SINCE 2005 WHEN THE COMMUNITY WAS DEVASTATED BY HURRICANE RITA, THIS SAME SERVICE AREA HAS BEEN IMPACTED BY NINE TROPICAL STORMS AND HURRICANES, MOST NOTABLY HURRICANE HARVEY IN 2017, AS REPORTED BY THE NATIONAL HURRICANE CENTER. BASED ON A COMMUNITY NEEDS ASSESSMENT COMPLETED BY STC IN 2020, THERE ARE APPROXIMATELY 2 LOCAL BEHAVIORAL HEALTH HOSPITALS, 15 FEDERALLY QUALIFIED HEALTH CLINICS, 4 PUBLIC HEALTH DEPARTMENTS, AND A VA CLINIC IN BEAUMONT. STC HAS APPLIED TO TEXAS HEALTH AND HUMAN SERVICES COMMISSION TO PURSUE CERTIFIED COMMUNITY BEHAVIORAL HEALTH CLINIC (CCBHC) STATUS. THE CCBHC MODEL OF CARE ALIGNS WITH THE STC MISSION OF HELPING PEOPLE HELP THEMSELVES BY PROVIDING RESOURCES AND SUPPORTS. IN ADDITION TO THE NEED FOR THE COMPREHENSIVE SERVICE ARRAY WHICH A CCBHC MODEL OF CARE WILL ENSURE, THE INDIVIDUALS LIVING IN THE SERVICE AREA CLEARLY NEED THE BENEFIT OF CARE COORDINATION TO ASSIST THEM IN ACCESSING THE RIGHT CARE AT THE RIGHT TIME TO ADDRESS THE HEALTH CONCERNS OF THE WHOLE PERSON. TRANSCUTANEOUS ELECTRICAL SPINAL CORD NEUROMODULATION (TESCON) OFFERS A DIAMETRICALLY DIFFERENT APPROACH TO CLASSIC THERAPIES FOR NEUROGENIC BLADDER: BY DELIVERING LOW-INTENSITY SUB-MOTOR THRESHOLD ELECTRICAL PULSES TO THE SPINAL CORD, TESCON ACTIVATES LUMBAR NEURAL NETWORKS RENDERED DYSFUNCTIONAL BY THE SPINAL CORD INJURY AND RESTORES SOME COMMUNICATION BETWEEN THE LOWER URINARY TRACT AND HIGHER NEURAL CENTERS RESPONSIBLE FOR MICTURITION CONTROL. RESULTS FROM OUR PRELIMINARY STUDIES INDICATE THAT TESCON-MEDIATED NEUROMODULATION IMPROVES BLADDER CAPACITY, DIMINISHES URINARY INCONTINENCE AND, REMARKABLY, RESTORES BLADDER SENSATION EVEN IN INDIVIDUALS, WHOSE SPINAL CORD INJURY IS CONSIDERED CLINICALLY COMPLETE. IN THIS PHASE II APPLICATION, WE PROPOSE TO BUILD A COMMERCIAL VERSION OF THE TESCON PROTOTYPE UNDER DESIGN CONTROLS AND USE THE DEVICE IN A CLINICAL VALIDATION STUDY. WE PROPOSE A MULTICENTER RANDOMIZED CONTROL TRIAL TO DEMONSTRATE THE BENEFICIAL EFFECTS OF TESCON ON THE PARALYZED LOWER URINARY TRACT. BY RECRUITING SIXTY INDIVIDUALS WITH SPINAL CORD INJURY AND COMPARING THE EFFECTS OF SHAM AND TRUE STIMULATION ON THEIR LOWER URINARY TRACT, WE AIM TO SHOW THAT TESCON HAS A SIGNIFICANT ROLE IN MANAGING AND REHABILITATING URINARY FUNCTION IN THE CLINICAL SETTING (PRIMARY OUTCOME). THE ACADEMIC ROOTS OF OUR COMPANY AND THE TEAM WE HAVE ASSEMBLED (ENGINEERS, ENTREPRENEURS AND CLINICIANS) ENSURE THAT OUR STUDY DESIGN, IMPLEMENTATION AND ANALYSIS WILL BE BOTH IMPACTFUL AND RIGOROUS. SUCCESSFUL COMPLETION OF THIS STUDY WILL PROVIDE EVIDENCE FOR A FDA 510(K) SUBMISSION. POPULATION-BASED STUDIES FROM AROUND THE WORLD REPORT PREVALENCE ESTIMATES RANGING FROM 1.5 TO 4 PER 1000 INDIVIDUALS. GLOBALLY, APPROXIMATELY 17 MILLION PEOPLE HAVE CP. IT IS A DIAGNOSIS THAT INCLUDES A WIDE RANGE OF