| Department of Health and Human Services | TRANSFORMING MATERNAL HEALTH (TMAH) MODEL - THE LOUISIANA DEPARTMENT OF HEALTH (LDH) BUREAU OF HEALTH SERVICES FINANCING (BHSF), WHICH SERVES AS THE STATE MEDICAID AGENCY FOR LOUISIANA, REQUESTS $17,000,000 IN ORDER TO IMPLEMENT THE TRANSFORMING MATERNAL HEALTH (TMAH) MODEL IN A TEST REGION (“LA MATERNAL CARE TRANSFORMATION ZONE”) THAT IS COMPRISED OF CALDWELL, EAST CARROLL, FRANKLIN, JACKSON, LINCOLN, MADISON, MOREHOUSE, OUACHITA, RICHLAND, TENSAS, UNION, WEST CARROLL, EAST BATON ROUGE, EAST FELICIANA, IBERVILLE, POINTE COUPEE, WEST BATON ROUGE, AND WEST FELICIANA PARISHES (COUNTIES) OVER A TEN-YEAR GRANT PERIOD. THE COMPARABLE REGION WILL INCLUDE ORLEANS, ST. BERNARD, PLAQUEMINES, BIENVILLE, BOSSIER, CADDO, CLAIBORNE, DESOTO, NATCHITOCHES, RED RIVER, SABINE, AND WEBSTER PARISHES (COUNTIES). THESE TEST AND COMPARISON REGIONS ARE SIMILARLY SITUATED WITH REGARDS TO BIRTH VOLUME, RACIAL AND ECONOMIC CHARACTERISTICS AND AVAILABILITY OF SERVICES. LDH REQUESTS $8 MILLION TO ACCOMPLISH ACTIVITIES DURING THE PRE-IMPLEMENTATION PERIOD (MODEL YEARS 1-3), AND $9 MILLION DURING THE IMPLEMENTATION PERIOD (MODEL YEARS 4-10). GIVEN THAT APPROXIMATELY 60% OF ALL BIRTHS IN LOUISIANA ARE COVERED THROUGH MEDICAID, A COMPLETE TRANSFORMATION OF LOUISIANA’S FRAGMENTED SYSTEM OF CARE IS NECESSARY TO IMPROVE MATERNAL HEALTH CARE AND BIRTH OUTCOMES WHILE REDUCING HEALTH DISPARITIES FOR ALL. THE LDH WILL BUILD ON EXISTING EFFORTS OVER THE GRANT PERIOD IN ORDER TO IMPLEMENT THE TMAH MODEL IN THE LA MATERNAL CARE TRANSFORMATION ZONE WITH FIDELITY IN ORDER TO SIGNIFICANTLY REDUCE MATERNAL AND INFANT MORTALITY AND INCREASE ACCESS TO COORDINATED, HIGH QUALITY PERINATAL CARE. SPECIFICALLY, THE LDH WILL LEVERAGE TMAH FUNDING TO: 1) IMPROVE ITS DATA INFRASTRUCTURE TO BEST MEET THE NEEDS OF STAKEHOLDERS AND STRATEGICALLY ADDRESS GAPS IN MATERNAL HEALTH CARE, 2) INCREASE HOME MONITORING OF DIABETES AND HYPERTENSION; AND 3) INCREASE RISK-ASSESSMENT, SCREENING, REFERRALS, AND FOLLOW UP FOR PERINATAL DEPRESSION, ANXIETY, SUBSTANCE USE DISORDER, AND HEALTH RELATED SOCIAL NEEDS (HRSNS). IN PARTNERSHIP WITH CMS, LDH WILL MEASURE THE EFFECTIVENESS OF THE TMAH MODEL ON IMPROVING MATERNAL HEALTH OUTCOMES IN THE LA MATERNAL CARE TRANSFORMATION ZONE IN COMPARISON WITH A SIMILARLY SITUATED LOUISIANA REGION. BOTH THE LA MATERNAL CARE TRANSFORMATION ZONE AND COMPARISON REGION HAVE ABOUT 11,000 BIRTHS ANNUALLY. JUST UNDER 50% OF BIRTHS IN EACH REGION ARE TO BLACK WOMEN, WHILE ABOUT 40% ARE TO WHITE WOMEN. ABOUT 10% OF BIRTHS IN EACH REGION ARE TO PEOPLE WHO IDENTIFY AS NEITHER BLACK NOR WHITE. BOTH THE TEST AND COMPARISON REGIONS CONTAIN SEVERAL PARISHES THAT ARE IDENTIFIED BY THE MARCH OF DIMES IN A 2021 REPORT AS MATERNITY CARE DESERTS OR AREAS OF LOW ACCESS. (TEST REGION: CALDWELL, FRANKLIN, JACKSON, RICHLAND, TENSAS, UNION, WEST CARROLL PARISHES; COMPARISON REGION: DESOTO, SABINE, BIENVILLE, PLAQUEMINES AND ST. BERNARD PARISHES). BOTH THE TEST AND COMPARISON REGION HAVE SIGNIFICANT RACIAL DISPARITIES IN PERINATAL HEALTH OUTCOMES AND BOTH HAVE OUTCOMES SIGNIFICANTLY WORSE THAN THE NATIONAL AVERAGE. | $3M | FY2025 | Jan 2025 – Dec 2034 |

