ADVANCING SEXUAL HEALTH AND EQUITY IN PHILADELPHIA WITH YOUTH EXPERIENCING POVERTY - AS DESCRIBED IN GREATER DETAIL BELOW, ACCESSMATTERS’ AIMS TO REACH 1655 YOUTH AGES 13-19 IN PHILADELPHIA, AND OUR PRIORITY POPULATION IS YOUTH LIVING AT OR BELOW 150% OF THE FEDERAL POVERTY LEVEL (FPL), MANY OF WHOM ARE SYSTEM-IMPACTED AND ARE LIVING WITH COMPLEX TRAUMA. THIS POPULATION WAS PRIORITIZED FOR THE FOLLOWING REASONS. FIRST, THE PROPORTION OF CHILDREN IN PHILADELPHIA LIVING IN HOUSEHOLDS WITH INCOME AT OR BELOW 150% OF THE FEDERAL POVERTY LEVEL IS NEARLY TWICE THAT OF THE U.S. GENERALLY. SECOND, THE BURDEN OF TRAUMA THAT YOUNG PHILADELPHIANS FACE IS DISPROPORTIONALLY LARGE COMPARED TO THE REST OF THE COUNTRY, AND TRAUMA IS LINKED TO WORSE HEALTH OUTCOMES. THIRD, SEXUAL AND REPRODUCTIVE HEALTH (SRH) OUTCOMES FOR PHILADELPHIA’S YOUTH – AND PARTICULARLY FOR YOUTH LIVING IN POVERTY – ARE SIGNIFICANTLY POORER THAN THOSE FOR THE AVERAGE YOUTH IN PENNSYLVANIA AND IN THE U.S. FOURTH, THERE IS AN IDENTIFIED NEED FOR EXPANDED ACCESS TO SRH SERVICES AND EDUCATION FOR YOUTH IN PHILADELPHIA, AS IS SUPPORTED BY OUR WORK WITH KEY MEMBERS OF THE COMMUNITY. THE PROPOSED PROJECT WILL UTILIZE ACCESSMATTERS’ HEALTHCARE NETWORK MANAGEMENT EXPERIENCE AND EXPERTISE IN EDUCATION AND ENGAGEMENT AROUND ADOLESCENT SRH TO BRING TOGETHER COMMUNITY ORGANIZATIONS AND OTHER STAKEHOLDERS TO DIRECTLY ADDRESS THE SRH NEEDS OF THE YOUTH OF PHILADELPHIA, LEADING TO REDUCTIONS IN KEY MARKERS SUCH AS UNINTENDED PREGNANCY AND SEXUALLY TRANSMITTED INFECTIONS (STIS). THROUGH THREE SETTINGS – EDUCATIONAL, CLINICAL AND COMMUNITY-BASED – WE AIM TO REACH 1655 YOUTH, 250 PARENTS/CAREGIVERS, AND 200 PROFESSIONALS PER YEAR.

SPECIFICALLY, WE WILL DEVELOP THE AI PLATFORM Μ3.AI AND APPLY IT TO OUD DRUG DISCOVERY. THIS PLATFORM WILL GENERATE MULTI-CELL TYPE, MULTI- OBJECTIVE, MULTI-TARGET LEAD COMPOUNDS TO RESTORE OUD PATIENT OMICS PROFILES TO A NON-ADDICTIVE STATE. Μ3.AI COMBINES FOUR AI MODELS THAT 1) PREDICTS DOSE-DEPENDENT CHEMICAL-INDUCED CELL VIABILITY AND TRANSCRIPTOMICS FOR PHENOTYPE-BASED DRUG DISCOVERY, 2) PROVIDES ACCURATE GENOME-SCALE BINDING AFFINITY PREDICTIONS, 3) SEARCHES FOR MOLECULES WITH DESIRED EFFICACY AND SAFETY PROFILES, AND 4) PRIORITIZES CELL TYPE-SPECIFIC OUD TARGETS. ADDITIONALLY, WE WILL DEVELOP A HUMAN-IN-THE-LOOP ACTIVE LEARNING STRATEGY. THIS APPROACH INVOLVES CLOSE COLLABORATION BETWEEN MEDICINAL CHEMISTS AND AI SCIENTISTS TO ITERATIVELY REFINE Μ3.AI AND DISCOVER NEW POLYPHARMACOLOGICAL THERAPEUTICS. THIS STRATEGY UTILIZES REINFORCEMENT LEARNING FROM HUMAN FEEDBACK (RLHF) TO ENHANCE THE AI PLATFORM. IT SEAMLESSLY INTEGRATES AI-POWERED MOLECULAR GENERATION, PROPERTY