OUR SERVICES INCLUDE PRIMARY MEDICAL CARE, BEHAVIORAL HEALTH AND PSYCHIATRY SERVICES, 340B DISCOUNT MEDICATIONS, ADDICTION TREATMENT, AND ORAL HEALTH SERVICES. LRHC ALSO PROVIDES PRIMARY CARE SERVICES AT VALLEY VISTA (INPATIENT SUBSTANCE ABUSE REHABILITATION CENTER) AND BEHAVIORAL HEALTH SERVICES IN 6 AREA SCHOOLS. LRHC SERVES ALL RESIDENTS OF THE 16 TOWNS IN OUR REGION, WITH SPECIAL OUTREACH TO THE UNDERSERVED. OUR CLINICS HAVE MAINTAINED STATUS AS PATIENT-CENTERED MEDICAL HOMES SINCE 2013. PROPOSED PROJECT LRHC PROPOSES TO USE THE CDS FUNDING TO BUILD A ~3400 SQUARE FOOT DENTAL CLINIC THAT INCORPORATES THE LATEST INFECTION PREVENTION FEATURES AT 55 MAIN STREET NORTH, WELLS RIVER, VT. THIS FACILITY WILL HAVE 6 OPERATORIES AND HOUSE 2 DENTISTS, 2 HYGIENISTS, AND SUPPORT STAFF, AND EVENTUALLY PROVIDE ~9000 VISITS FOR ~3500 PATIENTS. THIS NEW CLINIC WILL FILL A SIGNIFICANT UNMET NEED IN THIS AREA WHERE THERE ARE NO DENTISTS TAKING NEW PATIENTS WITHIN A 30-60 MINUTE DRIVING RADIUS. THE PROPERTY IS OWNED BY LRHC AND IS ADJACENT TO ITS MEDICAL CLINIC IN WELLS RIVER, ON A SITE THAT FORMERLY HAD A GAS STATION AND MINI-MART. LRHC HAS BEEN WORKING WITH THE STATE TO CLEAN UP DECADES OF PROPERTY CONTAMINATION AND IT HAS NOW BEEN DECLARED SAFE AND FIT FOR ITS PROPOSED USE. THE PROJECT WILL INVOLVE SIGNIFICANT RENOVATION OF THE CURRENT 25 00 SQ. FT. MINI-MART BUILDING WITH CONSTRUCTION OF A 900 SQ. FT. ADDITION. THE TOTAL PROJECT COSTS ARE PROJECTED TO BE $1,558,581, WITH THE FEDERAL SHARE BEING $1,334,000.
