THE PROGRAM SUPPORTS HOLISTIC, SURVIVOR-CENTERED APPROACHES TO PROVIDING TRANSITIONAL HOUSING AND SUPPORT SERVICES DESIGNED TO ENABLE A MINOR, AN ADULT, OR A DEPENDENT OF SUCH MINOR OR ADULT LOCATE AND SECURE PERMANENT HOUSING, SECURE EMPLOYMENT, AND INTEGRATE INTO A COMMUNITY. THE HUMAN DEVELOPMENT COMMISSION IS A NON-PROFIT ORGANIZATION LOCATED IN LAPEER, MICHIGAN, A RURAL REGION OF THE STATE. THE ORGANIZATION WILL PROVIDE SCATTERED SITE RESIDENCES FOR 18 SURVIVORS AND THEIR FAMILIES THROUGH PRIVATE LANDLORD HOUSING UNITS. THE HUMAN DEVELOPMENT COMMISSION WILL COLLABORATE WITH ITS 1 PARTNER, GENESEE SHIAWASSEE THUMB MICHIGAN WORKS! (GST), TO PROVIDE A HOLISTIC, VICTIM-CENTERED AND MULTIDISCIPLINARY APPROACH TO TRANSITIONAL HOUSING NEEDS IN THE COMMUNITY. THIS NEW PROJECT WILL USE FUNDS TO PROVIDE BOTH HOUSING AND SUPPORTIVE SERVICES TO MOVE SURVIVORS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, OR STALKING WHO ARE HOMELESS OR IN NEED OF HOUSING ASSISTANCE TO PERMANENT HOUSING. THE PROJECT WILL ASSIST CLIENTS FOR AN ANTICIPATED MINIMUM OF 6 AND A MAXIMUM LENGTH OF 24 MONTHS. SERVICES OFFERED WILL FOCUS ON EXPANDING ECONOMIC JUSTICE. THE GRANT ACTIVITIES WILL BE TAILORED TO MEET THE NEEDS OF THE SURVIVORS. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. SOME OF THEIR ACTIVITIES WILL INCLUDE TUTORING ELEMENTARY SCHOOL-AGED CHILDREN, DELIVERING MEALS TO THE HOMEBOUND ELDERLY, TRANSPORTING NON-MOBILE SENIORS TO HEALTH CARE APPOINTMENTS, ASSISTING LOCAL HEALTH DEPARTMENTS WITH PRESCRIPTION TAKE-BACK EVENTS, MAKING REASSURANCE TELEPHONE CALLS TO THE FRAIL (OFTEN HOMEBOUND) ELDERLY, AND VISITING WITH ISOLATED AND/OR VICTIMIZED SENIORS. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. AT THE END OF THE THREE-YEAR GRANT, THE FOLLOWING OUTCOMES ARE ANTICIPATED. IN THE PRIMARY FOCUS AREA OF HEALTHY FUTURES WILL BE 370 INDIVIDUALS WHO REPORT HAVING INCREASED SOCIAL SUPPORT OR IMPROVED CAPACITY FOR INDEPENDENT LIVING (H9A), 850 INDIVIDUALS WHO REPORT INCREASED FOOD SECURITY (H12), AND 150 INDIVIDUALS WITH INCREASED HEALTH KNOWLEDGE (H17). THIS TOTAL INCLUDES A MINIMUM OF 15 INDIVIDUALS RECEIVING COMPANIONSHIP SERVICES, 875 INDIVIDUALS RECEIVING NUTRITION/FOOD SUPPORT, 75 INDIVIDUALS RECEIVING TRANSPORTATION SERVICES, AND 75 INDIVIDUALS RECEIVING OPIOID/DRUG INTERVENTION AS A RESULT OF CNCS-SUPPORTED SERVICES. IN THE EDUCATION FOCUS AREA, THERE WILL BE 10 STUDENTS IN GRADES K-12 THAT PARTICIPATED IN TUTORING SERVICES WHO WILL DEMONSTRATE IMPROVED ACADEMIC ENGAGEMENT OR SOCIAL-EMOTIONAL SKILLS (ED27C).

HDC'S FOSTER GRANDPARENT PROGRAM SERVES THE "THUMB" AREA OF MICHIGAN'S LOWER PENINSULA WHICH INCLUDES THE FGP SERVICE AREA OF HURON, SANILAC, AND TUSCOLA COUNTIES. INCORPORATED IN 1965, HDC IS A NONPROFIT, COMMUNITY-BASED ORGANIZATION THAT IS COMMITTED TO HELPING PEOPLE ACHIEVE SELF-SUFFICIENCY AS CONDITIONS OF ABUSE, HOMELESSNESS, HUNGER, JOBLESSNESS, AND OTHER LIFE CRISES ARE AMELIORATED. THE AGENCY ADMINISTERS NEARLY 70 COMMUNITY SERVICE PROGRAMS, AND SERVICE DELIVERY IS DESIGNED TO CHANGE THE LIVES OF INDIVIDUALS, FAMILIES, AND COMMUNITIES, ESPECIALLY LOW-INCOME HOUSEHOLDS AND NEIGHBORHOODS. ADOPTED IN FEBRUARY 2011, HDC'S MISSION STATEMENT "RESTORING HOPE BY HELPING PEOPLE AND CHANGING LIVES", IS A CONSTANT REMINDER OF THE AGENCY'S COMMITMENT TO MOVE PEOPLE OUT OF POVERTY AND INTO SELF-SUFFICIENCY. HDC WILL FULFILL AN ESTIMATED 34 VOLUNTEER SERVICE YEARS. OF THIS NUMBER, 34 VOLUNTEERS WILL BE PLACED IN OUTCOME ASSIGNMENTS. SOME OF THEIR ACTIVITIES WILL INCLUDE ASSISTING IN CLASSROOMS, SCHOOL BASED MENTORING, WORKING WITH STUDENTS IN POVERTY, AND ACADEMIC SKILL BUILDING. THE PRIMARY FOCUS AREA OF THIS PROJECT IS EDUCATION. AT THE END OF THE THREE-YEAR GRANT, 12 STUDENTS WILL SHOW SUCCESS IN SCHOOL READINESS AND 155 STUDENTS WILL SHOW ACADEMIC IMPROVEMENT IN K-12 SUCCESS. THE CNCS FEDERAL INVESTMENT OF $14,727 WILL BE SUPPLEMENTED BY THE STATE OF MICHIGAN'S AGING AND ADULT SERVICES AGENCY IN THE AMOUNT TO $148,264.