SYMPTOMS WITH HETEROGENEOUS ETIOLOGIES OF NEURAL AND CARDIOVASCULAR ORIGINS. A WIDE RANGE OF FUNCTIONAL DISORDERS TYPICALLY BECOME OBVIOUS IN CP DURING EARLY DEVELOPMENT. FOR EXAMPLE, THE MAJORITY OF CHILDREN CLASSIFIED AS GROSS MOTOR FUNCTIONAL CLASSIFICATION SCALE (GMFCS) LEVEL I ARE EXPECTED TO REACH THEIR MOTOR POTENTIAL BETWEEN 7-9 YEARS AND REMAIN STABLE AFTER THAT UNTIL AGE 21 WHERE THEY MAY DECLINE DUE TO PAIN, WEAKNESS AND STIFFNESS. WHILE MOVEMENTS SUCH AS STEPPING AND POSTURE ARE AMONGST THE MOST RECOGNIZABLE IMPAIRMENTS, ABSENCE OR ABNORMAL SENSATIONS, PERCEPTION, IMPAIRED COGNITION, COMMUNICATION SKILLS, EPILEPSY, SPASTICITY, POOR CONTROL OF FINE MOVEMENTS AND MARKED SECONDARY MUSCULOSKELETAL PROBLEMS CAN EMERGE WITH TIME. PRESENTLY, ALL AVAILABLE INTERVENTIONS ARE DESIGNED TO MINIMIZE THE SEVERITY OF SYMPTOMS RATHER THAN CORRECTING THE NEUROLOGICAL ETIOLOGY. TWO COMMONLY PREFERRED TREATMENTS, SELECTIVE DORSAL ROOT RHIZOTOMY AND REPETITIVE ADMINISTRATION OF BOTULINUM TOXIN A (BOTOX) DIRECTLY TARGET THE NEUROMUSCULAR SYSTEM TO COUNTER SPASTICITY. WHILE THESE TWO STRATEGIES MINIMIZE SOME OF THE DISRUPTIVE MOVEMENTS ASSOCIATED WITH SPASTICITY; THE LONG-TERM BENEFITS OF THESE INTERVENTIONS ARE LESS CLEAR AND BOTH CAUSE IRREVERSIBLE CHANGES TO THE NEUROMUSCULAR SYSTEM THAT MAY BE DETRIMENTAL TO FUTURE FUNCTION. IN THE PRESENT PROPOSAL WE HAVE MERGED IDEAS AND CONCEPTS DERIVED FROM BOTH RECENT CLINICAL OBSERVATIONS OF CP AND DECADES OF STUDIES OF SPINAL CORD INJURY TO ARRIVE AT A SUBJECT-SPECIFIC REHABILITATION STRATEGY DRIVEN BY ACTIVITY- BASED MECHANISMS DURING TRANSCUTANEOUS SPINAL NEUROMODULATION. THE OBJECTIVES OF OUR NEUROMODULATION PROCEDURES ARE TO AMPLIFY THE PLASTICITY OF THE NEUROMUSCULAR SYSTEM AND HELP CHILDREN GAIN THE ABILITY TO PERFORM COORDINATED MOVEMENTS THROUGH ACTIVITY-DEPENDENT PLASTICITY. WE HYPOTHESIZED THAT TRANSCUTANEOUS SPINAL NEUROMODULATION DURING ACTIVITY-BASED NEUROREHABILITATION THERAPY (ABNT) CAN TRANSFORM THE NEURAL NETWORKS IN CHILDREN DIAGNOSED WITH SPASTIC CP TO IMPROVE VOLUNTARY POSTURAL AND LOCOMOTOR ACTIVITY AS CAPTURED ON THE GMFM88 SCORES. THIS NOVEL SYSTEM CAN PRODUCE A WIDE VARIETY OF ORGANIC, INORGANIC, AND POLYMER FIBER COMPOSITIONS ON DEMAND TO CREATE A VARIETY OF MATERIALS FOR NUMEROUS INDUSTRIES, INCLUDING WATER FILTRATION, AIR TREATMENT, THERMAL INSULATION, WOUND DRESSING, TISSUE ENGINEERING, CELL SCAFFOLDING, TEXTILES, AND BATTERY ENERGY STORAGE. THIS SMALL BUSINESS INNOVATION RESEARCH (SBIR) PHASE II PROJECT WILL DEVELOP A PLATFORM PROCESS TO MANUFACTURE CUSTOM NANOFIBERS USING ALTERNATING CURRENT ELECTROSPINNING, A SCALABLE AND COST-EFFICIENT METHOD THAT SUPPORTS OUTPUT EFFICIENCIES THOUSANDS OF TIMES HIGHER THAN STANDARD DIRECT CURRENT ELECTROSPINNING METHODS WHILE ALLOWING EASIER HANDLING OF THE FIBERS. IDEALLY, RT