PREDICTIONS, ORGANIC SYNTHESIS, IN VITRO ASSAYS, AND COMPOUND TESTING IN CELL LINE MODELS. IN PHASE I, ESTABLISHED ON THE SOLID EVIDENCES FROM PATIENT-DERIVED SINGLE CELL RNA-SEQ STUDIES, WE WILL TARGET OXYCODONE DYSREGULATES TYPE I INTERFERON SIGNALING IN NEURONS AND ASTROCYTES. OUR GOAL IS TO RESTORE THE EXPRESSION OF MULTIPLE DYSREGULATED GENES (STAT1, ISG15, BST2, IFI44L, IFITM3, IFI27, PARP14, PLSCR1, B2M, P2RY14, ETC.) WORKING WITH ONE OF THE LEADING CIVIC ENGAGEMENT ORGANIZATIONS, ENCORE.ORG, WE ARE AT THE FOREFRONT IN THE NATIONAL MOVEMENT TO TRANSFORM MODELS OF COMMUNITY VOLUNTEER ENGAGEMENT AND HEALTHY AGING. OVER THE LAST FIVE YEARS, EM HAS PLACED NEARLY 1,000 INDIVIDUALS AGE 50+ IN OVER 500 SOCIAL PURPOSE ORGANIZATIONS THROUGH OUR VARIOUS MATCHING PROGRAMS. EM IS THE CONDUIT CONNECTING THE SKILLS, TALENTS AND PASSION OF THESE INDIVIDUALS TO BUILD CAPACITY AND RESOURCES IN THE SOCIAL SECTOR. EM VIEWS SENIOR CORPS RSVP AS A POWERFUL PATHWAY FOR OLDER ADULTS TO ENGAGE IN, STRENGTHEN AND BUILD RESILIENCY IN OUR COMMUNITY. SINCE 2014, EM HAS BEEN THE SPONSORING AGENCY FOR THE RSVP PROGRAM THAT ENCOMPASSES THE GREATER PHOENIX AREA AND SURROUNDING COMMUNITIES TO THE NORTH AND WEST OF PHOENIX. OUR PLAN IS TO CONTINUE THE PROGRAMMATIC COMPONENTS THAT HAVE BEEN HIGHLY SUCCESSFUL IN TERMS OF OUTCOMES, PARTNERSHIPS AND INTERGENERATIONAL VOLUNTEER ENGAGEMENT. THE MARICOPA COUNTY CENSUS POPULATION WAS 4,167,947 (2015) WITH 14.2% OF RESIDENTS 65+ YEARS. BY 2020, IT IS PROJECTED THAT 25% OF MARICOPA COUNTY RESIDENTS WILL BE 65+ YEARS. THE SERVICE AREA FOR THE PROPOSED PROGRAM INCLUDES MARICOPA COUNTY CITIES EXCEPT TEMPE, MESA, CHANDLER, GILBERT, GUADALUPE AND QUEEN CREEK -- FROM HERE ON WILL BE REFERRED TO AS MARICOPA COUNTY. THIS MARICOPA COUNTY REGION IS CURRENTLY SERVED BY EM. APPROXIMATELY 13% OF THE POPULATION IN THIS SERVICE AREA ARE INDIVIDUALS 65 AND OLDER. FOR THE PROPOSED RSVP PROJECT, WE ANTICIPATE AN ESTIMATED 190 RSVP VOLUNTEERS WILL SERVE. OF THIS NUMBER, 130 RSVP VOLUNTEERS WILL BE PLACED IN OUTCOME ASSIGNMENTS. SERVICE ACTIVITIES WILL BE FOCUSED IN TWO AREAS. EDUCATION (K-12 SUCCESS) WHICH INCLUDES SCHOOL AND COMMUNITY-BASED TUTORING/MENTORING IN EARLY LITERACY, MATH AND COLLEGE AND CAREER READINESS AND HEALTHY FUTURES (AGING IN PLACE) WHICH INCLUDES TRANSPORTATION AND FOOD DELIVERY. THE PRIMARY FOCUS AREA OF THIS PROJECT IS EDUCATION (K-12 SUCCESS). AT THE END OF THE THREE-YEAR GRANT, WE ANTICIPATE AT LEAST 80% OF THE STUDENTS WHO COMPLETE THE SCHOOL-BASED EARLY LITERACY AND MATH TUTORING WILL SHOW IMPROVED ACADEMIC PERFORMANCE. THE CNCS FEDERAL INVESTMENT OF $95,034 WILL BE SUPPLEMENTED BY $40,720 FOR THE GRANT PERIOD.