BEHAVIORAL HEALTH DISEASE RATES AND CO-MORBIDITIES FOR TUNICA-BILOXI ARE ELEVATED WHEN COMPARED TO NATIONAL AVERAGES, EXPERIENCING INCREASED BEHAVIORAL HEALTH DISEASE RATES AND CO-MORBIDITIES. ACCORDING TO THE LOUISIANA DEPARTMENT OF HEALTH, NON-MEDICAL PRESCRIPTION DRUG USE AMONG TUNICA-BILOXI TRIBAL MEMBERS IS ESTIMATED AT 15.9%; WHILE ILLICIT DRUG USE IS APPROXIMATELY 18.4% FOR TRIBAL MEMBERS LIVING ON THE RESERVATION FOR 2020. THIS GREATLY EXCEEDS THE NATIONAL AVERAGE AS WELL AS THE AVERAGE AMONG AMERICAN INDIANS PER SAMHSA’S 2017 NATIONAL SURVEY ON DRUG USE HEALTH. SIMILARLY, ALCOHOL ABUSE AMONG TUNICA-BILOXI TRIBAL ADULTS IS 22.4% PER THE TUNICA-BILOXI SOCIAL SERVICES DEPARTMENT, WHICH IS ABOVE THE NATIONAL AVERAGE AND JUST BELOW THE AVERAGE FOR AI/AN POPULATIONS. OTHER TRIBAL SURVEY DATA HAS DEMONSTRATED THAT TUNICA-BILOXI HAS A HIGH PREVALENCE OF DEPRESSION, AUTISM, ATTENTION DEFICIT (HYPERACTIVITY) DISORDER, AND POST-TRAUMATIC STRESS DISORDER. IT IS KNOWN THAT WHEN ONE BEHAVIORAL HEALTH DISPARITY OCCURS, OTHER DISPARITIES GO UNTREATED. THE 2019 SAMHSA NATIONAL SURVEY ON DRUG USE AND HEALTH, FOUND THAT APPROXIMATELY 34.5% OF YOUTH AGED 16 TO 25 THAT WERE DIAGNOSED WITH A MENTAL HEALTH ISSUE ALSO HAVE A SUBSTANCE ABUSE DISORDER THAT WAS NOT TREATED. COLLECTIVELY, THESE MEASURES HIGHLIGHT THE UNIQUE CONSTELLATION OF BEHAVIORAL HEALTH DISPARITIES THAT CONTRIBUTE TO POOR HEALTH OUTCOMES FOR TUNICA-BILOXI CITIZENS. AN INTEGRATED APPROACH TO BEHAVIORAL HEALTHCARE, CAN ASSIST IN ADDRESSING MANY OF THESE ISSUES. INTEGRATING BEHAVIORAL HEALTHCARE SERVICES THROUGH THE BH2I PROGRAM WILL ALLOW THE TRIBE TO ACHIEVE THE BELOW KEY GOALS: 1. ALTHOUGH THE AVERAGE HOUSEHOLD INCOME FOR TUNICA-BILOXI TRIBAL MEMBERS IS $18,518 (LOUISIANA WORKFORCE COMMISSION 2017 LABOR STATISTICS), NEARLY 50% OF TRIBAL HOUSEHOLDS FALL IN THE LOW-TO-MODERATE INCOME RANGE BASED ON U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT LMI HOUSEHOLD DATA (HOUSEHOLD INCOME LESS THAN $23,850 FOR A FAMILY OF FOUR) WHICH LIMITS ACCESS TO HEALTHCARE AND SUBSTANCE ABUSE TREATMENT SERVICES. WITH A SUBSTANCE ABUSE RATE OF 15.7% AMONG AMERICAN INDIANS NATIONALLY AND AN ILLICIT SUBSTANCE ABUSE RATE OF 22.4% (AMONG TUNICA-BILOXI TRIBAL MEMBERS BETWEEN THE AGES OF 16-35), IT IS IMPERATIVE THAT THE TUNICA-BILOXI TRIBE OF LOUISIANA PROVIDE COMPREHENSIVE OPIOID ABUSE ABATEMENT PROGRAM. UNDER THE CURRENT TUNICA-BILOXI SUBSTANCE ABUSE PROGRAM, TRIBAL MEMBERS SEEKING ASSISTANCE ARE ABLE TO ACCESS TREATMENT AND SUPPORT SERVICES DURING THE RECOVERY PROCESS. ALTHOUGH THIS PROGRAM HAS BEEN AROUND FOR THREE YEARS AND HAS HELPED MANY TRIBAL MEMBERS, MANY MORE TRIBAL MEMBERS SUFFER FROM OPIOID ABUSE ISSUES. FURTHERMORE, WITH THE RISE OF PRESCRIPTION DRUG ABUSE SUCH OPIOIDS AND BENZOS, MANY TRIBAL MEMBERS ARE BECOMING ADDICTED TO LEGAL DRUGS INCLUDING TRIBAL YOUTH. DUE TO DRUG ARRESTS THAT HAVE BEEN MADE ON THE RESERVATION INVOLVING OPIOIDS, THE RATE OF SUBSTANCE ABUSE CASES ON THE RESERVATION HAS INCREASED BY 9% FROM 2016 TO 2020 (TUNICA-BILOXI TRIBAL POLICE DEPARTMENT). THE KEY GOALS OF THE TUNICA-BILOXI SUBSTANCE ABUSE PROGRAM ARE TO: 1. DECREASE THE RATE OF OPIOID AND SUBSTANCE ABUSE AMONG TUNICA-BILOXI TRIBAL MEMBERS BY PROVIDING COMPREHENSIVE, CULTURALLY APPROPRIATE, WRAP AROUND SUBSTANCE ABUSE TREATMENT SERVICES TO TRIBAL MEMBERS WITHIN OUR SERVICE AREA; AND 2. THROUGH THE IMPLEMENTATION OF THE TRIBAL OPIOID RESOURCE GRANT, THE TUNICA-BILOXI TRIBE INTENDS TO BUILD INTERNAL CAPACITY AND GROW EXTERNAL PARTNERSHIPS TO EDUCATE AND TRAIN STAFF THAT