ABLATION IS PERFORMED IN CONJUNCTION WITH NONINVASIVE ELECTROPHYSIOLOGY (EP) IMAGING, SUCH AS ECG IMAGING (ECGI), SO THAT THE ENTIRE PROCEDURE IS NONINVASIVE; HOWEVER, CURRENTLY NONINVASIVE EP IMAGING IS MUCH LESS ACCURATE THAN RT AND NEEDS TO BE SIGNIFICANTLY IMPROVED TO REALIZE THE FULL POTENTIAL OF RT ABLATION. IN THIS PROJECT, WE PROPOSE TO UTILIZE MAGNETOCARDIOGRAPHY (MCG) TO DEVELOP AN MCG IMAGING (MCGI) SYSTEM. IN COMPARISON TO ECG, MCG PROVIDES FUNDAMENTAL ADVANTAGES FOR SOURCE LOCALIZATION AND IMAGING DUE TO THE FAVORABLE TRANSMISSION PROPERTIES OF MAGNETIC SIGNALS. FROM A PRACTICAL STANDPOINT, MCG IS MORE CONVENIENT BECAUSE IT IS A CONTACTLESS TECHNIQUE, WHEREAS ECGI REQUIRES APPLICATION OF A LARGE ARRAY OF ELECTRODES. THE PROPOSED SYSTEM WILL BE BASED ON A REVOLUTIONARY TYPE OF MAGNETIC SENSOR, KNOWN AS AN OPTICALLY-PUMPED MAGNETOMETER (OPM), WHICH WAS FIRST BROUGHT TO MARKET BY OUR COMPANY. OPMS ARE FAR MORE PRACTICAL THAN SQUID MAGNETOMETERS BECAUSE THEY ARE SMALLER, LESS EXPENSIVE, AND OPERATE AT ROOM TEMPERATURE. THIS WILL ENABLE CONSTRUCTION OF AN MCGI SYSTEM WITH A CONFORMAL SENSOR ARRAY THAT COVERS THE ENTIRE TORSO. THE OVERALL GOAL IS TO CONSTRUCT AN MCGI SYSTEM WITH IMPROVED ACCURACY AND EASE-OF-USE, COMPARED TO CURRENT ECGI/MCGI SYSTEMS. WE PURSUE THIS GOAL BY FIRST DESIGNING AND BUILDING AN AUTOMATIC, MRI-COMPATIBLE, PISTON-BASED SET OF SYRINGE PUMPS THAT RESPOND TO SUBJECT BREATHING DYNAMICS BY INJECTING SUPPLEMENTAL HPX OR OXYGEN (OR ANY OTHER DESIRED AGENT) INTO THE INHALED GAS STREAM. BASED ON AN EXISTING PRODUCT USED FOR HPX IN VENTILATED SMALL-ANIMAL STUDIES, THIS DEVICE WILL ACCUMULATE AND RESPOND TO PASSIVELY-OBTAINED PNEUMOTACH MEASURES OF AIRFLOW, ALLOWING BOLUS OR CONSTANT- CONCENTRATION DELIVERY AS DESIRED, AS WELL AS SYNCHRONIZATION WITH THE MR IMAGING DATA, THE HEARTBEAT, AND EXHALED GAS CONCENTRATIONS, WITHOUT THE NEED FOR PATIENT COMPLIANCE WITH BREATHING MANEUVERS. ACDF SURGERY REMOVES 1-4 INTERVERTEBRAL DISCS AND REPLACES THEM WITH INTERBODY SPACERS FILLED WITH BONE GRAFT. THIS DISTRACTS AND DECOMPRESS THE NERVE ROOTS WHILE FUSING THE ADJACENT VERTEBRAE. POSTOPERATIVELY, IT IS CRITICAL TO RECOGNIZE WHEN THE VERTEBRAE HAVE FUSED, AN OUTCOME THAT DETERMINES SAFE RETURN TO ACTIVITY; CONVERSELY, DELAYED FUSION MAY INDICATE NEED FOR ADDITIONAL INTERVENTIONS (E.G., PROLONGED COLLAR USAGE, MODIFIED PHYSICAL THERAPY, ELECTRICAL STIMULATION, AND INJECTIONS SUCH AS RHPTH). IMPROPER MANAGEMENT CAN LEAD TO POOR OUTCOMES, WORSE PAIN, SURGICAL REVISION AND NEUROLOGICAL DEFICITS. UNFORTUNATELY, PATIENTS HEAL AT RATES THAT VARY GREATLY, AND SOME PATIENTS WILL NOT HEAL PROPERLY WITH 7% NEEDING COSTLY REVISION SURGERY ($123,000 HOSPITAL CHARGES). WE PROPOSE TO DEVELOP AN X-RAY VISIBLE INTERBODY SPACER INDICATING BIOMECHANICAL LOAD (X-VISIBL) FUSION