COMMUNITY PROJECT FUNDING/CONGRESSIONALLY DIRECTED SPENDING - NON-CONSTRUCTION - CHCRR’S SCHOOL-BASED ORAL HEALTH CARE PROJECT WILL PROVIDE FREE ORAL HEALTH SERVICES FOR SCHOOL-AGE CHILDREN AGES FIVE TO 10 (GRADES K-3) IN THE RUTLAND CITY PUBLIC SCHOOLS. THE PROGRAM, WHICH WILL BEGIN OPERATIONS IN FALL 2022, IS INTENDED TO BUILD GOOD ORAL HEALTH PRACTICES FOR CHILDREN AND FAMILIES, PROVIDE PREVENTIVE SERVICES, FACILITATE TREATMENT FOR EMERGING CONDITIONS, AND LINK CHILDREN TO A DENTAL HEALTH HOME AT CHCRR’S NEWLY-ESTABLISHED COMMUNITY KIDS DENTAL CLINIC IN RUTLAND. THIS PROJECT WILL ADDRESS THE NEEDS OF AN ESTIMATED 330 CHILDREN IN THE CITY’S THREE SCHOOLS SERVING ELEMENTARY AGE STUDENTS. THESE RURAL, LOW-INCOME CHILDREN ARE MORE THAN TWICE AS LIKELY TO HAVE UNTREATED TOOTH DECAY AS CHILDREN WITH FAMILY INCOMES GREATER THAN 200% OF THE FEDERAL POVERTY LEVEL. ONE OF THE SCHOOLS INCLUDED IN THE PROJECT SERVES THE 3RD-MOST IMPOVERISHED POPULATION OF STUDENTS IN THE STATE. TOOTH DECAY, IF LEFT UNTREATED, CAN LEAD TO PROBLEMS IN EATING, SPEAKING, AND LEARNING. STRONG EVIDENCE DEMONSTRATES THE EFFECTIVENESS OF PREVENTIVE DENTAL CARE SERVICES, AND INCREASING LOW-INCOME CHILDREN’S ACCESS TO THESE SERVICES IS A CRITICAL HEALTH GOAL. THE PROGRAM WILL BE STAFFED BY A DENTAL HYGIENIST, DENTAL ASSISTANT, AND PEDIATRIC DENTIST, AND MANAGED BY CHCRR’S DENTAL PRACTICE MANAGER. THE HYGIENIST WILL VISIT EACH OF THE THREE SCHOOLS MONTHLY AND PROVIDE ON-SITE SCREENINGS, FLUORIDE TREATMENTS, AND X-RAYS AS NEEDED. SHE WILL COORDINATE TELE-DENTAL VISITS AS APPROPRIATE WITH THE PEDIATRIC DENTIST TO CONDUCT FULL EXAMS, MAKE DIAGNOSES, AND PROVIDE DIRECTION FOR TREATMENT.

COMMUNITY PROJECT FUNDING/CONGRESSIONALLY DIRECTED SPENDING - CONSTRUCTION - COMMUNITY KIDS DENTAL WAS ESTABLISHED TO RESPOND TO URGENT NEED IN THE RUTLAND REGION. ON JUNE 5, 2021 THE SOLE PEDIATRIC DENTAL PROVIDER IN THE RUTLAND AREA ACCEPTING MEDICAID CLOSED HIS PRACTICE WITHOUT NOTICE, LEAVING CLOSE 5,000 PATIENTS WITHOUT A PROVIDER. CHCRR BEGAN RECEIVING 70-100 CALLS PER DAY FOR PEDIATRIC DENTAL CARE. CHCRR HAS HAD A LONG COMMITMENT TO DENTAL CARE AS A PART OF COMPREHENSIVE HEALTH CARE, OPERATING TWO DENTAL CLINICS, IN RUTLAND AND SHOREHAM. THESE PRACTICES WERE ALREADY AT CAPACITY, AND, DUE TO THE COVID-19 PANDEMIC, HAVE BACKLOGS OF APPOINTMENTS. AT THAT TIME CHCRR DID NOT HAVE A PEDIATRIC DENTIST ON STAFF. CHCRR RESPONDED TO THIS CRISIS BY ACQUIRING AND ADAPTING A NEW LOCATION TO ESTABLISH COMMUNITY KIDS DENTAL IN RUTLAND, WHICH OPENED IN JANUARY 2022. THE CLINIC IS LOCATED BELOW COMMUNITY DENTAL ON ALLEN STREET AND IS STAFFED BY A PEDIATRIC DENTIST, TWO DENTAL HYGIENISTS, TWO DENTAL ASSISTANTS, AND AN