ARE RESPONSIBLE FOR OPIOID AND SUBSTANCE ABUSE TREATMENT AND PREVENTION IN USING CULTURALLY APPROPRIATE SERVICES WHEN TREATING NATIVE AMERICANS THAT INVOLVE EVIDENCE-BASED PRACTICES. THE TRIBAL RESERVATION IS IN AVOYELLES PARISH (COUNTY); AND THE TRIBAL CATCHMENT AREA INCLUDES BOTH AVOYELLES PARISH AND THE NEIGHBORING PARISH OF RAPIDES. THERE ARE 1,580 ENROLLED TUNICA-BILOXI TRIBAL MEMBERS. APPROXIMATELY 42% LIVE EITHER ON OR NEAR THE RESERVATION AND DESIGNATED TRIBAL LANDS LOCATED IN CENTRAL LOUISIANA. ALTHOUGH THE AVERAGE HOUSEHOLD INCOME FOR TUNICA-BILOXI TRIBAL MEMBERS IS $18,518 (LOUISIANA WORKFORCE COMMISSION 2017 LABOR STATISTICS), NEARLY 50% OF TRIBAL HOUSEHOLDS FALL IN THE LOW-TO-MODERATE INCOME RANGE BASED ON U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT LMI HOUSEHOLD DATA (HOUSEHOLD INCOME LESS THAN $23,850 FOR A FAMILY OF FOUR) WHICH LIMITS ACCESS TO HEALTHCARE AND SUBSTANCE ABUSE TREATMENT SERVICES. BASED ON REQUESTS FOR TREATMENT SERVICES FROM TRIBAL MEMBERS, POLICE DEPARTMENT ARRESTS AND CALLS, COMMUNITY INTERACTION BY THE SOCIAL SERVICES DEPARTMENT, AND COMMUNITY COMPLAINTS, SUBSTANCE ABUSE HAS BEEN DEEMED AN IMPORTANT ISSUE FOR OUR COMMUNITY. TRIBAL MEMBERS FACING SUBSTANCE ABUSE ISSUES ARE IN DESPERATE NEED OF BOTH CULTURALLY APPROPRIATE TREATMENT SERVICES AND SUPPORTIVE SERVICES. THE USE OF PRESCRIPTION DRUGS, OPIOIDS, BENZODIAZEPINES AND METHAMPHETAMINES IS ESPECIALLY PREVALENT AMONG TRIBAL MEMBERS IN OUR COMMUNITY. ACCORDING TO THE TRIBAL POLICE DEPARTMENT, NON-MEDICAL PRESCRIPTION DRUG USE AMONG TUNICA-BILOXI TRIBAL MEMBERS IS 8.3%; WHILE ILLICIT DRUG USE IS APPROXIMATELY 22.4% (TUNICA-BILOXI SOCIAL SERVICES DEPARTMENT). THESE RATES GREATLY EXCEED THE NATIONAL AVERAGE AS WELL AS THE NATIONAL AVERAGE AMONG AMERICAN INDIANS PER THE DEPARTMENT OF HEALTH AND HUMAN SERVICES OFFICE OF SUBSTANCE ABUSE AND MENTAL HEALTH ADMINISTRATION. THIS PROJECT WILL SERVE OVER 100 TRIBAL MEMBERS THROUGH DIRECT SERVICES AND OUTREACH EFFORTS. GOAL NUMBER 1: DECREASE THE RATE OF OPIOID AND SUBSTANCE ABUSE AMONG TUNICA-BILOXI TRIBAL MEMBERS BY PROVIDING COMPREHENSIVE, CULTURALLY APPROPRIATE, WRAP AROUND SUBSTANCE ABUSE TREATMENT SERVICES TO TRIBAL MEMBERS WITHIN OUR SERVICE AREA. IN ADDITION, ENSURE THAT TRIBAL YOUTH HAVE ACCESS TO SUBSTANCE ABUSE TREATMENT SERVICES. GOAL 2. INCREASE ACCESS TO MENTAL HEALTH AND COUNSELING SERVICES AVAILABLE TO TRIBAL MEMBERS WITH OPIOID AND SUBSTANCE ABUSE ADDICTIONS. THERE IS A COMMUNITY HEALTH DIVISON THAT IS BEING EXPANDED TO INTEGRATE CLINICAL CARE WITH COMMUNITY HEALTH TO FOCUS ON HEALTH PROMOTION AND DISEASE PREVENTION USING A WRAP AROUND SERVICE MODEL THAT ADDRESSES DETERMINANTS OF HEALTH AT VARIOUS LEVELS OF SOCIO-ECOLOGICAL FACTORS. | Department of Health and Human Services | TUNICA-BILOXI TRIBAL SELF-GOVERNANCE PROGRAM - TSGP ABSTRACT SUMMARY THE TUNICA-BILOXI TRIBE IS SEEKING FUNDING UNDER THE INDIAN HEALTH SERVICE’S (IHS) HHS-2024- IHS-TSGP-0001 TO SUPPORT THE RESEARCH AND ANALYSIS OF SELF-GOVERNANCE. AN AWARD SUCH AS HHS- 2024-IHS-TSGP-0001 WILL ASSIST THE TRIBE WITH THE RESOURCES TO SUPPLEMENT THE HEALTH PROGRAMS FUNDS TO CONDUCT THIS ANALYSIS OF PSFAS AND TRIBAL SHARES. THE TBTLA HAS BEEN CONTRACTING FOR PROGRAM FUNDING FOR 36 YEARS UNDER TITLE I OF THE IHS ISDEAA CONTRACTING. A SIGNIFICANT NUMBER OF HOURS WILL BE REQUIRED TO COLLECT DATA, CONDUCT ANALYSIS, MEET