DEVICE TO ASSIST CLINICIANS IN TRACKING AND DETECTING BONY FUSION. IF SUCCESSFUL, THIS PROJECT WILL VALIDATE A SIMPLE INDICATOR THAT REPORTS LOAD ON THE DEVICE TO ASSESS FUSION OF THE ADJACENT BONES. MEASUREMENTS ARE MADE USING FLEXION/EXTENSION RADIOGRAPHY WHICH IS ALREADY ROUTINELY USED IN PATIENT FOLLOW-UP BUT IS CURRENTLY INSUFFICIENTLY SENSITIVE TO DETECT DELAYED FUSION AND NON-UNION. MONITORING IMPLANT LOAD WILL PROVIDE THE PATIENT AND MEDICAL TEAM CRITICAL INFORMATION TO SELECT EARLY TARGETED INTERVENTIONS INCLUDING PROLONGED BRACE OR COLLAR USAGE, MODIFIED PHYSICAL THERAPY AND RETURN-TO-WORK PROTOCOLS, ULTRASOUND OR ELECTRICAL STIMULATION, MEDICATIONS OR INJECTIONS SUCH AS RHPTH, AND INFORM LONG TERM CARE TO AVOID DEVICE FAILURE AND ASSOCIATED PAIN, DISABILITY AND REOPERATIONS. SPINDLETOP CENTER CCBHC PROJECT - SPINDLETOP CENTER HAS SERVED AS THE LOCAL MENTAL HEALTH AUTHORITY FOR A FOUR-COUNTY AREA INCLUDING JEFFERSON, HARDIN, ORANGE, AND CHAMBERS COUNTIES IN SOUTHEAST TEXAS FOR OVER 45 YEARS. SINCE 2005 WHEN THE COMMUNITY WAS DEVASTATED BY HURRICANE RITA, THIS SAME SERVICE AREA HAS BEEN IMPACTED BY NINE TROPICAL STORMS AND HURRICANES, MOST NOTABLY HURRICANE HARVEY IN 2017, AS REPORTED BY THE NATIONAL HURRICANE CENTER. BASED ON A COMMUNITY NEEDS ASSESSMENT COMPLETED BY STC IN 2020, THERE ARE APPROXIMATELY 2 LOCAL BEHAVIORAL HEALTH HOSPITALS, 15 FEDERALLY QUALIFIED HEALTH CLINICS, 4 PUBLIC HEALTH DEPARTMENTS, AND A VA CLINIC IN BEAUMONT. STC HAS APPLIED TO TEXAS HEALTH AND HUMAN SERVICES COMMISSION TO PURSUE CERTIFIED COMMUNITY BEHAVIORAL HEALTH CLINIC (CCBHC) STATUS. THE CCBHC MODEL OF CARE ALIGNS WITH THE STC MISSION OF HELPING PEOPLE HELP THEMSELVES BY PROVIDING RESOURCES AND SUPPORTS. IN ADDITION TO THE NEED FOR THE COMPREHENSIVE SERVICE ARRAY WHICH A CCBHC MODEL OF CARE WILL ENSURE, THE INDIVIDUALS LIVING IN THE SERVICE AREA CLEARLY NEED THE BENEFIT OF CARE COORDINATION TO ASSIST THEM IN ACCESSING THE RIGHT CARE AT THE RIGHT TIME TO ADDRESS THE HEALTH CONCERNS OF THE WHOLE PERSON. IN FISCAL YEAR 2020, STC SERVED 12,718 UNDUPLICATED INDIVIDUALS IN ALL SERVICES COMBINED WITH 45.9% OF ADULTS UNINSURED AND 16% OF CHILDREN UNINSURED. STC PROPOSES TO UTILIZE THIS GRANT FUNDING TO INCREASE ACCESS TO CARE FOR CHILDREN AND ADULTS WITH SERIOUS MENTAL ILLNESS OR SERIOUS EMOTIONAL DISTURBANCE AND INDIVIDUALS WITH SUBSTANCE USE DISORDERS WHO ARE IDENTIFIED AS LOW-INCOME OR UNINSURED. STC PROPOSES TO SERVE APPROXIMATELY 13,650 UNDUPLICATED INDIVIDUALS OVER THE LIFE OF THE GRANT.

