TRAINING WILL BE PROVIDED USING MHP SALUD’S LEAD (LISTEN, EMPOWER, ADVANCE, DELIVER) CURRICULUM, WHICH IS APPROVED BY THE FLORIDA CERTIFICATION BOARD FOR CHW CERTIFICATION. FOLLOWING TRAINING, NEW CHWS TRAINED THROUGH THE PROGRAM WILL GAIN EXPERIENCE IN THE CHW FIELD BY EITHER A) PARTICIPATING IN FIELD EXPERIENCE PLACEMENTS WITH LOCAL FLORIDA ORGANIZATIONS, OR B) BEING REFERRED TO A REGISTERED APPRENTICESHIP PROGRAM (RAP). EXISTING CHWS PARTICIPATING IN THE PROGRAM WILL HAVE THE OPPORTUNITY TO EARN CONTINUING EDUCATION UNITS (CEUS) TO MAINTAIN CHW CERTIFICATION OR UPSKILL THROUGH MHP SALUD’S CHW SUPERVISOR TRAINING. ALL CHWS TRAINED THROUGH THE PROGRAM WILL RECEIVED INDIVIDUALIZED SUPPORT AND CASE MANAGEMENT TO ENSURE THEY ARE ABLE TO COMPLETE TRAINING AND OBTAIN EMPLOYMENT AS A CHW IN AN UNDERSERVED FLORIDA COMMUNITY. - OBJECTIVES: O OBJECTIVE 1: MHP SALUD WILL TRAIN 200 CHWS OVER THE PROJECT PERIOD (Y1: 60, Y2: 70, Y3: 70). ?OBJECTIVE 1A: OF THE CHWS TRAINED THROUGH THE PROJECT, AT LEAST 150 WILL BE NEW CHWS (Y1: 45, Y2: 53, Y3: 52). ?OBJECTIVE 1B: 75% OF NEW CHWS TRAINED THROUGH THE PROJECT WILL PARTICIPATE IN A FIELD EXPERIENCE PLACEMENT OR BE REFERRED TO A RAP. O OBJECTIVE 2: MHP SALUD WILL EXPAND ITS CHW LEAD CURRICULUM TO INCLUDE ADDITIONAL MODULES ON CHW AND PUBLIC HEALTH CORE COMPETENCIES AS WELL AS SPECIFIC HEALTH TOPICS. O OBJECTIVE 3: 90% OF PARTICIPANTS ENROLLED IN MHP SALUD’S CHW TRAINING PROGRAM WILL COMPLETE THE PROGRAM WITH A PASSING SCORE. O OBJECTIVE 4: 75% OF CHWS TRAINED THROUGH THE PROJECT WILL OBTAIN FLORIDA CHW CERTIFICATION WITHIN ONE YEAR OF COMPLETING TRAINING AND FIELD PLACEMENT. O OBJECTIVE 5: 90% OF EXISTING CHWS SEEKING CEUS THROUGH THE PROJECT WILL COMPLETE THE TRAINING WITH A PASSING SCORE. O OBJECTIVE 6: 90% OF CHWS PARTICIPATING IN MHP SALUD’S CHW SUPERVISOR TRAINING WILL COMPLETE THE PROGRAM WITH A PASSING SCORE. ?OBJECTIVE 6A: 80% OF CHWS THAT COMPLETE MHP SALUD’S CHW SUPERVISOR TRAINING WILL REPORT AN INCREASE IN THEIR CONFIDENCE TO PERFORM MANAGEMENT AND LEADERSHIP SKILLS. IN THE SIX-COUNTY PROPOSED TARGET LOCATION, THAT EQUATES TO APPROXIMATELY 88,000 OF ELIGIBLE CHILDREN THAT ARE NOT ENROLLED IN MEDICAID/CHIP. AS A CURRENT CONNECTING KIDS TO COVERAGE HEALTHY KIDS 2022 GRANTEE, MHP SALUD’S HEALTH OUTCOME PROGRAM WILL CONTINUE ITS CURRENT WORK IN THE RIO GRANDE VALLEY AND INCREASE ITS CAPACITY BY EXPANDING TO EL PASO AND WEBB COUNTIES. THROUGH THIS PROJECT, FOUR COMMUNITY HEALTH WORKERS/PROMOTORES(AS) DE SALUD WILL LEAD COMMUNITY OUTREACH AND PROVIDE CULTURALLY AND LINGUISTICALLY