A SUMMARY OF THE PROPOSED ACTIVITY AND ITS INTENDED IMPACT: DEVELOPING CAPACITY TO IMPROVE THE INTEGRATION AND COORDINATION OF CARE ACROSS PRIMARY CARE, DENTAL, AND BEHAVIORAL HEALTH FOR PEOPLE LIVING WITH HIV/AIDS (PLWHA) WILL IMPROVE OVERALL HEALTH AND WELLNESS. EVCHC WILL IMPLEMENT A FOUR-PRONGED APPROACH TO DEVELOP CAPACITY IN AN INNOVATIVE CARE MODEL THAT WILL IMPROVE INTEGRATION OF HIV PRIMARY CARE WITH ORAL HEALTH AND BEHAVIORAL HEALTH SERVICES AND ULTIMATELY IMPROVE RETENTION OF PATIENTS, COMPLIANCE WITH CARE, AND IMPROVED HEALTH FOR PLWHA: 1) ADD A 1.0 FTE CARE COORDINATOR TO INCREASE THE LINKAGE TO DENTAL AND BEHAVIORAL HEALTH SERVICES FOR PLWHA; 2) IMPLEMENT WELL HEALTH AS A COMMUNICATION AND APPOINTMENT PLATFORM FOR PLWHA; 3) IMPLEMENT A CONTINUOUS QUALITY IMPROVEMENT (CQI) STRATEGY; AND 4) IMPLEMENT A PRINT AND SOCIAL MEDIA MARKETING STRATEGY FOR PLWHA. THESE OBJECTIVES WILL ALLOW EVCHC TO REACH ITS GOAL TO INCREASE RETENTION OF CARE TO 90 PERCENT, INCREASE PRESCRIPTION OF ART TO 82 PERCENT, AND INCREASE VIRAL LOAD SUPPRESSION RATE TO 80 PERCENT FOR ALL PLWHA.
