FRANCIS HOSPITAL AND MEDICAL GROUP (SFH). THE NUMBER OF DISTINCT PATIENTS SERVED IN THIS AREA HAS INCREASED BETWEEN 3% AND 11% YEAR OVER YEAR FOR THE LAST 5 YEARS. FY18 = 673; FY19 = 736; FY20 = 762; FY 21 = 864; FY22 = 740 (PARTIAL FISCAL YEAR 10/1/2022-5/18/2022, 4 MONTHS REMAINING). WE ARE PROJECTING AN EVEN HIGHER INCREASE IN FY22, AS ANOTHER LOCAL HEALTHCARE PROVIDER OFFERING ONCOLOGY SERVICES IN OUR COMMUNITY CLOSED THEIR CLINIC ON 3/31/2022, AND WE HAVE RECEIVED AN INFLUX OF NEW PATIENTS. WE NEED ADDITIONAL CLINIC SPACE IN ORDER TO PROPERLY CARE FOR THESE PATIENTS. ON THE SFH CAMPUS THERE IS A VACANT 4,344 SF BUILDING THAT WAS FORMERLY LEASED TO A DIALYSIS PROVIDER WHO MOVED TO A NEW LOCATION IN FY20. THE PROPERTY TO BE UTILIZED FOR THIS PROJECT IS LOCATED AT 3501 LUDINGTON STREET IN ESCANABA, MICHIGAN. THE BUILDING SITS ADJACENT TO SFH, WHO OWNS THE BUILDING. SFH WILL REMODEL THIS SPACE AND MOVE ALL SFH CANCER CARE SERVICES TO THIS BUILDING. IN ADDITION, A 1,246 SF ADDITION WILL BE ADDED TO THE BUILDING TO ACCOMMODATE THE NEEDS OF THIS SERVICE PROVIDING A TOTAL OF 5,590 SF OF SPACE. THIS RENOVATION AND EXPANSION WILL PROVIDE OUR CANCER CARE TEAM WITH THE SPACE NEEDED TO PROVIDE TIMELY ACCESS TO CARE FOR OUR ONCOLOGY AND HEMATOLOGY PATIENTS. ADDITIONAL PATIENT SPACE THAT THIS PROJECT WILL PROVIDE INCLUDES: - INCREASING FROM 3 EXAM ROOMS TO 5. - INCREASING FROM 1 PATIENT TOILET TO 3. - INCREASING FROM 9 TREATMENT BAYS TO 10. - CREATION OF A LAB DRAW STATION. - EXPANDED WAITING ROOM CAPACITY. THE NEWLY DESIGNED SPACE WILL BE LAID OUT IN A CIRCULAR FASHION PREVENTING OVERLAPPING PATIENT FLOW WITH PATIENT RESTROOMS AT BOTH SIDES OF THE CLINIC. THIS CIRCULAR DESIGN WILL ALSO PROVIDE CLOSED-LOOP COMMUNICATION ENSURING THE CLINIC RUNS SMOOTHLY. A PRIVATE INFUSION ROOM WILL ALLOW FOR THOSE PATIENTS IN CONTACT PRECAUTIONS TO BE PROPERLY SEGREGATED FROM THE REMAINING VULNERABLE PATIENT POPULATION. AN EXAM ROOM THAT DOUBLES AS A CONSULTATION ROOM WILL ALLOW FOR PROPER SPACE FOR PROVIDERS TO HAVE DIFFICULT CONVERSATIONS WITH THEIR PATIENTS WITH THEIR SUPPORT PERSONS PRESENT. A ROOM THAT HOUSES A BED STYLE SET UP WILL ALLOW FOR MORE COMFORTABLE INFUSIONS FOR PATIENTS AND ALSO PROVIDE AN AREA TO PERFORM BEDSIDE PROCEDURES. ADDITIONAL EXAM ROOMS WILL ALLOW SCHEDULERS TO MAXIMIZE AVAILABILITY IN PROVIDERS’ SCHEDULES, BRINGING IN MORE PATIENTS WITHOUT INCREASING