MULTIDIMENSIONAL PREDICTIVE MODELING TO UNDERSTAND MECHANISMS OF EXERCISE RESPONSE HETEROGENEITY IN OLDER ADULTS - AGING-RELATED FUNCTIONAL DECLINES ARE THOUGHT TO BE CAUSED BY HALLMARK BIOLOGICAL PROCESSES THAT ULTIMATELY COMPROMISE HEALTHSPAN AND QUALITY OF LIFE (QOL). EXERCISE IS A MULTIPOTENT TREATMENT WITH PROMISE TO MITIGATE MOST AGING HALLMARKS, BUT THERE IS SUBSTANTIAL VARIABILITY IN INDIVIDUAL EXERCISE RESPONSIVENESS. THIS INTER- INDIVIDUAL RESPONSE HETEROGENEITY (IRH) WAS FIRST IDENTIFIED IN THE CONTEXT OF ENDURANCE TRAINING (ET) AND LATER WE ESTABLISHED IT WITH RESISTANCE TRAINING (RT). WE PROPOSE THE INNOVATIVE, BUT LOGICAL, STRATEGY TO USE COMBINED ET AND RT ALIGNED WITH HHS GUIDELINES TO MAXIMIZE HEALTH BENEFITS IN AGING ADULTS. LOW CARDIORESPIRATORY FITNESS (CRF; VO2MAX) AND LOW FUNCTIONAL MUSCLE QUALITY (FMQ; STRENGTH/MUSCLE MASS) ARE MULTI-SYSTEM MANIFESTATIONS OF THE DETERIORATION OF THE CELLULAR HALLMARKS OF AGING. IMPORTANTLY, BOTH CRF AND FMQ ARE MODIFIABLE WITH ET AND RT. THUS, OUR DESIGN PREMISE IS THAT COMBINED ET+RT IS AN EXCELLENT STRATEGY FOR ELUCIDATING FACTORS UNDERLYING IRH, AS IT FORGES A PATH TOWARD UNDERSTANDING AND MITIGATING IRH IN AGING WITH DIRECT TRANSLATABILITY TO BEST-EVIDENCE PUBLIC HEALTH RECOMMENDATIONS. WHILE SOURCES OF IRH ARE A CONSTELLATION OF MODIFIABLE AND NON-MODIFIABLE FACTORS, WE WILL TEST THE HYPOTHESIS THAT FACTORS CENTRAL TO AGING ITSELF – AGING HALLMARKS SUCH AS PROTEOSTASIS, MITOCHONDRIAL ENERGETICS, INFLAMMATION, AND CIRCADIAN CLOCK FUNCTION – ARE CHIEF CONTRIBUTORS TO THE MULTIDIMENSIONAL CIRCUITRY THAT DETERMINES WHETHER AN INDIVIDUAL ACHIEVES THE MINIMUM CLINICALLY IMPORTANT DIFFERENCE (MCID) IN CRF AND/OR FMQ WITH EXERCISE TRAINING. WE WILL ALSO TEST THE HYPOTHESIS THAT ALTERED EXERCISE DOSING COUPLED WITH LIFESTYLE RECOMMENDATIONS WILL PROMOTE ATTAINMENT OF MCIDS AMONG OLDER ADULTS WHO DO NOT INITIALLY RESPOND. WITH CRF AND FMQ AS PRIMARY CLINICAL OUTCOMES, WE WILL USE A 2-PHASE SEQUENTIAL MULTIPLE ASSIGNMENT RANDOMIZED TRIAL (SMART) OF COMBINED ET+RT WITH CLINICAL PHENOTYPING AND BLOOD/MUSCLE MOLECULAR AND CELLULAR ANALYSES. SPECIFIC AIM 1: INTERROGATION. IN SMART PHASE I, N=200 PARTICIPANTS STRATIFIED BY SEX AND AGE GROUP (60-69 VS ≥70 Y) WILL ENABLE TESTING OF BOTH AS BIOLOGICAL VARIABLES. AFTER 12 WK OF ET+RT INCLUDING ACUTE RESPONSE STUDIES, INDIVIDUAL RESPONDER CLASSIFICATION WILL BE DEFINED BY MCIDS FOR CRF AND FMQ. THE IMPACT OF AGING HALLMARKS (MUSCLE PROTEOSTASIS, MITOCHONDRIAL FUNCTION, INFLAMMATION, CIRCADIAN CLOCK FUNCTION) WILL BE