TAILORED INDIVIDUAL ENROLLMENT ASSISTANCE. TWO OF THE CHWS WILL REMAIN IN THE RGV AREA AND ONE CHW WILL BE ASSISTING INDIVIDUALS IN EL PASO AND WEBB COUNTIES, RESPECTIVELY. ADDITIONALLY, CHWS WILL REFER PARTICIPANTS IN NEED OF FURTHER SERVICES TO ADDITIONAL RESOURCES AS ISSUES ARE IDENTIFIED. WE WILL WORK WITH PARENTS, CURRENTLY RECEIVING MEDICAID TO FURTHER ENGAGE AND PROMOTE OUR INDIVIDUALIZED APPLICATION ASSISTANCE. OVER THE THREE-YEAR PERIOD, AN ESTIMATED 2,755 (Y1: 800; Y2: 1,050; Y3: 905) CHILDREN, ADULTS AND PREGNANT INDIVIDUALS WILL RECEIVE INDIVIDUALIZED ASSISTANCE WITH MEDICAID OR CHIP APPLICATIONS IF APPLICABLE. STAFF WILL ALSO DELIVER TRAININGS TO SUBSTANCE MISUSE PROVIDERS AND DEVELOP A MEDIA CAMPAIGN TAILORED TO REACH YOUTH IN THE REGION. MHP SALUD IS APPLYING FOR $997,493.73 OVER THE 5-YEAR PROJECT PERIOD. PROPOSED ACTIVITIES DELIVERED IN COLLABORATION WITH LOCAL COMMUNITY PARTNERS WILL SUPPORT THE PROJECT GOAL, WHICH IS TO PROVIDE CULTURALLY AND LINGUISTICALLY TAILORED PREVENTION EDUCATION AND NAVIGATION SERVICES TO HISPANIC/LATINO YOUTH AGES 13-24 LIVING IN ORANGE COUNTY. IN ORANGE COUNTY, NEARLY 1 IN 3 YOUTH ARE HISPANIC/LATINO. HIV AND SUBSTANCE MISUSE ARE ESPECIALLY PREVALENT IN THE AREA. BETWEEN 2009-2017, THERE WAS A 33% INCREASE IN HIV AMONG YOUTH 13-24. ROUGHLY 23% OF HIGH SCHOOL STUDENTS CURRENTLY DRINK ALCOHOL; 6% HAVE USED COCAINE; AND 4% HAVE INJECTED ILLEGAL DRUGS. AND WHEN EXAMINING DEMOGRAPHICS OF YOUTH, HISPANIC/LATINOS HAVE THE HIGHEST RATE OF USE COMPARED TO OTHER GROUPS.3-5 THE PROMOTOR(A) WILL CONDUCT OUTREACH, PROVIDING INFORMATION AND DISSEMINATING TAILORED RESOURCES. OUTREACH EFFORTS, WHICH WILL BE SUPPORTED BY COMMUNITY LEADERS, WILL REACH APPROXIMATELY 7,800 COMMUNITY RESIDENTS. CLASSES WILL FOLLOW THE CUIDATE CURRICULUM, A TAILORED HIV PREVENTION PROGRAM. AN ESTIMATED 540 (Y1:90; Y2:120; Y3:120; Y4:120; Y5:90) YOUTH WILL PARTICIPATE. PARTICIPANTS WILL DEMONSTRATE THE FOLLOWING STATISTICALLY SIGNIFICANT OUTCOMES: INCREASE IN POSITIVE ATTITUDES REGARDING HIV RISK-REDUCTION BEHAVIORS; IMPROVEMENTS IN BELIEFS REGARDING HIV RISK-REDUCTION BEHAVIORS; INCREASE IN SELF-EFFICACY REGARDING HIV RISK-REDUCTION BEHAVIORS; INCREASE IN KNOWLEDGE REGARDING HIV RISK-REDUCTION BEHAVIORS; AND AN INCREASE IN THE PROPORTION OF PARTICIPANTS TESTED FOR HIV. TO ADDRESS EXISTING STIGMA SURROUNDING SUBSTANCE MISUSE AND HIV, THE AGENCY’S MARKETING DIVISION WILL DEVELOP A POSITIVE AND EMPOWERING AWARENESS CAMPAIGN TO IMPROVE KNOWLEDGE AND AWARENESS OF HIV AND SUBSTANCE MISUSE AMONG LOCAL HISPANIC/LATINO