CONGRESSIONALLY DIRECTED SPENDING FOR CONSTRUCTION PROJECTS - CHICAGO FAMILY HEALTH CENTER, INC. (CFHC) IS A CURRENT HEALTH CENTER PROGRAM AWARD RECIPIENT (GRANT NUMBER H80CS00514) AND FEDERALLY QUALIFIED HEALTH CENTER PROVIDING CARE TO THE MEDICALLY UNDERSERVED—REGARDLESS OF ABILITY TO PAY—ON CHICAGO’S SOUTH AND SOUTHEAST SIDES. OUR MISSION IS TO PROMOTE HEALTH, PREVENT DISEASE, AND PROVIDE TREATMENT THROUGH THE DELIVERY OF QUALITY, ACCESSIBLE PRIMARY HEALTHCARE THAT IS CULTURALLY SENSITIVE, AFFORDABLE, AND RESPONSIVE TO COMMUNITY AND INDIVIDUAL NEEDS. CFHC SERVES AS A CRITICAL HEALTHCARE SAFETY NET FOR 34,000 PATIENTS ANNUALLY ACROSS 6 CLINICS, WORKING TO ACHIEVE HEALTH EQUITY BY ADDRESSING SOCIAL DRIVERS OF HEALTH AND DELIVERING AFFORDABLE, RESPONSIVE HEALTHCARE. AMONG THOSE WE SERVE, 53% ARE BLACK, 41% LATINX, 4% OTHER/UNKNOWN, AND 2% WHITE; 67% QUALIFY AS LOW-TO-MODERATE INCOME WITH 53% LIVING BELOW 100% OF THE FEDERAL POVERTY LEVEL; 74% RELY ON MEDICAID AND 10% ARE UNINSURED; AND 20% ARE BEST SERVED IN A LANGUAGE OTHER THAN ENGLISH. DUE TO THE GROWING PATIENT BASE, IT HAS BECOME NECESSARY FOR CFHC TO BUILD OUR IN-HOUSE PHARMACY. WE WILL TRANSITION FROM PHARMACY FACILITATION TO A FULL-SERVICE PHARMACY IN AN AREA LARGER AND MORE ACCESSIBLE IN OUR LOBBY. A HIGH-QUALITY, LOW-COST PHARMACY IS DEPENDENT UPON APPROPRIATE EQUIPMENT. WITH THIS EXPANSION WE WILL BE ABLE TO MEET THE NEEDS OF OUR PATIENT BASE BY IMMEDIATE DISBURSEMENT OF MEDICATIONS AND FOLLOW-UP PRESCRIPTION NEEDS. THE ABILITY FOR OUR PATIENTS TO COME TO OUR ON-SITE PHARMACY ALLOWS MORE PATIENT/PROVIDER INTERACTION. WE WILL BE ABLE TO CLOSE THE GAPS AND CHART THE EFFICIENCY ON MEDICATION PRESCRIBED AND TAKEN. OUR PATIENTS WILL BE INCLINED TO COME TO OUR SAFE ON-SITE PHARMACY DUE TO THEIR FAMILIARITY WITH STAFF AND COMFORT WITH PROVIDERS. WE WILL FOLLOW ALL FEDERAL, STATE AND LOCAL GUIDELINES CONCERNING CONSTRUCTION OF THE SITE. CFHC OWNS THE PROPERTY WHERE THE CHANGES WILL BE MADE. WE WILL ENDEAVOR TO USE MINORITY OWNED BUSINESSES/CONTRA CTORS AND UNION SHOPS AS APPLIED. OUR TIMELINE FOR THE PROJECT IS FIVE MONTHS FROM ACQUISITION OF FUNDS. WE WILL UTILIZE EXISTING STRUCTURAL ELEMENTS AND ATTACH TO PRESENT INFRASTRUCTURE MUCH AS POSSIBLE. THIS PROJECT WILL HAVE AN IMMEDIATE IMPACT UPON FULL UTILIZATION.
