IN RECENT YEARS IT HAS CONSISTENTLY EARNED MULTIPLE BADGES FROM HRSA FOR THE QUALITY OF ITS SERVICES AND IT HAS EARNED THE HIGHEST LEVEL OF APPROVAL FROM THE JOINT COMMISSION AS A PRIMARY CARE MEDICAL HOME. NHCAC ALSO MAINTAINS APPROVAL TO RECEIVE AND SPEND FEDERAL FUNDING FOR HEALTH CARE SERVICES WITHIN ITS SCOPE. IT PROVIDES THOSE SERVICES IN FIVE LOCATIONS IN HUDSON COUNTY – JERSEY CITY, HARRISON, WEST NEW YORK, AND TWO SITES IN UNION CITY. ADDITIONALLY, NHCAC HAS HEALTH CENTERS IN BERGEN AND PASSAIC COUNTIES. IN CONCERT WITH THE ENDING THE HIV EPIDEMIC INITIATIVE, NHCAC ACKNOWLEDGES THAT HUDSON COUNTY HAS A SUBSTANTIVE NUMBER OF HIV DIAGNOSIS IN THIS SUBSET OF OUR SERVICE AREA. THE NJ ENHANCED HIV REPORTING SYSTEM FOR HUDSON COUNTY REPORTS THAT 5,055 COUNTY RESIDENTS WERE LIVING WITH HIV/AIDS AS OF 12/31/2020. OF THOSE, APPROXIMATELY 4,979 RESIDED IN THE NORTHERN REGION OF HUDSON COUNTY. IN RESPONSE, NHCAC HAS BEEN A CONSISTENT PARTNER OF THE HUDSON COUNTY TRANSITIONAL GRANT AREA’S (TGA) ADMINISTRATION OF THE RYAN WHITE HIV/AIDS PROGRAM AS AN HIV TESTING FACILITY. BETWEEN 2018 AND 2020, MORE THAN 40,000 HIV TESTS WERE ADMINISTERED. TO FURTHER THAT SUPPORT, NHCAC WILL IMPLEMENT DIRECT SERVICE DELIVERY TO ERADICATE HIV BY EMPLOYING THE ENDING THE HIV EPIDEMIC: A PLAN FOR AMERICA’S KEY STRATEGIES. NORTH HUDSON HEALTH CENTERS ARE A KEY ENTRY POINT, FOR PEOPLE WITH HIV WHO ARE UNDIAGNOSED. SUPPORTED BY COMMUNITY PARTNERS, NHCAC WILL DIAGNOSE PEOPLE WITH HIV AS EARLY AS POSSIBLE AFTER TRANSMISSION AND PREVENT NEW HIV TRANSMISSION USING PRE-EXPOSURE PROPHYLAXIS (PREP) AS ITS PROVEN INTERVENTION MODEL. NEXT, NHCAC HEALTHCARE WORKERS WILL CONNECT PATIENTS DIAGNOSED WITH HIV TO TREATMENT AND CASE MANAGEMENT SERVICES AT THE CENTER FOR COMPREHENSIVE CARE AT THE JERSEY CITY MEDICAL CENTER. THEN, NORTH HUDSON IS PREPARED TO RESPOND QUICKLY TO POTENTIAL HIV OUTBREAKS BY PROVIDING ACCESS TO PREP THROUGH INFORMED OUTREACH FACILITATED BY THE HUDSON COUNTY HIV/AIDS PLANNING COUNCIL AND HUDSON PRIDE CENTER. NHCAC FURTHER INTENDS TO WRAP ITS INTEGRATED HEALTH CARE SERVICES AROUND ITS PATIENTS TO PROVIDE COMPREHENSIVE CARE INCLUDING ADULT MEDICINE, WOMEN’S SERVICES AND FAMILY PLANNING, PEDIATRICS, DENTISTRY, SUBSTANCE ABUSE COUNSELING, MENTAL HEALTH SERVICES, PODIATRY CARE MANAGEMENT AND SOCIAL SERVICES. FINALLY, NHCAC WILL SUPPORT PREP ACCESS THROUGH ITS ESTABLISHED PARTICIPATION IN THE 340B DRUG DISCOUNT PROGRAM AT ITS WEST NEW YORK IN-HOUSE PHARMACY, A RANGE OF COMMUNITY PHARMACY PARTNERS AND UTILIZATION OF THE READY, SET, PREP PROGRAM. PURPOSE: THE PURPOSE OF HUD’S COMPREHENSIVE HOUSING COUNSELING (CHC) GRANT IS TO PROVIDE DIRECT FUNDING SUPPORT TO HUD’S NATION-WIDE NETWORK OF APPROVED HOUSING COUNSELING AGENCIES.
