FOLLOWING OBJECTIVES: 1. INCREASE ACCESS TO BEHAVIORAL HEALTH PROVIDERS: - HIRE A FULL-TIME EQUIVALENT (FTE) PSYCHIATRIST AND ADDITIONAL SOCIAL WORKERS WITH EXPERTISE IN REFUAH'S DIVERSE POPULATIONS. - DESIGNATE A BEHAVIORAL HEALTH (BH) DIRECTOR AND MENTOR TO PROVIDE SUBJECT MATTER EXPERTISE AND ONGOING TRAINING FOR BH STAFF. 2. LEVERAGE EXISTING BEHAVIORAL HEALTH INFRASTRUCTURE: - UTILIZE EXISTING BH INFRASTRUCTURE, WHICH INCLUDES INTEGRATED BH IN WOMEN'S HEALTH AND FAMILY MEDICINE WITH PRACTICES LIKE WARM HAND-OFFS TO INCREASE ACCESS AND COMPREHENSIVE CARE. - LEVERAGE ROBUST ENABLING SERVICES TO ADDRESS SOCIAL DETERMINANTS OF HEALTH, INCLUDING FOOD INSECURITY, UNSTABLE HOUSING, LIMITED HEALTH INSURANCE, LINGUISTIC BARRIERS, FINANCIAL STRAIN, BROADBAND ISSUES, AND LIMITED ACCESS TO TRANSPORTATION. 3. EXPAND TELEPSYCHIATRY SERVICES: - DESIGNATE EXISTING SPACE AT REFUAH’S LIBERTY, NY SITE TO CREATE A MENTAL HEALTH HUB, ENHANCING TELEPSYCHIATRY SERVICES IN RURAL SULLIVAN COUNTY. THIS HUB WILL CONNECT PATIENTS TO TELEPSYCHIATRY AND ONSITE SOCIAL WORKERS. 4. RECOVERY AND SUPPORT SERVICES: HIRE ADDITIONAL RECOVERY AND SUPPORT STAFF TO PROVIDE ROBUST ENABLING SERVICES AND COMMUNITY OUTREACH AND EDUCATION PROGRAMS TO ADDRESS STIGMAS AND DISCRIMINATION ASSOCIATED WITH SEEKING BEHAVIORAL HEALTH SERVICES, PARTICULARLY IN UNDERSERVED POPULATIONS. REFUAH HAS PROVIDED COMPREHENSIVE HEALTHCARE SERVICES TO LOW-INCOME NEW YORKERS SINCE 1992 AND BECAME A SECTION 330(E) HEALTH CENTER PROGRAM GRANTEE IN 2002. BOTH COUNTIES ARE DESIGNATED AS MEDICALLY UNDERSERVED AREAS (MUA) AND HEALTH PROFESSIONAL SHORTAGE AREAS. LOCAL AND REGIONAL COMMUNITY HEALTH NEEDS ASSESSMENTS IDENTIFY ACCESS TO QUALITY, AFFORDABLE CARE AS TOP HEALTH PRIORITIES. WITH PROJECT FUNDING, REFUAH WILL EXPAND HOURS AT FOUR SERVICE SITES, ADDRESSING THE CRITICAL NEED FOR ACCESSIBLE HEALTHCARE. THE CENTER HAS BEEN PROVIDING EXPANDED HOURS AT ITS FLAGSHIP LOCATION IN SPRING VALLEY, NY, FOR SEVERAL YEARS. AT THIS SITE, REFUAH OFFERS OVER 100 HOURS OF PATIENT CARE PER WEEK, INCLUDING EXPANDED HOURS THROUGH ITS URGENT CARE AND WOMEN'S HEALTH URGENT CARE. THIS ALLOWS THE TARGET POPULATION, WHICH INCLUDES THE WORKING POOR, TO ACCESS CARE OUTSIDE OF BUSINESS HOURS. IT INCREASES ACCESS TO PRIMARY CARE AND KEEPS PATIENTS OUT OF THE ER. IT ALSO ALLOWS THEM TO SEEK URGENT CARE AT THEIR TRUSTED MEDICAL HOME TO ENSURE CONTINUITY AND QUALITY. THE IMPACT OF THESE EXPANDED HOURS WILL BE SIGNIFICANT, IMPROVING HEALTH OUTCOMES BY REDUCING EMERGENCY ROOM VISITS, INCREASING EARLY DETECTION AND MANAGEMENT OF CHRONIC CONDITIONS, AND PROVIDING TIMELY MENTAL HEALTH SERVICES. REFUAH OFFERS COMPREHENSIVE PRIMARY CARE SERVICES, AS WELL