KANE-LEWIS, CHIEF EXECUTIVE OFFICER PHONE (VOICE AND FAX): (TEL) 207 524-3501 (FAX) 207 524-2093 E-MAIL: LKLEWIS@DFDRUSSELL.ORG WEBSITE WWW.DFDRUSSELL.ORG CONGRESSIONAL DISTRICTS: ME - ALL AMOUNT OF FUNDING REQUESTED: $600,000 THE DFD RUSSELL MEDICAL CENTER (DFDRMC) WAS STARTED IN 1975 AS AN INDEPENDENT PUBLIC/PRIVATE PARTNERSHIP BY CONCERNED RESIDENTS OF LEEDS, MAINE AND THE 6 SURROUNDING TOWNS FOLLOWING THE DEATH OF THE AREA’S ONLY PHYSICIAN, DR. DANIEL FRANK DAVIS RUSSELL. IN 2021 THE BRIDGTON LOCATION WAS ADDED BECAUSE A RURAL HEALTH CENTER OPERATED BY THE HOSPITAL WAS CLOSING. IT IS GUIDED BY A BOARD OF DIRECTORS COMPRISED OF PATIENTS AND RESIDENTS FROM THE SERVICE AREAS. DFDRMC’S GOALS TO BE ACCOMPLISHED THROUGH THE BEHAVIORAL HEALTH SERVICE EXPANSION ARE: 1) INCREASE THE NUMBER OF PATIENTS RECEIVING MENTAL HEALTH SERVICES, AND INCREASE THE NUMBER OF PATIENTS RECEIVING SUD SERVICES, INCLUDING TREATMENT WITH MEDICATIONS FOR OPIOID USE DISORDER (MOUD). 2) TO ENHANCE THE LEVEL OF INTEGRATION WHERE A TEAM OF PRIMARY CARE PROVIDERS IN EACH OF OUR FOUR LOCATIONS HAVE ACCESS TO A FULL-TIME BEHAVIORAL HEALTH PROVIDER TO ASSIST WITH BRIEF INTERVENTIONS FOR PATIENTS WITH COMPLEX MENTAL HEALTH ISSUES, SUBSTANCE ABUSE CONDITIONS OR TO WHOM THEY CAN PROVIDE A WARM HAND OFF FOR PATIENTS. 3) TO INCREASE PRIMARY CARE SCREENING AND BEHAVIORAL COUNSELING INTERVENTIONS (INCLUDING SCREENING, BRIEF INTERVENTION AND REFERRAL FOR TREATMENT OR SBIRT) FOR SUBSTANCE ABUSE AND DEPRESSION, USING THE 5-AS BEHAVIORAL COUNSELING FRAMEWORK. SCREENING WILL INCORPORATE THE DEPRESSION SCREENING AND FOLLOWUP MEASURE. 4) IN ALIGNMENT WITH MAINE’S “MAINECARE BEHAVIORAL HOMES INITIATIVE” AND “MAINECARE OPIOD HEALTH HOMES INITIATIVE”, OFFER INTEGRATED MENTAL HEALTH AND SUBSTANCE ABUSE SERVICES (CAR E COORDINATION, SERVICES AND SUPPORT) TO THE CURRENTLY UNDERSERVED SPMI (SERIOUS AND PERSISTENT MENTAL ILLNESS) AND SED (SERIOUS EMOTIONAL DISTURBANCE) POPULATIONS, BASED ON CARE COORDINATION, SERVICES AND SUPPORT. 5) IMPROVE PATIENT ACCESS TO AND QUALITY OF CARE BY INCREASING ACCESS TO PSYCHIATRIC NURSE PRACTITIONERS TO PROVIDE MEDICATION MANAGEMENT AND CONSULTATION TO THE PRIMARY CARE PROVIDERS FOR PATIENTS WHO HAVE SERIOUS AND PERSISTENT MENTAL ILLNESS, THOSE WITH POST-TRAUMATIC STRESS DISORDER, AND PATIENTS WITH SUBSTANCE ABUSE DISORDERS. THE ADDITION OF A FULL-TIME LICENSED CLINICAL SOCIAL WORKER WITH SUBSTANCE ABUSE EXPERTISE IN BRIDGTON WILL INCREASE SERVICES BY ALLOWING DAILY BEHAVIORAL HEALTH COVERAGE IN ALL 4 LOCATIONS. WE WILL CONVENE A TEAM OF PRIMARY CARE, LICENSE CLINICAL SOCIAL WORKERS, PSYCHIATRIC NURSE PRACTITIONERS, COMMUNITY HEALTH WORKERS, RN, AND MEDICAL ASSISTANTS TO ENSURE CONTINUITY OF CARE AND ATTAINMENT OF PROJECT GOALS OF INCREASED ACCESS AND NUMBERS OF PATIENTS SERVED.
