FY 2024 BEHAVIORAL HEALTH SERVICE EXPANSION - SINCE 1982, ACCESS COMMUNITY HEALTH CENTERS (ACCESS) HAS BEEN ON A MISSION TO IMPROVE THE HEALTH AND LIVES OF PEOPLE IN SOUTHERN WISCONSIN REGARDLESS OF FINANCIAL OR INSURANCE STATUS THROUGH MEDICAL, BEHAVIORAL HEALTH, DENTAL, PHARMACY, AND ANCILLARY SERVICES. FIVE CLINICS ARE IN FEDERALLY RECOGNIZED OPPORTUNITY ZONES WHERE LARGE CONCENTRATIONS OF MEDICALLY UNDERSERVED POPULATIONS EXPERIENCE SOCIAL AND ECONOMIC BARRIERS TO HEALTHCARE. DESPITE HIGH RANKINGS IN THE AVAILABILITY OF MENTAL HEALTH PROVIDERS (422 TO EVERY 100,000), THERE IS LIMITED SUPPORT FOR MEDICALLY UNDERSERVED POPULATIONS. AS A RESULT, THE PREVALENCE OF DEPRESSION, ANXIETY, AND SUBSTANCE USE DISORDER IS HIGHEST AMONG MEDICAID AND MEDICARE BENEFICIARIES, ADOLESCENTS, AND MEMBERS OF DIVERSE ETHNIC, RACIAL, AND LGBTQ+ IDENTITIES IN THE SERVICE AREA. BEHAVIORAL HEALTH IS ONE OF THE COSTLIEST CHRONIC HEALTH CONDITIONS, ACCOUNTING FOR AN ESTIMATED $201 BILLION IN HEALTHCARE COSTS NATIONALLY. SEVENTY PERCENT OF PATIENTS WITH BEHAVIORAL HEALTH DISORDERS HAVE A MEDICAL CO-MORBIDITY AND 30 PERCENT OF ADULTS WITH A PHYSICAL CONDITION ALSO HAVE A BEHAVIORAL HEALTH DISORDER. HEALTHCARE SYSTEMS SEPARATING PHYSICAL AND MENTAL HEALTH SERVICES IS A MAJOR DRIVER OF INCREASED HEALTHCARE COSTS AND FRAGMENTATION, AND OFTEN FAIL TO MEET THE PHYSICAL, MENTAL, AND BEHAVIORAL HEALTH NEEDS OF PATIENTS. ALTERNATIVELY, ACCESS HAS EMBRACED A HOLISTIC MODEL THAT PROMOTES THE INTEGRATION AND CO-LOCATION OF THESE CRUCIAL AREAS OF CARE THROUGH BEHAVIORAL HEALTH INTEGRATION (BHI). SIGNIFICANT VALUE-ADDED BENEFITS FROM THE BHI MODEL INCLUDE CLINICAL, OPERATIONAL, FINANCIAL, AND PUBLIC HEALTH IMPROVEMENTS THAT CAN BE CATALYZED WITH ADDITIONAL HRSA SUPPORT. HARNESSING TWO PRIMARY INTEGRATED HEALTHCARE MODELS RECOGNIZED BY THE CENTER OF MEDICARE AND MEDICAID SERVICES, ACCESS HAS USED A PRIMARY CARE BEHAVIORAL MODEL SINCE 2006 WHICH FORGED WITH A COLLABORATIVE CARE MODEL (COCM) IN AUGUST 2023. IN 2015, ACCESS REC EIVED HRSA FUNDING THAT SUPPORTED BHI WORKFORCE DEVELOPMENT TO REACH MORE PATIENTS WITH A BROAD RANGE OF MENTAL HEALTH AND SUBSTANCE USE DISORDER SERVICES WITHIN THE PRIMARY CARE SETTING. USING A “NO WRONG DOOR” APPROACH, ACCESS’ BHI MODEL FOCUSES ON PROVIDING THE RIGHT CARE, AT THE RIGHT TIME, IN THE RIGHT PLACE. BEHAVIORAL HEALTH CONSULTANTS (BHC) PLAY A KEY ROLE BY COLLABORATING WITH THE PRIMARY CARE PROVIDER (PCP) TO DEVELOP A WELL-ROUNDED TREATMENT PLAN THAT ADDRESSES THE PSYCHOLOGICAL NEEDS OF PATIENTS WHERE THE PCP MAY LACK EXPERTISE. CO-LOCATING BEHAVIORAL HEALTH WITHIN THE PRIMARY CARE SETTING SEEKS TO ACHIEVE THE QUADRUPLE AIM OF PATIENT EXPERIENCE, POPULATION HEALTH, CARE TEAM WELL-BEING, AND COST EFFICIENCY. WHILE ACCESS’ MULTIDISCIPLINARY BHI TEAM HAS GROWN OVER THE PAST DECADE, THE NUMBER OF PATIENTS THAT RECEIVE CARE EACH YEAR HAS INCREASED 30-FOLD. IN 2023, ACCESS PROVIDED OVER 100,000 MEDICAL, 42,000 DENTAL, AND CLOSE TO 7,000 BEHAVIORAL HEALTH VISITS TO PEOPLE FROM DANE AND SURROUNDING COUNTIES. AS THE AREA IN AND AROUND THE COUNTY OF DANE AND CITY OF MADISON CONTINUES TO EXPERIENCE UNPRECEDENTED POPULATION GROWTH, ACCESS ANTICIPATES REACHING 35,000 PEOPLE ANNUALLY BY 2025 AND PROVIDING 8,000 BEHAVIORAL HEALTH VISITS TO ADDRESS MENTAL HEALTH AND SUBSTANCE USE DISORDER CONCERNS. THROUGH THIS FUNDING OPPORTUNITY, THE INTEGRATED BEHAVIORAL HEALTH TEAM STRUCTURE WILL INCREASE CAPACITY TO MEET THE COMPREHENSIVE BIOPSYCHOSOCIAL NEEDS OF PATIENTS. WITH THE ADDITION OF TWO BEHAVIORAL HEALTH CARE COORDINATORS, A BEHAVIORAL HEALTH CONSULTANT, THREE PRIMARY CARE PROVIDERS, TWO SOCIAL WORK FELLOWS, AND A REGISTERED NURSE SPECIALIST, ACCESS WILL HAVE GREATER IMPACT ON POPULATION HEALTH THROUGH IMPROVED SUSTAINABILITY, CAPACITY, AND TEAMWORK. DURING THE GRANT PERIOD, BEHAVIORAL HEALTH INTEGRATION EXPANSION WILL RESULT IN AN 11% INCREASE IN THE NUMBER OF PATIENTS RECEIVING MOUD SERVICES AS WELL AS AN 11% INCREASE IN THE NUMBER OF PATIENTS RECEIVING MENTAL HE ALTH SERVICES.