AS ONSITE DENTAL, BEHAVIORAL HEALTH AND A BROAD RANGE OF SPECIALTY SERVICES (PODIATRY, UROLOGY, OPHTHALMOLOGY, ETC.) TO NEARLY 80,000 PATIENTS ANNUALLY. REFUAH’S TARGET POPULATION INCLUDES LOW-INCOME SERVICE AREA RESIDENTS AND FAMILIES LIVING AT OR BELOW 200% OF THE FEDERAL POVERTY GUIDELINES (FPG) AND THE UN/UNDERINSURED. PROJECT DESCRIPTION: REFUAH’S AXCS PROJECT CENTERS ON TRAINING A TEAM OF LOCALLY SOURCED, CULTURALLY, AND LINGUISTICALLY RESPONSIVE NAVIGATORS (1.5 FTE) AND COMMUNITY HEALTH WORKERS (1.5 FTE). THIS SUPPORT TEAM WILL BE WELL POSITIONED TO MAKE IN-ROADS INTO THE SERVICE AREAS’ HARDEST TO REACH AND MOST AT-RISK COMMUNITIES INCLUDING HAITIAN AND HISPANIC IMMIGRANTS, ULT RAORTHODOX JEWS, LOW-INCOME AND RURAL RESIDENTS, AND AGING POPULATIONS. TO FURTHER ENSURE ACCESS, REFUAH WILL USE A PORTION OF AXCS FUNDING TO RETROFIT A MOBILE UNIT (OWNED AND LISTED ON THE CENTER’S FORM 5B) TO BE USED AS A CANCER SCREENING CLINIC. THIS MOBILE SCREENING CLINIC WILL TRAVEL THROUGHOUT ROCKLAND AND SULLIVAN COUNTY, INTO GEOGRAPHICALLY ISOLATED OR TRANSPORTATION DEPENDENT COMMUNITIES AND OFFER ACCESSIBLE, HIGH QUALITY SCREENING SERVICES FOR BREAST, CERVICAL AND COLORECTAL CANCERS. NCI PARTNER: TO EXECUTE THE PROPOSED PROJECT, REFUAH WILL COLLABORATE WITH COLUMBIA UNIVERSITY HERBERT IRVING COMPREHENSIVE CANCER CENTER (HICCC), A NATION CANCER INSTITUTE (NCI)- DESIGNATED CANCER CENTER. COLLABORATIVE EFFORTS WILL DEVELOP A TEAM OF LOCALLY SOURCED AND CULTURALLY COMPETENT NAVIGATORS AND COMMUNITY HEALTH WORKERS TO IMPLEMENT A PEER-WORKER MODEL AND INCREASE SCREENING RATES FOR BREAST, CERVICAL AND COLORECTAL CANCERS. NEED: THE NEED WITHIN THE SERVICE AREA IS IMMEDIATE. CANCER SCREENING PREVALENCE FELL DRAMATICALLY DURING THE COVID-19 PANDEMIC AND REMAINS BELOW PRE-PANDEMIC LEVELS. A STUDY PUBLISHED IN JAMA ONCOLOGY IN APRIL 2021 ESTIMATED THAT THROUGH JULY 2020 THE DISRUPTIONS STEMMING FROM THE PANDEMIC LED AMERICANS TO MISS MORE THAN 9 MILLION SCREENINGS FOR BREAST, COLORECTAL AND PROSTATE CANCERS. ADDITIONALLY, THE TOTAL NUMBER OF CANCER SCREENINGS RECEIVED BY THE CDC’S NATIONAL BREAST AND CERVICAL CANCER EARLY DETECTION PROGRAM DECLINED BY 87% FOR BREAST CANCER AND 84% FOR CERVICAL CANCER DURING APRIL 2020 COMPARED WITH THE PREVIOUS 5-YEAR AVERAGES FOR THAT MONTH. COLORECTAL CANCER SCREENING RATES DROPPED NEARLY 18%. IN JUNE 2020, THE NATIONAL CANCER INSTITUTE (NCI) PROJECTED THAT PANDEMIC-DRIVEN MISSED SCREENINGS WOULD CONTRIBUTE TO 10,000 DEATHS FROM BREAST AND COLORECTAL CANCERS. WITH AXCS FUNDING, REFUAH WILL PARTNER WITH HICCC TO INCREASE SCREENING RATES AMONG HIGH-RISK POPULATIONS AND, WHEN INDICATED, CONNECT PATIENTS WITH FOLLOW-UP REFERRAL S WITHIN 30 DAYS.

