FY 2024 BEHAVIORAL HEALTH SERVICE EXPANSION - THIS BHSE INITIATIVE IS PROPOSING TO SIGNIFICANTLY EXPAND ACCESS TO MOUD AND SUD/OUD TREATMENT AS WELL AS MENTAL HEALTH COUNSELING FOR THESE TWO TARGET POPULATIONS – THE UNSHELTERED POPULATION AS WELL AS YOUTH AND YOUNG ADULTS ACCESSING SERVICES AT OUR TEEN CLINIC AND THREE SBHCS. AS THERE ARE NO OTHER SUBSTANCE USE TREATMENT PROGRAMS IN WESTERN SONOMA COUNTY ANYMORE SINCE THE COUNTY HAS STOPPED PROVIDING THESE SERVICES, WCHC IS IN A UNIQUE POSITION TO EXPAND ACCESS TO MOUD AND SUD/OUD TREATMENT WITHIN A PRIMARY CARE SETTING IN THE LOCAL COMMUNITIES THAT WE SERVE. AS PART OF THIS BHSE INITIATIVE, WCHC WILL EXPAND THE ROLE OF DR. GARRISON-JAKEL TO BECOME A 0.2 FTE ADDICTION MEDICINE CLINICAL DIRECTOR THAT WILL BE ABLE TO DEDICATE TIME TOWARDS SECURING TRAININGS AVAILABLE THROUGH SUTTER HEALTH AND CONNECTIONS TO ADDICTION MEDICINE EXPERTS FOR OUR EXISTING 14 PROVIDERS SO THEY RECEIVE THE TRAINING AND EXPERTISE NEEDED TO BEGIN OFFERING MOUD TREATMENT TO PATIENTS THROUGHOUT OUR NETWORK OF CLINICS. AS THE NEW ADDICTION MEDICINE CLINICAL DIRECTOR, DR. GARRISON-JAKEL WILL PROVIDE CLINICAL SUPERVISION FOR OUR MAT PROVIDERS AND FAMILY PRACTICE PHYSICIANS, NURSE PRACTITIONERS, AND PSYCHIATRIC NURSE PRACTITIONERS AS THEY RECEIVE TRAINING AND BEGIN PROVIDING MOUD TREATMENT. IN ADDITION, WCHC WILL HIRE A 0.4 FTE ADDICTION MEDICINE PROVIDER THAT WILL BE LOCATED AT THE HCH CLINIC UNDER THE DIRECTION OF DR. GARRISON-JAKEL TO PROVIDE IMMEDIATE INCREASED ACCESS TO MOUD TREATMENT FOR THE HOMELESS POPULATION AS WELL AS ANY PATIENTS THAT IS IN NEED OF MOUD / SUD SERVICES AT WCHC. IN ADDITION, WCHC WILL EXPAND ACCESS TO MENTAL HEALTH SERVICES BY HIRING 2.0 FTE BEHAVIORAL HEALTH SPECIALISTS TO PROVIDE MUCH NEEDED CAPACITY TO SERVE ADDITIONAL PATIENTS WITH ONE-ON-ONE COUNSELING, GROUP VISITS, MENTAL HEALTH ASSESSMENTS AND INTAKES, AS WELL AS FAMILY SYSTEMS THERAPY. WCHC WILL HIRE A 1.0 FTE SBHC COMMUNITY HEALTH WORKER TO PROVIDE EXTENSIVE LINKAGES TO SOCIAL SUPP ORTS, TRACK REFERRALS AND CARE COORDINATION, ASSIST WITH SCHEDULING APPOINTMENTS, AND ENSURE PATIENTS WHO ARE NEWLY ACCESSING MENTAL HEALTH SERVICES AND SUD/OUD TREATMENT ARE BEING LINKED WITH FOOD, TRANSPORTATION, HOUSING, UTILITY ASSISTANCE, AND OTHER RECOVERY-BASED AND SUPPORTIVE SERVICES AVAILABLE WITHIN WESTERN SONOMA COUNTY. IN ADDITION, WCHC WILL HIRE A 1.0 FTE SBHC FAMILY CASE MANAGER THAT WILL PROVIDE A HIGH LEVEL OF CASE MANAGEMENT FOR YOUTH WHO PRESENT AT THE TEEN CLINIC AND SBHCS AS WELL AS THEIR FAMILIES. THE FAMILY CASE MANAGER WILL BE HIGHLY TRAINED TO WORK WITH YOUTH AND STUDENTS WHO ARE SCREENED POSITIVE FOR A SUD/OUD OR A MENTAL HEALTH CONCERN.

