TRANSITION TO HOPE (BEBASHI) PROPOSES A HIGH-IMPACT HIV PREVENTION, TESTING, AND LINKAGE PROGRAM TO HIGH-RISK MSM AND TRANSGENDER PEOPLE OF COLOR - SPECIFICALLY AFRICAN AMERICAN. BEBASHI IS THE FIRST BLACK-LED ORGANIZATION IN THE COUNTRY TO ADDRESS THE HIV/AIDS EPIDEMIC IN URBAN COMMUNITIES OF COLOR. BEBASHI AIMS TO ADDRESS EACH OF THE FOUR PILLARS OF THE ENDING THE HIV EPIDEMIC PLAN THROUGH CONDUCTING HIV TESTING IN COMMUNITY-BASED AND HEALTHCARE SETTINGS FOR PHILADELPHIA?S MOST AT-RISK INDIVIDUALS - MSM AND TRANSGENDER PEOPLE OF COLOR ? OVER THE FIVE-YEAR CONTRACT PERIOD. SERVICES WILL INCLUDE HIV TESTING; RISK REDUCTION EDUCATION; DISTRIBUTION OF CONDOMS AND BARRIERS; PREP ASSESSMENT EDUCATION AND REFERRAL; STREET OUTREACH AND RECRUITMENT; AND ONLINE RECRUITMENT. THE ORGANIZATION PROPOSES HIRING A FULL-TIME PREVENTION MANAGER, WHO WILL OVERSEE THE PROJECT AS WELL AS TEST WHEN NEEDED, FOUR (4) FULL-TIME PREVENTION NAVIGATORS WHO WILL CONDUCT HIV TESTS, STI SCREENINGS AND TESTS, AS WELL AS PROVIDE NAVIGATION SERVICES TO HIV NEGATIVE HIGH-RISK INDIVIDUALS. ALL POSITIVES FOUND THROUGH TESTING WILL BE REFERRED TO BEBASHI'S ON-SITE MEDICAL CASE MANAGEMENT TEAM FOR CASE MANAGEMENT SERVICES. A PART-TIME NURSE PRACTITIONER WILL BE HIRED FOR THE BEBASHI WELLNESS CLINIC TO ASSUME ALL HIV, STI AND OTHER TREATMENT DUTIES. FINALLY, BEBASHI WILL UTILIZE THE PREP SQUAD, A GROUP OF YOUNG BLACK AND BROWN LGBTQ+ YOUTH WHO ACT AS A STREET OUTREACH TEAM AS EDUCATORS AND RECRUITERS.BEBASHI PLANS TO COLLABORATE WITH THE COLOURS ORGANIZATION TO PROVIDE EXTENDED TESTING HOURS AND A DROP-IN CENTER FOR YOUNG LGBTQ+ PEOPLE OF COLOR.THROUGH THE DURATION OF THIS FIVE-YEAR CONTRACT, BEBASHI AIMS TO ACCOMPLISH THE FOLLOWING PR IMARY OUTCOMES OVER THE FIVE-YEAR PROJECT PERIOD: 1) TEST A TOTAL OF 10,700 MSM AND TRANSGENDER INDIVIDUALS USING FOURTH GENERATION HIV TESTS; 2) IDENTIFY 60 NEW HIV POSITIVES; 3) RELINK 70 INDIVIDUALS WHO HAVE BEEN OUT OF CARE; 4) INCREASE THE NUMBER OF PERSONS AWARE OF THEIR HIV STATUS BY 80% EACH YEAR FOR THOSE WHO HAVE NEVER BEEN TESTED AND/OR INDIVIDUALS WHO HAVE NOT BEEN TESTED IN ONE CALENDAR YEAR OR LONGER; 5) ENSURE THAT 100% OF PERSONS TESTED FOR HIV ARE ALSO SCREENED FOR STIS AND HEPATITIS C; 6) ENSURE THAT 100% OF NEW HIV POSITIVES ARE LINKED TO THE BEBASHI WELLNESS CLINIC OR A PROVIDER OF CHOICE FOR ARTAS; 7) REFER 100% OF PERSONS LOST TO CARE TO THE BEBASHI WELLNESS CLINIC OR A PROVIDER OF CHOICE FOR ARTAS; 8) REFER 100% OF PERSONS LOST TO CARE TO A BEBASHI MEDICAL CASE MANAGER; 9) REFER 100% OF NEW POSITIVE PERSONS TO A BEBASHI MEDICAL CASE MANAGER; 10) ENSURE 85% TREATMENT ADHERENCE FOR HIV POSITIVE INDIVIDUALS; 11) ENSURE BEBASHI?S VIRAL SUPPRESSION RATE REMAINS ABOVE 85%; AND 12) REFER 75% HIV NEGATIVE PERSONS TO PREP/PEP.