DETERMINED, AND WE WILL DECIPHER THE MULTIDIMENSIONAL CIRCUITRY OF IRH VIA INTEGRATED MODELING OF: (I) TARGETED CELLULAR/MOLECULAR OUTCOMES; (II) INTER-TISSUE COMMUNICATION; (III) EPIGENOMICS; (IV) FREE-LIVING BEHAVIOR; AND (V) CLINICAL OUTCOMES. SPECIFIC AIM 2: MITIGATION. IN SMART PHASE II (10 WK), INDIVIDUALS NOT ATTAINING ONE OR BOTH MCIDS WILL UNDERGO AN ADDITIONAL 10 WK OF TRAINING WITH TARGETED AUGMENTATION. THOSE ATTAINING BOTH MCIDS IN PHASE I WILL BE RANDOMIZED TO: (I) CONTINUE THE SUPERVISED TRAINING; OR (II) RETURN TO FREE-LIVING. THE LATTER WILL ENABLE US TO TEST SUSTAINABILITY OVER 10 WK. WE ANTICIPATE THESE AIMS WILL FOSTER SIGNIFICANT ADVANCES IN THE UNDERSTANDING OF FACTORS IMPACTING EXERCISE RESPONSE VARIABILITY AMONG OLDER ADULTS BY DETERMINING THE ROLE OF AGING HALLMARKS AND DEFINING MULTIDIMENSIONAL CIRCUITRY OF INFLUENTIAL FEATURES.

ADJUVANT NITRATE TO BOOST EXERCISE-INDUCED HEALTH BENEFITS IN OLDER ADULTS - AGING-RELATED DECLINES IN HEALTH AND INTRINSIC CAPACITY ARE LINKED TO BIOLOGICAL PROCESSES PROFOUNDLY AFFECTING ALL ORGAN SYSTEMS AND TISSUES INCLUDING SKELETAL MUSCLE. EXERCISE IS A MULTIPOTENT COUNTERMEASURE TO THESE PROCESSES. HOWEVER, THERE IS SIGNIFICANT INTER-INDIVIDUAL RESPONSE HETEROGENEITY (IRH) IN THE MAGNITUDE OF EXERCISE-INDUCED HEALTH BENEFITS. BOOSTING EXERCISE RESPONSIVENESS AMONG OLDER ADULTS IS THUS A TOP PRIORITY. WE HAVE FOUND IN OLDER ADULTS THAT SUPPLEMENTATION WITH NITRIC OXIDE (NO) PRECURSORS NITRATE AND NITRITE IS SAFE, WELL TOLERATED, AND IMPROVES: (I) PHYSICAL PERFORMANCE; (II) VASCULAR ENDOTHELIAL FUNCTION; (III) MITOCHONDRIAL FUNCTION; (IV) OXIDATIVE STRESS; AND (V) MUSCLE INFLAMMATION. WE SUSPECT DEFICITS AMONG SOME OLDER ADULTS IN NITRATE BIOAVAILABILITY AND/OR EXERCISE-INDUCED NO PRODUCTION MAY IN PART DRIVE SUBOPTIMAL EXERCISE RESPONSIVENESS. GIVEN THE POWERFUL AND SUSTAINING IMPACT OF EXERCISE-INDUCED HEALTH BENEFITS ON MORBIDITY AND MORTALITY, OUR FUNDAMENTAL PREMISE IS THAT EXERCISE RESPONSIVENESS OF EVERY OLDER ADULT SHOULD BE MAXIMIZED. IN THE BOOST-X TRIAL WE WILL DETERMINE IF INCREASING NITRATE BIOAVAILABILITY VIA DIETARY SUPPLEMENTATION IS AN EFFICACIOUS APPROACH TO MORE OLDER ADULTS ATTAINING THESE ESSENTIAL HEALTH BENEFITS. LOW CARDIORESPIRATORY FITNESS (CRF, VO2MAX) AND LOW FUNCTIONAL MUSCLE QUALITY (FMQ; STRENGTH/MUSCLE MASS) ARE MULTI-SYSTEM MANIFESTATIONS OF DEGENERATIVE AGING THAT INCLUDE VASCULAR ENDOTHELIAL AND MITOCHONDRIAL DYSFUNCTION, OXIDATIVE STRESS, AND INFLAMMATION. CRF AND FMQ ARE MODIFIABLE WITH ENDURANCE (ET) AND RESISTANCE (RT) TRAINING. THIS PLANNED TRIAL WILL BE THE FIRST TO EVALUATE THE EFFICACY OF COUPLING A NITRATE-RICH NUTRITIONAL SUPPLEMENT WITH A COMBINED ET+RT PRESCRIPTION IN LINE WITH DHHS WEEKLY EXERCISE GUIDELINES. WE WILL TEST THE CENTRAL HYPOTHESIS THAT DAILY NITRATE SUPPLEMENTATION WILL AUGMENT EXERCISE TRAINING-INDUCED GAINS IN CRF AND FMQ ACCOMPANIED BY IMPROVEMENTS IN VASCULAR ENDOTHELIAL AND MITOCHONDRIAL FUNCTION. AIM 1: ASSESS THE CLINICAL IMPACT OF NITRATE SUPPLEMENTATION ON THE PROPORTION OF OLDER ADULTS ATTAINING EXERCISE-INDUCED HEALTH BENEFITS. N=226 PARTICIPANTS STRATIFIED BY SEX AND AGE GROUP (60-69 VS ≥70 Y) WITH RANDOM ASSIGNMENT TO NITRATE (400 MG, 2X/DAY) (N=113) VS. PLACEBO (NITRATE- DEPLETED) (N=113) DURING 12 WK OF 3X/WK ET+RT. 1˚ OUTCOMES: MCID ATTAINMENT FOR CRF AND FMQ. EXPECTED RESULTS: DAILY NITRATE WILL REDUCE THE PROPORTION OF OLDER ADULTS NOT ATTAINING CRF AND FMQ MCIDS BY A FULL 1/3. AIM 2: ASSESS POTENTIAL MECHANISMS UNDERPINNING ADAPTATIONS TO EXERCISE +/- NITRATE SUPPLEMENTATION. WE WILL TEST/INTEGRATE IN VIVO, EX VIVO, IN VITRO AND MOLECULAR MAPPING ASSAYS CONSIDERED CENTRAL TO EXERCISE +/- NITRATE ADAPTATIONS AND LEVERAGE A MULTIDIMENSIONAL LEARNING FRAMEWORK TO BETTER UNDERSTAND THE BIOCIRCUITRY UNDERPINNING MCID ATTAINMENT. 1˚ OUTCOMES: VASCULAR ENDOTHELIAL FUNCTION, MUSCLE MITOCHONDRIAL FUNCTION. EXPECTED RESULTS: MCID ATTAINMENT WILL BE CLOSELY LINKED TO IMPROVEMENTS IN VASCULAR ENDOTHELIAL FUNCTION AND MUSCLE MITOCHONDRIAL FUNCTION. THE ACUTE MOLECULAR RESPONSES TO EXERCISE WILL BE DIFFERENTIALLY REGULATED BY NITRATE SUPPLEMENTATION, AND THE NOVEL BIOCIRCUITRY WILL REVEAL KEY FEATURES THAT INFLUENCE ADAPTABILITY.

PUBLIC HEALTH OFFICIALS USE SUCH MODELS TO INFORM PANDEMIC RESPONSE POLICIES AND ADVISORIES. YET THESE MODELS REQUIRE A RIGOROUS SCIENTIFIC FOUNDATION ABOUT HUMAN PSYCHOLOGY TO BETTER PREDICT PEOPLE?S RESPONSES TO INFORMATION AND POLICIES ABOUT PANDEMICS. THE RECENT COVID-19 PANDEMIC ILLUSTRATES THE CENTRAL ROLE OF HUMAN DECISION MAKING AND BEHAVIOR IN THE SPREAD OF SUCH A TRANSMISSIBLE DISEASE. PEOPLE?S DECISIONS REGARDING SOCIAL ISOLATION, SOCIAL DISTANCING, MASK WEARING, HAND WASHING, AND VACCINATION ARE CORRELATED WITH THE RATE AT WHICH THE COVID-19 VIRUS SPREADS OR THE SERIOUSNESS OF GETTING INFECTED. PEOPLE HAVE DIFFERENT INDIVIDUAL MINDSETS, AND THESE CAN VARY ACROSS DIFFERENT REGIONS AND SUBGROUPS, SO DIFFERENT GROUPS OF PEOPLE RESPOND DIFFERENTLY TO MESSAGING AND MANDATES AND THOSE RESPONSES CHANGE OVER TIME. THERE IS ALSO AN ONGOING SCIENTIFIC DEBATE ABOUT