WITH AGENCY OFFICIALS, CULMINATING REPORTS, AND FINANCIAL SUMMARIES. THIS PLANNING PROCESS IS LONG OVERDUE FOR ASSUMPTION OF EXPANDING PSFAS, AND TO ENJOY THE GROWTH SELF-GOVERNANCE COMPACTING PROVIDES. THE STRONG HEALTH LEADERSHIP CAPABILITIES IS PRIMED FOR SUCCESS. TRANSFORMING MATERNAL HEALTH (TMAH) MODEL - THE LOUISIANA DEPARTMENT OF HEALTH (LDH) BUREAU OF HEALTH SERVICES FINANCING (BHSF), WHICH SERVES AS THE STATE MEDICAID AGENCY FOR LOUISIANA, REQUESTS $17,000,000 IN ORDER TO IMPLEMENT THE TRANSFORMING MATERNAL HEALTH (TMAH) MODEL IN A TEST REGION (“LA MATERNAL CARE TRANSFORMATION ZONE”) THAT IS COMPRISED OF CALDWELL, EAST CARROLL, FRANKLIN, JACKSON, LINCOLN, MADISON, MOREHOUSE, OUACHITA, RICHLAND, TENSAS, UNION, WEST CARROLL, EAST BATON ROUGE, EAST FELICIANA, IBERVILLE, POINTE COUPEE, WEST BATON ROUGE, AND WEST FELICIANA PARISHES (COUNTIES) OVER A TEN-YEAR GRANT PERIOD. THE COMPARABLE REGION WILL INCLUDE ORLEANS, ST. BERNARD, PLAQUEMINES, BIENVILLE, BOSSIER, CADDO, CLAIBORNE, DESOTO, NATCHITOCHES, RED RIVER, SABINE, AND WEBSTER PARISHES (COUNTIES). THESE TEST AND COMPARISON REGIONS ARE SIMILARLY SITUATED WITH REGARDS TO BIRTH VOLUME, RACIAL AND ECONOMIC CHARACTERISTICS AND AVAILABILITY OF SERVICES. LDH REQUESTS $8 MILLION TO ACCOMPLISH ACTIVITIES DURING THE PRE-IMPLEMENTATION PERIOD (MODEL YEARS 1-3), AND $9 MILLION DURING THE IMPLEMENTATION PERIOD (MODEL YEARS 4-10). GIVEN THAT APPROXIMATELY 60% OF ALL BIRTHS IN LOUISIANA ARE COVERED THROUGH MEDICAID, A COMPLETE TRANSFORMATION OF LOUISIANA’S FRAGMENTED SYSTEM OF CARE IS NECESSARY TO IMPROVE MATERNAL HEALTH CARE AND BIRTH OUTCOMES WHILE REDUCING HEALTH DISPARITIES FOR ALL. THE LDH WILL BUILD ON EXISTING EFFORTS OVER THE GRANT PERIOD IN ORDER TO IMPLEMENT THE TMAH MODEL IN THE LA MATERNAL CARE TRANSFORMATION ZONE WITH FIDELITY IN ORDER TO SIGNIFICANTLY REDUCE MATERNAL AND INFANT MORTALITY AND INCREASE ACCESS TO COORDINATED, HIGH QUALITY PERINATAL CARE. SPECIFICALLY, THE LDH WILL LEVERAGE TMAH FUNDING TO: 1) IMPROVE ITS DATA INFRASTRUCTURE TO BEST MEET THE NEEDS OF STAKEHOLDERS AND STRATEGICALLY ADDRESS GAPS IN MATERNAL HEALTH CARE, 2) INCREASE HOME MONITORING OF DIABETES AND HYPERTENSION; AND 3) INCREASE RISK-ASSESSMENT, SCREENING, REFERRALS, AND FOLLOW UP FOR PERINATAL DEPRESSION, ANXIETY, SUBSTANCE USE DISORDER, AND HEALTH RELATED SOCIAL NEEDS (HRSNS). IN PARTNERSHIP WITH CMS, LDH WILL MEASURE THE EFFECTIVENESS OF THE TMAH MODEL ON IMPROVING MATERNAL HEALTH OUTCOMES IN THE LA MATERNAL CARE TRANSFORMATION ZONE IN COMPARISON WITH A SIMILARLY SITUATED LOUISIANA REGION. BOTH THE LA MATERNAL CARE TRANSFORMATION ZONE AND COMPARISON REGION HAVE ABOUT 11,000 BIRTHS ANNUALLY. JUST UNDER 50% OF BIRTHS IN EACH REGION ARE TO BLACK WOMEN, WHILE ABOUT 40% ARE TO WHITE WOMEN. ABOUT 10% OF BIRTHS IN EACH REGION ARE TO PEOPLE WHO IDENTIFY AS NEITHER BLACK NOR WHITE. BOTH THE TEST AND COMPARISON REGIONS CONTAIN SEVERAL PARISHES THAT ARE IDENTIFIED BY THE MARCH OF DIMES IN A 2021 REPORT AS MATERNITY CARE DESERTS OR AREAS OF LOW ACCESS. (TEST REGION: CALDWELL, FRANKLIN, JACKSON, RICHLAND, TENSAS, UNION, WEST CARROLL PARISHES; COMPARISON REGION: DESOTO, SABINE, BIENVILLE, PLAQUEMINES AND ST. BERNARD PARISHES). BOTH THE TEST AND COMPARISON REGION HAVE SIGNIFICANT RACIAL DISPARITIES IN PERINATAL HEALTH OUTCOMES AND BOTH HAVE OUTCOMES SIGNIFICANTLY WORSE THAN THE NATIONAL AVERAGE.