DEVELOPMENT OF A NOVEL, REGENERATIVE THERAPY TO REVERSE SYNAPSE LOSS IN ALZHEIMER'S DISEASE - PROJECT SUMMARY/ABSTRACT THE MISSION OF THE SBIR PROGRAM IS TO “ENCOURAGE DOMESTIC SMALL BUSINESSES TO ENGAGE IN FEDERAL RESEARCH/RESEARCH AND DEVELOPMENT (R/R&D) THAT HAS THE POTENTIAL FOR COMMERCIALIZATION.” A MISSION OF THE NIA IS TO “SUPPORT RESEARCH ON ALZHEIMER’S DISEASE, AND PROMOTE RESEARCH ON THE ETIOLOGY, TREATMENT, AND DIAGNOSIS OF AD”. THE PROPOSED DEVELOPMENT PROGRAM IS NECESSARY TO ENABLE THE FILING AND ACCEPTANCE OF AN IND WITH THE FDA, AN EARLY PRELUDE TO COMMERCIALIZATION. COMMERCIALIZATION READINESS OF WEARABLE OPTICALLY PUMPED MAGNETOENCEPHALOGRAPHY - ABSTRACT MAGNETOENCEPHALOGRAPHY (MEG) HAS ESTABLISHED ITSELF AS AN INDISPENSABLE TOOL FOR FUNCTIONAL BRAIN IMAGING, OFFERING UNPARALLELED SPATIAL AND TEMPORAL RESOLUTION WHILE BEING NON-INVASIVE. DESPITE ITS ADVANTAGES, CURRENT MEG SYSTEMS BASED ON SUPERCONDUCTING QUANTUM INTERFERENCE DEVICES (SQUIDS) ARE COST-PROHIBITIVE AND LACK ADAPTABILITY, PARTICULARLY FOR PEDIATRIC POPULATIONS. THIS PROJECT FOCUSES ON THE DEVELOPMENT OF A WEARABLE MEG SYSTEM BASED ON OPTICALLY PUMPED MAGNETOMETER (OPM) TECHNOLOGY, A NOVEL ALTERNATIVE TO TRADITIONAL SQUID MAGNETOMETERS. THIS INNOVATION ELIMINATES THE NEED FOR CRYOGENIC COOLING, THEREBY OVERCOMING SIGNIFICANT LIMITATIONS ASSOCIATED WITH TRADITIONAL MEG SYSTEMS. USING OPMS HAS ENABLED THE DEVELOPMENT OF A COST-EFFECTIVE, FLEXIBLE, AND HIGH-RESOLUTION MEG SYSTEM (OPM-MEG). TRANSCUTANEOUS ELECTRICAL SPINAL CORD NEUROMODULATION (TESCON) OFFERS A DIAMETRICALLY DIFFERENT APPROACH TO CLASSIC THERAPIES FOR NEUROGENIC BLADDER: BY DELIVERING LOW-INTENSITY SUB-MOTOR THRESHOLD ELECTRICAL PULSES TO THE SPINAL CORD, TESCON ACTIVATES LUMBAR NEURAL NETWORKS RENDERED DYSFUNCTIONAL BY THE SPINAL CORD INJURY AND RESTORES SOME COMMUNICATION BETWEEN THE LOWER URINARY TRACT AND HIGHER NEURAL CENTERS RESPONSIBLE FOR MICTURITION CONTROL. RESULTS FROM OUR PRELIMINARY STUDIES INDICATE THAT TESCON-MEDIATED NEUROMODULATION IMPROVES BLADDER CAPACITY, DIMINISHES URINARY INCONTINENCE AND, REMARKABLY, RESTORES BLADDER SENSATION EVEN IN INDIVIDUALS, WHOSE SPINAL CORD INJURY IS CONSIDERED CLINICALLY COMPLETE. IN THIS PHASE II APPLICATION, WE PROPOSE TO BUILD A COMMERCIAL VERSION OF THE TESCON PROTOTYPE UNDER DESIGN CONTROLS AND USE THE DEVICE IN A CLINICAL VALIDATION STUDY. WE PROPOSE A MULTICENTER RANDOMIZED CONTROL TRIAL TO DEMONSTRATE THE BENEFICIAL EFFECTS OF TESCON ON THE PARALYZED LOWER URINARY TRACT. BY RECRUITING SIXTY INDIVIDUALS WITH SPINAL CORD INJURY AND COMPARING THE EFFECTS OF SHAM AND TRUE STIMULATION ON THEIR LOWER URINARY TRACT, WE AIM TO SHOW THAT TESCON HAS A SIGNIFICANT ROLE IN MANAGING AND REHABILITATING URINARY FUNCTION IN THE CLINICAL SETTING (PRIMARY OUTCOME). THE ACADEMIC ROOTS OF OUR COMPANY AND THE TEAM WE HAVE ASSEMBLED (ENGINEERS, ENTREPRENEURS AND CLINICIANS) ENSURE THAT OUR STUDY DESIGN, IMPLEMENTATION AND ANALYSIS WILL BE BOTH IMPACTFUL AND RIGOROUS. SUCCESSFUL COMPLETION OF THIS STUDY WILL PROVIDE EVIDENCE FOR A FDA 510(K) SUBMISSION. FINALLY, IF TESCON IS SUCCESSFUL IN THE CASE OF A NEUROLOGICAL IMPAIRMENT AS DEVASTATING AS A SPINAL CORD INJURY, WE ARE CONFIDENT THAT WE WILL BE ABLE TO BRING THIS TECHNOLOGY TO A MUCH LARGER POPULATION OF PATIENTS SUFFERING FROM LOWER URINARY TRACT DYSFUNCTION DUE TO BOTH NEUROLOGICAL AND NON-NEUROLOGICAL CONDITIONS, SUCH AS STROKE, MULTIPLE SCLEROSIS, PARKINSON’S DISEASE AND IDIOPATHIC OVERACTIVE BLADDER.

