THE FUNDING WILL ENHANCE CHAG?S CAPACITY TO INCREASE HIV TESTING AND REFERRALS TO PARTNER SERVICES, LINK PERSONS WITH HIV TO HIV MEDICAL CARE AND ANTI-RETROVIRUS TREATMENT (ART), PROVIDE INTERNAL LINKAGE TO ESSENTIAL SERVICES, INCLUDING LIFE POINTS, CHAG?S SYRINGE SERVICES PROGRAM, AND RE-ENGAGE THOSE HIV INFECTED INDIVIDUALS LOST TO CARE. THE PROPOSED PROGRAM WILL BE BUILT ACROSS THE CDC FOUR PILLARS: DIAGNOSE, TREAT, PREVENT, AND RESPOND. DIAGNOSING WILL INCREASE THE NUMBER OF PERSONS WHO ARE AWARE OF THEIR HIV STATUS, AND THOUGH INTEGRATED SCREENINGS, THE NUMBER OF PERSONS AWARE OF HEPATITIS AND/OR STI STATUS. TREATING WILL INCREASE RECEIPT OF HIV MEDICAL CARE AND ART FOR THOSE NEWLY DIAGNOSED WITH HIV, AND THOSE PREVIOUSLY DIAGNOSED BUT NOT-IN-CARE.