TUNICA-BILOXI BEHAVIORAL HEALTH INTEGRATION INITIATIVE - THE TUNICA-BILOXI TRIBE IS SEEKING FUNDING UNDER THE INDIAN HEALTH SERVICE’S (I.H.S.) BEHAVIORAL HEALTH INTEGRATION INITIATIVE (BH2I) TO SUPPORT INTEGRATION OF THE TRIBE’S BEHAVIORAL HEALTH SERVICES AND CAPACITIES WITH PRIMARY CARE AND COMMUNITY BASED SERVICES OF THE TRIBE AND OTHER COMMUNITY PARTNERS. BEHAVIORAL HEALTH DISEASE RATES AND CO-MORBIDITIES FOR TUNICA-BILOXI ARE ELEVATED WHEN COMPARED TO NATIONAL AVERAGES, EXPERIENCING INCREASED BEHAVIORAL HEALTH DISEASE RATES AND CO-MORBIDITIES. ACCORDING TO THE LOUISIANA DEPARTMENT OF HEALTH, NON-MEDICAL PRESCRIPTION DRUG USE AMONG TUNICA-BILOXI TRIBAL MEMBERS IS ESTIMATED AT 15.9%; WHILE ILLICIT DRUG USE IS APPROXIMATELY 18.4% FOR TRIBAL MEMBERS LIVING ON THE RESERVATION FOR 2020. THIS GREATLY EXCEEDS THE NATIONAL AVERAGE AS WELL AS THE AVERAGE AMONG AMERICAN INDIANS PER SAMHSA’S 2017 NATIONAL SURVEY ON DRUG USE HEALTH. SIMILARLY, ALCOHOL ABUSE AMONG TUNICA-BILOXI TRIBAL ADULTS IS 22.4% PER THE TUNICA-BILOXI SOCIAL SERVICES DEPARTMENT, WHICH IS ABOVE THE NATIONAL AVERAGE AND JUST BELOW THE AVERAGE FOR AI/AN POPULATIONS. OTHER TRIBAL SURVEY DATA HAS DEMONSTRATED THAT TUNICA-BILOXI HAS A HIGH PREVALENCE OF DEPRESSION, AUTISM, ATTENTION DEFICIT (HYPERACTIVITY) DISORDER, AND POST-TRAUMATIC STRESS DISORDER. IT IS KNOWN THAT WHEN ONE BEHAVIORAL HEALTH DISPARITY OCCURS, OTHER DISPARITIES GO UNTREATED. THE 2019 SAMHSA NATIONAL SURVEY ON DRUG USE AND HEALTH, FOUND THAT APPROXIMATELY 34.5% OF YOUTH AGED 16 TO 25 THAT WERE DIAGNOSED WITH A MENTAL HEALTH ISSUE ALSO HAVE A SUBSTANCE ABUSE DISORDER THAT WAS NOT TREATED. COLLECTIVELY, THESE MEASURES HIGHLIGHT THE UNIQUE CONSTELLATION OF BEHAVIORAL HEALTH DISPARITIES THAT CONTRIBUTE TO POOR HEALTH OUTCOMES FOR TUNICA-BILOXI CITIZENS. AN INTEGRATED APPROACH TO BEHAVIORAL HEALTHCARE, CAN ASSIST IN ADDRESSING MANY OF THESE ISSUES. INTEGRATING BEHAVIORAL HEALTHCARE SERVICES THROUGH THE BH2I PROGRAM WILL ALLOW THE TRIBE TO ACHIEVE THE BELOW KEY GOALS: 1. IMPROVE CARE COORDINATION OF BEHAVIORAL HEALTHCARE WITH INTERNAL AND LOCAL PARTNERS 2. EXPAND BEHAVIORAL HEALTH CARE SERVICES THROUGH THE USE OF CULTURALLY APPROPRIATE EVIDENCE-BASED (EBP) AND PRACTICE-BASED MODELS LIKE TRAUMA-INFORMED CARE TO ADDRESS SUBSTANCE ABUSE ISSUES. 3. IMPROVE BEHAVIORAL HEALTHCARE INFRASTRUCTURE

ONCE COMPLETED, THE PROJECT WILL AIDE IN THE CREATION OF NETWORKING, CONNECTING, AND STRENGTHENING RELATIONSHIPS AMONGST COMMUNITY PARTNERSHIPS, ENHANCING ECONOMIC GROWTH, PROVIDING COMPETITIVE WAGES TO ALL EMPLOYEES, AND PROVIDING JOB OPPORTUNITIES THROUGHOUT THE REGION.

THIS WILL BE ACCOMPLISHED THROUGH THE FOLLOWING GOAL: DECREASE THE RATE OF SUBSTANCE AND ALCOHOL ABUSE AMONG TUNICA-BILOXI TRIBAL MEMBERS BY PROVIDING COMPREHENSIVE, CULTURALLY APPROPRIATE, PROSECUTORIAL SERVICES WITHIN THE LOCAL SERVICE AREA. THROUGH THE EXPANSION OF TRIBAL COURT PROSECUTOR CAPACITY, THE TUNICA-BILOXI TRIBE WILL BE ABLE TO COMPREHENSIVELY ABLE TO ADDRESS SUBSTANCE/ALCOHOL MISUSE, RECIDIVISM AND OTHER CRIMINOGENIC NEEDS AMONG TRIBAL MEMBERS INVOLVED FROM THE CRIMINAL JUSTICE SYSTEM BY PROVIDING EVIDENCE-BASED CULTURAL SOLUTIONS AS ALTERNATIVES TO CURRENT METHODS EMPLOYED BY PARISH/STATE CRIMINAL JUSTICE SYSTEMS. TO THIS END, THE TRIBAL COURT WILL INCREASE STAFFING AND SERVICES TO HANDLE CASES UNDER TRIBAL JURISDICTION, EXPAND TYPES OF CASES UNDER TRIBAL COURT SUCH AS DOMESTIC VIOLENCE AND SEXUAL ASSAULT IN ADDITION TO CURRENT CIVIL AND CRIMINAL CASES AND HEALING-TO-WELLNESS COURT ACTIVITIES.

