THE PENOBSCOT NATION'S POPULATION IS 2,398, WITH APPROX. 600 PEOPLE LIVING ON THE PENOBSCOT INDIAN RESERVATION. TRIBAL MEMBERS MAKE UP APPROXIMATELY 800 HOUSEHOLDS. THIS POPULATION HAS A MEDIAN HOUSEHOLD INCOME OF $36,607/YR., COMPARED TO THE MAINE MEDIAN OF $63,182/YR., WITH 19.1% LIVING BELOW THE FEDERAL POVERTY LINE (COMPARED TO 11.5% IN MAINE) AND 10% UNEMPLOYMENT RATE (COMPARED TO 2.8% IN MAINE). HIGH-SPEED INTERNET IS WIDELY RECOGNIZED AS A NECESSITY FOR FULL PARTICIPATION IN TODAYS SOCIETY. EMPLOYERS, EDUCATORS, BUSINESSES, HEALTHCARE PROVIDERS, AND CIVIC INSTITUTIONS EXPECT PEOPLE TO HAVE ACCESS TO COMPUTERS AND BROADBAND CONNECTIVITY. THE PENOBSCOT NATION HAS NEGOTIATED A CONTRACT WITH SPECTRUM, A REGIONAL INTERNET PROVIDER, TO A COST OF $25 PER MONTH PER HOUSEHOLD FOR THOSE LIVING ON TRIBAL LAND240 HOUSEHOLDS, AND $25 PER MONTH FOR TRIBAL MEMBER HOUSEHOLDS LIVING OFF TRIBAL LAND560. THESE NEWLY NEGOTIATED MONTHLY COSTS ARE DOWN FROM $134.99 PER MONTH PER HOUSEHOLD. THE DENTAL CLINIC WILL BE CO-LOCATED WITH PCHC’S PRIMARY CARE PRACTICE, SEAPORT COMMUNITY HEALTH CENTER (SCHC), AND AVAILABLE TO SCHC’S PRIMARY CARE PATIENTS AS WELL AS NON-PATIENTS. THE DENTAL PRACTICE WILL OFFER COMPREHENSIVE DENTAL SERVICES. DENTAL CARE ACCESS IN WALDO COUNTY IS EXTREMELY LIMITED, ESPECIALLY FOR MAINECARE, UNINSURED AND UNDERINSURED PATIENTS. THE U.S. HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) DESIGNATES THE BELFAST, WALDO COUNTY AREA AS A “HEALTH PROVIDER SHORTAGE AREA” IN BOTH MENTAL HEALTH AND DENTAL FIELDS AND CONSIDERS THE ENTIRE COUNTY TO BE A “MEDICALLY UNDERSERVED AREA.” ACCORDING TO COUNTY RURAL HEALTH PROFILE DATA, NEARLY 30% OF WALDO COUNTY RESIDENTS ARE AT 150% OR BELOW OF THE FEDERAL POVERTY LEVEL, INCLUDING 11% OF SENIORS, A SIGNIFICANTLY HIGHER RATE THAN THE STATE AVERAGE OF 8.7%. MEDIAN HOUSEHOLD INCOME IN WALDO COUNTY IS NEARLY 13% LOWER THAN THE STATE AVERAGE ($42,221), IN A STATE WHERE MEDIAN INCOME LAGS THE NATIONAL AVERAGE SIGNIFICANTLY. THERE IS A HIGHER RATE OF PUBLICLY INSURED AND UNINSURED PATIENTS IN WALDO COUNTY THAN IN THE REST OF MAINE. THOUGH PRIVATE PRACTICE DENTISTS EXIST IN THE REGION, THEY ACCEPT FEW, IF ANY MAINECARE PATIENTS AND DO NOT HAVE A SLIDING FEE SCALE FOR LOW INCOME, SELF-PAY PATIENTS. WITHOUT ACCESS TO COMPREHENSIVE DENTAL CARE, ORAL DISEASES PROGRESS RAPIDLY, BECOMING EMERGENT AND LEADING TO LOSS OF TEETH, WHICH EXACERBATES OTHER HEALTHCARE CONDITIONS. LACKING PREVENTIVE CARE, PEOPLE OVERUTILIZE HOSPITAL EMERGENCY ROOMS AND OTHER MEDICAL FACILITIES TO HELP MANAGE DENTAL PAIN, DRIVING UP THE TOTAL COST OF CARE TO THE HEALTHCARE SYSTEM. DENTAL PAIN IS AMONG THE TOP DRIVERS OF EMERGENCY ROOM UTILIZATION. ATTEMPTS TO SELF-MANAGE PREVENTABLE DENTAL PAIN ASSOCIATED WITH ORA L DISEASE CAN ALSO LEAD TO OPIOID ADDICTION AND OTHER SUBSTANCE USE DISORDERS (SUDS). MOREOVER, POOR ORAL HYGIENE LEADS TO POOR NUTRITION, LOW CONFIDENCE, LOWER EMPLOYMENT RATES, AND A DOWNWARD SPIRAL IN HEALTH AND WELLBEING AND SOCIOECONOMIC STATUS. IN NOVEMBER 2021, PCHC, MAINE’S LARGEST FQHC, PURCHASED THE FORMER BANK OF AMERICA COMPLEX IN BELFAST, MAINE IN ORDER TO SECURE SPACE TO EXPAND ITS PRIMARY CARE CLINIC. SCHC HAS PROVIDED INTEGRATED PRIMARY CARE IN THE REGION FOR OVER 40 YEARS, BUT IN THE PAST DECADE DEMAND FOR PRIMARY, MENTAL HEALTH AND SUBSTANCE USE DISORDER SERVICES HAS GROWN EXPONENTIALLY. PCHC RECENTLY RENOVATED AND NOW HOUSES SCHC’S EXPANDED PRIMARY CARE CLINIC ON THE FIRST FLOOR OF BUILDING 5 OF THE COMPLEX. CDS FUNDS WILL BE USED TO RENOVATE A PORTION OF THE SECOND FLOOR OF THIS BUILDING, APPROXIMATELY 4,500 SF, AND TO PURCHASE EQUIPMENT FOR A NEW DENTAL CLINIC. THE SCOPE OF THESE RENOVATIONS INCLUDES NEW INTERIOR WALLS, NEW ELECTRICAL, NEW HVAC DISTRIBUTION, AND THE INSTALLATION OF THE DENTAL EQUIPMENT NECESSARY FOR AN 8-CHAIR DENTAL CLINIC. THIS PROJECT WILL NOT REQUIRE RENOVATIONS TO THE EXTERIOR OF THE FACILITY OR GROUND DISTURBANCE. THIS REQUEST FOR CDS FUNDING WILL SIGNIFICANTLY IMPROVE ACCESS TO ORAL HEALTHCARE IN WALDO COUNTY FOR ALL, REGARDLESS OF ABILITY TO PAY. THE CLINIC WILL SERVE A PROJECTED 7,200 PATIENTS EACH YEAR. WE WILL ALSO PROVIDE ACUTE/WALK-IN DENTAL CARE SERVICES, SIGNIFICANTLY REDUCING ED UTILIZATION FOR EMERGENT DENTAL NEEDS. THE RENOVATION PROJECT WILL ADD APPROXIMATELY 15 FTE CONSTRUCTION JOBS. THE LONG-TERM GOAL OF THIS PROGRAM IS TO RAISE SCIENCE LITERACY ACROSS THE NATION AND PROVIDE CAREER READINESS TRAINING FOR THE NEXT GENERATION OF SCIENTISTS. PENOBSCOT COUNTY, WHERE THE RESERVATION IS LOCATED, HAS SEEN OVERDOSE FATALITIES PER 100,000 NEARLY DOUBLE FROM 2019 (34.8) TO 2020 (62.0) (MAINE SHARED COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) 2022). WHILE THE PUBLIC INTEREST IN THE OPIOID CRISIS SEEMS TO HAVE DECREASED, THE NUMBER OF OPIOID FATALITIES AND OPIOID-RELATED EMERGENCY VISITS