SUCH SUPPORT SPANS DAY-TO-DAY NETWORK SUPPORT TO NEXTGEN CHERT IMPLEMENTATION, MANAGEMENT, AND GOVERNANCE. AS ONE OF THE TOP FULL-SERVICE HCCNS IN THE COUNTRY, OSIS OFFERS MEMBERS ACCESS TO A TEAM OF EXPERT CONSULTANTS, 24/7 IT SUPPORT, AND THE SHARED KNOWLEDGE OF ALL OTHER CHCS IN THE NETWORK. OSIS CURRENTLY SERVES 62 FQHCS, LOOK-ALIKES, PLANNED PARENTHOODS, TRIBAL HEALTH GROUPS AND/OR BEHAVIORAL HEALTH ORGANIZATIONS ACROSS THIRTY-FOUR STATES. THE TWENTY-SEVEN PARTICIPATING HEALTH CENTERS (PHC) IN THE PROPOSED PROJECT REPRESENT A WIDE RANGE OF FQHCS(25) AND LOOK-ALIKES(2), INCLUDING SMALL, LARGE, URBAN, RURAL AND VARIED RATES OF RACIAL/ETHNIC MINORITY POPULATIONS. PHCS WILL BE SUPPORTED IN LEVERAGING HIT USING THE SAME CEHRT PLATFORM WHILE FOCUSING ON CLINICAL QUALITY OF CARE TO DELIVER PATIENT-CENTERED CARE AND SUPPORTING PROVIDER AND STAFF WELL-BEING. OSIS MEMBERS AND OTHER CHCS ARE NOT UNLIKE OTHER PRIMARY CARE PRACTICES FACED WITH REALITIES THAT HAVE DETERRED PROGRESS TOWARD INTEROPERABILITY AND OTHER HIT ADVANCES. THEY ENCOUNTERED CHALLENGES COMMON ACROSS THE COUNTRY RESULTING IN DELAYS TOWARD FUNCTIONAL HIT EXCHANGE ON REGIONAL LEVELS. WHILE NEARLY ALL OSIS PHCS ORDER AND RECEIVE ELECTRONIC LAB R ESULTS, MANY ARE STILL STRUGGLING WITH TRANSMITTING AND RECEIVING ELECTRONIC SUMMARIES OF CARE THAT ARE EASILY DIGESTIBLE FOR HEALTHCARE PROVIDERS. AN EVEN GREATER GAP EXISTS ACROSS MANY STATES IN COMMUNITY RECORDS OR ON-DEMAND ACCESS TO LONGITUDINAL RECORDS. WHILE THE MEDICAID PROMOTING INTEROPERABILITY (PI) HAS ENDED, MANY HEALTH CENTERS STRUGGLED TO ACHIEVE THRESHOLDS RELATED TO PATIENT ENGAGEMENT AND INTEROPERABILITY MEASURES AND WERE UNABLE TO ATTEST IN THE FINAL YEARS OF THE PROGRAM. TO FURTHER THE GOALS OF THE HRSA HEALTH CENTER PROGRAM, PROJECTS IMPLEMENTED UNDER THIS COOPERATIVE AGREEMENT WILL STRENGTHEN THE FOUNDATION NEEDED TO MAXIMIZE USE OF INFORMATION EXCHANGE AND REALIZE IMPROVED HEALTH INDICATORS. EHR OPTIMIZATION, HEALTH INFORMATION EXCHANGE CONNECTIVITY, SUPPORT FOR DISCRETE DATA COLLECTION, ANALYSIS AND REPORTING WILL POSITION HEALTH CENTERS FOR BETTER RESULTS. ACTIVITIES UNDER THE PROPOSED PROJECT WILL INCLUDE EDUCATIONAL SESSIONS, TRAINING MATERIALS, TECHNICAL ASSISTANCE, SUPPORT AND GUIDANCE FOR TARGETED ACTIVITIES TO ASSIST CHCS IN DELIVERING HIGH-QUALITY, CULTURALLY COMPETENT, EQUITABLE AND COMPREHENSIVE PRIMARY CARE.