YOUTH. O OBJECTIVE 3: 90% OF PARTICIPANTS ENROLLED IN MHP SALUD’S CHW TRAINING PROGRAM WILL COMPLETE THE PROGRAM WITH A PASSING SCORE. O OBJECTIVE 4: 75% OF CHWS TRAINED THROUGH THE PROJECT WILL OBTAIN FLORIDA CHW CERTIFICATION WITHIN ONE YEAR OF COMPLETING TRAINING AND FIELD PLACEMENT. O OBJECTIVE 5: 90% OF EXISTING CHWS SEEKING CEUS THROUGH THE PROJECT WILL COMPLETE THE TRAINING WITH A PASSING SCORE. O OBJECTIVE 6: 90% OF CHWS PARTICIPATING IN MHP SALUD’S CHW SUPERVISOR TRAINING WILL COMPLETE THE PROGRAM WITH A PASSING SCORE. ?OBJECTIVE 6A: 80% OF CHWS THAT COMPLETE MHP SALUD’S CHW SUPERVISOR TRAINING WILL REPORT AN INCREASE IN THEIR CONFIDENCE TO PERFORM MANAGEMENT AND LEADERSHIP SKILLS. HEALTH OUTCOME - CONNECT AND CARE - HISPANIC/LATINO CHILDREN, PARENTS, AND PREGNANT INDIVIDUALS IN HIDALGO, CAMERON, STARR, WILLACY, EL PASO, AND WEBB COUNTIES ARE IN NEED OF ASSISTANCE IN APPLYING FOR MEDICAID AND CHIP TO INCREASE THE RATE OF MEDICAL COVERAGE IN THE TARGET LOCATION. NATIONALLY, 89.8 PERCENT OF UNINSURED CHILDREN ARE ELIGIBLE FOR MEDICAID OR CHIP BUT NOT CURRENTLY ENROLLED. IN THE SIX-COUNTY PROPOSED TARGET LOCATION, THAT EQUATES TO APPROXIMATELY 88,000 OF ELIGIBLE CHILDREN THAT ARE NOT ENROLLED IN MEDICAID/CHIP. AS A CURRENT CONNECTING KIDS TO COVERAGE HEALTHY KIDS 2022 GRANTEE, MHP SALUD’S HEALTH OUTCOME PROGRAM WILL CONTINUE ITS CURRENT WORK IN THE RIO GRANDE VALLEY AND INCREASE ITS CAPACITY BY EXPANDING TO EL PASO AND WEBB COUNTIES. THROUGH THIS PROJECT, FOUR COMMUNITY HEALTH WORKERS/PROMOTORES(AS) DE SALUD WILL LEAD COMMUNITY OUTREACH AND PROVIDE CULTURALLY AND LINGUISTICALLY TAILORED INDIVIDUAL ENROLLMENT ASSISTANCE. TWO OF THE CHWS WILL REMAIN IN THE RGV AREA AND ONE CHW WILL BE ASSISTING INDIVIDUALS IN EL PASO AND WEBB COUNTIES, RESPECTIVELY. ADDITIONALLY, CHWS WILL REFER PARTICIPANTS IN NEED OF FURTHER SERVICES TO ADDITIONAL RESOURCES AS ISSUES ARE IDENTIFIED. WE WILL WORK WITH PARENTS, CURRENTLY RECEIVING MEDICAID TO FURTHER ENGAGE AND PROMOTE OUR INDIVIDUALIZED APPLICATION ASSISTANCE. OVER THE THREE-YEAR PERIOD, AN ESTIMATED 2,755 (Y1: 800; Y2: 1,050; Y3: 905) CHILDREN, ADULTS AND PREGNANT INDIVIDUALS WILL RECEIVE INDIVIDUALIZED ASSISTANCE WITH MEDICAID OR CHIP APPLICATIONS IF APPLICABLE. ADDITIONALLY, OVER THE THREE-YEAR PERIOD, AN ESTIMATED 3,060 (Y1: 945; Y2: 1,115; Y3: 1,000) CHILDREN, ADULTS AND PREGNANT INDIVIDUALS WILL RECEIVE ASSISTANCE WITH RENEWAL APPLICATIONS FOR MEDICAID OR CHIP. HEALTH OUTCOME - HEALTHY LIVING INITIATIVE - MHP SALUD’S PROJECT, HEALTH OUTCOME – HEALTHY LIVING INITIATIVE WILL