CONTINUE TO RISE. THE NATIONS PENOBSCOT NATION HEALTH DEPARTMENTS (PNHD) OPIOID TREATMENT PROGRAM (ALSO KNOWN AS THE SUBOXONE PROGRAM), HAS NEARLY 100 PATIENTS PARTICIPATING AND ARE ENROLLING NEW PATIENTS REGULARLY. THE PROJECT WILL HIRE A PUBLIC DEFENDER TO REPRESENT PARTICIPANTS IN THE TRIBAL COURT AND/OR THE STATE OR FEDERAL SYSTEMS USING BRAIDED SERVICES INCORPORATING LEGAL, PSYCHOLOGICAL, AND SOCIOLOGICAL SUPPORT. THE PROJECT NAME IS TRIBAL STATE COLLABORATION TO ENHANCE THE DIVERSITY OF RECOVERY SERVICES IN RURAL AREAS OF NORTHERN MAINE. THE POPULATION TO BE SERVED INCLUDES ADULT AMERICAN INDIAN OR ALASKAN NATIVE TRIBAL NATION MEMBERS OF EVERY GENDER IDENTITY, THEIR SPOUSES/ PARTNERS WHO HAVE LIVED WITH THE TRIBAL NATION MEMBERS FOR SIX MONTHS OR LONGER, AND DESCENDANTS OF NATIVE AMERICANS WHO ARE NOT ON THE CENSUS WHO ARE DIAGNOSED WITH A SUBSTANCE USE DISORDER AND CHARGED WITH CRIMES WITHIN THE JURISDICTION OF THE PENOBSCOT NATION TRIBAL COURT OCCURRING ON THE NATION’S RESERVATIONS; WITHIN THE PENOBSCOT, KENNEBEC, CUMBERLAND, AND TWO ADJACENT COUNTY DISTRICTS; SUPERIOR COURT; OR FEDERAL COURT. THE STRATEGIES INCLUDE OFFERING BEST PRACTICES USED DURING THE CURRENT SAMSHA GRANT TO ADDRESS THE UNMET NEEDS OF DESCENDANTS NOT ON THE CENSUS AND THE SPOUSES/PARTNERS WHO HAVE LIVED WITH TRIBAL MEMBERS FOR SIX MONTHS OR LONGER. ADDRESSING THESE UNMET NEEDS WILL REDUCE THE TRIBAL MEMBER’S EXPOSURE TO SUBSTANCES, PARAPHERNALIA, AND THE USE OF ADDICTIVE SUBSTANCES. THE STRATEGIES ALSO INCLUDE OFFERING PROGRAM PARTICIPANTS ACCESS TO CULTURALLY SPECIFIC DETOXIFICATION AND RECOVERY FACILITIES IN, AND OUTSIDE, OF MAINE AND COVERING THE COSTS NOT PAID BY INSURANCE OR MAINE CARE. FINALLY, THE PROGRAM INCLUDES HIRING A FULL TIME PUBLIC DEFENDER TO REPRESENT PARTICIPANTS CHARGED IN THE TRIBAL COURT AND/OR THE STATE OR FEDERAL JUDICIAL SYSTEMS TO COORDINATE SERVICES, PROVIDE CULTURALLY SPECIFIC REPRESENTATION, AND REDUCE THE NUMBER OF FELONY CONVICTIONS. THE PROJECT WILL MINIMIZE PRE-TRIAL INCARCERATION, THE CONFINEMENT OF A PARTICIPANT IN JAIL FOR EXTENDED PERIODS WAITING FOR A PUBLIC DEFENDER TO ARRANGE BAIL OR BED TO BED TREATMENT. THE PROJECT GOALS AND MEASURABLE OBJECTIVES INCLUDE HELPING SEVENTY FIVE MEMBERS OF FEDERALLY RECOGNIZED TRIBES, THEIR SPOUSES/PARTNERS OF SIX MONTHS OR LONGER, AND DESCENDANTS OF NATIVE AMERICANS NOT ON THE CENSUS END INTERGENERATIONAL PATTERNS OF SUBSTANCE ABUSE, CRIMINAL BEHAVIOR, INCARCERATION, AND RELATED FAMILY AND COMMUNITY BREAKDOWN, BY EXPANDING ACCESS TO CULTURALLY SPECIFIC CARE, CASE MANAGEMENT, AND WRAPAROUND BRAIDED SERVICES. THE PROJECT WILL ESTABLISH AN INTERJURISDICTIONAL, MULTISYSTEM MODEL THAT REDUCES INCARCERATION RATES, REDUCES THE STIGMATIZATION OF INDIVIDUALS WITH A SUBSTANCE USE DISORDER, INCREASES ACCESS TO CULTURALLY SPECIFIC INTERVENTIONS AND TREATMENT SERVICES, AND CLOSES GAPS IN THE CONTINUUM OF SUBSTANCE ABUSE AND CO-OCCURRING DISORDER TREATMENT. THE INTERVENTIONS WILL DECREASE THE RELAPSE RATE FOR SEVENTY FIVE PARTICIPANTS BY OFFERING ACCESS TO CULTURALLY SPECIFIC TREATMENT FACILITIES IN MAINE AND SURROUNDING STATES, REGARDLESS OF THE PARTICIPANTS’ INSURANCE STATUS OR THE FACILITY’S ACCEPTANCE OF MAINE CARE. SASP WAS CREATED BY THE VIOLENCE AGAINST WOMEN AND DEPARTMENT OF JUSTICE REAUTHORIZATION ACT OF 2005 (VAWA 2005) AND IS THE FIRST FEDERAL FUNDING STREAM DEDICATED SOLELY TO THE PROVISION OF DIRECT INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT. OVERALL, THE PURPOSE OF SASP IS TO PROVIDE INTERVENTION, ADVOCACY, ACCOMPANIMENT (E.G., ACCOMPANYING VICTIMS TO COURT, MEDICAL FACILITIES, POLICE DEPARTMENTS), SUPPORT SERVICES, AND RELATED ASSISTANCE FOR ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, NON-OFFENDING FAMILY AND HOUSEHOLD MEMBERS OF VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE SEXUAL ASSAULT. TSASP SUPPORTS PROJECTS TO CREATE, MAINTAIN, AND EXPAND SUSTAINABLE SEXUAL ASSAULT SERVICES PROVIDED BY TRIBES, TRIBAL ORGANIZATIONS, AND NONPROFIT TRIBAL ORGANIZATIONS WITHIN INDIAN COUNTRY AND ALASKA NATIVE VILLAGES. WITH THIS FUNDING, THE PENOBSCOT NATION CURRENTLY HAS 2,365 ENROLLED MEMBERS (2023 TRIBAL CENSUS). OVER 1,400 TRIBAL MEMBERS LIVE IN MAINE AND ARE WITHIN SERVICEABLE DISTANCE, AND MANY VISIT THE RESERVATION REGULARLY. THERE ARE 425 TRIBAL MEMBERS LIVING ON THE RESERVATION AT INDIAN ISLAND, WHICH IS LOCATED IN PENOBSCOT COUNTY. INDIAN ISLAND HAS A TOTAL POPULATION OF 571 INCLUDING NON-ENROLLED PENOBSCOTS (1%), OTHER INDIAN RESIDENTS (3%) AND NON- INDIAN RESIDENTS (19%). INDIAN ISLAND AND IS PART OF THE GREATER OLD TOWN AREA WITHIN A 20-MILE RADIUS OF BANGOR, ME, AI/AN MAKE UP 0.9% OF THE COUNTYS POPULATION. PENOBSCOT COUNTY IS A DESIGNATED HEALTH PROFESSIONAL SHORTAGE AREA (HPSA) FOR PRIMARY CARE, MENTAL HEALTH AND DENTAL SERVICES, AN INDICATION OF THE DEPTH OF HEALTH CARE