ACCESSMATTERS CONTINUES TO EVOLVE IN A CHANGING HEALTHCARE ENVIRONMENT TO MEET THE UNIQUE NEEDS OF WICY CLIENTS THROUGHOUT PHILADELPHIA. FOR OVER THIRTY YEARS, ACCESSMATTERS’ RWPD NETWORK, THE LARGEST IN PHILADELPHIA, HAS IMPROVED ACCESS TO PRIMARY HIV MEDICAL CARE FOR HIV POSITIVE WICY THROUGH THE PROVISION OF COORDINATED, COMPREHENSIVE, CULTURALLY AND LINGUISTICALLY COMPETENT SERVICES. THESE SERVICES ARE PROVIDED IN PART DIRECTLY BY ACCESSMATTERS AND IN PART THROUGH CONTRACTUAL PARTNERSHIPS WITH A NETWORK OF WELL-ESTABLISHED AND RESPECTED CLINICAL AND SUPPORT SERVICE PROVIDERS. TO FURTHER ITS MISSION, ACCESSMATTERS IS REQUESTING $150,000 TO IMPLEMENT GENDER-AFFIRMING CARE TRAINING TO ITS NETWORK IN ORDER TO IMPROVE THE QUALITY OF HIV SERVICES PROVIDED TO TRANSGENDER AND GENDER DIVERSE (TGD) CLIENTS. FALLING UNDER HIV CARE INNOVATION: GENDER-AFFIRMING CARE AND FOCUSED ON STIGMA AND DISCRIMINATION, THIS PROGRAM BUILDS UPON ACCESSMATTERS’ ROLE AND PREVIOUS SUPPLEMENTAL FUNDING BY CONTINUING TO GROW THE CAPACITY OF RYAN WHITE PARTS A, C, AND D FUNDED ORGANIZATIONS TO PROMOTE LINKAGE TO AND LIFELONG RETENTION IN HIV MEDICAL CARE AMONG PEOPLE LIVING WITH HIV. PHILADELPHIA’S HIV INCIDENCE AND PREVALENCE RATES REMAIN CONSISTENTLY HIGHER THAN THOSE OF THE COMMONWEALTH OF PENNSYLVANIA, THE UNITED STATES OVERALL, AND SIMILAR METROPOLITAN AREAS. AS IS TRUE ACROSS THE COUNTRY, THERE ARE SIGNIFICANT DISPARITIES IN WHO IS MOST IMPACTED BY HIV/AIDS IN THE CITY. WICY ARE DEEPLY IMPACTED BY THE EPIDEMIC, WITH FURTHER DISPARITIES AMONG WICY WHO IDENTIFY AS TGD. AS WILL BE DEMONSTRATED IN THE PROJECT NARRATIVE, ACCESSMATTERS’ EXPERT TRAINING TEAM AND ITS CONTRACTED PARTNER WILL PROVIDE CAPACITY-BUILDING TRAININGS TO THE RWPD NETWORK IN ORDER TO IMPROVE THE QUALITY OF CLINICAL SERVICES PROVIDED TO TGD CLIENTS. THESE TRAININGS WILL BE INFORMED BY BOTH CLIENTS AND SUB-RECIPIENT STAFF IN ORDER TO ENSURE THAT MEANINGFUL CHANGES CAN BE MADE THROUGHOUT THE COURSE OF THE PROJECT. EM VIEWS SENIOR CORPS RSVP AS A POWERFUL PATHWAY FOR OLDER ADULTS TO ENGAGE IN, STRENGTHEN AND BUILD RESILIENCY IN OUR COMMUNITY. SINCE 2014, EM HAS BEEN THE SPONSORING AGENCY FOR THE RSVP PROGRAM THAT ENCOMPASSES THE GREATER PHOENIX AREA. OUR PLAN IS TO CONTINUE THE PROGRAMMATIC COMPONENTS THAT HAVE BEEN HIGHLY SUCCESSFUL IN TERMS OF OUTCOMES, PARTNERSHIPS AND INTERGENERATIONAL VOLUNTEER ENGAGEMENT. THE MARICOPA COUNTY CENSUS POPULATION WAS 4,410,824 (2018) WITH 15.2% OF RESIDENTS 65+ YEARS. BY 2020, IT IS PROJECTED THAT 25% OF MARICOPA COUNTY RESIDENTS WILL BE 65+ YEARS. THE PROPOSED SERVICE AREA FOR THE PROGRAM IS MARICOPA COUNTY INCLUDING TEMPE, MESA, CHANDLER, GILBERT, GUADALUPE AND QUEEN CREEK WITH NO ADDITIONAL FUNDING REQUESTED. THIS BEST SUPPORTS PARTNERING