FOR OVER 30 YEARS, AHI HAS LED HEALTH PLANNING, HEALTH PROMOTION AND HEALTH INNOVATION THROUGH PARTNERSHIPS WITH HEALTH CARE PROVIDERS, COMMUNITY-BASED ORGANIZATIONS, AND RESIDENTS OF A 9-COUNTY, 11 THOUSAND SQUARE MILE REGION OF RURAL UPSTATE NEW YORK. AHI IS A JOINT VENTURE OF MULTIPLE HEALTH CARE PROVIDERS INCLUDING 3 HOSPITALS, A LARGE HEALTH SYSTEM AND A FEDERALLY QUALIFIED HEALTH CENTER WITH 21 LOCATIONS - ACTIVITIES THAT BUILD, STRENGTHEN, AND MAINTAIN THE NECESSARY COMPETENCIES AND RESOURCES NEEDED TO SUSTAIN OR IMPROVE HEALTH SERVICES DELIVERY IN RURAL POPULATIONS. OUR INITIATIVE WILL BUILD UPON AND STRENGTHEN EXISTING CAPACITIES BY LEVERAGING THE EXPERTISE OF A REGIONAL NONPROFIT HEALTH WORKFORCE DEVELOPMENT ORGANIZATION THAT ADDRESSES HEALTH WORKFORCE CHALLENGES AND NEEDS IN OUR RURAL REGION THAT WILL CREATE A ONE-STOP INTERNET BASED PLATFORM FOR INFORMATION ON AVAILABLE CASE MANAGEMENT (CM) TRAINING AND EMPLOYMENT OPPORTUNITIES, BY SUPPORTING LOCAL PROVIDERS THROUGH CM APPRENTICESHIPS THAT ENSURE ON-THE-JOB TRAINING COMPLEMENTED BY FORMAL COURSEWORK OFFERED BY A REGIONAL INSTITUTE OF HIGHER EDUCATION AND TARGETED COVID AND LONG-COVID TRAINING DEVELOPED BY THE REGIONAL AHEC. TOGETHER THIS NETWORK OF PROVIDERS, HEALTH EDUCATORS AND WORKFORCE EXPERTS WILL ENSURE IMPROVED AND INCREASED AVAILABILITY OF CARE MANAGERS TO SUPPORT RESIDENTS AND PROVIDERS THROUGHOUT THE REGION. - ORGANIZATIONAL EXPERTISE AND CAPACITY AS IT RELATES TO THE SCOPE OF WORK PROPOSED. EACH OF THE PARTNERS INVOLVED IN OUR NETWORK BRING BOTH EXPERTISE AND CAPACITY, AS DEFINED BY THEIR INTEREST IN PARTICIPATING WITH US IN THIS VENTURE. WITH THESE GRANT FUNDS WE WILL BE ABLE TO CREATE AND SUPPORT A NETWORK THAT LINKS EDUCATIONAL ENTITIES, CLINICAL SITES, PAYERS AND KEY WORKFORCE STAKEHOLDERS TO INCREASE THE CM WORKFORCE IN OUR RURAL COMMUNITIES. ADIRONDACK HEALTH INSTITUTE IS REQUESTING A FUNDING PREFERENCE BASED ON ALL COUNTIES IN THE PROPOSED TARGET AREA BEING DESIGNATED AS HPSAS; ALONG WITH A FOCUS ON PRIMARY CARE, WELLNESS AND PREVENTION.