NEEDS IN THE AREA. THROUGH TSASP FUNDING THE PROPOSED PROJECT WILL STRENGTHEN AND EXPAND EXISTING PROGRAMMING IN MEETING NEEDS BY: 1. INCREASING TRIBAL CAPACITY TO RESPONSE TO SEXUAL ASSAULT; AND 2. THE PROGRAM SUPPORTS VICTIM-CENTERED APPROACHES TO PROVIDING TRANSITIONAL HOUSING, SHORT-TERM HOUSING ASSISTANCE, AND VOLUNTARY SUPPORT SERVICES TO ASSIST WITH LOCATING AND SECURING PERMANENT HOUSING, OBTAINING EMPLOYMENT, AND INTEGRATING INTO A COMMUNITY. PENOBSCOT INDIAN NATION, LOCATED IN INDIAN ISLAND, MAINE, WILL IMPLEMENT THE TRANSITIONAL HOUSING SERVICES PROJECT THROUGH THE PROVISION OF VICTIM-CENTERED APPROACHES TO TRANSITIONAL HOUSING NEEDS, IN COLLABORATION WITH WABANAKI WOMENS COALITION. THE PROJECT WILL PROVIDE EIGHT PRIVATE LANDLORD, SCATTERED SITE TRANSITIONAL HOUSING RESIDENCES FOR EIGHT SURVIVORS AND THEIR FAMILIES IN A TRIBAL REGION OF MAINE. THIS NEW PROJECT WILL USE FUNDS TO PROVIDE ONLY HOUSING SERVICES TO MOVE SURVIVORS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, OR STALKING WHO ARE HOMELESS OR IN NEED OF HOUSING ASSISTANCE TO PERMANENT HOUSING. THE PROJECT WILL ASSIST CLIENTS FOR AN ANTICIPATED MINIMUM OF 6 MONTHS AND A MAXIMUM LENGTH OF 24 MONTHS. RECENTLY OUR TEAM DESCRIBED A SYSTEM THAT USES BEADS TO RAPIDLY ENHANCE THE DISCOVERY OF PROBES THAT TARGET SPECIFIC DOMAINS WITHIN A PROTEIN. IN THIS PROGRAM OUR TEAM DEVELOPS THIS METHOD, AS A TECHNIQUE TO DISCOVER MOLECULAR PROBES TO STUDY THE NEUROLOGICAL ROLES OF THE IMMUNOPHILINS. THE EXTRACELLULAR AMYLOID PLAQUES AND THE INTRACELLULAR NEUROFIBRILLARY TANGLES (TAU) THAT DEFINE THE NEUROPATHOLOGY OF ALZHEIMER'S DISEASE ARE BOTH REGULATED BY THE LARGE IMMUNOPHILINS. CYCLOPHILIN 40 (CYP40), FK506-BINDING PROTEINS FKBP51 AND FKBP52 PLAY IMPORTANT, BUT ANTAGONISTIC ROLES. CYP40 DISAGGREGATES A-SYNUCLEIN AND OVEREXPRESSION RESCUES TAU-INDUCED COGNITIVE DEFICITS AND UNRAVELS NEUROTOXIC PLAQUES. BOTH CYP40 AND FKBP51 PROMOTE THE STABILISATION OF MICROTUBULES WHILE FKBP52 HAS BEEN SHOWN TO CAUSE MEMORY IMPAIRMENTS IN A TAU TRANSGENIC MOUSE MODEL AND TO PROMOTE TAU PATHOGENESIS. A LIKELY MECHANISM INVOLVES THE OPPOSING ROLES PLAYED BY CYP40 AND FKPB52 IN REGULATING MICROTUBULE DEPOLYMERISATION. FKBP52 HAS BEEN SHOWN TO BIND AND INHIBIT TUBULIN FORMATION VIA DIRECT BINDING BY ITS TPR DOMAIN. AS THE AMINO ACID IDENTITY BETWEEN THESE THREE LARGE IMMUNOPHILIN TPR DOMAINS IS LESS THAN 26%, IT WILL BE POSSIBLE TO IDENTIFY SMALL MOLECULE BINDERS THAT DISTINGUISH BETWEEN THE TPR DOMAINS OF CYP40, FKBP51 AND FKBP52. MODULATING THE EFFECTS OF THE LARGE IMMUNOPHILINS WILL PROVIDE A NEW FORM OF THERAPY TO TREAT NEURODEGENERATIVE DISEASES CAUSED BY PLAQUE FORMATION INCLUDING ALZHEIMER'S DISEASE. THE PENOBSCOT INDIAN NATION HAS BEEN A MARGINALIZED COMMUNITY IN THE STATE OF MAINE FOR CENTURIES. THE PENOBSCOT NATION'S POPULATION IS 2,398, WITH APPROX. 600 PEOPLE LIVING ON THE PENOBSCOT INDIAN RESERVATION. TRIBAL MEMBERS MAKE UP APPROXIMATELY 800 HOUSEHOLDS. THIS POPULATION HAS A MEDIAN HOUSEHOLD INCOME OF $36,607/YR., COMPARED TO THE MAINE MEDIAN OF $63,182/YR., WITH 19.1% LIVING BELOW THE FEDERAL POVERTY LINE (COMPARED TO 11.5% IN MAINE) AND 10% UNEMPLOYMENT RATE (COMPARED TO 2.8% IN MAINE). HIGH-SPEED INTERNET IS WIDELY RECOGNIZED AS A NECESSITY FOR FULL PARTICIPATION IN TODAYS SOCIETY. EMPLOYERS, EDUCATORS, BUSINESSES, HEALTHCARE PROVIDERS, AND CIVIC INSTITUTIONS EXPECT PEOPLE TO HAVE ACCESS TO COMPUTERS AND BROADBAND CONNECTIVITY. THE PENOBSCOT NATION HAS NEGOTIATED A CONTRACT WITH SPECTRUM, A REGIONAL INTERNET PROVIDER, TO A COST OF $25 PER MONTH PER HOUSEHOLD FOR THOSE LIVING ON TRIBAL LAND240 HOUSEHOLDS, AND $25 PER MONTH FOR TRIBAL MEMBER HOUSEHOLDS LIVING OFF TRIBAL LAND560. THESE NEWLY NEGOTIATED MONTHLY COSTS ARE DOWN FROM $134.99 PER MONTH PER HOUSEHOLD. THROUGH THIS GRANT, FOR THE DURATION OF THE GRANT PERIOD, WE WILL COVER THE $25 COST PER MONTH PER HOUSEHOLD THAT REMAINS TO PROVIDE FULL INTERNET CONNECTIVITY TO ALL TRIBAL MEMBER HOUSEHOLDS. CURRENTLY, WE HAVE A 20-HOUR-PER-WEEK FUNDED DIGITAL NAVIGATOR POSITION THROUGH THE NATIONAL DIGITAL EQUITY CENTER (NDEC). THROUGH THIS GRANT, WE WILL MAKE THIS POSITION FULL-TIME. THE DENTAL CLINIC WILL BE CO-LOCATED WITH PCHC’S PRIMARY CARE PRACTICE, SEAPORT COMMUNITY HEALTH CENTER (SCHC), AND AVAILABLE TO SCHC’S PRIMARY CARE PATIENTS AS WELL AS NON-PATIENTS. THE DENTAL PRACTICE WILL OFFER COMPREHENSIVE DENTAL SERVICES. DENTAL CARE ACCESS IN WALDO COUNTY IS EXTREMELY LIMITED, ESPECIALLY FOR MAINECARE, UNINSURED AND UNDERINSURED PATIENTS. THE U.S. HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) DESIGNATES THE BELFAST, WALDO COUNTY AREA AS A “HEALTH PROVIDER SHORTAGE AREA” IN BOTH MENTAL HEALTH AND DENTAL FIELDS AND CONSIDERS THE ENTIRE COUNTY TO BE A “MEDICALLY UNDERSERVED AREA.” ACCORDING TO COUNTY RURAL HEALTH PROFILE DATA, NEARLY 30% OF WALDO