SPINAL CORD INNOVATION IN PEDIATRICS TO TREAT CEREBRAL PALSY (SCIP CLINICAL STUDY) - SUMMARY CEREBRAL PALSY (CP) IS THE MOST COMMON MOTOR DISABILITY IN CHILDHOOD. POPULATION-BASED STUDIES FROM AROUND THE WORLD REPORT PREVALENCE ESTIMATES RANGING FROM 1.5 TO 4 PER 1000 INDIVIDUALS. GLOBALLY, APPROXIMATELY 17 MILLION PEOPLE HAVE CP. IT IS A DIAGNOSIS THAT INCLUDES A WIDE RANGE OF SYMPTOMS WITH HETEROGENEOUS ETIOLOGIES OF NEURAL AND CARDIOVASCULAR ORIGINS. A WIDE RANGE OF FUNCTIONAL DISORDERS TYPICALLY BECOME OBVIOUS IN CP DURING EARLY DEVELOPMENT. FOR EXAMPLE, THE MAJORITY OF CHILDREN CLASSIFIED AS GROSS MOTOR FUNCTIONAL CLASSIFICATION SCALE (GMFCS) LEVEL I ARE EXPECTED TO REACH THEIR MOTOR POTENTIAL BETWEEN 7-9 YEARS AND REMAIN STABLE AFTER THAT UNTIL AGE 21 WHERE THEY MAY DECLINE DUE TO PAIN, WEAKNESS AND STIFFNESS. WHILE MOVEMENTS SUCH AS STEPPING AND POSTURE ARE AMONGST THE MOST RECOGNIZABLE IMPAIRMENTS, ABSENCE OR ABNORMAL SENSATIONS, PERCEPTION, IMPAIRED COGNITION, COMMUNICATION SKILLS, EPILEPSY, SPASTICITY, POOR CONTROL OF FINE MOVEMENTS AND MARKED SECONDARY MUSCULOSKELETAL PROBLEMS CAN EMERGE WITH TIME. PRESENTLY, ALL AVAILABLE INTERVENTIONS ARE DESIGNED TO MINIMIZE THE SEVERITY OF SYMPTOMS RATHER THAN CORRECTING THE NEUROLOGICAL ETIOLOGY. TWO COMMONLY PREFERRED TREATMENTS, SELECTIVE DORSAL ROOT RHIZOTOMY AND REPETITIVE ADMINISTRATION OF BOTULINUM TOXIN A (BOTOX) DIRECTLY TARGET THE NEUROMUSCULAR SYSTEM TO COUNTER SPASTICITY. WHILE THESE TWO STRATEGIES MINIMIZE SOME OF THE DISRUPTIVE MOVEMENTS ASSOCIATED WITH SPASTICITY; THE LONG-TERM BENEFITS OF THESE INTERVENTIONS ARE LESS CLEAR AND BOTH CAUSE IRREVERSIBLE CHANGES TO THE NEUROMUSCULAR SYSTEM THAT MAY BE DETRIMENTAL TO FUTURE FUNCTION. IN THE PRESENT PROPOSAL WE HAVE MERGED IDEAS AND CONCEPTS DERIVED FROM BOTH RECENT CLINICAL OBSERVATIONS OF CP AND DECADES OF STUDIES OF SPINAL CORD INJURY TO ARRIVE AT A SUBJECT-SPECIFIC REHABILITATION STRATEGY DRIVEN BY ACTIVITY- BASED MECHANISMS DURING TRANSCUTANEOUS SPINAL NEUROMODULATION. THE OBJECTIVES OF OUR NEUROMODULATION PROCEDURES ARE TO AMPLIFY THE PLASTICITY OF THE NEUROMUSCULAR SYSTEM AND HELP CHILDREN GAIN THE ABILITY TO PERFORM COORDINATED MOVEMENTS THROUGH ACTIVITY-DEPENDENT PLASTICITY. WE HYPOTHESIZED THAT TRANSCUTANEOUS SPINAL NEUROMODULATION DURING ACTIVITY-BASED NEUROREHABILITATION THERAPY (ABNT) CAN TRANSFORM THE NEURAL NETWORKS IN CHILDREN DIAGNOSED WITH SPASTIC CP TO IMPROVE VOLUNTARY POSTURAL AND LOCOMOTOR ACTIVITY AS CAPTURED ON THE GMFM88 SCORES. THIS STRATEGY COULD PROVIDE A SPECIFIC PHYSIOLOGICAL TARGET OF FUNDAMENTAL SIGNIFICANCE IN THE CONTROL OF MOVEMENT THAT CAN BE USED TO IMPROVE FUNCTION AND PROVIDE NEW CARE PATHWAYS FOR CHILDREN WITH CP.