WAIT TIMES DUE TO LIMITED EXAM ROOMS. THE NEW SPACE WILL ALSO PROVIDE THE ROOM NEEDED FOR PEOPLE WORKING IN THE CLINIC. PROVIDERS WILL HAVE ADDITIONAL SPACE THAT WILL GIVE THEM PRIVACY FOR PHONE CALLS AND MEETINGS. THE NURSE NAVIGATOR WILL HAVE AN OFFICE THAT WILL ALLOW HER TO HAVE PRIVATE CONVERSATIONS WITH PATIENTS TO SUPPORT THEM AND PROPERLY PLAN THEIR CARE. STORAGE SPACE WILL BE CREATED TO ALLOW FOR THE PROPER STORAGE OF SUPPLIES AND EQUIPMENT NEEDED TO PROVIDE THE BEST CARE. A BREAK/LOCKER ROOM WILL BE DEVELOPED WHERE STAFF CAN STORE THEIR PERSONAL ITEMS AND TAKE NECESSARY BREAKS. IN ADDITION TO MEETING ONCOLOGY SPACE NEEDS, MOVEMENT OF THE CLINIC WILL ALSO FREE UP MUCH NEEDED SPACE IN SFH’S EXISTING MEDICAL OFFICE BUILDING FOR ADDITIONAL RURAL HEALTH OR SPECIALTY CARE PROVIDERS TO MEET THE NEEDS OF OUR COMMUNITY. OSF HEALTHCARE SYSTEM WAS AWARDED A HRSA-20-118 ADVANCED NURSING EDUCATION NUSE PRACTITIONER RESIDENCY INTEGRATION PROGRAM (ANE-NPRIP) GRANT (FAIN# T5839873); A HRSA-21-142 RURAL HEALTH CLINIC VACCINE CONFIDENCE PROGRAM GRANT (FAIN# G2943191); AND A HRSA-23-009 ADVANCED NURSING EDUCATION NURSE PRACTITIONER RESIDENCY AND FELLOWSHIP (ANE-NPRF) PROGRAM GRANT (FAIN# T5949646). OSF HEALTHCARE SYSTEM WAS ALSO A WINNER OF HRSA'S PROMOTING PEDIATRIC PRIMARY PREVENTION (P4) CHALLENGE IN 2022. OSF ST. MARY MEDICAL CENTER (SMMC) IS A RURAL HOSPITAL WITHIN THE OSF HEALTHCARE SYSTEM LOCATED IN GALESBURG, ILLINOIS. WE ARE REQUESTING $749,845 OF RRPD FUNDS FOR THE START-UP OF A NEW FAMILY MEDICINE RESIDENCY WITH THE MATERNAL HEALTH AND OBSTETRICS PATHWAY. THE GOAL OF THIS PROJECT IS TO DEVELOP A ROBUST TRAINING PROGRAM THAT DEVELOPS RESIDENTS WITH EXPERIENCE TREATING MOTHERS AND FAMILIES IN RURAL, UNDERSERVED AREAS. OVER THE PAST DECADE, FOUR HOSPITALS IN KNOX COUNTY AND SURROUNDING AREAS SERVED BY SMMC HAVE CLOSED THEIR LABOR AND DELIVERY PROGRAMS. SMMC, ALONG WITH OSF AMBULATORY SITES, IS NOW THE ONLY SOURCE OF MATERNAL HEALTH SERVICES FOR PATIENTS IN THIS AREA. BROADLY, THERE IS A SHORTAGE OF PRIMARY CARE PHYSICIANS IN THE COUNTY, THE STATE OF ILLINOIS, AND THE COUNTRY AT LARGE. DUE TO THIS SHORTAGE, ACCESS TO CARE ACROSS POPULATIONS, BUT ESPECIALLY PRENATAL AND MATERNAL CARE, IS SEVERELY LIMITED. ADDITIONALLY, SMMC IS LOCATED IN A HRSA-DESIGNATED MEDICALLY UNDERSERVED POPULATION (LOW INCOME) IN A HEALTH PROFESSIONAL SHORTAGE AREA (HPSA). THERE