COUNTY RESIDENTS ARE AT 150% OR BELOW OF THE FEDERAL POVERTY LEVEL, INCLUDING 11% OF SENIORS, A SIGNIFICANTLY HIGHER RATE THAN THE STATE AVERAGE OF 8.7%. MEDIAN HOUSEHOLD INCOME IN WALDO COUNTY IS NEARLY 13% LOWER THAN THE STATE AVERAGE ($42,221), IN A STATE WHERE MEDIAN INCOME LAGS THE NATIONAL AVERAGE SIGNIFICANTLY. THERE IS A HIGHER RATE OF PUBLICLY INSURED AND UNINSURED PATIENTS IN WALDO COUNTY THAN IN THE REST OF MAINE. THOUGH PRIVATE PRACTICE DENTISTS EXIST IN THE REGION, THEY ACCEPT FEW, IF ANY MAINECARE PATIENTS AND DO NOT HAVE A SLIDING FEE SCALE FOR LOW INCOME, SELF-PAY PATIENTS. WITHOUT ACCESS TO COMPREHENSIVE DENTAL CARE, ORAL DISEASES PROGRESS RAPIDLY, BECOMING EMERGENT AND LEADING TO LOSS OF TEETH, WHICH EXACERBATES OTHER HEALTHCARE CONDITIONS. LACKING PREVENTIVE CARE, PEOPLE OVERUTILIZE HOSPITAL EMERGENCY ROOMS AND OTHER MEDICAL FACILITIES TO HELP MANAGE DENTAL PAIN, DRIVING UP THE TOTAL COST OF CARE TO THE HEALTHCARE SYSTEM. DENTAL PAIN IS AMONG THE TOP DRIVERS OF EMERGENCY ROOM UTILIZATION. ATTEMPTS TO SELF-MANAGE PREVENTABLE DENTAL PAIN ASSOCIATED WITH ORA L DISEASE CAN ALSO LEAD TO OPIOID ADDICTION AND OTHER SUBSTANCE USE DISORDERS (SUDS). MOREOVER, POOR ORAL HYGIENE LEADS TO POOR NUTRITION, LOW CONFIDENCE, LOWER EMPLOYMENT RATES, AND A DOWNWARD SPIRAL IN HEALTH AND WELLBEING AND SOCIOECONOMIC STATUS. IN NOVEMBER 2021, PCHC, MAINE’S LARGEST FQHC, PURCHASED THE FORMER BANK OF AMERICA COMPLEX IN BELFAST, MAINE IN ORDER TO SECURE SPACE TO EXPAND ITS PRIMARY CARE CLINIC. SCHC HAS PROVIDED INTEGRATED PRIMARY CARE IN THE REGION FOR OVER 40 YEARS, BUT IN THE PAST DECADE DEMAND FOR PRIMARY, MENTAL HEALTH AND SUBSTANCE USE DISORDER SERVICES HAS GROWN EXPONENTIALLY. PCHC RECENTLY RENOVATED AND NOW HOUSES SCHC’S EXPANDED PRIMARY CARE CLINIC ON THE FIRST FLOOR OF BUILDING 5 OF THE COMPLEX. CDS FUNDS WILL BE USED TO RENOVATE A PORTION OF THE SECOND FLOOR OF THIS BUILDING, APPROXIMATELY 4,500 SF, AND TO PURCHASE EQUIPMENT FOR A NEW DENTAL CLINIC. THE SCOPE OF THESE RENOVATIONS INCLUDES NEW INTERIOR WALLS, NEW ELECTRICAL, NEW HVAC DISTRIBUTION, AND THE INSTALLATION OF THE DENTAL EQUIPMENT NECESSARY FOR AN 8-CHAIR DENTAL CLINIC. THIS PROJECT WILL NOT REQUIRE RENOVATIONS TO THE EXTERIOR OF THE FACILITY OR GROUND DISTURBANCE. THIS REQUEST FOR CDS FUNDING WILL SIGNIFICANTLY IMPROVE ACCESS TO ORAL HEALTHCARE IN WALDO COUNTY FOR ALL, REGARDLESS OF ABILITY TO PAY. THE CLINIC WILL SERVE A PROJECTED 7,200 PATIENTS EACH YEAR. WE WILL ALSO PROVIDE ACUTE/WALK-IN DENTAL CARE SERVICES, SIGNIFICANTLY REDUCING ED UTILIZATION FOR EMERGENT DENTAL NEEDS. THE RENOVATION PROJECT WILL ADD APPROXIMATELY 15 FTE CONSTRUCTION JOBS. THE CLINIC WILL EMPLOY 13-15 FTE PERMANENT DENTAL TEAM MEMBERS. ADDING DENTAL SERVICES IS A WIN FOR THE COMMUNITY AND THE HEALTHCARE SYSTEM: IT WILL INCREASE JOBS, SUPPORT ECONOMIC DEVELOPMENT, EFFICIENTLY LEVERAGE TAXPAYER DOLLARS FLOWING TO FQHCS, AND REDUCE THE TOTAL COST OF CARE TO MAINE’S HEALTHCARE SYSTEM.

SBIR PHASE II: A WEB-APPLICATION THAT ENABLES HANDS-ON BIOTECH LABORATORY EDUCATION FOR STUDENTS REGARDLESS OF SOCIOECONOMIC STATUS -THE BROADER IMPACT/COMMERCIAL POTENTIAL OF THIS SMALL BUSINESS INNOVATION RESEARCH (SBIR) PHASE II PROJECT IS EQUITABLE ACCESS TO A QUALITY SCIENCE EDUCATION REGARDLESS OF SOCIOECONOMIC STATUS OR GEOGRAPHIC LOCATION. ACROSS THE NATION, SCIENCE ILLITERACY IS AN ONGOING ISSUE WITH ONLY 30% OF STUDENTS TESTING AS HAVING A BASIC KNOWLEDGE OF SCIENCE. AS A RESULT, THE U.S. DOES NOT HAVE ENOUGH TRAINED SCIENTISTS TO COMMERCIAL OR ACADEMIC LABORATORIES. THIS SITUATION HAS BEEN DEEMED A NATIONAL CRISIS AND IS CONSIDERED A RISK TO U.S. NATIONAL SECURITY. A QUALITY SCIENCE EDUCATION HAS LASTING BENEFITS TO STUDENTS INCLUDING MORE PERSUASIVE AND CLEAR COMMUNICATION SKILLS, AND THE ABILITY TO DESIGN AND DEVELOP CRITICAL APPROACHES TO PROBLEM-SOLVING. THE LONG-TERM GOAL OF THIS PROGRAM IS TO RAISE SCIENCE LITERACY ACROSS THE NATION AND PROVIDE CAREER READINESS TRAINING FOR THE NEXT GENERATION OF SCIENTISTS. THE PROGRAM MEETS ALL THREE DIMENSIONS OF NEXT GENERATION SCIENCE STANDARDS. A BOOK VERSION OF THE PROGRAM IS BEING ROLLED-OUT TO INDEPENDENT STUDY CHARTER SCHOOLS IN CALIFORNIA. THE ONLINE PROGRAM WILL BE TESTED IN SCHOOLS WHICH HAVE ADOPTED THE BOOKS. BOTH BOOK AND ONLINE VERSIONS WILL BE EXPANDED TO CHARTER SCHOOLS, PRIVATE SCHOOLS, AND EVENTUALLY PUBLIC SCHOOLS. MODULES OF THE PROGRAM MAY BE ADOPTED IN AREAS WITHOUT ACCESS TO HIGH-SPEED INTERNET. THIS SMALL BUSINESS INNOVATION RESEARCH (SBIR) PHASE II PROJECT ADDRESSES INEQUITIES IN SCIENCE EDUCATION BY PROVIDING AN INDEPENDENT STUDY APPLICATION THAT INCLUDES STEP-BY-STEP INSTRUCTION FOR A HANDS-ON CHEMICAL LABORATORY. ALL HANDS-ON LABORATORIES USE REAGENTS COMMONLY FOUND IN THE HOME FOR SAFETY AND ACCESSIBILITY REASONS. EXPERIMENTS ARE DESIGNED TO BE DONE WITHOUT THE