CHW-CCDN WILL FOCUS ON ESTABLISHING AND STRENGTHENING A CHW NETWORK TO ENHANCE COMMUNITY-BASED PRIMARY CARE DELIVERY AND ADDRESS HEALTH DISPARITIES AND TO PROMOTE HEALTH EQUITY. BY PROVIDING TRAINING, SUPPORT, AND RESOURCES THIS PROGRAM SEEKS TO EMPOWER CHWS AND MAXIMIZE THEIR IMPACT ON POPULATION HEALTH. THE PROPOSED CHW NETWORK WILL FOCUS ON THE FOLLOWING KEY COMPONENTS TO ACHIEVE NETWORK EFFICIENCY, INCREASE ACCESS, IMPROVE HEALTH CARE SERVICES AND OUTCOMES, AND TO STRENGTHEN THE RURAL HEALTH CARE SYSTEM: •TRAINING AND CAPACITY BUILDING. •COLLABORATION AND RESOURCE SHARING. •QUALITY ASSURANCE AND SUPERVISION. •TECHNOLOGY AND DATA INTEGRATION. •EVALUATION AND SUSTAINABILITY. LEGISLATIVE AIMS: CHW-CCDN IMPLEMENTATION WILL TARGET THE FOLLOWING LEGISLATIVE AIMS: 1) ACHIEVE EFFICIENCIES, 2) EXPAND ACCESS TO, COORDINATE, AND IMPROVE THE QUALITY OF BASIC HEALTH CARE SERVICES AND ASSOCIATED HEALTH OUTCOMES, AND 3) STRENGTHEN THE RURAL HEALTH CARE SYSTEM AS A WHOLE. PROPOSED SERVICE REGION: ALL TARGET SERVICE AREAS ARE IN IDAHO AND DESIGNATED RURAL: BLACKFOOT AND SHELLEY IN BINGHAM COUNTY, AND ARCO IN BUTTE COUNTY. TARGET POPULATION: THE POPULATION FOR THIS PROJECT COMPRISES SOUTHEAST RURAL IDAHOANS, INCLUDING ETHNIC MINORIT IES IN BINGHAM COUNTY AND BUTTE COUNTY. IN SOUTHEASTERN IDAHO 80% OF THE POPULATION IS WHITE, 4% ARE INDIGENOUS PEOPLE, 15% ARE HISPANIC. ALL COUNTIES INCLUDED IN THIS PROPOSAL (BINGHAM COUNTY AND BUTTE COUNTY) ARE RURAL AND DESIGNATED AS PRIMARY CARE HEALTH PROFESSIONAL SHORTAGE AREAS. NETWORK AND CONSORTIUM PARTNERS: BINGHAM HEALTHCARE IS A CAH/RHC, LOST RIVERS MEDICAL CENTER IS A CAH/RHC, PUBLIC HEALTH DISTRICT 6 IS THE SOUTHEASTERN PUBLIC HEALTH DISTRICT, THE IDAHO DIVISION OF PUBLIC HEALTH IS THE STATE’S PUBLIC HEALTH ADMINISTRATOR, CORNERSTONE WHOLE HEALTHCARE ORGANIZATION A PRIVATE NON-PROFIT, THE IDAHO COMMUNITY HEALTH WORKERS ASSOCIATION IS A PRIVATE NON-PROFIT, THE IDAHO STATE UNIVERSITY COMMUNITY HEALTH WORKERS TRAINING ACADEMY IS AN ACADEMIC INSTITUTION, AND STEELE MEMORIAL MEDICAL CENTER IS A CAH. EXPERIENCE IN SERVING RURAL UNDERSERVED POPULATIONS: BINGHAM HEALTHCARE IS A NON-PROFIT HEALTH SYSTEM ESTABLISHED IN 1950. THE BINGHAM HEALTHCARE SYSTEM HAS GROWN TO INTO AN INTEGRATIVE HEALTH SYSTEM THAT INCLUDES OVER 140 MEDICAL PROVIDERS AND OFFERS OVER 100 TYPES OF PATIENT SERVICES, INCLUDING INPATIENT, OUTPATIENT, CRITICAL CARE, EMERGENCY CARE, REHABILITATION, SAME-DAY SURGERY, AND TRANSITIONAL CARE SERVICES. BINGHAM HAS BECOME ONE OF THE LEADING AND AWARD-WINNING HEALTHCARE PROVIDERS IN RURAL SOUTHEAST IDAHO, SUPPORTING THE SURROUNDING RURAL/FRONTIER COMMUNITIES. AS THE LARGEST NETWORK OF DOCTORS, SURGEONS AND SPECIALISTS IN THE REGION, BINGHAM HEALTHCARE SERVICES INCLUDE A GROWING NUMBER OF HOSPITALS, SURGERY CENTERS, AND PHYSICIAN OFFICES IN UNDERSERVED COMMUNITIES.