IS A GREAT NEED FOR RURAL FAMILY MEDICINE PHYSICIANS, ESPECIALLY THOSE WITH OBSTETRICS TRAINING, IN OUR AREA; WE HOPE TO CLOSE THAT GAP PARTIALLY THROUGH THE DEVELOPMENT OF THIS PROGRAM AND RETENTION OF RESIDENTS AFTER GRADUATION. BROADLY, PROVIDING RESIDENTS WITH EXPERIENCE IN OUR RURAL COMMUNITY WILL EQUIP THEM WITH THE TOOLS TO TREAT A VARIETY AND LARGE VOLUME OF PATIENTS. THE OSF ST. MARY MEDICAL CENTER RURAL FAMILY MEDICINE RESIDENCY PROGRAM WILL TRAIN 12 RESIDENTS WITH MATRICULATION BEGINNING IN JULY 2027. RESIDENTS WILL SERVE PATIENTS IN GALESBURG AND SURROUNDING AREAS DURING THEIR TIME IN THE PROGRAM. THEY WILL SPEND AT LEAST 80% OF THEIR TIME AT SMMC AND WILL ALSO HAVE SELECTED ROTATIONS AT OTHER OSF HOSPITALS AND AT LOCAL AMBULATORY LOCATIONS FOR EXPERIENCES INCLUDING CANCER CARE, NEPHROLOGY, AND OTHER SPECIALTIES. THERE ARE ALSO OPPORTUNITIES TO COLLABORATE WITH PRACTICES IN ADJACENT COUNTIES AS NEEDED FOR ADDITIONAL ROTATIONS. WE WILL LEVERAGE THE EXPERTISE OF EXISTING HEALTHCARE FACILITIES WITHIN THE OSF SYSTEM, ESPECIALLY THOSE WITH EXISTING PROGRAMS SUCH AS SAINT FRANCIS MEDICAL CENTER IN PEORIA AND ST. JOSEPH MEDICAL CENTER IN BLOOMINGTON, TO DEVELOP A ROBUST RESIDENCY PROGRAM. FINALLY, BECAUSE THE PROGRAM WILL BE SPONSORED BY UNIVERSITY OF ILLINOIS COLLEGE OF MEDICINE PEORIA, WE WILL ALSO HAVE THE ABILITY TO RELY ON OUR PARTNERS WITHIN THE MEDICAL SCHOOL TO SUPPORT THE CLINICAL AND DIDACTIC NEEDS OF THE PROGRAM.

JOSEPH MEDICAL CENTER WILL USE GRANT FUNDS FOR THE RENOVATION OF SPACE WITHIN THE HOSPITAL TO HOUSE THE NEW FAMILY MEDICINE RESIDENCY PROGRAM. THIS SPACE WILL BE CONSTRUCTED TO MEET THE REQUIREMENTS NECESSARY FOR ACGME ACCREDITATION FOR THE FAMILY MEDICINE RESIDENCY PROGRAM, TO INSURE A HIGH QUALITY EDUCATION AND PHYSICAL LEARNING SPACE FOR RESIDENTS. THIS SPACE WILL ACCOMMODATE 6 RESIDENTS PER YEAR FOR EACH YEAR OF THE THREE YEAR RESIDENCY PROGRAM, FOR A TOTAL OF 18 RESIDENTS IN THE OSF ST. JOSEPH LEARNING ENVIRONMENT. OSF HAS IDENTIFIED SPACE (TOTAL OF 3,782 SQ FT) FOR THE PROGRAM IN OSF ST. JOSEPH’S EASTLAND MEDICAL PLAZA II (EMP 2), 2200 EAST WASHINGTON STREET, BLOOMINGTON, ILLINOIS 61704. THE HOSPITAL IS LOCATED CLOSE TO THE CORNER OF VETERANS PARKWAY AND EMPIRE IN BLOOMINGTON, WHICH IS A MAJOR INTERSECTION IN THE CENTER OF BLOOMINGTON – NORMAL. THE LOCATION IS FULLY OWNED BY OSF HEALTHCARE. EMP 2 IS CONNECTED TO THE