BETWEEN 2017-2021 CH SERVED 345 MEMBERS OF THE TARGET POPULATION. OF THESE, 91% IDENTIFIED AS WHITE; 6% AS BLACK; 0% AS ASIAN; 2% AS TWO OR MORE RACES; 1% AS OTHER RACES; AND 0% AS HISPANIC OR LATINO. THE MEDIAN AGE WAS 33; 100% WERE LOW-INCOME; 94% WERE UNEMPLOYED AT ENTRY; AND 59% WERE HIGH SCHOOL GRADUATES OR HIGHER. OF THE PARTICIPANTS SERVED AT CHRYSALIS DURING 2021, 74% WERE PREGNANT AND 90.7% HAD MINOR CHILDREN. MISUSED SUBSTANCES INCLUDED METHAMPHETAMINE (72.1%), FOLLOWED BY HEROIN (46.5%), MARIJUANA (32.6%), AND OTHER OPIATES (23%); 48.8% INDICATED FAIR OR POOR HEALTH; 74.4% REPORTED EXPERIENCING DEPRESSION AND/OR ANXIETY IN THE LAST 30 DAYS, 22% REPORTED BEING PRESCRIBED MEDICATIONS FOR MENTAL HEALTH; 6.8% RECEIVED OUTPATIENT TREATMENT IN THE PAST 30 DAYS. OVER 80% REPORTED VIOLENCE OR TRAUMA OVER THEIR LIFETIME. CHRYSALIS WILL UTILIZE A TRAUMA INFORMED APPROACH TO PROVIDE EVIDENCE-BASED PRACTICES SUCH AS GROUP CURRICULA, MEDICALLY ASSISTED TREATMENT, CBT, AND FAMILY AND PARENTING INTERVENTIONS CHOSEN FOR THEIR EFFECTIVENESS WITH THE TARGET POPULATION; IN-HOUSE AND COMMUNITY-BASED RECOVERY SUPPORT SERVICES; AND PREVENTION AND HARM REDUCTION STRATEGIES TO PROMOTE LONG-TERM RECOVERY AND SAFE, HEALTHY, AND STABLE FAMILIES. GOALS AND OBJECTIVES FOR THE PROJECT INCLUDE: 1) TO ENSURE PROJECT IMPLEMENTATION, EVALUATION, AND CONTINUOUS QUALITY IMPROVEMENT. 2) TO DECREASE THE USE AND/OR ABUSE OF PRESCRIPTION AND ILLICIT DRUGS AND OTHER HARMFUL DRUGS AMONG PPW. OBJ: A. CONDUCT ONGOING COMPREHENSIVE CLINICAL SCREENING AND ASSESSMENTS TO DETERMINE THE APPROPRIATE ASAM LEVEL OF CARE (LOC) AND INFORM TREATMENT PLANNING AND SERVICE DELIVERY IN A CONTINUOUS CARE MODEL; B. PROVIDE COMPREHENSIVE SUD TREATMENT SERVICES, RECOVERY SUPPORT SERVICES, AND HARM REDUCTION INTERVENTIONS AT THE APPROPRIATE LOC RESULTING IN IMPROVED TREATMENT OUTCOMES. 3) TO DECREASE THE PERINATAL EFFECTS OF MATERNAL SUD, INCLUDING SYMPTOMOLOGY OF NAS AMONG BABIES BORN TO WOMEN WITH A PRIMARY OR SECONDARY DIAGNOSIS OF OUD RECEIVING RESIDENTIAL TREATMENT. OBJ: A. ENSURE QUALITY PRENATAL, POSTPARTUM, AND ONGOING MEDICAL CARE, INCLUDING MAT IF APPROPRIATE AND DESIRED, FOR WOMEN RECEIVING RESIDENTIAL TREATMENT; B. PROVIDE EVIDENCE-BASED PRENATAL PARENTING CLASSES TO 100% OF PREGNANT WOMEN RECEIVING RESIDENTIAL TREATMENT; C. 100% OF PREGNANT CLIENTS WILL RECEIVE PRENATAL CARE, AND 100% OF INFANTS AND CHILDREN PARTICIPATING PROGRAM WILL RECEIVE COMPREHENSIVE, INDIVIDUALIZED SERVICES. 