LOUIS, MO 63111 PROJECT DIRECTOR NAME: ROBERT MASSIE, DDS, CEO CONTACT PHONE NUMBERS: PHONE (314) 481-1615 EXTENSION 1315; FAX (314) 353-1310 EMAIL ADDRESS: BMASSIE@FCHCSTL.ORG WEBSITE: WWW.FAMILYCAREHEALTHCENTERS.ORG GRANT PROGRAM FUNDS REQUESTED: $325,000 FROM FISCAL YEAR 2022 ENDING THE HIV EPIDEMIC RATES OF HIV TRANSMISSION ARE HIGH IN THE ST. LOUIS REGION AND THE USE OF PREP IS LOW AMONG UNDERSERVED COMMUNITIES; LACK OF EDUCATION, STIGMA AROUND HIV AND LACK OF MONEY TO AFFORD PREP CONTRIBUTE TO THE GREAT NEED. FAMILY CARE HEALTH CENTERS (FCHC) CURRENTLY PROVIDES COMPREHENSIVE PRIMARY CARE, INCLUDING ON-SITE BEHAVIORAL HEALTH, PHARMACY, DENTAL AND OUTREACH SERVICES, TO OVER 22,000 PATIENTS. APPROXIMATELY 15% OF CURRENT PATIENTS IDENTIFY AS LATINO, 35% IDENTIFY AS AFRICAN-AMERICAN, 40% IDENTIFY AS CAUCASIAN AND 10% IDENTIFY AS A WIDE VARIETY OF OTHER RACES/ETHNICITIES. THIS PROPOSED PROJECT, COMPREHENSIVE PRIMARY CARE HIV TREATMENT AND PREVENTION, WILL SERVE UNDERSERVED RESIDENTS OF THE ST. LOUIS REGION INCLUDING EXISTING FCHC PATIENTS AS WELL AS NEW PATIENTS. THE FOUR STRATEGIES OF THE INITIATIVE, DIAGNOSE, TREAT, PREVENT AND RESPOND, WILL BE WOVEN INTO EXISTING STRATEGY TO TREAT ALL PATIENTS COMPREHENSIVELY THROUGH PRIMARY CARE. THE PROJECT WILL TEST ALL PATIENTS AT RISK FOR HIV; THE ONES THAT TEST POSITIVE WILL IMMEDIATELY ENTER CARE THAT WILL TREAT THE COMPREHENSIVELY WITH PRIMARY CARE, BEHAVIORAL HEALTH CARE AND APPROPRIATE MEDICATIONS. PATIENTS THAT TEST NEGATIVE WILL BE STRONGLY ENCOURAGED TO BEGIN PREP. MONITORING OF POSITIVE TESTS WILL ALLOW A QUICK RESPONSE TO HIV OUTBREAKS IN THE COMMUNITY AND DEPLOY PREVENTION AND TREATMENT SERVICES QUICKLY.