NEED FOR A TRAINED SCIENCE TEACHER IN THE ROOM. STUDIES SHOW THAT HANDS-ON LEARNING IS MORE IMPACTFUL THAN COMPUTER SIMULATIONS. LOW-COST SCIENTIFIC EQUIPMENT IS ALSO BEING DEVELOPED. A HAND-POWERED SPINNING CENTRIFUGE IS UNDER FINAL DESIGN. DURING PHASE II, A DRUG DISCOVERY MODULE WILL BE DEVELOPED WHERE STUDENTS CAN FIND A POTENTIAL CURE FOR A DISEASE IN THEIR OWN KITCHEN OR CLASSROOM. AIMS ALSO INCLUDE MODIFYING THE ONLINE APPLICATION TO MAKE IT EASILY EXPANDABLE, SCALABLE AND MAINTAINABLE FOR THE LONG TERM. THE APPLICATION DEVELOPMENT INCLUDES TECHNICAL OPTIMIZATION FOR RELIABLE DEPLOYMENT NATIONWIDE AND NEW INTERFACES TO ALLOW EDUCATORS TO UPLOAD CONTENT DIRECTLY INTO THE APPLICATION. DESCRIPTION:THIS AGREEMENT PROVIDES FUNDING UNDER THE INFLATION REDUCTION ACT (IRA). THE AGREEMENT PROVIDES FUNDING TO THE PENOBSCOT INDIAN NATION. SPECIFICALLY, THE PROJECT WILL WORK WITH A CONTRACTOR TO INSTALL UP TO 180 ENERGY EFFICIENT HEATING SYSTEMS IN TRIBAL HOUSING UNITS. THE COOPERATIVE AGREEMENT IS A PARTNERSHIP AMONG THE WABANAKI ALLIANCE. THE RECIPIENT WILL WORK WITH OTHER WABANAKI NATIONS AS SUBRECIPIENTS TO MANAGE THEIR OWN BUDGETS. EACH NATION WILL WORK WITH THE COMMUNITY TO IDENTIFY CANDIDATE HOMES FOR HEAT PUMP INSTALLATION. EACH NATION WILL WORK WITH THEIR OWN CONTRACTOR TO COMPLETE THE INSTALLATIONS. THE PROJECT WILL INCLUDE COMMUNITY OUTREACH AND EDUCATION RELATED TO CLIMATE RESILIENCE AND IMPROVING SUSTAINABILITY FOR MEMBERS OF WABANAKI NATIONS.ACTIVITIES:THE ACTIVITIES WILL ESTABLISH A CO-MANAGED PLAN AMONG FOUR THE WABANAKI TRIBES AS A CONSORTIUM. THE PENOBSCOT INDIAN NATION WILL BE THE GRANTEE AND MANAGE THE GRANT. THE CONSORTIUM WILL ESTABLISH CRITERIA FOR RECEIVING A HEAT PUMP TO ENSURE UNITS WITH THE GREATEST NEED ARE ADDRESSED. THE CONTRACT IMPLEMENTATION WILL BE EQUALLY DIVIDED AS SUB-AWARDS, DUE TO THE GEOGRAPHICAL DISTANCE BETWEEN THE FOUR TRIBES OF THE WABANAKI ALLIANCE. THIS SPREADS THE MANAGEMENT AND FUNDING AMONGST FOUR TRIBES AND HELPS TO ENSURE THAT HEAT PUMPS WILL BE DEPLOYED TO THE HOUSEHOLDS WITH THE GREATEST NEED. EACH TRIBE'S PROGRAM MANAGER WILL BE RESPONSIBLE FOR REPORTING THE PROGRAM EVALUATION AND SUBMITTING DATA TO THE PENOBSCOT INDIAN NATION FOR COMPILATION AND COLLABORATION. A PROPERTY NEEDS ASSESSMENT AND SITE VISIT WILL BE INCORPORATED IN THE APPLICATION PROCESS. THIS PROVIDES AN OPPORTUNITY FOR THE PROGRAM MANAGER TO IDENTIFY ADDITIONAL NEEDS TO BE ADDRESSED WITHIN THE HOME. THIS NEEDS ASSESSMENT WILL ALSO HELP DETERMINE THE SIZE OF THE HEAT PUMP THAT IS NEEDED FOR THE HOME. THIS IS CRUCIAL TO MAINTAINING A BUDGET WHILE INSTALLING THE MOST HEAT PUMPS IN HOMES. EACH TRIBE WILL MANAGE AN OPEN BIDDING PROCESS THAT WILL ENGAGE QUALIFIED VENDORS TO INSTALL THE HEAT PUMPS IN THE SELECTED HOMES. THE PROCUREMENT PROCESS WILL INCLUDE NATIVE PREFERENCE TO INCREASE ACCESS TO CONTRACTS FOR TRIBAL CITIZENS. CONTRACTS WILL BE BUNDLED BY TRIBE AND WILL VARY DEPENDING ON THE TRIBE. THE WABANAKI ALLIANCE WILL BE WORKING WITH EFFICIENCY MAINE TO MAXIMIZE THE VARIOUS INCENTIVES, REBATES, AND TAX CREDITS AVAILABLE FOR HEAT PUMPS. THIS PROGRAM IS EXPECTED TO INSTALL AT LEAST 45 HEAT PUMPS PER TRIBE, DEPENDING ON INCENTIVES AND CONTRACTOR BIDS. PENOBSCOT COUNTY, WHERE THE RESERVATION IS LOCATED, HAS SEEN OVERDOSE FATALITIES PER 100,000 NEARLY DOUBLE FROM 2019 (34.8) TO 2020 (62.0) (MAINE SHARED COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) 2022). WHILE THE PUBLIC INTEREST IN THE OPIOID CRISIS SEEMS TO HAVE DECREASED, THE NUMBER OF OPIOID FATALITIES AND OPIOID-RELATED EMERGENCY VISITS CONTINUE TO RISE. THE NATIONS PENOBSCOT NATION HEALTH DEPARTMENTS (PNHD) OPIOID TREATMENT PROGRAM (ALSO KNOWN AS THE SUBOXONE PROGRAM), HAS NEARLY 100 PATIENTS PARTICIPATING AND ARE ENROLLING NEW PATIENTS REGULARLY. GOAL 1: TO BREAK THE CYCLE OF CRIMINAL BEHAVIOR, ALCOHOL AND/OR DRUG USE, INCARCERATION, OTHER PENALTIES, AND FAMILY AND COMMUNITY BREAKDOWN, BY EXPANDING SUBSTANCE USE DISORDER TREATMENT, CASE MANAGEMENT AND WRAPAROUND RECOVERY SERVICES AVAILABLE TO NATIVE AMERICANS THROUGH THE HTWC. GOAL 2: TO ESTABLISH AN INTERJURISDICTIONAL, MULTISYSTEM HTWC MODEL FOR PENOBSCOT COUNTY THAT REDUCES NATIVE AMERICAN INCARCERATION RATES, INCREASES ACCESS TO CULTURALLY RESPONSIVE INTERVENTIONS AND TREATMENT SERVICES, AND CLOSES GAPS IN THE CONTINUUM OF SUD AND CO-OCCURRING DISORDER TREATMENT. GOAL 3: TO INCREASE THE NUMBER OF HTWC PARTICIPANTS WHO SUCCESSFULLY COMPLETE THE PROGRAM, BECOME, AND REMAIN SOBER, AND ARE RESTORED AS PRODUCTIVE FAMILY AND COMMUNITY MEMBERS BY CONDUCT A PROGRAM PROCESS AND OUTCOME EVALUATION AND IMPLEMENTING RECOMMENDED PROGRAM MODIFICATIONS. GOAL 4: INCREASE THE SUCCESS RATE AND DECREASE THE RELAPSE RATE OF HTWC PARTICIPANTS BY IMPLEMENTING A WELL-TRAINED CULTURALLY RESPONSIVE PEER RECOVERY PROGRAM FOR HTWC PARTICIPANTS AND GRADUATES.

