THE ORGANIZATION WAS ESTABLISHED IN 1966 AS PART OF THE JOHNSON ADMINISTRATION'S WAR ON POVERTY AND HAS EVOLVED TO BECOME NORTHERN KENTUCKY'S PREEMINENT MULTIPURPOSE HUMAN SERVICES AGENCY. NKCAC HELPS OVER 20,000 LOW-INCOME INDIVIDUALS AND FAMILIES PER YEAR DEVELOP THE KNOWLEDGE, OPPORTUNITIES AND RESOURCES THEY NEED TO ACHIEVE SELF-RELIANCE. WITH SERVICES SPANNING EIGHT NORTHERN KENTUCKY COUNTIES THAT INCLUDE AFFORDABLE HOUSING, CHILD ABUSE PREVENTION, EARLY CHILDHOOD EDUCATION, JOB TRAINING, FINANCIAL EMPOWERMENT, WEATHERIZATION, NEIGHBORHOOD CENTERS AND SENIOR EMPLOYMENT PROGRAMMING, NKCAC IS GOVERNED BY A 21-MEMBER BOARD REPRESENTING DIVERSE SECTORS. NKCAC WILL PARTNER WITH AVIATRA, WHICH OFFERS CLASSES, COACHING, COMMUNITY AND OTHER RESOURCES TO HELP ENTREPRENEURS SUCCEED, TO RECRUIT SMALL BUSINESSES IN NORTHERN KENTUCKY TO PARTICIPATE IN THE R2H PROGRAM. AVIATRA WILL PROVIDE BUSINESS COACHING AND PROFITABILITY PLANNING TO ENSURE PARTICIPATING BUSINESSES ARE VIABLE AND PREPARED FOR WORKFORCE. CED FUNDS WILL BE USED TO PROVIDE LOANS TO AT LEAST 10 PARTICIPATING BUSINESSES FOR CAPITAL EXPENSES LIKE PURCHASING EQUIPMENT, ADDING CAPACITY AND HIRING. AT THE END OF THE THREE-YEAR GRANT, 68 (85%) STUDENTS WILL REALIZE A DEMONSTRATED GAIN IN LITERACY SKILLS; 68 (85%) STUDENTS WILL REALIZE A DEMONSTRATED GAIN IN NUMERACY SKILLS; AND 72 (90%) STUDENTS WILL REALIZE A DEMONSTRATED GAIN IN SOCIAL AND EMOTIONAL DEVELOPMENT. THE AMERICORPS FEDERAL INVESTMENT OF $260,000 WILL BE SUPPLEMENTED BY $28,889 IN NON-FEDERAL RESOURCES. NORTHERN KENTUCKY COMMUNITY ACTION COMMISSION (NKCAC) WILL MATCH FOSTER GRANDPARENT PROGRAM (FGP) VOLUNTEERS WITH CHILDREN AGED 3-5 ENROLLED IN HEAD START (HS) PROGRAMS OPERATED BY NKCAC. THE FGP VOLUNTEERS WILL MEET WITH STUDENTS FOR APPROXIMATELY 10 HOURS WEEKLY EITHER IN THE HEAD START CENTER, IN THE STUDENTS HOME, OR THROUGH TELESERVICE. FOSTER GRANDPARENTS WILL BE PLACED IN SEVEN HEAD START PROGRAMS OPERATED BY THE APPLICANT IN BOONE, CAMPBELL, KENTON, AND PENDLETON COUNTIES IN NORTHERN KENTUCKY AND IN-HOME PLACEMENTS WILL BE ORCHESTRATED BY THE HEAD START CENTER. FOSTER GRANDPARENTS WILL ACT AS ROLE MODELS, MENTORS, AND FRIENDS TO HEAD START CHILDREN WITH SPECIAL OR EXCEPTIONAL NEEDS, OR THOSE WHO ARE IN CIRCUMSTANCES THAT LIMIT THEIR ACADEMIC, SOCIAL, OR EMOTIONAL DEVELOPMENT. FOSTER GRANDPARENTS WILL TUTOR, MENTOR, AND COMFORT CHILDREN. THEY WILL HELP CHILDREN DEVELOP BASIC LEARNING SKILLS, SHARE LIFE EXPERIENCES, PROVIDE ONE-ON-ONE ATTENTION IN AREAS OF READING, WRITING, AND MATH, AND PROVIDE CHILDREN WITH EMOTIONAL SUPPORT AND EMPATHY. THE PROPOSED PROGRAM WILL PAIR LOW-INCOME SENIORS WITH LOW-INCOME STUDENTS TO HELP PREPARE FOR SUCCESS IN KINDERGARTEN AND BEYOND.

THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. FAYETTE COUNTY PREVENTION COALITION - THE COMMUNITY ACTION COMMISSION OF FAYETTE COUNTY (CAC), ON BEHALF OF THE FAITH IN RECOVERY FAYETTE COUNTY PREVENTION COALITION (PC), SUBMITS THIS APPLICATION FOR THE DRUG FREE COMMUNITIES GRANT PROGRAM. NESTLED IN SOUTHWESTERN OHIO, ROUGHLY ONE HOUR FROM COLUMBUS, CINCINNATI AND DAYTON AND ACCESSIBLE BY SEVERAL MAJOR HIGHWAYS, INCLUDING INTERSTATE 71; U.S. ROUTE 22, 35 & 62; & OHIO ROUTES 41,38,207,729, 734, AND 753, THE 28,903 RESIDENTS IN FAYETTE FELL INTO THE OPIOID EPIDEMIC SEEMINGLY OVERNIGHT BEGINNING 2014, QUICKLY CLIMBING TO THE TOP OF THE CHARTS FOR OPIOID OVERDOSE IN 2017, REACHING #2 IN OHIO, 7TH NATIONWIDE. FROM 2017-2021, DRUG FREE COMMUNITIES FUNDING SUPPORTED THE COALITION REDUCING YOUTH MARIJUANA, NICOTINE AND PRESCRIPTION DRUG PAST 30-DAY USE RATES BY .92%, 3.3% & 4.89% RESPECTIVELY. DURING THE 2022 OHYES! (YOUTH RISK BEHAVIOR SURVEY) YOUTH PAST 30-DAY USE RATES OF 7-12TH GRADERS WERE: NICOTINE (2.2%) AND MARIJUANA (5.5%), ALCOHOL (10.36%), AND PRESCRIPTION DRUGS (1.1%). IN THE SECTION BELOW WE OUTLINE OUR STRATEGIES AND GOALS. GOAL 1: INCREASE COMMUNITY COLLABORATION AROUND PREVENTING YOUTH SUBSTANCE USE, AS EVIDENCED BY, IN YEAR 5, OR BY 9/29/29, FAYETTE COUNTY DEMONSTRATING BEING AT LEVEL 9, A “HIGH LEVEL OF COMMUNITY OWNERSHIP” AFTER DEMONSTRATING A READINESS OF “INITIATION: IN 2023, COMPARED TO “PRE-PLANNING” IN THE YEAR ORIGINALLY FUNDED BY DFC. WE WILL ALSO MAINTAIN AT LEAST 24 COALITION MEMBERS AT ALL TIMES. WE WILL ACCOMPLISH THIS THROUGH COALITION MEETINGS, AN ANNUAL COLOR RUN, CONSTANT COMMUNITY PROMOTIONS THROUGH A VARIETY OF MEDIA SOURCES LIKE SOCIAL MEDIA, THE PAPER AND TELEVISION, YOUTH COALITION, CADCA TRAINING, TAKING OUR YOUTH TO THE STATEWIDE RALLY, & AN ANNUAL VOLUNTEER RECOGNITION AND DATA SHARING CEREMONY. GOAL 2: WE WILL ALSO REDUCE YOUTH ALCOHOL PAST 30-DAY USE BY 4% BY SEPTEMBER 29, 2029, THROUGH THE FOLLOWING: CONNECTING YOUTH WITH MENTAL HEALTH CONDITIONS TO TREATMENT, DEVELOPING A POSITIVE SOCIAL NORMS CAMPAIGN, CONTINUING IDENTIFICATION CHECKS AND ALSO DOING A PROJECT STICKER SHOCK TO REDUCE SOCIAL ACCESS, AND GETTING FAMILIES TO FOLLOW SOCIAL HOST LAWS. GOAL 3: WE WILL REDUCE YOUTH MARIJUANA USE BY 3% BY SEPTEMBER 29, 2029, THROUGH THE FOLLOWING: HAVING EVIDENCE-BASED PREVENTION FOR ALL FAYETTE COUNTY YOUTH, THE SAFE STORAGE & DISPOSAL OF MARIJUANA, CONNECTING YOUTH TO PROJECT TEEN INTERVENE INSTEAD OF BEING EXPELLED FROM SCHOOL AND BY HAVING RESOURCE OFFICERS ON EACH CAMPUS. WE WILL ALSO PROVIDE TRIPLE P PARENTING IN THE EVENINGS TO TARGET LOW-INCOME PARENTS AND STRENGTHENING FAMILIES AT ROSE AVENUE COMMUNITY CENTER FOR GRANDPARENTS PARENTING BECAUSE OF INCARCERATION AND/OR ADDICTION. WE WILL ALSO USE THE CANNABIS TOOLKIT AS A COMMUNITY EDUCATION PIECE. GOAL 4: WE WILL REDUCE YOUTH NICOTINE USE BY 1% BY 9/29/29, THROUGH EARLY INTERVENTION PROGRAMMING AT THE DISTRICTS INSTEAD OF EXPULSION AND EVIDENCE-BASED PREVENTION. GOAL 5: LAST, WE WILL REDUCE YOUTH PRESCRIPTION DRUG USE BY .5% BY 9/29/29 THROUGH PRESCRIBER EDUCATION AND DRUG TAKE BACK.

THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. LIMITATION ON USE OF FUNDS: NO ASSISTANCE PROVIDED UNDER PROGRAM (OR ANY STATE OR LOCAL GOVERNMENT FUNDS USED TO SUPPLEMENT THIS ASSISTANCE) MAY BE USED TO REPLACE STATE OR LOCAL FUNDS PREVIOUSLY USED, OR DESIGNATED FOR USE, TO ASSIST HOMELESS PERSONS OR PERSONS AT-RISK OF HOMELESSNESS.; EXPECTED OUTCOMES: DECREASE IN THE NUMBER INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS, MORE SPECIFICALLY USING PERFORMANCE INDICATORS SUCH AS THE LENGTH OF TIME HOMELESS, RETURNS TO HOMELESSNESS OVER TIME, AND EXITS TO PERMANENT HOUSING.
