THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. THE ORGANIZATION WILL PROVIDE 15 SCATTERED SITE RESIDENCES FOR 15 SURVIVORS AND THEIR FAMILIES THROUGH PRIVATE LANDLORD HOUSING UNITS. OSWEGO COUNTY OPPORTUNITIES WILL COLLABORATE WITH ITS EIGHT PARTNERS, THE OSWEGO COUNTY DEPARTMENT OF SOCIAL SERVICES, OSWEGO COUNTY DIVISION OF MENTAL HYGIENE, FARNHAM FAMILY SERVICES, CONNEXTCARE, CITY OF OSWEGO RENTAL ASSISTANCE PROGRAM, CITY OF FULTON COMMUNITY DEVELOPMENT AGENCY, OSWEGO COUNTY WORKFORCE NEW YORK, AND THE HOUSING & HOMELESS COALITION OF CENTRAL NEW YORK TO PROVIDE A HOLISTIC, VICTIM-CENTERED AND MULTIDISCIPLINARY APPROACH TO TRANSITIONAL HOUSING NEEDS IN THE COMMUNITY. PURPOSE: THE CONTINUUM OF CARE (COC) PROGRAM IS DESIGNED TO PROMOTE COMMUNITY-WIDE COMMITMENT TO THE GOAL OF ENDING HOMELESSNESS; PROVIDE FUNDING FOR EFFORTS BY NONPROFIT PROVIDERS, STATES, AND LOCAL GOVERNMENTS TO QUICKLY HOUSE HOMELESS INDIVIDUALS AND FAMILIES WHILE MINIMIZING THE TRAUMA AND DISLOCATION CAUSED TO HOMELESS INDIVIDUALS, FAMILIES, AND COMMUNITIES BY HOMELESSNESS; PROMOTE ACCESS TO AND EFFECTIVE UTILIZATION OF MAINSTREAM PROGRAMS BY HOMELESS INDIVIDUALS AND FAMILIES; AND OPTIMIZE SELF-SUFFICIENCY AMONG INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. RURAL COMMUNITIES OPIOID RESPONSE-IMPLEMENTATION - IF FUNDED, THE ACTIVITIES PERFORMED UNDER THE RCORP-IMPLEMENTATION AWARD WILL BUILD ON THE PLANNING ACTIVITIES SUCH AS THE NEEDS ASSESSMENT AND GAPS ANALYSIS COMPLETED IN THE CURRENT RCORP PLANNING GRANT. THE OSWEGO COUNTY OPIOID RESPONSE CONSORTIUM HAS ACKNOWLEDGED THAT IN ORDER TO COMBAT OPIOID USE DISORDERS AND MISUSE IN OSWEGO COUNTY THAT WE MUST COORDINATE AND COLLABORATE OUR EFFORTS TO HAVE THE MAXIMUM EFFECTIVENESS ON REDUCING THE MORTALITY AND MORBIDITY OF OPIOID USE. THE NEEDS THAT WERE BROUGHT FORWARD AS A RESULT OF THE NEEDS AND GAPS ANALYSIS UNDER THE RCORP PLANNING GRANT PROVIDED A FOUNDATION FOR THE ACTIVITIES THAT WILL BE CONDUCTED UNDER THE RCORP-IMPLEMENTATION AWARD SUCH AS; INCREASING COMMUNITY WIDE NALOXONE TRAINING AND DISTRIBUTION, INCREASING PROVIDERS SCREENING FOR SUD, INCREASING PREVENTION ACTIVITIES, ENHANCEMENT OF TREATMENT SERVICES, DEVELOPMENT AND DISTRIBUTION OF RECOVERY RESOURCES, AND ADDRESSING COMMUNITY WIDE STIGMA SURROUNDING SUD. THE CONSORTIUM HAS A STANDARD MEETING SCHEDULE, A HEALTHY NETWORK OF MEMBERS, AND THE WILLINGNESS AND DEDICATION TO REDUCE THE MORTALITY AND MORBIDITY OF OPIOID USE IN OSWEGO COUNTY. RURAL PROGRAM FUNDS ARE USED TO SUPPORT PROGRAMS THAT: A) IDENTIFY, ASSESS, AND APPROPRIATELY RESPOND TO CHILD, YOUTH, AND ADULT VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING IN RURAL COMMUNITIES; B) ESTABLISH AND EXPAND VICTIM SERVICES IN RURAL COMMUNITIES FOR CHILD, YOUTH, AND ADULT VICTIMS; AND/OR C) INCREASE THE SAFETY AND WELL-BEING OF WOMEN AND CHILDREN IN RURAL