NEONATAL ABSTINENCE SYNDROME CARE - ABSTRACT SUMMARY SUCCESSFUL OUTCOMES WHEN CARING FOR BABIES AFFECTED BY NEONATAL ABSTINENCE SYNDROME (NAS), AND THEIR MOTHERS EXPERIENCING SUBSTANCE USE DISORDER (SUD), REQUIRE A COMPREHENSIVE PROGRAM BRIDGING THE CONTINUUM OF CARE. THE LONG-TERM GOALS OF THE PROGRAM INCLUDE; PROACTIVE SCREENING TO IDENTIFY PREGNANT MOTHERS AT RISK, A DEDICATED SUD OBSTETRICAL NAVIGATOR AND COMPREHENSIVE OBSTETRICAL SERVICES. THE INTEGRATED MODEL OF CARE INCLUDES A TRAUMA INFORMED APPROACH, AND DEDICATED HOSPITAL ROOMS WHERE FAMILIES CAN STAY TOGETHER, AS THEY LEARN AND PRACTICE THE SPECIAL CARE NEEDS THAT HELP NAS THRIVE. SERVICES WILL BE ALIGNED WITH COMMUNITY PARTNERSHIPS WITH THE DEPARTMENT OF HEALTH, MONTGOMERY AND BUCKS COUNTIES HEALTH AND HUMAN SERVICES, THE CITY OF PHILADELPHIA’S OPIOID ADDICTION EFFORTS AND A BROAD RANGE OF PRIVATE AND PUBLIC ADDICTION SERVICES. SINCE 2013, HOLY REDEEMER HOSPITAL HAS CARED FOR OVER 400 BABIES WITH NAS, AND THEIR MOTHERS. IN 2021, WE PROVIDED COMPREHENSIVE CARE TO 50 BABIES WITH NAS AND THEIR MOTHERS AFFECTED WITH SUD, 10 MORE THAN LAST YEAR. THE MAJORITY #36 (72%) WERE FROM ZIP CODES IN PHILADELPHIA’S FOCAL POINT OF THE OPIOID EPIDEMIC, AND 14 FROM BUCKS AND MONTGOMERY COUNTIES. FIRST AND FOREMOST, AS THE INCIDENCE OF NAS AND MATERNAL OPIOID USE INCREASES, RESEARCH DEMONSTRATES THAT TRAUMA-INFORMED CARE IS A BEST PRACTICE FOR ADDRESSING CARE AND THE COMPLEX CHALLENGES OF ADDICTION. IN ORDER TO FACILITATE A TRAUMA INFORMED CULTURE IN OBSTETRICS WE AIM TO TRAIN AT LEAST 200 CAREGIVERS, WHO INTERACT WITH PATIENTS EXPERIENCING SUD. THIS INCLUDES, BUT NOT LIMITED TO, INPATIENT AND OUTPATIENT CAREGIVERS IN THE AREAS OF OB/GYN PRACTICES, NEONATAL INTENSIVE CARE UNIT (NICU), LABOR & DELIVERY, POST-PARTUM UNIT, OPERATING ROOMS, LACTATION SPECIALISTS, PASTORAL CARE, ENVIRONMENTAL SERVICES, DIETARY AND REGISTRATION STAFF. PATIENT NAVIGATION INCLUDES ASSESSING THE PSYCHOSOCIAL, PHYSICAL AND EMOTIONAL NEEDS OF FAMILIES CONFRONTING SUD. THE NAVIGATOR IS A RESOURCE FOR PATIENTS AND FAMILIES, AND THE COMMUNITY OF OBSTETRIC PATIENTS, PROVIDING AWARENESS, PROPER SCREENING AND RISK BEHAVIOR MODIFICATION GUIDANCE. THEY SERVE AS THE CENTRAL POINT OF CONTACT FOR OBSTETRIC PATIENTS CONFRONTED WITH, OR AT RISK OF, AN SUD AND ACT AS THE LIAISON BETWEEN PATIENT AND CLINICIANS. PATIENT ACCESS TO SERVICES PROVIDED WITHIN THE HEALTH SYSTEM, AND RESOURCES AVAILABLE IN THE COMMUNITY WILL BE PARAMOUNT BY THE END OF THE ONE-YEAR GRANT PERIOD, AT LEAST 100 (FOR A TOTAL OF 200) OBSTETRIC STAFF WILL BE TRAINED TO REACH THE GOAL OF CREATING TRAUMA-INFORMED, TRAUMA-RESPONSIVE AND TRAUMA-RESILIENT CARE. OVER 90 PERCENT OF OBSTETRIC PATIENTS WILL BE SCREENED FOR “RISK” AND “SOCIAL DETERMINANTS OF HEALTH” IN OUR FOUR OBSTETRIC PRACTICES. AT LEAST 40 FAMILIES EXPERIENCING NAS AND SUD WILL RECEIVE COMPRESSIVE CARE THROUGH THE CONTINUUM WITH APPROPRIATE SUPPORT SERVICES, INCLUDING DIETARY, BEHAVIORAL HEALTH, AND FOLLOW UP IN THE NICU CLINIC FOR DEVELOPMENTAL EVALUATION.