AGENCIES WHO PROVIDE SERVICES THROUGHOUT THE COUNTY . IF THIS IS NOT ALLOWABLE, WE WILL SERVE MARICOPA COUNTY CITIES EXCLUDING TEMPE, MESA, CHANDLER, GILBERT, GUADALUPE AND QUEEN CREEK.., WE ANTICIPATE 190 RSVP VOLUNTEERS WILL SERVE IN THE PROPOSED RSVP PROJECTS. OF THIS NUMBER, 141 RSVP VOLUNTEERS WILL BE PLACED IN OUTCOME ASSIGNMENTS. SERVICE ACTIVITIES WILL BE FOCUSED IN TWO AREAS. EDUCATION (K-12 SUCCESS) WHICH INCLUDES SCHOOL AND COMMUNITY-BASED TUTORING/MENTORING IN EARLY LITERACY, MATH AND COLLEGE AND CAREER READINESS AND HEALTHY FUTURES (AGING IN PLACE) SPECIFICALLY TRANSPORTATION. THE PRIMARY FOCUS AREA OF THIS PROJECT IS EDUCATION (K-12 SUCCESS). AT THE END OF THE THREE-YEAR GRANT, WE ANTICIPATE AT LEAST 80% OF THE STUDENTS WHO COMPLETE THE SCHOOL-BASED EARLY LITERACY AND MATH TUTORING WILL SHOW IMPROVED ACADEMIC PERFORMANCE. TRACE MATTERS SCIENTIFIC LLC PROPOSES TO DESIGN DEVELOP AND PROTOTYPE A MINIATURE AMBIENT DESORPTION IONIZATION AND EXTRACTION SOURCE (MADIE) AS A COMPACT ALLINONE INSTRUMENT FOR OPERATION UNDER THE AMBIENT MARTIAN ENVIRONMENT TO SEQUENTIALLY DESORB IONIZE AND EXTRACT ANALYTES FROM MARTIAN SAMPLES. THE MADIE WILL ENABLE IN SITU INTERROGATION OF THE MARTIAN MINERALOGICAL SAMPLES WITH NO SAMPLE PREPARATION AND/OR SEPARATION WHEN COUPLED TO A MASS SPECTROMETER. AT THE CORE OF THE PROPOSED MADIE WILL BE A SELFTUNING PLASMA IONIZATION MODULE FOR SAMPLE IONIZATION CONSISTING OF AN AMBIENT CARBON DIOXIDE PLASMA SOURCE AND A TUNING CIRCUIT; AN ION FUNNEL MODULE FOR EFFICIENT ION EXTRACTION; AND A LASER DIODE MODULE FOR TIMERESOLVED SAMPLE DESORPTION. THE AMBIENT PLASMA SOURCE WILL UTILIZE THE MARTIAN ATMOSPHERE WHICH IS MAINLY COMPOSED OF CARBON DIOXIDE TO FORM A REDUCEDPRESSURE CARBON DIOXIDE INDUCTIVELY COUPLED PLASMA (ICP) TO IONIZE THE PLUME OF SAMPLED ANALYTES. THE TUNING CIRCUIT WILL COMPENSATE ANY PLASMA VARIATION AND MAINTAIN THE PLASMA SOURCE AT RESONANCE DURING OPERATION. THE ION FUNNEL MODULE WHICH WILL BE DERIVED BY A RADIO FREQUENCY POWER SUPPLY WILL EFFICIENTLY EXTRACT THE IONS AND GUIDE THEM INTO THE MASS SPECTROMETER TO INCREASE SENSITIVITY. THE LASER DIODE MODULE WILL PRODUCE A PLUME OF SAMPLE ANALYTES FROM THE SAMPLE SURFACE WITH HIGH SPATIAL RESOLUTION AT BOTH ULTRAVIOLET (UV) AND INFRARED (IR) WAVELENGTHS. DURING PHASE I THE SELFTUNING PLASMA IONIZATION MODULE WILL BE DESIGNED AND CONSTRUCTED AND THE ION FUNNEL WILL BE DESIGNED AND SIMULATED WITH SIMION AND THEN PROTOTYPED. COMMERCIALLYAVAILABLE LASER MODULES AT VARIOUS WAVELENGTHS WILL BE SELECTED AND EXPERIMENTALLY VALIDATED. THESE MODULES WILL THEN BE PIECED TOGETHER TO DEVELOP A BREADBOARD PROTOTYPE OF THE MADIE BY THE CONCLUSION OF PHASE I. PRELIMINARY EXPERIMENTS WILL BE CONDUCTED TO TEST THE EFFICIENCY OF THE MADIE.
