A STATE-FUNDED COMMUNITY HEALTH CENTER SINCE 1996, AND A FEDERALLY QUALIFIED HEALTH CENTER SINCE 2004, WHN IS COMPRISED OF SIX SERVICE LOCATIONS, AND AN ADMINISTRATIVE CENTER. 1. WHN - TRAFALGAR (OPENED 1996) 2. WHN - HOPE (OPENED 2006) 3. WHN - COUNTYLINE (OPENED 2008) 4. WHN - FRANKLIN (OPENED 2014) 5. WHN - EPLER PARKE (OPENED 2015) 6. WHN BUSINESS CENTER (OPENED 2016) WHN'S SERVICE AREA CONSISTS OF FOUR (4) CENTRAL INDIANA COUNTIES: THE SOUTHERN THIRD OF MARION COUNTY, JOHNSON, MORGAN, AND BARTHOLOMEW COUNTIES. SINCE THE COVID-19 PANDEMIC, DEMAND FOR BEHAVIORAL HEALTH AND PSYCHIATRIC SERVICES IN WHN’S SERVICE AREA HAS INCREASED SHARPLY. AT THE SAME TIME, ACCESS TO THESE CLINICIAN SERVICES HAS BECOME MORE DIFFICULT AND WAIT TIMES TO GET IN TO SEE THESE MEDICAL PROFESSIONALS HAS GROWN SIGNIFICANTLY SINCE 2020. ADDITIONALLY, ALCOHOLISM APPEARS TO BE ON THE RISE IN THE COMMUNITIES THAT WHN SERVES, ESPECIALLY WITHIN IMMIGRANT POPULATIONS, AND THE PREVALENCE OF FENTANYL AND CARFENTANIL IN INDIANA IS NEAR EPIDEMIC LEVELS. AS A GRANT RECIPIENT, WHN WILL EXPAND ACCESS TO BEHAVIORAL HEALTH AND SUBSTANCE USE DISORDER (SUD) SERVICES DIRECTLY BY EMPLOYING (1) ADDITIONAL THERAPIST WITH SUD / ADDICTIONS EXPERTISE, (2) PSYCHIATRIC NURSE PRACTITIONERS, (2) CERTIFIED RECOVERY SPECIALISTS, (2) COMMUNITY HEALTH WORKERS, AND ADMINISTRATIVE / CLINICAL STAFF TO SUPPORT THESE PROFESSIONALS. THROUGH THIS BEHAVIORAL HEALTH & SUD EXPANSION, WHN WILL ACHIEVE THE FOLLOWING GOALS: 1. INCREASE PATIENT ACCESS TO COUNSELING & THERAPY SERVICES; 2. REDUCE WAIT TIMES & INCREASE PATIENT ACCESS TO PSYCHIATRIC SERVICES; 3. IMPROVE CARE COORDINATION & COMMUNICATION BETWEEN PRIMARY CARE PROVIDERS AND BH PROVIDERS; 4. INCREASE THE NUMBER OF PRIMARY CARE PATIENTS RECEIVING SBIRT (SCREEN, BRIEF INTERVENTION, REFERRAL TO TREATMENT) SCREENINGS; 5. INCREASE THE NUMBER OF PATIENTS REFERRED TO WHN PROVIDERS FOR ASSESSMENT OF SUD AND ADDICTION ISSUES; 6. INCREASE THE NUMBER OF PATIENTS RECEIVING MAT AND MOUD SERVICES FROM A WHN MEDICAL PROVIDER; 7. INCREASE ONE-ON-ONE NON-CLINICAL SUPPORT FOR PATIENTS STRUGGLING WITH SUBSTANCE ABUSE (SUD) AND/OR ADDICTION ISSUES; 8. FACILITATE GROUP SUPPORT FOR PATIENTS STRUGGLING WITH SUBSTANCE ABUSE AND/OR ADDICTION ISSUES BY HOSTING CULTURALLY-APPROPRIATE ADDICTION GROUPS IN ENGLISH, SPANISH, BURMESE & PUNJABI LANGUAGES; AND 9. ASSIST PATIENTS WITH SDOH BARRIERS TO IMPROVE THEIR OVERALL MENTAL HEALTH WELLBEING, CONNECT THEM WITH COMMUNITY RESOURCES, AND ELIMINATE BARRIERS TO RECOVERY. BY ACHIEVING THESE GOALS AND IMPROVING ACCESS TO INTEGRATED BH AND PSYCHIATRIC SERVICES, WHN WILL IMPROVE THE HEALTH AND WELLBEING OF THE COMMUNITIES THAT IT SERVES. THE RECIPIENTS OF THIS WORK WILL BE WHN’S PATIENTS AND HOOSIERS & THEIR FAMILIES IN BARTHOLOMEW CO., JOHNSON CO., MORGAN CO., AND SOUTHERN MARION CO.