OFFICE MANAGER. RURAL, LOW-INCOME CHILDREN ARE MORE THAN TWICE AS LIKELY TO HAVE UNTREATED TOOTH DECAY AS CHILDREN WITH FAMILY INCOMES GREATER THAN 200% OF THE FEDERAL POVERTY LEVEL. TOOTH DECAY, IF LEFT UNTREATED, CAN LEAD TO PROBLEMS IN EATING, SPEAKING, AND LEARNING. COMMUNITY PROJECT FUNDING/CONGRESSIONALLY DIRECTED SPENDING - CONSTRUCTION - RUTLAND COUNTY IS HOME TO THE ONE OF THE LARGEST POPULATIONS OF RESIDENTS 85+ IN VERMONT. IN THIS LARGELY RURAL AREA, PORTIONS OF WHICH ARE MEDICALLY-UNDERSERVED AREAS/POPULATIONS, SENIORS FACE CHALLENGES IN ACCESSING HEALTH CARE, INCLUDING TRANSPORTATION, LACK OF SPECIALTY SERVICES, AND LACK OF SUPPORTS FOR MANAGING CHRONIC CONDITIONS. WITH A SHORTAGE OF PROVIDERS AND SERVICES THAT ARE GEOGRAPHICALLY AND ECONOMICALLY WITHIN REACH, ELDERS MISS DOCTORS APPOINTMENTS, SKIP MEDICATIONS, AND EXPERIENCE UNNECESSARY DECLINE IN HEALTH CONDITIONS. CHRONIC DISEASES (INCLUDING COPD, DIABETES, CARDIOVASCULAR DISEASE, AND CANCER) CLAIM THE LIVES OF MORE THAN 59% OF COUNTY RESIDENTS EACH YEAR. IN ADDITION, CHRONIC ILLNESSES ARE ASSOCIATED WITH SIGNIFICANTLY INCREASED RISK OF HAVING OR DEVELOPING A MENTAL HEALTH CONDITION. EXPANDED SPECIALTY CARE SERVICES ARE NEEDED FOR THOSE WITH MEMORY AND EARLY DEMENTIA DIAGNOSES IN ORDER TO ADDRESS HIGH RATES OF ALZHEIMER’S DISEASE DEATHS PER YEAR IN RUTLAND COUNTY. THIS PROJECT WILL CREATE A NEW GERIATRIC SERVICES CLINIC IN RUTLAND TO PROVIDE SPECIALTY SERVICES FOR ELDERS IN THE REGION. THIS EXPANSION OF COMMUNITY HEALTH CENTERS OF THE RUTLAND REGION (CHCRR) CURRENT GERIATRIC SERVICES PROGRAM WILL DOUBLE THE NUMBER OF CURRENT PROVIDERS AND INCREASE ACCESS TO SERVICES THAT MEET THE COMPLEX NEEDS OF OLDER ADULTS, INCLUDING CHRONIC DISEASE TREATMENT AND MANAGEMENT, MEMORY CARE, MENTAL HEALTH CARE, AND PHYSICAL REHABILITATION. THE PROJECT IS THE SECOND PHASE OF A TWO-PHASE RENOVATION AND MODIFICATION OF COMMUNITY HEALTH NORTH MAIN, A 17,972 SQUARE FOOT BUILDING ON 1.6 ACRES, LOCATED AT 231 NORTH MAIN STREET, RUTLAND. BUILT IN 1983, IT WAS PURCHASED IN SEPTEMBER 2021 BY CHCRR. THE BUILDING SERVED FROM 1993 TO 2021 AS ADMINISTRATIVE OFFICES FOR THE U.S. FORESTRY SERVICE. CHCRR IS ADAPTING THE SITE TO MEET EXPANDING ADMINISTRATIVE AND CLINICAL NEEDS, INCLUDING THE CREATION OF A NEW GERIATRIC SERVICES CLINIC. THE PROJECT WIL L: 1) INCREASE ACCESS TO SPECIALTY SERVICES FOR ELDERS, IMPROVING QUALITY OF CARE AND OUTCOMES; 2) IMPROVE ACCESS TO CHRONIC AND PREVENTIVE CARE, REDUCING COSTLY RELIANCE ON EMERGENCY DEPARTMENTS, HOSPITAL BEDS, AND NURSING HOMES; AND 3) MAKE A SOUND INVESTMENT IN ENSURING THAT CARE IS ACCESSIBLE FOR THE PROJECTED DOUBLING OF THE 65-AND-OVER POPULATION IN THE NEXT THREE DECADES.