RECOGNIZING THAT EVERY CHILD HAS UNIQUE NEEDS, THE TRIBE AIMS TO OFFER INDIVIDUALIZED CARE AND PLANNING TO HELP FAMILIES NAVIGATE COMPLEX SYSTEMS OF SUPPORT. A DEDICATED CASE MANAGEMENT COORDINATOR WOULD STREAMLINE ACCESS TO SERVICES, OFFERING FAMILIES CRITICAL SUPPORT THAT PROMOTES STABILITY AND WELL-BEING. AS PART OF THIS EFFORT, THE TUNICA-BILOXI YOUTH COORDINATOR WILL PLAY A CENTRAL ROLE IN WORKING DIRECTLY WITH TRIBAL YOUTH BY DELIVERING AFTER-SCHOOL PROGRAMMING, PROVIDING CASE MANAGEMENT, AND CONDUCTING COMMUNITY OUTREACH TO INCREASE YOUTH ENGAGEMENT. THE AFTER-SCHOOL PROGRAM WILL BE OPEN TO CHILDREN AGES 516 AND HOSTED AT THE TUNICA-BILOXI GYMNASIUM, MONDAY THROUGH FRIDAY. THE PROGRAM IS DESIGNED TO OFFER A SAFE AND SUPPORTIVE ENVIRONMENT FOCUSED ON ACADEMIC ENRICHMENT, SOCIAL DEVELOPMENT, AND CULTURAL CONNECTION. THIS INITIATIVE WILL ALSO SERVE AS A PLATFORM TO IDENTIFY CHILDREN IN NEED OF ADDITIONAL SUPPORT AND ENSURE TIMELY INTERVENTION THROUGH COORDINATED RESPONSES. TUNICA-BILOXI TRIBAL OPIOID RESPONSE PROJECT - THE TUNICA-BILOXI TRIBE OF LOUISIANA IS SEEKING FUNDING UNDER SAMHSA’S TRIBAL OPIOID RESPONSE PROGRAM TO ENHANCE ITS SUBSTANCE ABUSE PROGRAM, SPECIFICALLY OPIOID TREATMENT AND PREVENTION. THE TUNICA-BILOXI TRIBE OF LOUISIANA IS ONE OF FOUR FEDERALLY RECOGNIZED TRIBES IN LOUISIANA AND HAS BEEN A FEDERALLY RECOGNIZED TRIBE SINCE 1981. THERE ARE OVER 1,600 ENROLLED TUNICA-BILOXI TRIBAL MEMBERS. APPROXIMATELY 40% LIVE EITHER ON OR NEAR THE RESERVATION AND DESIGNATED TRIBAL LANDS LOCATED IN CENTRAL LOUISIANA. ALTHOUGH THE AVERAGE HOUSEHOLD INCOME FOR TUNICA-BILOXI TRIBAL MEMBERS IS $18,518 (LOUISIANA WORKFORCE COMMISSION 2017 LABOR STATISTICS), NEARLY 50% OF TRIBAL HOUSEHOLDS FALL IN THE LOW-TO-MODERATE INCOME RANGE BASED ON U.S. DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT LMI HOUSEHOLD DATA (HOUSEHOLD INCOME LESS THAN $23,850 FOR A FAMILY OF FOUR) WHICH LIMITS ACCESS TO HEALTHCARE AND SUBSTANCE ABUSE TREATMENT SERVICES. WITH A SUBSTANCE ABUSE RATE OF 15.7% AMONG AMERICAN INDIANS NATIONALLY AND AN ILLICIT SUBSTANCE ABUSE RATE OF 22.4% (AMONG TUNICA-BILOXI TRIBAL MEMBERS BETWEEN THE AGES OF 16-35), IT IS IMPERATIVE THAT THE TUNICA-BILOXI TRIBE OF LOUISIANA PROVIDE COMPREHENSIVE OPIOID ABUSE ABATEMENT PROGRAM. UNDER THE CURRENT TUNICA-BILOXI SUBSTANCE ABUSE PROGRAM, TRIBAL MEMBERS SEEKING ASSISTANCE ARE ABLE TO ACCESS TREATMENT AND SUPPORT SERVICES DURING THE RECOVERY PROCESS. ALTHOUGH THIS PROGRAM HAS BEEN AROUND FOR THREE YEARS AND HAS HELPED MANY TRIBAL MEMBERS, MANY MORE TRIBAL MEMBERS SUFFER FROM OPIOID ABUSE ISSUES. FURTHERMORE, WITH THE RISE OF PRESCRIPTION DRUG ABUSE SUCH OPIOIDS AND BENZOS, MANY TRIBAL MEMBERS ARE BECOMING ADDICTED TO LEGAL DRUGS INCLUDING TRIBAL YOUTH. DUE TO DRUG ARRESTS THAT HAVE BEEN MADE ON THE RESERVATION INVOLVING OPIOIDS, THE RATE OF SUBSTANCE ABUSE CASES ON THE RESERVATION HAS INCREASED BY 9% FROM 2016 TO 2020 (TUNICA-BILOXI TRIBAL POLICE DEPARTMENT). THE KEY GOALS OF THE TUNICA-BILOXI SUBSTANCE ABUSE PROGRAM ARE TO: 1. DECREASE THE RATE OF OPIOID AND SUBSTANCE ABUSE AMONG TUNICA-BILOXI TRIBAL MEMBERS BY PROVIDING COMPREHENSIVE, CULTURALLY APPROPRIATE, WRAP AROUND SUBSTANCE ABUSE TREATMENT SERVICES TO TRIBAL MEMBERS WITHIN OUR SERVICE AREA; AND 2. INCREASE ACCESS TO MENTAL HEALTH AND COUNSELING SERVICES AVAILABLE TO TRIBAL MEMBERS WITH OPIOID AND SUBSTANCE ABUSE ADDICTIONS.