THE GRANTS TO INDIAN TRIBAL GOVERNMENTS PROGRAM (REFERRED TO AS THE TRIBAL GOVERNMENTS PROGRAM) WAS AUTHORIZED TO ASSIST TRIBAL GOVERNMENTS AND AUTHORIZED DESIGNEES OF TRIBAL GOVERNMENTS TO RESPOND TO DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, SEX TRAFFICKING, AND STALKING IN THEIR COMMUNITIES. THE TRIBAL GOVERNMENTS PROGRAM SUPPORTS PROJECTS THAT: (1) DECREASE THE INCIDENCE OF VIOLENT CRIME AGAINST INDIAN WOMEN; (2) STRENGTHEN THE CAPACITY OF INDIAN TRIBES TO EXERCISE THEIR SOVEREIGN AUTHORITY TO RESPOND TO VIOLENT CRIMES COMMITTED AGAINST INDIAN WOMEN; AND (3) ENSURE THAT PERPETRATORS OF VIOLENT CRIMES COMMITTED AGAINST INDIAN WOMEN ARE HELD ACCOUNTABLE FOR THEIR CRIMINAL BEHAVIOR. TRIBAL/STATE COLLABORATION TO ENHANCE THE DELIVERY OF RECOVERY SERVICES TO RURAL COMMUNITIES IN NORTHERN MAINE - PROJECT ABSTRACT SUMMARY THE PENOBSCOT NATION TRIBAL HEALING TO WELLNESS COURT WILL IMPLEMENT THE TRIBAL STATE COLLABORATION TO ENHANCE THE DIVERSITY OF RECOVERY SERVICES IN RURAL AREAS OF NORTHERN MAINE TO SERVE TRIBAL NATION MEMBERS, THEIR SPOUSES/ PARTNERS, AND DESCENDANTS. THE PROJECT WILL HIRE A PUBLIC DEFENDER TO REPRESENT PARTICIPANTS IN THE TRIBAL COURT AND/OR THE STATE OR FEDERAL SYSTEMS USING BRAIDED SERVICES INCORPORATING LEGAL, PSYCHOLOGICAL, AND SOCIOLOGICAL SUPPORT. THE PROJECT NAME IS TRIBAL STATE COLLABORATION TO ENHANCE THE DIVERSITY OF RECOVERY SERVICES IN RURAL AREAS OF NORTHERN MAINE. THE POPULATION TO BE SERVED INCLUDES ADULT AMERICAN INDIAN OR ALASKAN NATIVE TRIBAL NATION MEMBERS OF EVERY GENDER IDENTITY, THEIR SPOUSES/ PARTNERS WHO HAVE LIVED WITH THE TRIBAL NATION MEMBERS FOR SIX MONTHS OR LONGER, AND DESCENDANTS OF NATIVE AMERICANS WHO ARE NOT ON THE CENSUS WHO ARE DIAGNOSED WITH A SUBSTANCE USE DISORDER AND CHARGED WITH CRIMES WITHIN THE JURISDICTION OF THE PENOBSCOT NATION TRIBAL COURT OCCURRING ON THE NATION’S RESERVATIONS; WITHIN THE PENOBSCOT, KENNEBEC, CUMBERLAND, AND TWO ADJACENT COUNTY DISTRICTS; SUPERIOR COURT; OR FEDERAL COURT. THE STRATEGIES INCLUDE OFFERING BEST PRACTICES USED DURING THE CURRENT SAMSHA GRANT TO ADDRESS THE UNMET NEEDS OF DESCENDANTS NOT ON THE CENSUS AND THE SPOUSES/PARTNERS WHO HAVE LIVED WITH TRIBAL MEMBERS FOR SIX MONTHS OR LONGER. ADDRESSING THESE UNMET NEEDS WILL REDUCE THE TRIBAL MEMBER’S EXPOSURE TO SUBSTANCES, PARAPHERNALIA, AND THE USE OF ADDICTIVE SUBSTANCES. THE STRATEGIES ALSO INCLUDE OFFERING PROGRAM PARTICIPANTS ACCESS TO CULTURALLY SPECIFIC DETOXIFICATION AND RECOVERY FACILITIES IN, AND OUTSIDE, OF MAINE AND COVERING THE COSTS NOT PAID BY INSURANCE OR MAINE CARE. FINALLY, THE PROGRAM INCLUDES HIRING A FULL TIME PUBLIC DEFENDER TO REPRESENT PARTICIPANTS CHARGED IN THE TRIBAL COURT AND/OR THE STATE OR FEDERAL JUDICIAL SYSTEMS TO COORDINATE SERVICES, PROVIDE CULTURALLY SPECIFIC REPRESENTATION, AND REDUCE THE NUMBER OF FELONY CONVICTIONS. THE PROJECT WILL MINIMIZE PRE-TRIAL INCARCERATION, THE CONFINEMENT OF A PARTICIPANT IN JAIL FOR EXTENDED PERIODS WAITING FOR A PUBLIC DEFENDER TO ARRANGE BAIL OR BED TO BED TREATMENT. THE PROJECT GOALS AND MEASURABLE OBJECTIVES INCLUDE HELPING SEVENTY FIVE MEMBERS OF FEDERALLY RECOGNIZED TRIBES, THEIR SPOUSES/PARTNERS OF SIX MONTHS OR LONGER, AND DESCENDANTS OF NATIVE AMERICANS NOT ON THE CENSUS END INTERGENERATIONAL PATTERNS OF SUBSTANCE ABUSE, CRIMINAL BEHAVIOR, INCARCERATION, AND RELATED FAMILY AND COMMUNITY BREAKDOWN, BY EXPANDING ACCESS TO CULTURALLY SPECIFIC CARE, CASE MANAGEMENT, AND WRAPAROUND BRAIDED SERVICES. THE PROJECT WILL ESTABLISH AN INTERJURISDICTIONAL, MULTISYSTEM MODEL THAT REDUCES INCARCERATION RATES, REDUCES THE STIGMATIZATION OF INDIVIDUALS WITH A SUBSTANCE USE DISORDER, INCREASES ACCESS TO CULTURALLY SPECIFIC INTERVENTIONS AND TREATMENT SERVICES, AND CLOSES GAPS IN THE CONTINUUM OF SUBSTANCE ABUSE AND CO-OCCURRING DISORDER TREATMENT. THE INTERVENTIONS WILL DECREASE THE RELAPSE RATE FOR SEVENTY FIVE PARTICIPANTS BY OFFERING ACCESS TO CULTURALLY SPECIFIC TREATMENT FACILITIES IN MAINE AND SURROUNDING STATES, REGARDLESS OF THE PARTICIPANTS’ INSURANCE STATUS OR THE FACILITY’S ACCEPTANCE OF MAINE CARE. THROUGHOUT THE PROJECT, ATTENTION WILL BE GIVEN TOWARD MAINTAINING THE PENOBSCOT NATION TRIBAL HEALING TO WELLNESS COURT’S HIGH OPERATIONAL STANDARDS AS A NATIONAL DRUG COURT LEADER. FIFTEEN PEOPLE WILL BE SERVED ANNUALLY AND SEVENTY-FIVE WILL BE SERVED OVER THE ENTIRE PROJECT.

THE TRIBAL SEXUAL ASSAULT SERVICES PROGRAM (TSASP), WHICH IS AUTHORIZED BY 34 U.S.C. 12511(E), IS ONE OF FIVE GRANT PROGRAMS FUNDED WITHIN THE SEXUAL ASSAULT SERVICES PROGRAM (SASP). SASP WAS CREATED BY THE VIOLENCE AGAINST WOMEN AND DEPARTMENT OF JUSTICE REAUTHORIZATION ACT OF 2005 (VAWA 2005) AND IS THE FIRST FEDERAL FUNDING STREAM DEDICATED SOLELY TO THE PROVISION OF DIRECT INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT. OVERALL, THE PURPOSE OF SASP IS TO PROVIDE INTERVENTION, ADVOCACY, ACCOMPANIMENT (E.G., ACCOMPANYING VICTIMS TO COURT, MEDICAL FACILITIES, POLICE DEPARTMENTS), SUPPORT SERVICES, AND RELATED ASSISTANCE FOR ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, NON-OFFENDING FAMILY AND HOUSEHOLD MEMBERS OF VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE SEXUAL ASSAULT. TSASP SUPPORTS PROJECTS TO CREATE, MAINTAIN, AND EXPAND SUSTAINABLE SEXUAL ASSAULT SERVICES PROVIDED BY TRIBES, TRIBAL ORGANIZATIONS, AND NONPROFIT TRIBAL ORGANIZATIONS WITHIN INDIAN COUNTRY AND ALASKA NATIVE VILLAGES. WITH THIS FUNDING, THE PENOBSCOT NATION CURRENTLY HAS 2,365 ENROLLED MEMBERS (2023 TRIBAL CENSUS). OVER 1,400 TRIBAL MEMBERS LIVE IN MAINE AND ARE WITHIN SERVICEABLE DISTANCE, AND MANY VISIT THE RESERVATION REGULARLY. THERE ARE 425 TRIBAL MEMBERS LIVING ON THE RESERVATION AT INDIAN ISLAND, WHICH IS LOCATED IN PENOBSCOT COUNTY. INDIAN ISLAND HAS A TOTAL POPULATION OF 571 INCLUDING NON-ENROLLED PENOBSCOTS (1%), OTHER INDIAN RESIDENTS (3%) AND NON- INDIAN RESIDENTS (19%). INDIAN ISLAND AND IS PART OF THE GREATER OLD TOWN AREA WITHIN A 20-MILE RADIUS OF BANGOR, ME, AI/AN MAKE UP 0.9% OF THE COUNTYS POPULATION. PENOBSCOT COUNTY IS A DESIGNATED HEALTH PROFESSIONAL SHORTAGE AREA (HPSA) FOR PRIMARY CARE, MENTAL HEALTH AND DENTAL SERVICES, AN INDICATION OF THE DEPTH OF HEALTH CARE NEEDS IN THE AREA. THROUGH TSASP FUNDING THE PROPOSED PROJECT WILL STRENGTHEN AND EXPAND EXISTING PROGRAMMING IN MEETING NEEDS BY: 1. INCREASING TRIBAL CAPACITY TO RESPONSE TO SEXUAL ASSAULT; AND 2. TO ENHANCE SERVICES TO INDIAN WOMEN VICTIMIZED. THIS AWARDWILL BE A THREE-YEAR PROJECT.

