HEALTH CENTER PROGRAM - LOOK-ALIKE #: LACS33334 HISTORY/SERVICE AREA: NEIGHBORHEALTH CENTER, INC. (NHC), A FAITH-BASED, PRIVATE, NON-PROFIT 501(C)3 HEALTH CENTER PROGRAM LOOK-ALIKE IS APPLYING FOR NEW ACCESS POINT FUNDING. LOCATED IN RALEIGH, NORTH CAROLINA, NHC OPENED ITS FIRST CLINIC IN JUNE 2018 AND WAS DESIGNATED AS A LOOK-ALIKE IN JULY 2019. THE FOUNDATION FOR NEIGHBORHEALTH WAS LAID BY VOLUNTEERS IN 2012, WHO LED THE ORGANIZATION IN ASSESSING EXISTING HEALTHCARE RESOURCES AND IDENTIFYING NEEDS, ATTAINING NON-PROFIT 501(C)3 STATUS, RESEARCHING CLINIC MODELS AND METHODS, MEETING WITH COMMUNITY PARTNERS AND THE TARGET COMMUNITY, AND RAISING FUNDS. THE BOARD OF DIRECTORS DETERMINED THAT THE HEALTH CENTER PROGRAM MODEL BEST MET THEIR VISION FOR PROVIDING ACCESSIBLE, QUALITY CARE TO UNDERSERVED AND VULNERABLE POPULATIONS. TODAY, NHC OPERATES ONE HEALTH CENTER SITE; ITS SERVICES INCLUDE PRIMARY MEDICAL, OBSTETRICS, BEHAVIORAL HEALTH, AND ENABLING SERVICES. NO PATIENT IS DENIED CARE DUE TO INABILITY TO PAY. COMMUNITY SERVED /BARRIERS TO CARE: THE SERVICE AREA INCLUDES PARTS OF RALEIGH AND THE SURROUNDING ZIP CODES. THE POPULATION IS 711,901. BASED ON LOW-INCOME DATA, THE TARGET POPULATION IS 199,170, OR 28 PERCENT OF THE POPULATION (US CENSUS, 2021). WHILE THE REGION APPEARS WEALTHY AND WELL-RESOURCED OVERALL, IT IS HOME TO SIGNIFICANT INCOME INEQUALITY AND DISPARITIES IN HEALTHCARE ACCESS AND OUTCOMES. NHC’S TARGET POPULATION OF LOW-INCOME, UNINSURED, AND UNDERINSURED RESIDENTS EXPERIENCES SIGNIFICANT BARRIERS TO ACCESS AS DETAILED IN THE NEEDS ASSESSMENT. PROPOSED PROJECT: NEIGHBORHEALTH CENTER IS APPLYING FOR HEALTH CENTER FUNDING FOR ITS CURRENT SITES, WHICH INCLUDE AN ADMIN-ONLY SITE AND A MEDICAL, OB, AND BEHAVIORAL HEALTH CLINIC IN TWO SEPARATE SUITES AT 2605 BLUE RIDGE RD., RALEIGH, 27607, AND A NEW ACCESS POINT (NAP) AT 2101 GARNER ROAD, RALEIGH, 27610. THE NAP IS LOCATED IN THE ZIP CODE WHERE THE LARGEST PORTION OF NEIGHBORHEALTH’S PATIENTS RESIDE AND AN AREA WITH HIGH UNMET NEED. THE TOTAL STAFFING PLAN INCLUDES 92.10 FTE CLINICIANS, SUPPORT STAFF, AND ADMINISTRATORS, OF WHICH 17.0 FTE WILL BE AT THE NAP SITE. FUNDING FROM THIS GRANT WILL SUPPORT THE 5 NEW CLINICAL POSITIONS AT THE NAP: 1.0 FTE FAMILY PHYSICIAN, 2.0 FTE NURSE PRACTITIONERS, 2 FTE REGISTERED NURSES, AND 1.0 FTE LICENSED CLINICAL SOCIAL WORKER WHO WILL PROVIDE MENTAL HEALTH AND SUBSTANCE USE DISORDER SERVICES. UNDER THIS PROPOSAL, NEIGHBORHEALTH CENTER WILL SERVE 11,000 UNDUPLICATED PATIENTS IN 2026.
