THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. AT THE END OF THE THREE-YEAR GRANT, WE WILL BE TO SERVE 2000 CLIENTS THROUGH OUR HEALTHY FUTURES INITIATIVE. OUR GOALS ARE THAT: 140 HOMEBOUND CLIENTS WILL BE ABLE TO CONTINUE TO LIVE INDEPENDENTLY DUE TO THE NUTRITION SUPPORT THEY ARE RECEIVING; 60 CLIENTS WILL BE ABLE TO STATE THEY ARE MORE FOOD SECURE; 45 STUDENTS WILL HAVE IMPROVED ACADEMIC SKILLS, 35 STUDENTS WILL HAVE IMPROVED ACADEMIC ENGAGEMENT BEHAVIORS; 100 OLDER ADULTS IN THE COMMUNITY WILL HAVE INCREASED THEIR HEALTH KNOWLEDGE AND 5 INDIVIDUALS WILL HAVE BEEN TRANSITIONED INTO SAFE, AFFORDABLE HOUSING. OUR RSVP PROGRAM WILL PROVIDE SERVICES IN SEVERAL KEY CNCS FUNDING PRIORITIES.