4) TO IMPROVE PARENTING SKILLS, FAMILY FUNCTIONING, ECONOMIC STABILITY, AND QUALITY OF LIFE FOR PREGNANT AND PARENTING WOMEN, THEIR MINOR CHILDREN AND EXTENDED FAMILY MEMBERS THROUGH THE PROVISION OF COMPREHENSIVE SUD TREATMENT, RECOVERY SUPPORT SERVICES, AND HARM REDUCTION INTERVENTIONS. OBJ: A. PROVIDE EVIDENCE-BASED PARENTING INTERVENTIONS WHICH WILL RESULT IN IMPROVED PARENTING SKILLS: B. (CCI) RESEARCH FOCUSES ON THE CURRENT OPERATION AND INTERRELATED COMPONENTS OF ITS FOOD HUB THAT IS LOCATED IN NORTH HARTFORD'S PROMISE ZONE (NHPZ) IN CONNECTICUT (CT) THAT IS CATEGORIZED AS A LOW-INCOME, LOW-ACCESS (LILA) FOOD DESERT. IN CT, HARTFORD IS RANKED AS HAVING THE HIGHEST RATE OF FOOD INSECURITY OUT OF ALL 169 TOWNS AND THE 8TH WORST CITY NATIONALLY, AMONG CITIES WITH A SIMILAR POPULATION, FOR PROVIDING LOW-INCOME RESIDENTS WITH ACCESS TO HEALTHY FOODS. IN GREATER HARTFORD, 1 IN 7 ADULTS LACKS RELIABLE ACCESS TO AFFORDABLE, NUTRITIOUS FOOD, WHICH PUTS THEM AT AN INCREASED RISK FOR A VARIETY OF ADVERSE HEALTH OUTCOMES, INCLUDING HEART DISEASE, HYPERTENSION, AND TYPE 2 DIABETES. FOR THESE REASONS, CCI WILL ENGAGE IN COMMUNITY PARTICIPATION TO ENHANCE ITS COMMUNITY FOOD HUB TO MEET THE NEEDS OF THE NORTH HARTFORD COMMUNITY THROUGH FOOD DISTRIBUTION AND COMMUNITY OUTREACH, INCREASING THE SELF-RELIANCE OF LOW-INCOME COMMUNITIES IN PROVIDING FOR THEIR OWN FOOD NEEDS AND PROMOTING COMPREHENSIVE RESPONSE TO LOCAL FOOD ACCESS. FOOD HUB COMPONENTS INCLUDE A FOOD PANTRY, A HYDROPONIC GROW CENTER, RAISED BED GARDENS, FOOD FOREST AND A FARMER'S MARKET.CCI'S KEY PARTNERSHIP FOR THE PLANNING PROJECT IS WITH THE RUDD CENTER FOR FOOD POLICY AND HEALTH AT THE UNIVERSITY OF CONNECTICUT WHOSE MISSION IS TO PROMOTE SOLUTIONS TO FOOD INSECURITY, POOR DIET QUALITY AND WEIGHT BIAS THROUGH RESEARCH AND POLICY. WITH THE ASSISTANCE OF THE RUDD CENTER, CCI WILL ASSESS HOW PEOPLE CURRENTLY ACCESS ALL COMPONENTS OF CCI'S FOOD HUB AND IDENTIFY STRATEGIES FOR INCREASED OUTREACH AND ENGAGEMENT. CCI WILL ALSO FULLY EXAMINE EACH FOOD HUB COMPONENT IN TERMS OF OPERATIONS AND IMPACT IN THE COMMUNITY MEASURED BY KEY BENCHMARKS. CCI SEEKS TO ELEVATE THE RELEVANCE AND SUSTAINABILITY OF THE FOOD HUB BY INCORPORATING THIS INITIATIVE IN HARTFORD'S DEVELOPMENT PLANS WHICH INCLUDES THE PLAN OF CONSERVATION AND DEVELOPMENT (POCD), CONSOLIDATED PLAN ANDANNUAL ACTION PLANS FOR HUD FUNDING, AND COMPREHENSIVE ECONOMIC DEVELOPMENT STRATEGY (CEDS). USING A SOCIAL DETERMINANT OF HEALTH (SDOH) LENS, THE FOOD HUB WILL ALSO BE INCORPORATED INTO THE LOCAL HOSPITAL'S COMMUNITY HEALTH IMPROVEMENT PLANS (CHIP).CCI'S ULTIMATE GOALS ARE TO SHARE THE SUCCESSES OF OUR PARTNERSHIPS THAT WILL HELP DRIVE PUBLIC POLICY ON SUPPORTING LOCAL FOOD HUBS AND IMPROVE CREATE A FIVE-YEAR BUSINESS PLAN FOR THE FOOD HUB TO STRATEGICALLY IDENTIFY OPTIMAL LEVELS OF INVESTMENT TO ENSURE ITS SUSTAINABILITY. RESIDENTIAL TREATMENT PROGRAM FOR PREGNANT AND POSTPARTUM WOMEN AND THEIR CHILDREN - CHRYSALIS HOUSE, INC., (CH) LOCATED IN LEXINGTON, KY, REQUESTS $525,000 FOR YEAR ONE YEAR GRANT FUNDING OF A FIVE-YEAR PROJECT FOR RESIDENTIAL TREATMENT FOR PREGNANT AND POSTPARTUM (PPW) WOMEN. CH WILL PROVIDE