COMMUNITIES BY DEALING DIRECTLY AND IMMEDIATELY WITH DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING, AND CREATING AND IMPLEMENTING STRATEGIES TO INCREASE AWARENESS AND PREVENT THESE CRIMES. THE PROGRAM SUPPORTS HOLISTIC, SURVIVOR-CENTERED APPROACHES TO PROVIDING TRANSITIONAL HOUSING AND SUPPORT SERVICES DESIGNED TO ENABLE A MINOR, AN ADULT, OR A DEPENDENT OF SUCH MINOR OR ADULT LOCATE AND SECURE PERMANENT HOUSING, SECURE EMPLOYMENT, AND INTEGRATE INTO A COMMUNITY. OSWEGO COUNTY OPPORTUNITIES, INC. IS A NON-PROFIT ORGANIZATION LOCATED IN OSWEGO, NY, A RURAL REGION OF THE STATE. THE ORGANIZATION WILL PROVIDE 15 SCATTERED SITE RESIDENCES FOR 15 SURVIVORS AND THEIR FAMILIES THROUGH PRIVATE LANDLORD HOUSING UNITS. OSWEGO COUNTY OPPORTUNITIES WILL COLLABORATE WITH ITS EIGHT PARTNERS, THE OSWEGO COUNTY DEPARTMENT OF SOCIAL SERVICES, OSWEGO COUNTY DIVISION OF MENTAL HYGIENE, FARNHAM FAMILY SERVICES, CONNEXTCARE, CITY OF OSWEGO RENTAL ASSISTANCE PROGRAM, CITY OF FULTON COMMUNITY DEVELOPMENT AGENCY, OSWEGO COUNTY WORKFORCE NEW YORK, AND THE HOUSING & HOMELESS COALITION OF CENTRAL NEW YORK TO PROVIDE A HOLISTIC, VICTIM-CENTERED AND MULTIDISCIPLINARY APPROACH TO TRANSITIONAL HOUSING NEEDS IN THE COMMUNITY. THIS CONTINUATION PROJECT WILL USE FUNDS TO PROVIDE BOTH HOUSING AND SUPPORTIVE SERVICES TO MOVE SURVIVORS OF SEXUAL ASSAULT, DOMESTIC VIOLENCE, DATING VIOLENCE, OR STALKING WHO ARE HOMELESS TO PERMANENT HOUSING. THE PROJECT WILL ASSIST CLIENTS FOR AN ANTICIPATED MINIMUM OF SIX MONTHS AND A MAXIMUM LENGTH OF 24 MONTHS. SERVICES OFFERED WILL FOCUS ON COORDINATING SUPPORT WITH SUBSTANCE ABUSE PROFESSIONALS, AS WELL AS, MEETING THE NEEDS OF THE UNDERSERVED RURAL POPULATION IN THE REGION. THE RANGE OF OPTIONAL SUPPORT SERVICES INCLUDE RENTAL AND UTILITY ASSISTANCE, CASE MANAGEMENT, SAFETY PLANNING, TRANSPORTATION, CAREER COUNSELING, FINANCIAL AND CREDIT COUNSELING, SUPPORT GROUPS, JOB TRAINING, LIMITED LEGAL ASSISTANCE, AND HOUSING ADVOCACY. THE DEMONSTRATION HAS SEVEN PRIMARY OBJECTIVES: • BUILD NATIONAL MOMENTUM. • PROMOTE EQUITY IN THE DELIVERY AND OUTCOMES OF HOMELESS ASSISTANCE. • HIGHLIGHT THE IMPORTANCE OF YOUTH LEADERSHIP. • EVALUATE THE COORDINATED COMMUNITY APPROACH. • EXPAND CAPACITY. • EVALUATE PERFORMANCE MEASURES. • ESTABLISH A FRAMEWORK FOR THE FEDERAL PROGRAM AND TECHNICAL ASSISTANCE COLLABORATION. BEGINNING IN 2016, NEW YHDP COMMUNITIES ARE SELECTED THROUGH A COMPETITIVE PROCESS DEPENDING ON THE ANNUAL FUNDING PROVIDED BY CONGRESS. NEEDS ADDRESSED: OSWEGO COUNTY FACES SIGNIFICANT RURAL HEALTH CHALLENGES, INCLUDING PROVIDER SHORTAGES, FRAGMENTED CARE DELIVERY, AND DISPARITIES IN ACCESS TO PREVENTIVE AND SPECIALTY HEALTH SERVICES. THE REGION EXPERIENCES HIGH RATES OF CHRONIC DISEASE, BEHAVIORAL HEALTH CONCERNS, AND SOCIOECONOMIC BARRIERS THAT IMPEDE RESIDENTS' ABILITY TO RECEIVE TIMELY, COORDINATED CARE.