SERVICES HAVE INCLUDED: INDIVIDUAL CONSULTATIONS, SUPPORT GROUPS, WEIGHT LOSS PROGRAMS, MULTI-DISCIPLINARY DIABETES CLINICS, COOKING CLASSES FOR ADULTS AND YOUTH, FITNESS ROOM STAFFED BY A PERSONAL TRAINER, VARIOUS EXERCISE PROGRAMS INCLUDING; WATER AEROBICS, STEP AEROBICS, SWIMMING, BODY PUMP, ZUMBA, TAI CHI, WALKING CLUBS, FUN RUNS, AND YOGA. THE KEY DIABETES-RELATED HEALTH ISSUES IDENTIFIED BY OUR COMMUNITY AND LOCAL LEADERSHIP ARE OBESITY, HYPERLIPIDEMIA, AND HYPERTENSION. SDPI FUNDING WILL ENABLE THE PENOBSCOT NATION TO CONTINUE AND EXPAND TARGETED DIABETIC SERVICES TO ITS TRIBAL COMMUNITY.

COMMUNITY OPIOID INTERVENTION PROJECT - THE PENOBSCOT NATION HEALTH DEPARTMENT (PNHD) IS REQUESTING $173,330 IN FUNDING FROM THE INDIAN HEALTH SERVICES (IHS) FISCAL YEAR 2021 COMMUNITY OPIOID INTERVENTION PILOT PROJECT (COIPP). IF AWARDED, IHS WILL APPROVE UP TO $500,000 PER YEAR FOR A THREE-YEAR PROJECT PERIOD. THIS FUNDING REQUEST WILL FACILITATE THE PNHD?S EFFORTS TO DEVELOP AND EXPAND COMMUNITY AWARENESS AND EDUCATION ABOUT CULTURALLY-APPROPRIATE AND FAMILY-CENTERED OPIOID PREVENTION, TREATMENT AND RECOVERY ACTIVITIES. THE PENOBSCOT NATION HEALTH DEPARTMENT IS A TRIBALLY OPERATED TITLE V COMPACT FACILITY FUNDED, IN PART, BY INDIAN HEALTH SERVICES. THE ONLY TRIBAL FEDERALLY QUALIFIED HEALTH CENTER (FQHC) IN THE STATE OF MAINE, PNHD HAS PROVIDED MEDICATION-ASSISTED THERAPY (MAT) FOR THE TREATMENT OF OPIOID DEPENDENCE FOR OVER A DECADE. PNHD?S PLAN TARGETS CULTURALLY APPROPRIATE COMMUNITY AWARENESS AND EDUCATION THROUGH: A COMPREHENSIVE MARKETING PLAN TO RAISE AWARENESS OF EXISTING OPIOID-SPECIFIC SERVICES; THE IMPLEMENTATION OF A CONSISTENT, CULTURALLY-BASED ISLAND SCHOOL PROGRAM FOR STUDENTS IN GRADES 1-8; A SAFETY CAMPAIGN TO PROVIDE MEDICATION LOCK BOXES TO ALL HOUSEHOLDS ON THE RESERVATION AS WELL AS A COMPREHENSIVE ?TAKE-BACK? PROGRAM THAT WILL INCLUDE THE HEALTH CENTER, PHARMACY AND POLICE DEPARTMENT; YOUTH, FAMILY AND MAT PROGRAM PARTICIPANT ENGAGEMENT ACTIVITIES THAT WILL PROMOTE A CONNECTION TO THE EARTH, TRIBAL TRADITIONS AND A HEALTHY LIFESTYLE. PNHD?S PLAN WILL EXPAND MAT SERVICES THROUGH: PROVIDER CONTINUING EDUCATION; DISTRIBUTION AND EDUCATION ON THE USE OF NALOXONE; AND TELEMAT SERVICES. THE PLAN WILL ALSO STRENGTHEN THE EXISTING SUPPORT SYSTEM FOR RECOVERY BY INCREASING COMMUNITY PARTNERSHIPS.THE ADDITION OF A PROJECT COORDINATOR POSITION WILL ALLOW FOR THE PLANNING, TRACKING AND FACILITATION OF ALL ACTIVITIES WITHIN PNHD?S PLAN AND WILL ENSURE PROGRAM SUSTAINABILITY AFTER THE GRANT PERIOD.

THE TRANSITIONAL HOUSING ASSISTANCE GRANTS FOR VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING PROGRAM (TRANSITIONAL HOUSING PROGRAM, AUTHORIZED BY 34 U.S.C. 12351), HAS AS ITS PRIMARY PURPOSE THE PROVISION OF TRANSITIONAL HOUSING AND SUPPORT SERVICES TO AID MINORS, ADULTS, AND THEIR DEPENDENTS WHO ARE HOMELESS (AS DEFINED IN 34 U.S.C. 12473(6)) OR IN NEED OF TRANSITIONAL HOUSING OR OTHER HOUSING ASSISTANCE AS A RESULT OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, OR STALKING. THE PROGRAM IS INTENDED TO PROVIDE ASSISTANCE TO VICTIMS FOR WHOM EMERGENCY SHELTER SERVICES OR OTHER CRISIS INTERVENTION SERVICES ARE UNAVAILABLE OR INSUFFICIENT. THE PROGRAM SUPPORTS VICTIM-CENTERED APPROACHES TO PROVIDING TRANSITIONAL HOUSING, SHORT-TERM HOUSING ASSISTANCE, AND VOLUNTARY SUPPORT SERVICES TO ASSIST WITH LOCATING AND SECURING PERMANENT HOUSING, OBTAINING EMPLOYMENT, AND INTEGRATING INTO A COMMUNITY. PENOBSCOT INDIAN NATION, LOCATED IN INDIAN ISLAND, MAINE, WILL IMPLEMENT THE TRANSITIONAL HOUSING SERVICES PROJECT THROUGH THE PROVISION OF VICTIM-CENTERED APPROACHES TO TRANSITIONAL HOUSING NEEDS, IN COLLABORATION WITH WABANAKI WOMENS COALITION. THE PROJECT WILL PROVIDE EIGHT PRIVATE LANDLORD, SCATTERED SITE TRANSITIONAL HOUSING RESIDENCES FOR EIGHT SURVIVORS AND THEIR FAMILIES IN A TRIBAL REGION OF MAINE. THIS NEW PROJECT WILL USE FUNDS TO PROVIDE ONLY HOUSING SERVICES TO MOVE SURVIVORS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, OR STALKING WHO ARE HOMELESS OR IN NEED OF HOUSING ASSISTANCE TO PERMANENT HOUSING. THE PROJECT WILL ASSIST CLIENTS FOR AN ANTICIPATED MINIMUM OF 6 MONTHS AND A MAXIMUM LENGTH OF 24 MONTHS. SERVICES OFFERED WILL FOCUS ON PRIMARILY SERVING UNDERSERVED POPULATIONS IN THE REGION AND WILL BE TAILORED TO ADDRESS THE NEEDS OF VICTIMS WHO ARE SERVED. THE RANGE OF OPTIONAL SUPPORT SERVICES INCLUDES VICTIM ADVOCACY, HOUSING ADVOCACY, FINANCIAL/EMPLOYMENT COUNSELING, INDIVIDUAL/GROUP COUNSELING, TRANSPORTATION, 24/7 HOTLINE, HOSPITAL ACCOMPANIMENT, AND CRISIS INTERVENTION. THE PROJECT WILL PROVIDE FOLLOW-UP SERVICES FOR AT LEAST THREE MONTHS ONCE PERMANENT HOUSING IS SECURED.