LOW-INCOME PROJECT PARTICIPANTS COMPREHENSIVE, INDIVIDUALIZED SCREENINGS AND ASSESSMENTS THAT INFORM TREATMENT PLANNING AND SERVICE DELIVERY IN A CONTINUOUS CARE MODEL ACROSS A CONTINUUM OF SPECIALTY SUD RESIDENTIAL AND OUTPATIENT LEVELS OF CARE. BETWEEN 2017-2021 CH SERVED 345 MEMBERS OF THE TARGET POPULATION. OF THESE, 91% IDENTIFIED AS WHITE; 6% AS BLACK; 0% AS ASIAN; 2% AS TWO OR MORE RACES; 1% AS OTHER RACES; AND 0% AS HISPANIC OR LATINO. THE MEDIAN AGE WAS 33; 100% WERE LOW-INCOME; 94% WERE UNEMPLOYED AT ENTRY; AND 59% WERE HIGH SCHOOL GRADUATES OR HIGHER. OF THE PARTICIPANTS SERVED AT CHRYSALIS DURING 2021, 74% WERE PREGNANT AND 90.7% HAD MINOR CHILDREN. MISUSED SUBSTANCES INCLUDED METHAMPHETAMINE (72.1%), FOLLOWED BY HEROIN (46.5%), MARIJUANA (32.6%), AND OTHER OPIATES (23%); 48.8% INDICATED FAIR OR POOR HEALTH; 74.4% REPORTED EXPERIENCING DEPRESSION AND/OR ANXIETY IN THE LAST 30 DAYS, 22% REPORTED BEING PRESCRIBED MEDICATIONS FOR MENTAL HEALTH; 6.8% RECEIVED OUTPATIENT TREATMENT IN THE PAST 30 DAYS. OVER 80% REPORTED VIOLENCE OR TRAUMA OVER THEIR LIFETIME. CHRYSALIS WILL UTILIZE A TRAUMA INFORMED APPROACH TO PROVIDE EVIDENCE-BASED PRACTICES SUCH AS GROUP CURRICULA, MEDICALLY ASSISTED TREATMENT, CBT, AND FAMILY AND PARENTING INTERVENTIONS CHOSEN FOR THEIR EFFECTIVENESS WITH THE TARGET POPULATION; IN-HOUSE AND COMMUNITY-BASED RECOVERY SUPPORT SERVICES; AND PREVENTION AND HARM REDUCTION STRATEGIES TO PROMOTE LONG-TERM RECOVERY AND SAFE, HEALTHY, AND STABLE FAMILIES. GOALS AND OBJECTIVES FOR THE PROJECT INCLUDE: 1) TO ENSURE PROJECT IMPLEMENTATION, EVALUATION, AND CONTINUOUS QUALITY IMPROVEMENT. 2) TO DECREASE THE USE AND/OR ABUSE OF PRESCRIPTION AND ILLICIT DRUGS AND OTHER HARMFUL DRUGS AMONG PPW. OBJ: A. CONDUCT ONGOING COMPREHENSIVE CLINICAL SCREENING AND ASSESSMENTS TO DETERMINE THE APPROPRIATE ASAM LEVEL OF CARE (LOC) AND INFORM TREATMENT PLANNING AND SERVICE DELIVERY IN A CONTINUOUS CARE MODEL; B. PROVIDE COMPREHENSIVE SUD TREATMENT SERVICES, RECOVERY SUPPORT SERVICES, AND HARM REDUCTION INTERVENTIONS AT THE APPROPRIATE LOC RESULTING IN IMPROVED TREATMENT OUTCOMES. 3) TO DECREASE THE PERINATAL EFFECTS OF MATERNAL SUD, INCLUDING SYMPTOMOLOGY OF NAS AMONG BABIES BORN TO WOMEN WITH A PRIMARY OR SECONDARY DIAGNOSIS OF OUD RECEIVING RESIDENTIAL TREATMENT. OBJ: A. ENSURE QUALITY PRENATAL, POSTPARTUM, AND ONGOING MEDICAL CARE, INCLUDING MAT IF APPROPRIATE AND DESIRED, FOR WOMEN RECEIVING RESIDENTIAL TREATMENT; B. PROVIDE EVIDENCE-BASED PRENATAL PARENTING CLASSES TO 100% OF PREGNANT WOMEN RECEIVING RESIDENTIAL TREATMENT; C. 100% OF PREGNANT CLIENTS WILL RECEIVE PRENATAL CARE, AND 100% OF INFANTS AND CHILDREN PARTICIPATING PROGRAM WILL RECEIVE COMPREHENSIVE, INDIVIDUALIZED SERVICES. 