IDEALLY, RT ABLATION IS PERFORMED IN CONJUNCTION WITH NONINVASIVE ELECTROPHYSIOLOGY (EP) IMAGING, SUCH AS ECG IMAGING (ECGI), SO THAT THE ENTIRE PROCEDURE IS NONINVASIVE; HOWEVER, CURRENTLY NONINVASIVE EP IMAGING IS MUCH LESS ACCURATE THAN RT AND NEEDS TO BE SIGNIFICANTLY IMPROVED TO REALIZE THE FULL POTENTIAL OF RT ABLATION. IN THIS PROJECT, WE PROPOSE TO UTILIZE MAGNETOCARDIOGRAPHY (MCG) TO DEVELOP AN MCG IMAGING (MCGI) SYSTEM. IN COMPARISON TO ECG, MCG PROVIDES FUNDAMENTAL ADVANTAGES FOR SOURCE LOCALIZATION AND IMAGING DUE TO THE FAVORABLE TRANSMISSION PROPERTIES OF MAGNETIC SIGNALS. FROM A PRACTICAL STANDPOINT, MCG IS MORE CONVENIENT BECAUSE IT IS A CONTACTLESS TECHNIQUE, WHEREAS ECGI REQUIRES APPLICATION OF A LARGE ARRAY OF ELECTRODES. THE PROPOSED SYSTEM WILL BE BASED ON A REVOLUTIONARY TYPE OF MAGNETIC SENSOR, KNOWN AS AN OPTICALLY-PUMPED MAGNETOMETER (OPM), WHICH WAS FIRST BROUGHT TO MARKET BY OUR COMPANY. OPMS ARE FAR MORE PRACTICAL THAN SQUID MAGNETOMETERS BECAUSE THEY ARE SMALLER, LESS EXPENSIVE, AND OPERATE AT ROOM TEMPERATURE. THIS WILL ENABLE CONSTRUCTION OF AN MCGI SYSTEM WITH A CONFORMAL SENSOR ARRAY THAT COVERS THE ENTIRE TORSO. THE OVERALL GOAL IS TO CONSTRUCT AN MCGI SYSTEM WITH IMPROVED ACCURACY AND EASE-OF-USE, COMPARED TO CURRENT ECGI/MCGI SYSTEMS. PAIRED WITH A CLINICALLY PRACTICAL SET OF PHYSIOLOGICAL MONITORING MEASUREMENTS, WE INTEND THIS DEVELOPMENT TO SUPPORT A NEW PARADIGM FOR FUNCTIONAL LUNG IMAGING THAT IS ACCESSIBLE TO ALL PATIENTS REGARDLESS OF AGE OR DISEASE STATE, REMOVES SOURCES OF IRREPRODUCIBILITY AND BIAS ASSOCIATED WITH IMAGES ACQUIRED DURING EXTENDED BREATH-HOLD, AND OFFERS NEW OPPORTUNITIES FOR CONTRAST ASSOCIATED WITH THE DYNAMIC ASPECTS OF GAS- AND BLOOD-FLOW IN AN ORGAN THAT DURING NORMAL FUNCTIONING IS ALWAYS IN MOTION. WE PURSUE THIS GOAL BY FIRST DESIGNING AND BUILDING AN AUTOMATIC, MRI-COMPATIBLE, PISTON-BASED SET OF SYRINGE PUMPS THAT RESPOND TO SUBJECT BREATHING DYNAMICS BY INJECTING SUPPLEMENTAL HPX OR OXYGEN (OR ANY OTHER DESIRED AGENT) INTO THE INHALED GAS STREAM. BASED ON AN EXISTING PRODUCT USED FOR HPX IN VENTILATED SMALL-ANIMAL STUDIES, THIS DEVICE WILL ACCUMULATE AND RESPOND TO PASSIVELY-OBTAINED PNEUMOTACH MEASURES OF AIRFLOW, ALLOWING BOLUS OR CONSTANT- CONCENTRATION DELIVERY AS DESIRED, AS WELL AS SYNCHRONIZATION WITH THE MR IMAGING DATA, THE HEARTBEAT, AND EXHALED GAS CONCENTRATIONS, WITHOUT THE NEED FOR PATIENT COMPLIANCE WITH BREATHING MANEUVERS. THE COMBINED DATASET WILL SUPPORT IDENTIFICATION OF ABNORMAL VENTILATION, PERFUSION OR GAS DIFFUSION DURING