AS THE LEAD APPLICANT, HQP WILL SUBCONTRACT WITH SIX FQHC COMMUNITY HEALTH CENTER ORGANIZATIONS THAT WILL EMPLOY CERTIFIED ENROLLMENT COUNSELORS (CEC) TO IDENTIFY ELIGIBLE CHILDREN, PROVIDE ONE-ON-ONE ENROLLMENT SERVICES, VERIFY ENROLLMENT, AND CONDUCT ANNUAL RENEWAL FOLLOW-UP TO ENSURE RETENTION. IT IS ANTICIPATED THAT 15,000 CHILDREN WILL SUBMIT AN APPLICATION FOR HEALTH COVERAGE, 4,080 WILL BE NEWLY ENROLLED, AND 7,920 SUCCESSFULLY RETAINED OVER THE THREE-YEAR PERIOD. HQP WAS A GRANTEE OF CMS DURING CYCLE IV OF THE CONNECTING KIDS TO COVERAGE PROGRAM AND IS A CURRENT GRANTEE OF THE FY19 CKC PROJECT. ALL SIX PARTICIPATING HEALTH CENTERS SERVED AS SUBCONTRACTORS OF HQP DURING THE CYCLE IV AND FY19 PROJECTS AND ARE OPERATIONALLY READY TO CONTINUE PROVIDING SERVICES TO CHILDREN AND FAMILIES IN THE NEW THREE-YEAR PROJECT. PROJECT PARTNERS WILL IMPLEMENT CLINIC INREACH AND COMMUNITY OUTREACH STRATEGIES THAT HAVE PROVEN SUCCESSFUL DURING THE PREVIOUS TWO CYCLES TO ENROLL CHILDREN WHO REMAIN UNINSURED. HQP WILL PUT SPECIAL EMPHASIS ON VERIFYING ENROLLMENT TO ENSURE THAT A MINIMUM OF 80% OF APPLICANTS (BOTH NEW ENROLLMENTS AND RENEWALS) ARE PROCESSED SUCCESSFULLY THROUGH TO VERIFIED ENROLLMENT. PROJECT PARTNERS WILL ALSO PARTICIPATE IN CMS NATIONAL CAMPAIGN ACTIVITIES TO INCREASE ENROLLMENT IN MEDICAID AND PARTICIPATE IN TARGETED OUTREACH AND EDUCATION TO INFORM FAMILIES WITH CHILDREN OF THE NEED TO RENEW ENROLLMENT AS THE COVID-19 PUBLIC HEALTH EMERGENCY ENDS. THIS INITIATIVE FOCUSES ON DEVELOPING AND ENHANCING EBIS AT HEALTH CENTERS WITH HISTORICALLY LOW CRC SCREENING RATES. KEY STRATEGIES INCLUDE INTEGRATING PROVIDER REMINDERS, PATIENT EDUCATION CAMPAIGNS, AND PATIENT NAVIGATION SERVICES TO ADDRESS STRUCTURAL BARRIERS SUCH AS TRANSPORTATION AND ACCESS TO SPECIALTY CARE FOR UNINSURED INDIVIDUALS, ENSURING COLONOSCOPY COMPLETIONS. HQP WILL SUPPORT THE DEVELOPMENT OF AN INTERFACE BETWEEN ARCADIA, ITS POPULATION HEALTH MANAGEMENT PLATFORM, AND COLOGUARD, ENSURING PHCS HAVE SEAMLESS ACCESS TO SCREENING RESULTS FOR MORE EFFECTIVE TRACKING, FOLLOW-UP, AND REPORTING OF SCREENING COMPLETIONS. EXPECTED PROJECT OUTCOMES INCLUDE INCREASED CRC SCREENING PREVALENCE, HIGHER STOOL TEST RETURN RATES, IMPROVED COLONOSCOPY COMPLETION RATES, AND THE INTEGRATION OF SUSTAINABLE SCREENING PRACTICES ACROSS PARTICIPATING CLINICS. THIS INITIATIVE ALIGNS WITH HEALTHY PEOPLE 2030 GOALS AND CDC PRIORITIES BY ADDRESSING SCREENING DISPARITIES AND PROMOTING SYSTEMATIC CHANGES THAT ENSURE ACCESS TO CRC SCREENING AND FOLLOW-UP CARE. THROUGH TARGETED INTERVENTIONS AND COLLABORATIVE PARTNERSHIPS, HQP AND ITS PARTNERS WILL ENHANCE EARLY DETECTION, REDUCE CRC-RELATED MORTALITY, AND IMPROVE LONG-TERM HEALTH OUTCOMES FOR COMMUNITIES IN SOUTHERN CALIFORNIA.