IMPROVE HIV AND SUBSTANCE MISUSE PREVENTION EFFORTS FOR HISPANIC/LATINO YOUTH AGES 13-24 IN ORANGE COUNTY, FLORIDA. THE PROJECT WILL BE LED BY A COMMUNITY HEALTH WORKER/PROMOTOR(A) DE SALUD, WHO WILL CONDUCT OUTREACH AND DELIVER EVIDENCE-BASED EDUCATION. STAFF WILL ALSO DELIVER TRAININGS TO SUBSTANCE MISUSE PROVIDERS AND DEVELOP A MEDIA CAMPAIGN TAILORED TO REACH YOUTH IN THE REGION. MHP SALUD IS APPLYING FOR $997,493.73 OVER THE 5-YEAR PROJECT PERIOD. PROPOSED ACTIVITIES DELIVERED IN COLLABORATION WITH LOCAL COMMUNITY PARTNERS WILL SUPPORT THE PROJECT GOAL, WHICH IS TO PROVIDE CULTURALLY AND LINGUISTICALLY TAILORED PREVENTION EDUCATION AND NAVIGATION SERVICES TO HISPANIC/LATINO YOUTH AGES 13-24 LIVING IN ORANGE COUNTY. IN ORANGE COUNTY, NEARLY 1 IN 3 YOUTH ARE HISPANIC/LATINO. HIV AND SUBSTANCE MISUSE ARE ESPECIALLY PREVALENT IN THE AREA. BETWEEN 2009-2017, THERE WAS A 33% INCREASE IN HIV AMONG YOUTH 13-24. ROUGHLY 23% OF HIGH SCHOOL STUDENTS CURRENTLY DRINK ALCOHOL; 6% HAVE USED COCAINE; AND 4% HAVE INJECTED ILLEGAL DRUGS. AND WHEN EXAMINING DEMOGRAPHICS OF YOUTH, HISPANIC/LATINOS HAVE THE HIGHEST RATE OF USE COMPARED TO OTHER GROUPS.3-5 THE PROMOTOR(A) WILL CONDUCT OUTREACH, PROVIDING INFORMATION AND DISSEMINATING TAILORED RESOURCES. OUTREACH EFFORTS, WHICH WILL BE SUPPORTED BY COMMUNITY LEADERS, WILL REACH APPROXIMATELY 7,800 COMMUNITY RESIDENTS. CLASSES WILL FOLLOW THE CUIDATE CURRICULUM, A TAILORED HIV PREVENTION PROGRAM. AN ESTIMATED 540 (Y1:90; Y2:120; Y3:120; Y4:120; Y5:90) YOUTH WILL PARTICIPATE. PARTICIPANTS WILL DEMONSTRATE THE FOLLOWING STATISTICALLY SIGNIFICANT OUTCOMES: INCREASE IN POSITIVE ATTITUDES REGARDING HIV RISK-REDUCTION BEHAVIORS; IMPROVEMENTS IN BELIEFS REGARDING HIV RISK-REDUCTION BEHAVIORS; INCREASE IN SELF-EFFICACY REGARDING HIV RISK-REDUCTION BEHAVIORS; INCREASE IN KNOWLEDGE REGARDING HIV RISK-REDUCTION BEHAVIORS; AND AN INCREASE IN THE PROPORTION OF PARTICIPANTS TESTED FOR HIV. TO ADDRESS EXISTING STIGMA SURROUNDING SUBSTANCE MISUSE AND HIV, THE AGENCY’S MARKETING DIVISION WILL DEVELOP A POSITIVE AND EMPOWERING AWARENESS CAMPAIGN TO IMPROVE KNOWLEDGE AND AWARENESS OF HIV AND SUBSTANCE MISUSE AMONG LOCAL HISPANIC/LATINO YOUTH. TO STRENGTHEN EXISTING NETWORKS OF CARE, MHP SALUD STAFF WILL ADDITIONALLY DELIVER NINE TRAININGS TO PROVIDERS ON THE IMPORTANCE OF HIV TESTING AND CLINICAL CARE. PARTICIPANTS WILL DEMONSTRATE A STATISTICALLY SIGNIFICANT INCREASE IN KNOWLEDGE OF HIV TESTING; AN INCREASE IN KNOWLEDGE OF HIV CLINICAL CARE; AND AN INCREASE IN INTENTION TO COLLABORATE WITH HIV PROVIDERS.