THE TRIBAL RESERVATION IS IN AVOYELLES PARISH (COUNTY); AND THE TRIBAL CATCHMENT AREA INCLUDES BOTH AVOYELLES PARISH AND THE NEIGHBORING PARISH OF RAPIDES. THERE ARE 1,580 ENROLLED TUNICA-BILOXI TRIBAL MEMBERS. APPROXIMATELY 42% LIVE EITHER ON OR NEAR THE RESERVATION AND DESIGNATED TRIBAL LANDS LOCATED IN CENTRAL LOUISIANA. TRIBAL MEMBERS FACING SUBSTANCE ABUSE ISSUES ARE IN DESPERATE NEED OF BOTH CULTURALLY APPROPRIATE TREATMENT SERVICES AND SUPPORTIVE SERVICES. THE USE OF PRESCRIPTION DRUGS, OPIOIDS, BENZODIAZEPINES AND METHAMPHETAMINES IS ESPECIALLY PREVALENT AMONG TRIBAL MEMBERS IN OUR COMMUNITY. ACCORDING TO THE TRIBAL POLICE DEPARTMENT, NON-MEDICAL PRESCRIPTION DRUG USE AMONG TUNICA-BILOXI TRIBAL MEMBERS IS 8.3%; WHILE ILLICIT DRUG USE IS APPROXIMATELY 22.4% (TUNICA-BILOXI SOCIAL SERVICES DEPARTMENT). THESE RATES GREATLY EXCEED THE NATIONAL AVERAGE AS WELL AS THE NATIONAL AVERAGE AMONG AMERICAN INDIANS PER THE DEPARTMENT OF HEALTH AND HUMAN SERVICES OFFICE OF SUBSTANCE ABUSE AND MENTAL HEALTH ADMINISTRATION. THIS PROJECT WILL SERVE OVER 100 TRIBAL MEMBERS THROUGH DIRECT SERVICES AND OUTREACH EFFORTS. GOAL NUMBER 1: DECREASE THE RATE OF OPIOID AND SUBSTANCE ABUSE AMONG TUNICA-BILOXI TRIBAL MEMBERS BY PROVIDING COMPREHENSIVE, CULTURALLY APPROPRIATE, WRAP AROUND SUBSTANCE ABUSE TREATMENT SERVICES TO TRIBAL MEMBERS WITHIN OUR SERVICE AREA. IN ADDITION, ENSURE THAT TRIBAL YOUTH HAVE ACCESS TO SUBSTANCE ABUSE TREATMENT SERVICES. GOAL 2. INCREASE ACCESS TO MENTAL HEALTH AND COUNSELING SERVICES AVAILABLE TO TRIBAL MEMBERS WITH OPIOID AND SUBSTANCE ABUSE ADDICTIONS. THE HEALTH DEPARTMENT WILL PROVIDE SUBSTANCE ABUSE COUNSELING SERVICES TO A MINIMUM OF 8 TRIBAL MEMBERS WITH CO-OCCURRING SUBSTANCE ABUSE AND MENTAL HEALTH DISORDERS. THE HEALTH DEPARTMENT WILL PROVIDE OR REFER MENTAL HEALTH COUNSELING SERVICES TO TRIBAL MEMBERS WITH SUBSTANCE ABUSE ADDICTIONS THAT LACK ADEQUATE ACCESS TO SERVICES. IN ADDITION, THE SAP WILL PROVIDE COMPREHENSIVE CASE MANAGEMENT SERVICES TO TRIBAL MEMBERS RECEIVING SUBSTANCE/ALCOHOL ABUSE SERVICES TO SUPPORT RECOVERY. GENERAL NUTRITIONAL EDUCATION SUCH AS HEALTHY EATING HABITS AND WHAT FOODS TO AVOID IN ORDER TO CONTROL AND PREVENT DMII IS NEEDED IN OUR TRIBAL COMMUNITY. THERE IS A COMMUNITY HEALTH DIVISON THAT IS BEING EXPANDED TO INTEGRATE CLINICAL CARE WITH COMMUNITY HEALTH TO FOCUS ON HEALTH PROMOTION AND DISEASE PREVENTION USING A WRAP AROUND SERVICE MODEL THAT ADDRESSES DETERMINANTS OF HEALTH AT VARIOUS LEVELS OF SOCIO-ECOLOGICAL FACTORS. THE HEALTH DEPT. IS WORKING WITH THE LOCAL FARMERS MARKET TO PROVIDE FRESH PRODUCE AT A REDUCED COST TO OUR MEMBERS AS WELL AS IMPLEMENTING A HEALTH AND WELLNESS PASSPORT PROGRAM WHERE OUR MEMBERS ARE ENCOURAGED TO PARTICIPATE IN HEALTH AND WELLNESS CHECK-UPS WHERE INCENTIVES FOR COMPLETION CAN BE EARNED.THERE ARE TENTATIVE PLANS FOR A ANNUAL HEALTH FAIR TO BE HELD HERE ON THE TUNICA BILOXI RESERVATION. PURPOSE: RECIPIENTS OF THE CORONAVIRUS CAPITAL PROJECTS FUND (CCPF), WILL DESIGNATE FINANCIAL ASSISTANCE TOWARDS CARRYING OUT CRITICAL CAPITAL PROJECTS THAT DIRECTLY ENABLE WORK, EDUCATION, AND