THE DEGREE TO WHICH PANDEMIC INFORMATION OR MISINFORMATION, OR THE PERCEIVED CREDIBILITY OF INFORMATION SOURCES, INFLUENCES THE DEGREE TO WHICH PEOPLE CHANGE THEIR BEHAVIOR. TO ADDRESS THESE SCIENTIFIC NEEDS, THIS PROJECT INVOLVES ACTIVITIES TO DEVELOP A MULTIDISCIPLINARY RESEARCH CORE AND AGENDA AND TO DEVELOP A STRONG PLAN FOR A COHESIVE RESEARCH CENTER FOR PREDICTIVE INTELLIGENCE FOR PANDEMIC PREVENTION. THE ACTIVITIES INCLUDE EXPLORATORY RESEARCH ON COMPUTATIONAL MODELS OF HUMAN PSYCHOLOGY, INFORMATION FLOW AND INFLUENCE, AND RESULTING PANDEMIC TRANSMISSION. THE PROJECT WILL ALSO SUPPORT THE TRAINING AND MENTORING OF GRADUATE STUDENTS WHO REPRESENT THE NEXT GENERATION OF RESEARCHERS TACKLING THESE GLOBAL CHALLENGES. THIS PROJECT USES COMPUTATIONAL THEORIES AND MODELS TO EXAMINE THE FUNDAMENTAL INTERDEPENDENT EVOLUTION OF INFECTION, BEHAVIOR, AND INFORMATION AT MULTIPLE LEVELS AND DRAWING UPON MULTIPLE DISCIPLINES IN ORDER TO SUPPORT IMPROVED PANDEMIC INTELLIGENCE, PREDICTION, EXPLANATION, AND COUNTERMEASURES. THE PROJECT IS ORGANIZED INTO (1) INTERDISCIPLINARY, STRATEGIC RESEARCH THRUSTS TO ACCELERATE CONVERGENT SCIENCE TOWARDS THE GRAND CHALLENGE, (2) THREE INVITATIONAL MEETINGS TO DRAW IN DIVERSE RESEARCHERS TO ADDRESS FOCAL RESEARCH TOPICS AND RESEARCH QUESTIONS, TO FILL IN GAPS IN THE RESEARCH CHALLENGES, AND DEVELOP A STRONG RESEARCH AND EDUCATION AGENDA FOR A COHESIVE PIPP CENTER, AND (3) PILOT STUDIES TO DEMONSTRATE FEASIBILITY OF INTEGRATED COMPUTATIONAL MODELS OF INFORMATION, HUMAN PSYCHOLOGY, AND PANDEMIC TRANSMISSION. FOR THE PILOT RESEARCH, A MULTIDISCIPLINARY TEAM COMBINES EMPIRICAL ASSESSMENTS WITH COMPUTATIONAL COGNITIVE MODELS IN AN AGENT-BASED MODELING SYSTEM. FOR DATA THE INVESTIGATORS DRAW ON VACCINATION DISCUSSIONS IN MASS MEDIA, TWITTER, GEOLOCATED TIMESERIES DATA ON VACCINATION RATES, INFECTION, DEATH AND RECOVERY RATES, STATE AND NATIONAL MANDATES REGARDING COVID-19 POLICIES ABOUT VACCINATION AND MASK WEARING FROM FEBRUARY 2020 THROUGH DECEMBER 2021 IN THE UNITED STATES. THESE DATA WILL BE SEGMENTED BY STATE AND MAJOR CITIES WITHIN THOSE STATES. THIS AWARD IS SUPPORTED BY THE CROSS-DIRECTORATE PREDICTIVE INTELLIGENCE FOR PANDEMIC PREVENTION PHASE I (PIPP) PROGRAM, WHICH IS JOINTLY FUNDED BY THE DIRECTORATES FOR BIOLOGICAL SCIENCES (BIO), COMPUTER INFORMATION SCIENCE AND ENGINEERING (CISE), ENGINEERING (ENG) AND SOCIAL, BEHAVIORAL AND ECONOMIC SCIENCES (SBE). THIS PROJECT WILL DELIVER A LIGHTWEIGHT AND COMPACT PROTOTYPE ROBOT-BASED EXERCISE DEVICE. THE CURRENT GRASSHOPPER TESTBED DEVICE