(IHC) HAS A LONGSTANDING HISTORY IN INDIANA, SERVING THE STATE’S RURAL, URBAN, UNDERSERVED AND MINORITY POPULATIONS SINCE 1981. THE ORGANIZATION WAS ORIGINALLY FUNDED AS A HRSA MIGRANT HEALTH CENTER, AND HAS GROWN INTO A STATEWIDE HEALTH CENTER, SERVING VARIOUS REGIONS AND VULNERABLE POPULATIONS THROUGHOUT INDIANA. IN ADDITION TO CHC SITES, IHC HAS ONE HCH (HEALTHCARE FOR THE HOMELESS) FUNDED SITE, TWO INTEGRATED WIC PROGRAMS, FIVE FREE-STANDING WIC PROGRAMS, AND A MOBILE SITE. IHC ALSO HAS A TEACHING HEALTH CENTER RESIDENCY PROGRAM AND IS DEDICATED TO TRAINING HEALTH PROFESSIONALS TO INCREASE ACCESS IN INDIANA. IHC IS THE ONLY MIGRANT HEALTH CENTER IN THE STATE OF INDIANA, AND AS SUCH, IS VITAL TO MEETING THE NEEDS OF THIS SPECIAL POPULATION AND SERVING 475 MIGRANT PATIENTS IN 2023. ACCORDING TO THE 2023 UDS REPORT, IHC SERVED 37,225 PATIENTS, 379 OF WHICH WERE EXPERIENCING HOMELESSNESS. APPROXIMATELY 443 IHC PATIENTS RECEIVED MENTAL HEALTH SERVICES IN 2023 WHILE 0 PATIENTS RECEIVED SUBSTANCE USE DISORDER (SUD) SERVICES. IHC INTENDS TO LEVERAGE BHSE FUNDS TO INCREASE THE NUMBER OF PATIENTS RECEIVING BEHAVIORAL HEALTH SERVICES. IHC WILL UTILIZE FUNDS THROUGH HRSA TO BEGIN OFFERING COMPREHENSIVE BEHAVIORAL HEALTH SERVICES INCLUDING SUD SERVICES DIRECTLY. THESE WILL BE DESIGNED TO SUPPORT INDIVIDUALS AND THEIR FAMILIES AT EVERY STAGE OF THEIR LIFE IHC UNDERSTANDS THAT DIFFERENT AGE GROUPS HAVE UNIQUE NEEDS AND WILL ENSURE THAT SERVICES ARE TAILORED TO CATER TO SPECIFIC DEVELOPMENTAL STAGES. BEHAVIORAL HEALTH SERVICES INCLUDING SUD SERVICES WILL BE PROVIDED AT ALL IHC CLINICAL PHYSICAL SITES IN YEAR ONE AND EXPANDED TO THE MOBILE HEALTH UNIT IN YEAR 2, AS NOTED IN FORM 5B. SPECIALIZED BEHAVIORAL HEALTH AND SUD SERVICES ACROSS THE LIFESPAN WILL INCLUDE: - AGES 0-5: EARLY CHILDHOOD SUPPORT – BEHAVIORAL AND DEVELOPMENTAL MILESTONE ASSESSMENTS, PARENTING SUPPORT, PLAY THERAPY - AGES 6-12: CHILDREN – INDIVIDUAL COUNSELING, PLAY THERAPY, FAMILY THERAPY - AGES 13-18: ADOLESCENT – TEEN COUNSELING, SUBSTANCE USE COUNSELING, CRISIS INTERVENTION, SEXUAL HEALTH COUNSELING - AGES 18 AND ABOVE – INDIVIDUAL THERAPY, COUPLES COUNSELING, PSYCHIATRIC SERVICES, SEXUAL HEALTH COUNSELING, SUBSTANCE USE COUNSELING - AGES 12 AND ABOVE- SCREENING DEPRESSION USING PHQ-9, ALCOHOL USE USING AUDIT, AND SUBSTANCE USE USING DAST-10 IHC WILL LEVERAGE HRSA BHSE FUNDS TO INCREASE ACCESS TO BEHAVIORAL HEALTH SERVICES THROUGH THE EXPANSION OF MENTAL HEALTH AND SUBSTANCE USE DISORDER (SUD) SERVICES. PROJECT ACTIVITIES INCLUDE: - LEVERAGING COMMUNITY PARTNERSHIPS WITH ORGANIZATIONS WITH A DIRECT CONNECTION TO UNDERSERVED AND UNDERREPRESENTED POPULATIONS (RACIAL/ETHNIC MINORITIES, PEOPLE WHO ARE UNINSURED, ETC.). - INTEGRATING PRIMARY CARE AND BEHAVIORAL HEALTH AT EACH CLIENT ENCOUNTER, CONDUCTING ASSESSMENTS AND CONNECTING CLIENTS WITH SUPPORTIVE SERVICES AND SCHEDULE FOLLOW-UP APPOINTMENTS. - GO BEYOND PROVIDING MEDICAL CARE AND ADDRESSING THE SOCIAL DRIVERS OF HEALTH THROUGH COMMUNITY COLLABORATIONS (HOUSING INSECURITY, FOOD INSECURITY, FINANCIAL STRAIN). - EXPAND THE ROLE OF PRIMARY CARE PROVIDERS IN THE USE OF FDA-APPROVED MEDICATIONS FOR TREATMENT OF OPIOID USE DISORDER THROUGH TRAINING AND EDUCATION. - HIRE BEHAVIORAL HEALTH PROFESSIONALS TO ADD TO INTEGRATED IHC TEAMS TO EXPAND ACCESS. - BROADEN THE USE OF TELEHEALTH ACROSS IHC SITES TO ENHANCE ACCESS TO BEHAVIORAL HEALTH SERVICES AT ALL IHC LOCATIONS.