HEALTH MONITORING, INCLUDING REMOTE OPTIONS, IN RESPONSE TO THE PUBLIC HEALTH EMERGENCY WITH RESPECT TO THE CORONAVIRUS PANDEMIC (COVID-19). ACTIVITIES TO BE PERFORMED: UNDER THE CCPF PROGRAM, RECIPIENTS WILL CARRY OUT CRITICAL CAPITAL PROJECTS DIRECTLY ENABLING WORK, EDUCATION, AND HEALTH MONITORING, INCLUDING REMOTE OPTIONS, IN RESPONSE TO THE PUBLIC HEALTH EMERGENCY WITH RESPECT TO THE CORONAVIRUS DISEASE (COVID-19). THE CONSTRUCTION AND DEPLOYMENT OF BROADBAND INFRASTRUCTURE PROJECTS ARE ELIGIBLE FOR FUNDING UNDER THE CCPF PROGRAM IF THE INFRASTRUCTURE IS DESIGNED TO DELIVER, UPON PROJECT COMPLETION, SERVICE THAT RELIABLY MEETS OR EXCEEDS SYMMETRICAL DOWNLOAD AND UPLOAD SPEEDS OF 100 MBPS. END GOAL/EXPECTED OUTCOMES: RECIPIENTS WILL FUND INVESTMENTS IN ELIGIBLE CAPITAL PROJECTS THAT: 1) IMPROVE COMMUNITIES' PHYSICAL OR REMOTE CONNECTIVITY BY INVESTING IN CAPITAL ASSETS THAT DIRECTLY ENABLE WORK, EDUCATION, AND HEALTH MONITORING; 2) ADDRESS A NEED THAT RESULTS FROM OR WAS EXACERBATED BY THE COVID-19 PUBLIC HEALTH EMERGENCY; AND 3) ADDRESS A NEED FOR UNDERSERVED OR UNSERVED PEOPLE. INTENDED BENEFICIARIES: THE PRIMARY INTENDED BENEFICIARIES UNDER THE CCPF PROGRAM ARE THOSE WHO FACE CHALLENGES CAUSED BY COVID-19, ESPECIALLY IN RURAL AMERICA, LOW AND MODERATE-INCOME COMMUNITIES, INCLUDING, HOUSEHOLDS, BUSINESSES, NONPROFIT INSTITUTIONS/ORGANIZATIONS, AND OTHER KEY PUBLIC INSTITUTIONS IN THE ELIGIBLE ENTITIES' JURISDICTIONS. TUNICA-BILOXI TRIBAL SELF-GOVERNANCE PROGRAM - TSGP ABSTRACT SUMMARY THE TUNICA-BILOXI TRIBE IS SEEKING FUNDING UNDER THE INDIAN HEALTH SERVICE’S (IHS) HHS-2024- IHS-TSGP-0001 TO SUPPORT THE RESEARCH AND ANALYSIS OF SELF-GOVERNANCE. AN AWARD SUCH AS HHS- 2024-IHS-TSGP-0001 WILL ASSIST THE TRIBE WITH THE RESOURCES TO SUPPLEMENT THE HEALTH PROGRAMS FUNDS TO CONDUCT THIS ANALYSIS OF PSFAS AND TRIBAL SHARES. THE TBTLA HAS BEEN CONTRACTING FOR PROGRAM FUNDING FOR 36 YEARS UNDER TITLE I OF THE IHS ISDEAA CONTRACTING. A SIGNIFICANT NUMBER OF HOURS WILL BE REQUIRED TO COLLECT DATA, CONDUCT ANALYSIS, MEET WITH AGENCY OFFICIALS, CULMINATING REPORTS, AND FINANCIAL SUMMARIES. THIS PLANNING PROCESS IS LONG OVERDUE FOR ASSUMPTION OF EXPANDING PSFAS, AND TO ENJOY THE GROWTH SELF-GOVERNANCE COMPACTING PROVIDES. THE STRONG HEALTH LEADERSHIP CAPABILITIES IS PRIMED FOR SUCCESS. COLLECTIVELY, THESE OBJECTIVES AND MEASURES HIGHLIGHT THE UNIQUE CONSTELLATION OF DATA ANALYSIS THAT IS ESSENTIAL TO EXPAND PSFAS. THE PLANNING PROCESS THROUGH THE HHS-2024-IHS-TSGP-0001 COOPERATIVE AGREEMENT WILL ALLOW THE TRIBE TO ACHIEVE THE BELOW KEY GOALS: 1. TO COLLECT DATA AND MAKE AN INFORMED DECISION TO EXPAND PSFAS AND ASSOCIATED SHARES; 2. ACHIEVE SELF GOVERNANCE COMPACTING AND IMPLEMENTING PROVISIONS OF ISDEAA TITLE V; 3. IDENTIFY RESOURCES TO MAXIMIZE PROGRAM POTENTIAL TO IMPROVE SERVICES TO TRIBAL CITIZENS; 4. IMPROVE CARE COORDINATION OF HEALTHCARE FORGING PARTNERSHIPS INTERNAL AND EXTERNAL; 5. EXPAND HEALTH CARE SERVICES THROUGH THE USE OF CULTURALLY APPROPRIATE EVIDENCE-BASED AND PRACTICE-BASED MODELS TO ADDRESS SUBSTANCE ABUSE ISSUES; 6. IMPROVE HEALTHCARE INFRASTRUCTURE; 7.