CYCLOPHILINS AND FKBPS ARE MEMBERS OF A LARGE CLASS OF PEPTIDYL-PROLINE ISOMERASES (PPIASES) AND PLAY A CENTRAL ROLE IN MODERN MEDICINE WITH CLINICAL APPLICATIONS IN IMMUNOSUPPRESSION, INFECTIOUS DISEASE, AND CHEMOTHERAPY. ALTHOUGH DECADES OF EFFORT HAVE REVEALED THE REMARKABLE STRUCTURAL FEATURES OF IMMUNOPHILIN ENZYMES, MANY MEMBERS OF THIS FAMILY, INCLUDING CYP40, HAVE YET TO MEET THE CRITICAL LEVEL OF THERAPEUTIC EXAMINATION. RECENTLY OUR TEAM DESCRIBED A SYSTEM THAT USES BEADS TO RAPIDLY ENHANCE THE DISCOVERY OF PROBES THAT TARGET SPECIFIC DOMAINS WITHIN A PROTEIN. IN THIS PROGRAM OUR TEAM DEVELOPS THIS METHOD, AS A TECHNIQUE TO DISCOVER MOLECULAR PROBES TO STUDY THE NEUROLOGICAL ROLES OF THE IMMUNOPHILINS. THE EXTRACELLULAR AMYLOID PLAQUES AND THE INTRACELLULAR NEUROFIBRILLARY TANGLES (TAU) THAT DEFINE THE NEUROPATHOLOGY OF ALZHEIMER'S DISEASE ARE BOTH REGULATED BY THE LARGE IMMUNOPHILINS. CYCLOPHILIN 40 (CYP40), FK506-BINDING PROTEINS FKBP51 AND FKBP52 PLAY IMPORTANT, BUT ANTAGONISTIC ROLES. CYP40 DISAGGREGATES A-SYNUCLEIN AND OVEREXPRESSION RESCUES TAU-INDUCED COGNITIVE DEFICITS AND UNRAVELS NEUROTOXIC PLAQUES. BOTH CYP40 AND FKBP51 PROMOTE THE STABILISATION OF MICROTUBULES WHILE FKBP52 HAS BEEN SHOWN TO CAUSE MEMORY IMPAIRMENTS IN A TAU TRANSGENIC MOUSE MODEL AND TO PROMOTE TAU PATHOGENESIS. A LIKELY MECHANISM INVOLVES THE OPPOSING ROLES PLAYED BY CYP40 AND FKPB52 IN REGULATING MICROTUBULE DEPOLYMERISATION. FKBP52 HAS BEEN SHOWN TO BIND AND INHIBIT TUBULIN FORMATION VIA DIRECT BINDING BY ITS TPR DOMAIN. AS THE AMINO ACID IDENTITY BETWEEN THESE THREE LARGE IMMUNOPHILIN TPR DOMAINS IS LESS THAN 26%, IT WILL BE POSSIBLE TO IDENTIFY SMALL MOLECULE BINDERS THAT DISTINGUISH BETWEEN THE TPR DOMAINS OF CYP40, FKBP51 AND FKBP52. MODULATING THE EFFECTS OF THE LARGE IMMUNOPHILINS WILL PROVIDE A NEW FORM OF THERAPY TO TREAT NEURODEGENERATIVE DISEASES CAUSED BY PLAQUE FORMATION INCLUDING ALZHEIMER'S DISEASE. HERE, OUR TEAM ADAPTS AN OPTIMIZED WORKFLOW THAT USES BEADS, CALLED CONFOCAL NANOSCANNING (CONA) AS A TOOL TO IDENTIFY AND VALIDATE FLUORESCENT NATURAL PRODUCTS THAT BIND WITH HIGH OR MEDIUM AFFINITY TO CYP40, FKBP51 OR FKBP52. THE RESULTING MATERIALS ARE THEN USED TO ESTABLISH METHODS FOR CELLULAR IMAGING AS WELL AS ESTABLISH A HIGH-THROUGHPUT SCREEN FOR EACH OF THESE LARGE IMMUNOPHILINS.

DESCRIPTION:THE PRIMARY OBJECTIVES OF EPA'S CERCLA SECTION 128(A) STATE AND TRIBAL RESPONSE PROGRAM GRANTS ARE TO PROVIDE FINANCIAL SUPPORT TO STATES, US TERRITORIES, AND TRIBAL NATIONS TO (1) ESTABLISH OR ENHANCE THE FOUR STATUTORY ELEMENTS OF AN EFFECTIVE STATE OR TRIBAL RESPONSE PROGRAM, AS SPECIFIED IN CERCLA SECTION 128(A)(2), (2) MAINTAIN AND UPDATE, AT LEAST ANNUALLY, A PUBLIC RECORD OF SITES, PURSUANT TO CERCLA 128(B), THAT INCLUDES THE NAME AND LOCATION OF SITES AT WHICH RESPONSE ACTIONS HAVE BEEN COMPLETED DURING THE PREVIOUS YEAR AND THE NAME AND LOCATION OF SITES AT WHICH RESPONSE ACTIONS ARE PLANNED TO BE ADDRESSED IN THE NEXT YEAR, AND (3) CONDUCT A LIMITED NUMBER OF BROWNFIELD SITE ASSESSMENTS OR CLEANUPS THAT WILL HELP ESTABLISH OR ENHANCE THE STATE OR TRIBAL NATION'S RESPONSE PROGRAM. THE PURPOSE OF THIS AWARD IS TO ENHANCE THE CAPACITY OF THE PENOBSCOT INDIAN NATION'S BROWNFIELDS RESPONSE PROGRAM TO MEET THE SECTION 128(A) ELEMENTS. THE PENOBSCOT INDIAN NATION WILL OVERSEE AND PERFORM PLANNING, ASSESSMENT, AND CLEANUP OF BROWNFIELDS SITES THROUGHOUT THE STATE OF MAINE. ACTIVITIES:THE PENOBSCOT INDIAN NATION WILL PERFORM THE FOLLOWING ACTIVITIES: TIMELY SURVEY AND INVENTORY OF BROWNFIELD SITES, WHICH ARE PROPERTIES WHOSE EXPANSION, REDEVELOPMENT OR REUSE MAY BE COMPLICATED BY THE PRESENCE OF HAZARDOUS SUBSTANCES. IT ALSO INCLUDES OVERSIGHT AND ENFORCEMENT AUTHORITIES TO ENSURE THAT RESPONSE ACTIONS PROTECT HUMAN HEALTH AND THE ENVIRONMENT; RESOURCES TO PROVIDE MEANINGFUL PUBLIC INVOLVEMENT; MECHANISMS FOR APPROVAL OF CLEANUP PLANS, AND VERIFICATION OF COMPLETE RESPONSES. THE OBJECTIVE OF THIS PROJECT IS TO ASSIST IN REMEDIATING AND REUSING BROWNFIELDS SITES THROUGHOUT THE STATE OF MAINE.SUBRECIPIENT:NO SUBAWARDS ARE INCLUDED IN THIS ASSISTANCE AGREEMENT.OUTCOMES:THE PENOBSCOT INDIAN NATION WILL EXPAND PUBLIC EDUCATION AND OUTREACH OF THE THE PENOBSCOT INDIAN NATION VOLUNTARY CLEANUP PROGRAM TO SMALL AND DISADVANTAGED COMMUNITIES, AND IS PLANNING ON ASSESSING AND OVERSEEING THE CLEANUP OF TWO SITES.
