THE GOAL OF THE MOSHI PROJECT IS TO DELIVER HIGH QUALITY OPIOID USE DISORDER TREATMENT AND DETOX THROUGH BOTH TELEHEALTH AND MOBILE SOLUTIONS. THE DEVELOPMENT OF A MOBILE DETOX UNIT AND OUD/SUD TREATMENT DELIVERED VIA TELEHEALTH, WILL REACH BOTH RURAL AND FRONTIER HEALTHCARE SETTINGS TO ALLOW IDAHO RESIDENTS TO GAIN ACCESS TO SERVICES IN HEALTHCARE SHORTAGE AREAS. PROJECT PROPOSED SERVICES: THE MOSHI PROJECT WILL DEVELOP A REGIONAL RURAL VIRTUAL HEALTH NEIGHBORHOOD MODEL IN WHICH REGIONAL HUBS ARE LINKED TO SPOKE CARE SITES (HEALTH AND NON-HEALTH PROVIDERS) VIA CARE COMPACTS. THESE SITES WILL BE SUPPORTED THROUGH ROBUST TECHNICAL ASSISTANCE INCLUDING?1) DATA ANALYTICS RESOURCES, 2) POPULATION HEALTH TECHNOLOGY, 3) FINANCIAL MODELING, 4) CARE?MANAGEMENT, 5) TELEHEALTH STRATEGIES, 6) INTEGRATED BEHAVIORAL HEALTH, 7) MENTAL HEALTH, 8) SUBSTANCE USE AND OPIOID USE DISORDERS, AND 9) CLINICAL PHARMACY INTEGRATION. DESCRIPTION OF THE TARGET POPULATION: THE TARGET POPULATION ACROSS ALL THE TARGET SITES INCLUDES AN AVERAGE OF 13.24% OF INDIVIDUALS LIVING AT OR BELOW THE POVERTY LINE. 14.69% OF INDIVIDUALS LIVING WITHOUT HEALTH INSURANCE, ALL UNDER THE AGE OF 65. WITH 19.6% BEING OF HISPANIC ETHNICITY AND 1.93% BEING OF AMERICAN INDIAN DESCENT. THE MOSHI PROJECT WILL SERVE: 1) INDIVIDUALS AT RISK OF, DIAGNOSED WITH, IN TREATMENT FOR, AND/OR IN RECOVERY FOR SUD/OUD AND/OR CO-OCCURRING MENTAL DISORDERS; 2) INDIVIDUALS IN RURA L COMMUNITIES; 3) ALL AGES; 4) ALL INSURED (COMMERCIAL, MEDICAID, MEDICARE, & MILITARY); AND 5) ALL SAFETY NET POPULATIONS (WORKING POOR, UNINSURED, OR INDIGENT POPULATION).

FY 2024 BEHAVIORAL HEALTH SERVICE EXPANSION - OLE HEALTH, DBA COMMUNICARE+OLE WILL ADDRESS THE NEED FOR ADDITIONAL MENTAL HEALTH SERVICES AND SUBSTANCE USE DISORDER TREATMENT IN THE SERVICE AREA. THE PROPOSED PROGRAM WILL INCREASE THE NUMBER OF PATIENTS RECEIVING MENTAL HEALTH SERVICES AND THE NUMBER OF PATIENTS RECEIVING SUD SERVICES, INCLUDING TREATMENT WITH MEDICATIONS FOR OPIOID USE DISORDER (MOUD) BY REACHING PEOPLE WHO OTHERWISE MIGHT NOT SEEK BEHAVIORAL HEALTH CARE, INCLUDING THE UNHOUSED, AND THOSE WITHOUT HEALTH INSURANCE. THE ORGANIZATION WILL UTILIZE INTEGRATED BEHAVIORAL HEALTH SERVICES, EVIDENCE-BASED SERVICES, SUCH AS INDIVIDUAL AND FAMILY THERAPY, CASE MANAGEMENT, STREET MEDICINE, MOBILE HEALTH, AND COMMUNITY PARTNERSHIPS TO INCREASE THE NUMBER OF PERSONS SERVED. A FOCAL POINT OF THIS OPPORTUNITY IS THE INCLUSION