THE CENTER, TO BE LOCATED AT REFUAHHEALTH, A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) THAT PROVIDES COMPREHENSIVE HEALTHCARE SERVICES TO LOW-INCOME POPULATIONS THROUGHOUT ROCKLAND COUNTY AND THE LOWER HUDSON VALLEY IN NEW YORK, WILL FOCUS ON FERTILITY SERVICES, OFFERING WOMEN TREATMENT AND COUNSELING IN A LOCAL, CARING, AND COMPREHENSIVE ENVIRONMENT. CONSISTENT WITH ITS MISSION AS AN FQHC, SERVICES AT THE CENTER WILL BE MADE AVAILABLE TO ALL WOMEN, REGARDLESS OF ABILITY TO PAY. FUNDING WILL CONSTRUCT AN APPROX. 10,000 SQUARE FOOT FACILITY CONSISTING OF EXAM/CONSULT ROOMS, MAMMOGRAPHY, OUTPATIENT SURGICAL SPACE, AND AN EXPANDED MIDWIFERY-LED BIRTHING CENTER. KEEPING IN MIND THE DEEP EMOTIONAL ISSUES THAT CAN RESULT FROM INFERTILITY AND PREGNANCY LOSS, THE CENTER WILL BE DESIGNED WITH HOLISTIC DESIGN PRINCIPLES THAT WILL PROVIDE A SOOTHING AND PATIENT-FOCUSED EXPERIENCE. NEED: ACCORDING TO AMERICAN COMMUNITY SURVEY DATA, APPROXIMATELY 15% OF ROCKLAND COUNTY WOMEN WILL FACE INFERTILITY. CURRENTLY, THERE ARE VIRTUALLY NO LOCAL OPTIONS FOR FERTILITY SERVICES IN ROCKLAND COUNTY. EVEN THOUGH NEW YORK IS ONE OF THE MOST EXPANSIVE STATES FOR FERTILITY HEALTH INSURANCE COVERAGE - AND THE ONLY STATE WHICH OFFERS COVERAGE THROUGH MEDICAID - THE LACK OF A LOCAL OPTION MAKES IT DIFFICULT TO ACCESS TREATMENT. FERTILITY TREATMENT IS OFTEN A TIME-CONSUMING PROCESS, WITH CAREFULLY TIMED TREATMENTS BEING REQUIRED DAILY FOR SUSTAINED PERIODS OF TIMES. THE COST OF TRANSPORTATION AND THE NEED TO TAKE TIME OFF FROM WORK AND ARRANGE FAMILY CARE CAN MAKE TRAVEL TO OUT-OF-COUNTY PROVIDERS PROHIBITIVELY EXPENSIVE AND PRACTICALLY IMPOSSIBLE. THIS IS ALL IN ADDITION TO THE EMOTIONAL BURDEN FERTILITY PLACES ON WOMEN AND THEIR FAMILIES. SERVICES: THE CENTER WILL PROVIDE ACCESSIBLE FERTILITY CARE, HOUSE AN EXPANDED MIDWIFERY LED BIRTHING CENTER, AND OFFER COMPREHENSIVE WOMEN'S HEALTH SERVICES, MENTAL HEALTH SERVICES, MAMMOGRAPHY, AND GYN OUT-PATIENT SURGERY. BRINGING ALL OF THESE SERVICES TOGETHER UNDER ONE ROOF, IN A PATIENT-CENTERED LOCATION, WILL ENHANCE REFUAH'S EXISTING CAPABILITIES AS AN FQHC AND ALLOW THE CENTER TO REACH THE UNDERSERVED WOMEN OF ROCKLAND COUNTY. POPULATION GROUP(S) TO BE SERVED: REFUAHHEALTH IS A FEDERALLY-QUALIFIED HEALTH CENTER (FQHC) SERVING LOW INCOME FAMILIES, LIVING AT OR BELOW 200% OF FPG. AS AN FQHC, IT IS ACCESSIBLE TO PATIENTS REGARDLESS OF ABILITY TO PAY. IT IS ONE OF THE LARGEST PROVIDERS OF OUTPATIENT WOMEN'S HEALTH SERVICES IN ROCKLAND COUNTY. OVER 80% OF ITS PATIENTS ARE COVERED BY MEDICAID. REFUAH OFFERS UNINSURED PATIENTS CARE ON A SLIDING-FEE SCALE, WITH PATIENTS BELOW 100% OF THE FEDERAL POVERTY LEVEL PAYING ONLY A NOMINAL FEE. NO PATIENT IS EVER TURNED AWAY BECAUSE THEY CANNOT AFFORD CARE. IN PARTICULAR, WOMEN OF COLOR AND WOMEN OF LOWER SOCIO-ECONOMIC STATUS ARE DISPARATELY IMPACTED BY THE LACK OF LOCAL FERTILITY AND COMPREHENSIVE WOMEN’S HEALTH OPTIONS. HISTORICALLY MARGINALIZED POPULATIONS, INCLUDING BIPOC AND LGBTQ COMMUNITIES, IN PARTICULAR, HAVE FACED HEIGHTENED BARRIERS TO ACCESSING FERTILITY TREATMENT. THESE WOMEN, WHO ALREADY FACE OTHER BARRIERS TO ACCESSING HEALTHCARE, WOULD ESPECIALLY BENEFIT FROM A LOCAL, AFFORDABLE OPTION
