IT HAS BEEN OPERATING AS SUCH SINCE THEN SUPPORTING 40 MEMBER ORGANIZATIONS, 39 PROGRAM AWARDEES AND ONE FEDERALLY QUALIFIED HEALTH CENTER LOOK ALIKE. MPCA IS A NON-PROFIT 501(C)3 THAT OFFERS OPERATIONAL SUPPORT AND TRAINING TO MEMBER HEALTH CENTERS TO ENHANCE THE DELIVERY OF INTEGRATED CARE INCLUSIVE OF PRIMARY CARE, DENTAL, VISION, AND BEHAVIORAL HEALTH. THE MISSION OF MPCA IS TO ENHANCE INTEGRATED CARE THROUGH COMMUNITY HEALTH CENTERS WHILE INFLUENCING POLICY AT THE STATE AND NATIONAL LEVEL. ITS VISION IS QUALITY INTEGRATED CARE FOR ALL. MPCA EMBRACES CORPORATE VALUES THAT INCLUDE PUTTING PATIENTS FIRST IN ALL DECISION MAKING. MEMBER SUPPORT IS CRITICAL TO THE SUCCESS OF ALL MPCA ENDEAVORS. COLLABORATION AND INFORMATION SHARING AMONG THE MEMBERSHIP, PARTNERING ORGANIZATIONS, AND AGENCIES IS ESSENTIAL FOR THE IMPROVED HEALTH OF THE POPULATIONS SERVED. INNOVATION AND CONTINUOUS IMPROVEMENTS IN THE WORKPLACE CRITICAL TO MAINTAINING RESPONSIVENESS AND UTILITY TO CLIENTS ROUND OUT MPCA’S VALUES. MQIN, A BOARD COMMITTEE OF MPCA, IS PEER-LED THROUGH A GOVERNANCE STRUCTURE COMPRISED OF HEALTH CENTER MEMBERS. ITS’ VISION IS TO PROMOTE ENHANCED SHARING OF INFORMATION AND EXPERTISE AS WELL AS TO ADDRESS CRITICAL OPERATIONAL AND CLINICAL PERFORMANCE THROUGH GREATER PROGRAM INTEGRATION. IT SERVES AS A PLATFORM FOR PROVIDING NETWORKING SUPPORT AND TECHNICAL EXPERTISE TO A CON SORTIUM OF HEALTH CENTERS ACROSS THE STATE. MQIN PARTICIPANTS (76.92%) WERE RECOGNIZED NATIONALLY BY HRSA FOR THEIR ADVANCING HEALTH INFORMATION TECHNOLOGY FOR QUALITY AND EXCEEDED NATIONAL RATES FOR GOLD AND SILVER COMMUNITY HEALTH CENTER QUALITY RECOGNITION. THE HEALTHCARE SYSTEM IN THE UNITED STATES IS EXPERIENCING AN UNPRECEDENTED SHIFT FROM A VOLUME-BASED TO A VALUE-BASED PAYMENT SYSTEM. VALUE, DEFINED AS A FUNCTION OF COST AND QUALITY ACROSS A GIVEN POPULATION, WILL BE A DRIVER FOR PAYMENT, PATIENT CARE, AND CARE TEAM SATISFACTION. ULTIMATELY, THE FOCUS AREA OF THE MQIN IS HEALTH EQUITY UTILIZING THE QUADRUPLE AIM OF 1) IMPROVING PATIENT EXPERIENCE 2) IMPROVING THE HEALTH OF OUR POPULATION 3) REDUCING COSTS AND 4) PROVIDER EXPERIENCE. SERVICES WILL PROMOTE THE EFFECTIVE LEVERAGING OF IT SOLUTIONS FOR IMPROVEMENTS IN CLINICAL QUALITY, PATIENT-CENTERED CARE, AND PROVIDER AND STAFF WELL-BEING AT ALL MQIN SITES. PARTICIPATING HEALTH CENTERS WILL EXPERIENCE SUPPORT AND EXPERTISE THAT WILL ENABLE THEM TO DELIVER HIGH-QUALITY, CULTURALLY COMPETENT, EQUITABLE COMPREHENSIVE PRIMARY HEALTH CARE USING DIGITAL HEALTH TOOLS. THIS WILL IMPROVE THEIR CAPABILITY FOR DATA SHARING AND DATA ANALYSIS, THAT WILL IMPACT HEALTH OUTCOMES AND EFFECTIVELY ADVANCE HEALTH EQUITY. THE IMPACT OF THIS GRANT WILL BE ACCOMPLISHED THROUGH 299 SITES WITHIN 40 HEALTH CENTERS, SERVING A TOTAL PATIENT POPULATION OF OVER 623,340 (PER UDS 2020 DATA). THIS ENHANCED VALUE WILL ALLOW HEALTH CENTERS TO EFFECTIVELY COMPETE IN THE MARKETPLACE AND DEMONSTRATE TANGIBLE IMPROVEMENTS IN POPULATION HEALTH, CLINICAL OUTCOMES, HEALTH EQUITY, SOCIAL DETERMINANTS OF HEALTH, AND THE PATIENT AND PROVIDER/STAFF EXPERIENCE. IT IS MQIN’S OVERARCHING GOAL TO ACCOMPLISH THIS THROUGH DELIVERY VIA DIGITAL HEALTH TOOLS AND DATA SHARING AND ENHANCED IT INFRASTRUCTURE THROUGHOUT MICHIGAN.