HOSPITAL, WHICH IS CONVENIENT FOR THE RESIDENTS. THE MAJORITY OF THE SPACE (3,653 SQ FT) WAS FORMALLY USED AS A CARDIOLOGY CLINIC WITH OTHER AREAS (129 SQ FT) FORMALLY USED AS STORAGE AND A CONFERENCE ROOM. RENOVATING THIS AREA WILL ENABLE THE RESIDENCY PROGRAM TO HAVE ADEQUATE SPACE FOR LEARNING WHILE REMAINING CLOSE TO THE HOSPITAL. THE FAMILY MEDICINE RESIDENCY SPACE WILL INCLUDE OFFICES FOR SUPPORT STAFF (PROGRAM DIRECTOR, PROGRAM COORDINATOR, ETC.) AND FACULTY, CONFERENCE AND EDUCATION SPACE THAT INCLUDES TECHNOLOGY WITH ACCESS TO LEARNING FROM TH E OSF FLAGSHIP HOSPITAL (OSF SAINT FRANCIS MEDICAL CENTER) AND HEADQUARTERS IN PEORIA AS WELL AS OTHER SISTER HOSPITALS, BREAKROOMS, SHOWERS AND BATHROOMS, QUIET ROOMS, AND MODULAR WORK STATIONS WITH SPACE FOR CHARTING AND ACCESSING INFORMATION. NEED EARLY IN 2019 OSF ST. JOSEPH LEADERSHIP BEGAN DISCUSSIONS WITH SYSTEM LEADERSHIP AND THE UNIVERSITY OF ILLINOIS COLLEGE OF MEDICINE PEORIA REGARDING DEVELOPING A FAMILY MEDICINE RESIDENCY PROGRAM IN THE OSF EASTERN REGION (BLOOMINGTON-PONTIAC-URBANA-DANVILLE). BLOOMINGTON-NORMAL WAS IDENTIFIED AS THE CHOSEN LOCATION BECAUSE OF PRIMARY CARE NEED, MARKET DYNAMICS (GROWING), UN-OPPOSED ENVIRONMENT (MEANING RESIDENTS DO NOT HAVE TO COMPETE WITH OTHER GME RESIDENTS FOR CASES) AND AVAILABILITY OF FACULTY. OSF MEDICAL GROUP HAS A LARGE WELL-DEVELOPED PRIMARY CARE NETWORK IN AND AROUND THE BLOOMINGTON NORMAL MARKET; HOWEVER MANY OF THE PHYSICIANS ARE AT THE AGE OF RETIREMENT OR WILL BE AT THAT AGE WITHIN THE NEXT 5 YEARS. IN TODAY’S WORLD, IT IS VERY HARD TO RECRUIT PHYSICIANS TO DOWNSTATE ILLINOIS UNLESS THEY ARE FROM THE AREA OR TRAINED IN THE AREA. A FAMILY MEDICINE RESIDENCY PROGRAM WILL HELP OSF GROW FAMILY PRACTICE PHYSICIANS AND INSURE LONG TERM ACCESS TO PRIMARY CARE SERVICES FOR THE COMMUNITY. RESEARCH STUDIES HAVE DOCUMENTED A LINK BETWEEN THE NUMBER OF PRIMARY CARE PHYSICIANS AND APP’S IN AN AREA AND THE HEALTH STATUS OF THE POPULATION. THE 2020 POPULATION OF THE BLOOMINGTON-NORMAL AREA WAS ESTIMATED TO BE 201,268. IN CONTRAST TO THE REST OF ILLINOIS, THE AREA EXPERIENCED AN INCREASE IN POPULATION FROM 2010, WHEN THE POPULATION WAS 186,444. THE POPULATION AGES 65 YEARS AND OVER INCREASED FROM 20,720 (2010) TO 31,689 (2020), A CHANGE OF 10,969 (52.9%). THE POPULATION IS 77.3% WHITE; 9.7% BLACK; 8.6% ASIAN; 6.2% HISPANIC OR LATINO; AND 2.9% TWO OR MORE RACES.