OF COMMUNITY HEALTH WORKERS (CHW) ACTING AS PEER SUPPORT ADVOCATES THAT WILL BE THE BRIDGE BETWEEN THE PATIENT, THE SUD BH THERAPIST, AND THE PRIMARY CARE PRESCRIBER. THIS PROGRAM WILL ADDRESS BARRIERS TO INDIVIDUALS SUCH AS COMMUNITY BIAS/STIGMA, LACK OF AFFORDABLE PROGRAMS, AND LACK OF TRANSPORTATION. THE BILINGUAL COMMUNITY HEALTH WORKERS WITH LIVED EXPERIENCE WILL ASSIST PATIENTS IN NAVIGATING SERVICES AVAILABLE IN THE COMMUNITY, ADDRESS STIGMA AND FACILITATE TRUST FOR INDIVIDUALS NEEDING BEHAVIORAL HEALTH CARE, OR TREATMENT FOR DRUG OR ALCOHOL DEPENDENCE. ANOTHER KEY COMPONENT OF THIS PROPOSAL IS THE INCLUSION OF BH STAFF TRAINING IN EMDR FOR THE CERTIFICATION PROCESS AND USE OF EMDR WITH CLIENTS. EMDR IS A NON-INVASIVE, EVIDENCE-BASED METHOD OF PSYCHOTHERAPY THAT HELPS VICTIMS RECOVER FROM THE EFFECTS OF PSYCHOLOGICAL TRAUMA THROUGH ADAPTIVE INFORMATION PROCESSING. EMDR THERAPY HAS BEEN LISTED AS AN EFFECTIVE TREATMENT BY THE AMERICAN PSYCHIATRIC ASSOCIATION, DEPARTMENTS OF DEFENSE AND VETERANS AFFAIRS, AND THE SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES. THE OVERALL APPROACH OF THIS PROGRAM WILL ALSO INCREASE THE SUPPORT GIVEN TO PRIMARY CARE PROVIDERS BY THE INCLUSION OF CHWS, RESULTING IN THE GREATER INTEGRATION OF BH IN PRIMARY CARE, AND ADDITIONAL HEALTH CENTER PRIMARY CARE PROVIDERS PRESCRIBING MEDICALLY ASSISTED TREATMENT.

CC+OLE IMPLEMENTS PRODUCE RX, A PATIENT-CENTERED INITIATIVE TO CONNECT CONNECT LOW-INCOME INDIVIDUALS WITH ELEVATED BLOOD SUGAR TO THE AGRICULTURAL BOUNTY OF YOLO COUNTY. PRODUCE RX LEVERAGES CC+OLE'S HEALTHY LIVING WITH DIABETES, SWEET SUCCESS AND FOOD IS MEDICINE ACTIVITIES TO PROVIDE PATIENTS WITH HIGH-QUALITY SELF-MANAGEMENT AND FOOD SKILLS EDUCATION WITH PRODUCE PRESCRIPTIONS.OVER THE PROJECT PERIOD, 580 CC+OLE PATIENTS WILL PARTICIPATE IN REGULAR EDUCATION ACTIVITIES AND RECEIVE PRODUCE GROWN BY LOCAL SMALL AND BEGINNING FARMERS. THE PROJECT WILL INCREASE PARTICIPANTS' COMSUMPTION OF LOCALLY-GROWN FRUITS AND VEGETABLES, EXPAND THEIR FOOD LITERACY, REDUCE FOOD INSECURITY, IMPROVE HEALTH OUTCOMES INCLUDING REDUCED A1C, AND BOOST SALES OPPORTUNITIES FOR SMALL AND BEGINNING FARMERS.PARTICIPANTS ENROLL IN ONE OF FOUR PRODUCE PRESCRIPTION INTERVENTION MODELS BASED ON THE SITE AT WHICH THEY RECEIVE HEALTHCARE SERVICES, INCLUDING BRICK AND MORTAR, FARMERS MARKETS, AND PRODUCE BOXES. UC BERKELEY EVALUATORS USE MIXED METHODS TO CONDUCT PROCESS AND OUTCOME EVALUATION.