WHICH HAS UNDERGONE PRELIMINARY TESTING AND DEVELOPMENT WILL BE USED TO ACCELERATE DEVELOPMENT. THE DEVICE WILL SUPPORT SEVERAL DIFFERENT EXERCISES INCLUDING ANAEROBIC AND AEROBIC. BY USING MOTORS THAT ARE COMPUTER CONTROLLED THE APPLIED RESISTIVE FORCES AND CAN BE ADJUSTED ALMOST INSTANTANEOUSLY. IT WILL BE POSSIBLE TO CONNECT A TOUCHSCREEN-BASED USER INTERFACE WILL ALLOW THE USER TO SELECT THE EXERCISE AND SETTING AND PROVIDE REAL-TIME FEEDBACK DURING THE ACTIVITY. DESPITE BEING SMALL COMPACT AND LIGHTWEIGHT THE DEVICE WILL BE CAPABLE OF PROVIDING A COMPLETE CUSTOMIZED STRENGTH AND AEROBIC WORKOUT WITH FEATURES NOT AVAILABLE ON ANY COMMERCIALLY AVAILABLE EXERCISE EQUIPMENT. THE DEVICE WILL COME WITH AN API FOR DEVELOPERS TO PROGRAM DIFFERENT TYPES OF EXERCISE. PROLONGED EXPOSURE TO MICROGRAVITY HAS BEEN SHOWN TO HAVE DELETERIOUS EFFECTS ON THE ENTIRE HUMAN MUSCULOSKELETAL SYSTEM. SPECIFICALLY LONG DURATION SPACEFLIGHT ADVERSELY AFFECTS THE MASS AND STRENGTH OF ASTRONAUTS MUSCLE AND BONE. THE CURRENT COUNTERMEASURES FOR REDUCED MUSCLE MASS AND STRENGTH INCLUDE A ROUTINE IN-FLIGHT EXERCISE PROGRAM. ONE OF THE MOST UTILIZED EXERCISE DEVICES IS CALLED THE ADVANCED RESISTIVE EXERCISE DEVICE (ARED) WHICH IS CURRENTLY IN PLACE ON THE INTERNATIONAL SPACE STATION (ISS). ALTHOUGH ARED IS CURRENTLY AN IMPORTANT COMPONENT IN MITIGATING MUSCLE LOSS AND BONE MINERAL DENSITY (BMD) REDUCTION IT IS FUNDAMENTALLY INCOMPATIBLE WITH MISSION SCENARIOS USING THE MULTI-PURPOSE CREW VEHICLE (MPCV) OR SIMILAR SPACECRAFT. THROUGH SIMPLE AND EASY RECONFIGURATION THE DEVICE WILL BE ABLE TO ACCOMMODATE 1ST-99TH PERCENTILE POPULATION TO PERFORM UPPER BODY LOWER BODY AND FULL BODY AEROBIC AND ANAEROBIC EXERCISES. IN ADDITION WE BELIEVE THAT THIS DEVICE WILL OFFER AN EXERCISE REGIMEN THAT IS SUPERIOR TO ARED AS WELL AS MOST GRAVITY BASED GYM EQUIPMENT. BECAUSE THE RESISTIVE FORCE THE DEVICE CREATES IS GENERATED BY MOTORS AND IS COMPUTER CONTROLLED AT THOUSANDS OF TIMES PER SECOND THE FORCE CAN BE CHANGED INSTANTANEOUSLY. THIS ALLOWS THE ECCENTRIC TO CONCENTRIC LOAD RATIO TO VARY FROM ZERO TO INFINITY DEPENDING ON THE EXERCISE PRESCRIPTION. IN ADDITION THE PROPOSED DEVICE CAN BE USED FREE FLOATING REMOVING THE NEED FOR VIBRATION ISOLATION FROM THE SPACECRAFT STRUCTURE. BECAUSE THE DEVICE WILL BE INSTRUMENTED WITH A FULL COMPLEMENT OF SENSORS TO MEASURE THE FORCES POSITIONS LOADS THIS LARGE AMOUNT OF DATA FROM THE DEVICE WILL HELP EXERCISE PHYSIOLOGISTS CONTINUE TO UNDERSTAND AND SOLVE THE CHALLENGES OF MAINTAINING CREW HEALTH IN SPACE.