IS A PRIVATE, NON-PROFIT FEDERALLY QUALIFIED HEALTH CENTER CONSISTING OF TEN PRIMARY CARE CLINICS THAT HAVE BEEN THE SOLE PROVIDERS OF PRIMARY MEDICAL, DENTAL AND BEHAVIORAL HEALTH SERVICES FOR RURAL COMMUNITY RESIDENTS IN WESTERN SONOMA COUNTY, CALIFORNIA FOR FIFTY YEARS. OUR MISSION IS TO PROVIDE COMPREHENSIVE AND HIGH QUALITY HEALTH CARE SERVICES TO THE COMMUNITIES OF WESTERN SONOMA COUNTY. WE ARE A COHESIVE TEAM OF HEALTH CARE PROVIDERS, SUPPORT STAFF, AND VOLUNTEERS DEDICATED TO WELLNESS, COMPASSION, AFFORDABILITY AND EXCELLENCE OF CARE. WCHC IS REQUESTING $150,000 TO SUPPORT THE PURCHASE OF NEW DENTAL EQUIPMENT TO BE UTILIZED AT THE RUSSIAN RIVER DENTAL CLINIC, WHERE THE RYAN WHITE PART C EARLY INTERVENTION PROGRAM IS BASED FOR THE PURPOSE OF DEVELOPING, ENHANCING, AND EXPANDING ORAL HEALTH SERVICES FOR PEOPLE WITH HIV. WCHC IS PROPOSING TO PURCHASE NEW DENTAL EQUIPMENT UNDER THE INFRASTRUCTURE DEVELOPMENT CATEGORY FOR DENTAL EQUIPMENT FOR EXPANDING DENTAL SERVICE CAPACITY. ENHANCING WCHC’S CAPACITY TO SERVE ADDITIONAL PATIENTS, REDUCE WAIT TIMES FOR APPOINTMENTS AND PROVIDE MORE COMPLEX DENTAL PROCEDURES FOR THE LOW-INCOME AND UN/UNDERINSURED HIV+ POPULATION THAT REDUCES COSTLY REFERRALS OUTSIDE THE REGION, ELIMINATES TRANSPORTATION CHALLENGES, AND IMPROVES ADHERENCE TO TREATMENT PLANS TO MAINTAIN POSITIVE ORAL HEALTH IS A CRITICAL CONCERN. FOR MORE THAN 40 YEARS, WCHC HAS PROVIDED DEDICATED ORAL HEALTH SERVICES TO INDIVIDUALS LIVING WITH HIV/AIDS AT ITS TWO DENTAL LOCATIONS AND IS THE ONLY SAFETY-NET DENTAL PROVIDER THAT SERVES THE HIV POPULATION ON A SLIDING FEE SCALE. COMPREHENSIVE DENTAL CARE, INCLUDING DIAGNOSTIC, PREVENTIVE, THERAPEUTIC AND RESTORATIVE SERVICES, IS PROVIDED FOR HIV PATIENTS ON-SITE AS AN INTEGRAL PART OF OUR HEALTH CENTER’S GENERAL DENTAL PROGRAM. THE AVAILABILITY OF COMMUNITY-BASED DENTAL SERVICES HAS BEEN INVALUABLE IN THE IDENTIFICATION OF PATIENTS WHO ARE IN NEED OF TREATMENT TO MAINTAIN AND RESTORE ORAL HEALTH AND ELIMINATE INFECTION, AND DIMINUTION OF WASTING SYNDROME.
