THE SENIOR COMPANION PROGRAM HAS FLOURISHED WHEN COMBINED WITH THE CENTER OF CONCERN'S LONG-STANDING COLLABORATION WITH SUBURBAN COOK COUNTY SOCIAL SERVICE AGENCIES, MUNICIPALITIES, FAITH-BASED ORGANIZATIONS, HOSPITALS, AND NURSING FACILITIES, ETC., THAT ENABLE OUR AGENCY TO EFFECTIVELY IDENTIFY AND ADDRESS THE NEEDS OF SENIOR RESIDENTS AND IS A CONDUIT FOR SECURING VALUABLE VOLUNTEER RESOURCES. THIS APPLICATION BEGINS OUR SECOND 3-YEAR GRANT CYCLE FOR THE CENTER OF CONCERN SENIOR COMPANION PROGRAM. WE PROPOSE TO HAVE A MINIMUM OF 56 UNDUPLICATED VOLUNTEERS PROVIDING AT A MINIMUM OF 56 VSYS BY THE END OF YEAR 3. OUR SENIOR COMPANION VOLUNTEERS WILL PROVIDE VITAL SERVICES THAT ALLOW SENIORS TO AGE IN PLACE. SENIOR COMPANIONS SERVE ONE-ON-ONE WITH THE FRAIL OLDER ADULTS AND OTHER HOMEBOUND ADULTS. SOME OF THE SERVICES PROVIDED WILL INCLUDE COMPANIONSHIP, TRANSPORTATION, MEDICAL SERVICES, OPIOID/DRUG INTERVENTION, ELDER JUSTICE, AND RESPITE SERVICES FOR CAREGIVERS. EXECUTIVE SUMMARY SINCE 1978, THE CENTER OF CONCERN HAS BEEN SERVING SUBURBAN COOK COUNTY RESIDENTS WITH A FOCUS ON LOW-INCOME SENIORS, THE DISABLED, AND OTHERS IN NEED. THE SENIOR COMPANION PROGRAM HAS FLOURISHED WHEN COMBINED WITH THE CENTER OF CONCERN'S LONG-STANDING COLLABORATION WITH SUBURBAN COOK COUNTY SOCIAL SERVICE AGENCIES, MUNICIPALITIES, FAITH-BASED ORGANIZATIONS, HOSPITALS, AND NURSING FACILITIES, ETC., THAT ENABLE OUR AGENCY TO EFFECTIVELY IDENTIFY AND ADDRESS THE NEEDS OF SENIOR RESIDENTS AND IS A CONDUIT FOR SECURING VALUABLE VOLUNTEER RESOURCES. THIS APPLICATION BEGINS OUR SECOND 3-YEAR GRANT CYCLE FOR THE CENTER OF CONCERN SENIOR COMPANION PROGRAM. WE PROPOSE TO HAVE A MINIMUM OF 56 UNDUPLICATED VOLUNTEERS PROVIDING AT A MINIMUM OF 56 VSYS BY THE END OF YEAR 3.
