JULIE SCHULLER CONTACT PHONE NUMBER: 414-897-5192 EMAIL ADDRESS: JULIE.SCHULLER@SSCHC.ORG WEBSITE: WWW.SSCHC.ORG FUNDING REQUEST: $1,250,000 FOR OVER 50 YEARS, SIXTEENTH STREET COMMUNITY HEALTH CENTERS (SSCHC) HAS PROVIDED SERVICES TO PEOPLE RESIDING ON MILWAUKEE’S SOUTH SIDE, AND IN 2012 EXPANDED TO WAUKESHA, WI. THE MISSION OF SSCHC IS TO IMPROVE THE HEALTH AND WELL-BEING OF THE INDIVIDUALS SERVED BY PROVIDING QUALITY PRIMARY HEALTH CARE, MENTAL HEALTH CARE, SOCIAL SERVICES, AND HEALTH EDUCATION IN A MANNER THAT IS FREE OF LINGUISTIC, CULTURAL, AND FINANCIAL BARRIERS. THESE FUNDS WILL HELP SSCHC BUILD OUT A LONG OVERDUE AND NEEDED PHARMACY DIRECTLY CONNECTED TO OUR FLAGSHIP CHAVEZ CLINIC ON CESAR CHAVEZ DRIVE (ZIP CODE 53204). THE 53204 ZIP CODE IS ONE OF THE MOST DENSELY POPULATED IN WI, YET IS LOCATED IN A PHARMACY DESERT WITH FOUR PHARMACIES. THE NATIONAL AVERAGE RATIO OF 1 PHARMACY: 5,000 PEOPLE SUGGESTS THAT WITH A POPULATION SIZE OF 40,486 RESIDENTS THERE SHOULD BE EIGHT PHARMACIES TO MEET THE NEED IN 53204. ADDITIONALLY, THE 53204 COMMUNITY IS LARGELY LATINO (70%) AND LOW-INCOME (70%) (UDS MAPPER), MOST OF WHOM PREFER A LANGUAGE OTHER THAN ENGLISH. THERE ARE SIGNIFICANT DISPARITIES IN MEDICATION ADHERENCE AMONG UNDERSERVED MINORITY GROUPS, INCLUDING LATINOS. THIS LACK OF ADHERENCE IS OFTEN ASSOCIATED WITH POOR COMMUNICATION AND CULTURAL INTERVENTIONS FROM HEALTH CARE PROVIDERS. FUNDING WILL BE USED TO CONSTRUCT A NEW 2,162 SQUARE FOOT PHARMACY SPACE AS A COMPONENT OF A NEW ADDITION TO THE EXISTING CHAVEZ CLINIC. THE SPACE WILL BE DIRECTLY ACCESSED FROM THE EXISTING CLINIC RECEPTION AND WAITING AREA. THE DESIGN GOAL IS TO MAXIMIZE PATIENT FLOW TO AND FROM THE PHARMACY AND TO MAXIMIZE THE PHARMACY’S VISIBILITY. A NEW PHARMACY WILL ALLOW SSCHC TO BE BETTER EQUIPPED TO IMPROVE MEDICA TION ADHERENCE AND COORDINATE CARE BETWEEN PROVIDER AND THE PHARMACY, ULTIMATELY RESULTING IN IMPROVED ACCESS AND OUTCOMES FOR OUR PATIENTS.