4) TO IMPROVE PARENTING SKILLS, FAMILY FUNCTIONING, ECONOMIC STABILITY, AND QUALITY OF LIFE FOR PREGNANT AND PARENTING WOMEN, THEIR MINOR CHILDREN AND EXTENDED FAMILY MEMBERS THROUGH THE PROVISION OF COMPREHENSIVE SUD TREATMENT, RECOVERY SUPPORT SERVICES, AND HARM REDUCTION INTERVENTIONS. OBJ: A. PROVIDE EVIDENCE-BASED PARENTING INTERVENTIONS WHICH WILL RESULT IN IMPROVED PARENTING SKILLS: B. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. (CCI) RESEARCH FOCUSES ON THE CURRENT OPERATION AND INTERRELATED COMPONENTS OF ITS FOOD HUB THAT IS LOCATED IN NORTH HARTFORD'S PROMISE ZONE (NHPZ) IN CONNECTICUT (CT) THAT IS CATEGORIZED AS A LOW-INCOME, LOW-ACCESS (LILA) FOOD DESERT. IN CT, HARTFORD IS RANKED AS HAVING THE HIGHEST RATE OF FOOD INSECURITY OUT OF ALL 169 TOWNS AND THE 8TH WORST CITY NATIONALLY, AMONG CITIES WITH A SIMILAR POPULATION, FOR PROVIDING LOW-INCOME RESIDENTS WITH ACCESS TO HEALTHY FOODS. IN GREATER HARTFORD, 1 IN 7 ADULTS LACKS RELIABLE ACCESS TO AFFORDABLE, NUTRITIOUS FOOD, WHICH PUTS THEM AT AN INCREASED RISK FOR A VARIETY OF ADVERSE HEALTH OUTCOMES, INCLUDING HEART DISEASE, HYPERTENSION, AND TYPE 2 DIABETES. FOR THESE REASONS, CCI WILL ENGAGE IN COMMUNITY PARTICIPATION TO ENHANCE ITS COMMUNITY FOOD HUB TO MEET THE NEEDS OF THE NORTH HARTFORD COMMUNITY THROUGH FOOD DISTRIBUTION AND COMMUNITY OUTREACH, INCREASING THE SELF-RELIANCE OF LOW-INCOME COMMUNITIES IN PROVIDING FOR THEIR OWN FOOD NEEDS AND PROMOTING COMPREHENSIVE RESPONSE TO LOCAL FOOD ACCESS. FOOD HUB COMPONENTS INCLUDE A FOOD PANTRY, A HYDROPONIC GROW CENTER, RAISED BED GARDENS, FOOD FOREST AND A FARMER'S MARKET.CCI'S KEY PARTNERSHIP FOR THE PLANNING PROJECT IS WITH THE RUDD CENTER FOR FOOD POLICY AND HEALTH AT THE UNIVERSITY OF CONNECTICUT WHOSE MISSION IS TO PROMOTE SOLUTIONS TO FOOD INSECURITY, POOR DIET QUALITY AND WEIGHT BIAS THROUGH RESEARCH AND POLICY. WITH THE ASSISTANCE OF THE RUDD CENTER, CCI WILL ASSESS HOW PEOPLE CURRENTLY ACCESS ALL COMPONENTS OF CCI'S FOOD HUB AND IDENTIFY STRATEGIES FOR INCREASED OUTREACH AND ENGAGEMENT. CCI WILL ALSO FULLY EXAMINE EACH FOOD HUB COMPONENT IN TERMS OF OPERATIONS AND IMPACT IN THE COMMUNITY MEASURED BY KEY BENCHMARKS. CCI SEEKS TO ELEVATE THE RELEVANCE AND SUSTAINABILITY OF THE FOOD HUB BY INCORPORATING THIS INITIATIVE IN HARTFORD'S DEVELOPMENT PLANS WHICH INCLUDES THE PLAN OF CONSERVATION AND DEVELOPMENT (POCD), CONSOLIDATED PLAN ANDANNUAL ACTION PLANS FOR HUD FUNDING, AND COMPREHENSIVE ECONOMIC DEVELOPMENT STRATEGY (CEDS). USING A SOCIAL DETERMINANT OF HEALTH (SDOH) LENS, THE FOOD HUB WILL ALSO BE INCORPORATED INTO THE LOCAL HOSPITAL'S COMMUNITY HEALTH IMPROVEMENT PLANS (CHIP).CCI'S ULTIMATE GOALS ARE TO SHARE THE SUCCESSES OF OUR PARTNERSHIPS THAT WILL HELP DRIVE PUBLIC POLICY ON SUPPORTING LOCAL FOOD HUBS AND IMPROVE CREATE A FIVE-YEAR BUSINESS PLAN FOR THE FOOD HUB TO STRATEGICALLY IDENTIFY OPTIMAL LEVELS OF INVESTMENT TO ENSURE ITS SUSTAINABILITY.