PHYSIOLOGICALLY RELEVANT CONDITIONS. SUBSEQUENTLY SYNCHRONOUS MONITORING AND CAPTURING OF INHALED AND EXHALED O2, CO2 AND XENON CONCENTRATIONS IS INCORPORATED THROUGHOUT THE BREATHING CYCLE. THIS PROJECT ALSO AIMS TO DEMONSTRATE FEASIBILITY OF THE DYNAMIC LUNG HPX MRI IN A SMALL PILOT STUDY. ACDF SURGERY REMOVES 1-4 INTERVERTEBRAL DISCS AND REPLACES THEM WITH INTERBODY SPACERS FILLED WITH BONE GRAFT. THIS DISTRACTS AND DECOMPRESS THE NERVE ROOTS WHILE FUSING THE ADJACENT VERTEBRAE. POSTOPERATIVELY, IT IS CRITICAL TO RECOGNIZE WHEN THE VERTEBRAE HAVE FUSED, AN OUTCOME THAT DETERMINES SAFE RETURN TO ACTIVITY; CONVERSELY, DELAYED FUSION MAY INDICATE NEED FOR ADDITIONAL INTERVENTIONS (E.G., PROLONGED COLLAR USAGE, MODIFIED PHYSICAL THERAPY, ELECTRICAL STIMULATION, AND INJECTIONS SUCH AS RHPTH). IMPROPER MANAGEMENT CAN LEAD TO POOR OUTCOMES, WORSE PAIN, SURGICAL REVISION AND NEUROLOGICAL DEFICITS. UNFORTUNATELY, PATIENTS HEAL AT RATES THAT VARY GREATLY, AND SOME PATIENTS WILL NOT HEAL PROPERLY WITH 7% NEEDING COSTLY REVISION SURGERY ($123,000 HOSPITAL CHARGES). WE PROPOSE TO DEVELOP AN X-RAY VISIBLE INTERBODY SPACER INDICATING BIOMECHANICAL LOAD (X-VISIBL) FUSION DEVICE TO ASSIST CLINICIANS IN TRACKING AND DETECTING BONY FUSION. IF SUCCESSFUL, THIS PROJECT WILL VALIDATE A SIMPLE INDICATOR THAT REPORTS LOAD ON THE DEVICE TO ASSESS FUSION OF THE ADJACENT BONES. MEASUREMENTS ARE MADE USING FLEXION/EXTENSION RADIOGRAPHY WHICH IS ALREADY ROUTINELY USED IN PATIENT FOLLOW-UP BUT IS CURRENTLY INSUFFICIENTLY SENSITIVE TO DETECT DELAYED FUSION AND NON-UNION. MONITORING IMPLANT LOAD WILL PROVIDE THE PATIENT AND MEDICAL TEAM CRITICAL INFORMATION TO SELECT EARLY TARGETED INTERVENTIONS INCLUDING PROLONGED BRACE OR COLLAR USAGE, MODIFIED PHYSICAL THERAPY AND RETURN-TO-WORK PROTOCOLS, ULTRASOUND OR ELECTRICAL STIMULATION, MEDICATIONS OR INJECTIONS SUCH AS RHPTH, AND INFORM LONG TERM CARE TO AVOID DEVICE FAILURE AND ASSOCIATED PAIN, DISABILITY AND REOPERATIONS. THIS PHASE I SMALL BUSINESS TECHNOLOGY TRANSFER PROJECT AIMS TO ASSESS TECHNICAL FEASIBILITY FOR A LOAD INDICATING CERVICAL INTERBODY SPACER. THE APPROACH IS INNOVATIVE IN PROVIDING A SENSOR TO CLEARLY MEASURE LOAD DURING FUSION WITH X-RAY READOUT THAT IS ALREADY PART OF THE STANDARD OF CARE. TO SHOW FEASIBILITY, WE MUST DEVELOP PROTOTYPES WITH MECHANICAL PROPERTIES THAT ALLOW/ENCOURAGE FUSION (ESPECIALLY STIFFNESS AND YIELD) AND THE PRECISION TO DETECT PHYSIOLOGICAL LOAD CHANGES DURING FUSION. THIS WILL BE ACCOMPLISHED WITH COMPUTER SIMULATIONS, MECHANICAL PROTOTYPE FABRICATION AND TESTING, AND RADIOGRAPHY IN CADAVERIC MODELS. THE RESEARCH IS RELEVANT TO PUBLIC HEALTH BECAUSE, IT PROVIDES AN OBJECTIVE NON-INVASIVE MEANS TO ASSESS BIOMECHANICAL FUSION AND ASSIST PHYSICIANS PRESCRIBING REHABILITATION PROTOCOLS AND ADJUNCTIVE THERAPIES.
